    The Use of Prasugrel and Ticagrelor in Pipeline Flow Diversion

    Background: Despite the routine clopidogrel/aