    Evaluation of spectral induced polarization field measurements in time and frequency domain

    Spectral induced polarization (SIP) measurements have been demonstrated to correlate with important parame-ters in hydrogeological and environmental investigations. Although SIP measurements were often collected in thefrequency domain (FDIP), recent developments have demonstrated the capabilities to solve for the frequency-de-pendence of the complex conductivity through measurements collected in the time domain (TDIP). Therefore,the aim of our field investigations is a comparison of the measured frequency-dependence at a broad frequencyrange resolved through FDIP and TDIP. In contrast to previous studies, we conducted measurements with dif-ferent instruments and measuring technologies for both FDIP and TDIP. This allows for investigating the robust-ness of different measurements and assessing various sources of errors, for the assessment of the advantages anddrawbacks from different measuring techniques. Our results demonstrate that data collected through differentinstruments are consistent. Apparent resistivity measurements as well as the inversion results revealed quantita-tively the same values for all instruments. The measurements of the IP effect are also comparable, particularlyFDIP readings in the low frequencies ( 0.1 s).However, data quality for higher frequencies in FDIP (i.e., early times in TDIP) show larger variations, whichreflects the differences between the instruments to deal with the electromagnetic contamination of the IP data.Concluded in general, the different instruments and measuring techniques can provide consistent responses forvarying signal-to-noise ratio and measuring configurations

    Tackling Lateral Variability Using Surface Waves: A Tomography-Like Approach

    Lateral velocity variations in the near-surface reflect the presence of buried geological or anthropic structures, and their identification is of interest for many fields of application. Surface wave tomography (SWT) is a powerful technique for detecting both smooth and sharp lateral velocity variations at very different scales. A surface-wave inversion scheme derived from SWT is here applied to a 2-D active seismic dataset to characterize the shape of an urban waste deposit in an old landfill, located 15 km South of Vienna (Austria). First, the tomography-derived inverse problem for the 2-D case is defined: under the assumption of straight rays at the surface connecting sources and receivers, the forward problem for one frequency reduces to a linear relationship between observed phase differences at adjacent receivers and wavenumbers (from which phase velocities are straightforwardly derived). A norm damping regularization constraint is applied to ensure a smooth solution in space: the choice of the damping parameter is made through a minimization process, by which only phase variations of the order of the average wavelength are modelled. The inverse problem is solved for each frequency with a weighted least-squares approach, to take into account the data error variances. An independent multi-offset phase analysis (MOPA) is performed using the same dataset, for comparison: pseudo-sections from the tomography-derived linear inversion and MOPA are very consistent, with the former giving a more continuous result both in space and frequency and less artefacts. Local dispersion curves are finally depth inverted and a quasi-2-D shear wave velocity section is retrieved: we identify a well-defined low velocity zone and interpret it as the urban waste deposit body. Results are consistent with both electrical and electromagnetic measurements acquired on the same line

    Mountain permafrost degradation documented through a network of permanent electrical resistivity tomography sites

    Mountain permafrost is sensitive to climate change and is expected to gradually degrade in response to the ongoing atmospheric warming trend. Long-term monitoring of the permafrost thermal state is a key task, but problematic where temperatures are close to 0 ∘C because the energy exchange is then dominantly related to latent heat effects associated with phase change (ice–water), rather than ground warming or cooling. Consequently, it is difficult to detect significant spatio-temporal variations in ground properties (e.g. ice–water ratio) that occur during the freezing–thawing process with point scale temperature monitoring alone. Hence, electrical methods have become popular in permafrost investigations as the resistivities of ice and water differ by several orders of magnitude, theoretically allowing a clear distinction between frozen and unfrozen ground. In this study we present an assessment of mountain permafrost evolution using long-term electrical resistivity tomography monitoring (ERTM) from a network of permanent sites in the central Alps. The time series consist of more than 1000 datasets from six sites, where resistivities have been measured on a regular basis for up to 20 years. We identify systematic sources of error and apply automatic filtering procedures during data processing. In order to constrain the interpretation of the results, we analyse inversion results and long-term resistivity changes in comparison with existing borehole temperature time series. Our results show that the resistivity dataset provides valuable insights at the melting point, where temperature changes stagnate due to latent heat effects. The longest time series (19 years) demonstrates a prominent permafrost degradation trend, but degradation is also detectable in shorter time series (about a decade) at most sites. In spite of the wide range of morphological, climatological, and geological differences between the sites, the observed inter-annual resistivity changes and long-term tendencies are similar for all sites of the network

    A novel thymidylate synthase from the Vibrionales, Alteromonadales, Aeromonadales, and Pasteurellales (VAAP) clade with altered nucleotide and folate binding sites

    Thymidylate synthase (TS, E.C. 2.1.1.45) is a crucial enzyme for de novo deoxythymidine monophosphate (dTMP) biosynthesis. The gene for this enzyme is thyA, which encodes the folate-dependent TS that converts deoxyuridine monophosphate group (dUMP) into (dTMP) using the cofactor 5,10-methylenetetrahydrofolate (mTHF) as a carbon donor. We identified the thyA gene in the genome of the Vibrio parahaemolyticus strain FIM-S1708+ that is innocuous to humans but pathogenic to crustaceans. Surprisingly, we found changes in the residues that bind the substrate dUMP and mTHF, previously postulated as invariant among all TSs known (Finer-Moore, Santi & Stroud, 2003). Interestingly, those amino acid changes were also found in a clade of microorganisms that contains Vibrionales, Alteromonadales, Aeromonadales, and Pasteurellales (VAAP) from the Gammaproteobacteria class. In this work, we studied the biochemical properties of recombinant TS from V. parahemolyticus FIM-S1708+ (VpTS) to address the natural changes in the TS amino acid sequence of the VAAP clade. Interestingly, the Km for dUMP was 27.3 ± 4.3 ”M, about one-fold larger compared to other TSs. The Km for mTHF was 96.3 ± 18 ”M, about three- to five-fold larger compared to other species, suggesting also loss of affinity. Thus, the catalytic efficiency was between one or two orders of magnitude smaller for both substrates. We used trimethoprim, a common antibiotic that targets both TS and DHFR for inhibition studies. The IC50 values obtained were high compared to other results in the literature. Nonetheless, this molecule could be a lead for the design antibiotics towards pathogens from the VAAP clade. Overall, the experimental results also suggest that in the VAAP clade the nucleotide salvage pathway is important and should be investigated, since the de novo dTMP synthesis appears to be compromised by a less efficient thymidylate synthase

    Evolving trends in the management of acute appendicitis during COVID-19 waves. The ACIE appy II study

    Background: In 2020, ACIE Appy study showed that COVID-19 pandemic heavily affected the management of patients with acute appendicitis (AA) worldwide, with an increased rate of non-operative management (NOM) strategies and a trend toward open surgery due to concern of virus transmission by laparoscopy and controversial recommendations on this issue. The aim of this study was to survey again the same group of surgeons to assess if any difference in management attitudes of AA had occurred in the later stages of the outbreak. Methods: From August 15 to September 30, 2021, an online questionnaire was sent to all 709 participants of the ACIE Appy study. The questionnaire included questions on personal protective equipment (PPE), local policies and screening for SARS-CoV-2 infection, NOM, surgical approach and disease presentations in 2021. The results were compared with the results from the previous study. Results: A total of 476 answers were collected (response rate 67.1%). Screening policies were significatively improved with most patients screened regardless of symptoms (89.5% vs. 37.4%) with PCR and antigenic test as the preferred test (74.1% vs. 26.3%). More patients tested positive before surgery and commercial systems were the preferred ones to filter smoke plumes during laparoscopy. Laparoscopic appendicectomy was the first option in the treatment of AA, with a declined use of NOM. Conclusion: Management of AA has improved in the last waves of pandemic. Increased evidence regarding SARS-COV-2 infection along with a timely healthcare systems response has been translated into tailored attitudes and a better care for patients with AA worldwide

    Canagliflozin and renal outcomes in type 2 diabetes and nephropathy

    BACKGROUND Type 2 diabetes mellitus is the leading cause of kidney failure worldwide, but few effective long-term treatments are available. In cardiovascular trials of inhibitors of sodium–glucose cotransporter 2 (SGLT2), exploratory results have suggested that such drugs may improve renal outcomes in patients with type 2 diabetes. METHODS In this double-blind, randomized trial, we assigned patients with type 2 diabetes and albuminuric chronic kidney disease to receive canagliflozin, an oral SGLT2 inhibitor, at a dose of 100 mg daily or placebo. All the patients had an estimated glomerular filtration rate (GFR) of 30 to <90 ml per minute per 1.73 m2 of body-surface area and albuminuria (ratio of albumin [mg] to creatinine [g], >300 to 5000) and were treated with renin–angiotensin system blockade. The primary outcome was a composite of end-stage kidney disease (dialysis, transplantation, or a sustained estimated GFR of <15 ml per minute per 1.73 m2), a doubling of the serum creatinine level, or death from renal or cardiovascular causes. Prespecified secondary outcomes were tested hierarchically. RESULTS The trial was stopped early after a planned interim analysis on the recommendation of the data and safety monitoring committee. At that time, 4401 patients had undergone randomization, with a median follow-up of 2.62 years. The relative risk of the primary outcome was 30% lower in the canagliflozin group than in the placebo group, with event rates of 43.2 and 61.2 per 1000 patient-years, respectively (hazard ratio, 0.70; 95% confidence interval [CI], 0.59 to 0.82; P=0.00001). The relative risk of the renal-specific composite of end-stage kidney disease, a doubling of the creatinine level, or death from renal causes was lower by 34% (hazard ratio, 0.66; 95% CI, 0.53 to 0.81; P<0.001), and the relative risk of end-stage kidney disease was lower by 32% (hazard ratio, 0.68; 95% CI, 0.54 to 0.86; P=0.002). The canagliflozin group also had a lower risk of cardiovascular death, myocardial infarction, or stroke (hazard ratio, 0.80; 95% CI, 0.67 to 0.95; P=0.01) and hospitalization for heart failure (hazard ratio, 0.61; 95% CI, 0.47 to 0.80; P<0.001). There were no significant differences in rates of amputation or fracture. CONCLUSIONS In patients with type 2 diabetes and kidney disease, the risk of kidney failure and cardiovascular events was lower in the canagliflozin group than in the placebo group at a median follow-up of 2.62 years

    COVID-19 symptoms at hospital admission vary with age and sex: results from the ISARIC prospective multinational observational study

    Background: The ISARIC prospective multinational observational study is the largest cohort of hospitalized patients with COVID-19. We present relationships of age, sex, and nationality to presenting symptoms. Methods: International, prospective observational study of 60 109 hospitalized symptomatic patients with laboratory-confirmed COVID-19 recruited from 43 countries between 30 January and 3 August 2020. Logistic regression was performed to evaluate relationships of age and sex to published COVID-19 case definitions and the most commonly reported symptoms. Results: ‘Typical’ symptoms of fever (69%), cough (68%) and shortness of breath (66%) were the most commonly reported. 92% of patients experienced at least one of these. Prevalence of typical symptoms was greatest in 30- to 60-year-olds (respectively 80, 79, 69%; at least one 95%). They were reported less frequently in children (≀ 18 years: 69, 48, 23; 85%), older adults (≄ 70 years: 61, 62, 65; 90%), and women (66, 66, 64; 90%; vs. men 71, 70, 67; 93%, each P < 0.001). The most common atypical presentations under 60 years of age were nausea and vomiting and abdominal pain, and over 60 years was confusion. Regression models showed significant differences in symptoms with sex, age and country. Interpretation: This international collaboration has allowed us to report reliable symptom data from the largest cohort of patients admitted to hospital with COVID-19. Adults over 60 and children admitted to hospital with COVID-19 are less likely to present with typical symptoms. Nausea and vomiting are common atypical presentations under 30 years. Confusion is a frequent atypical presentation of COVID-19 in adults over 60 years. Women are less likely to experience typical symptoms than men
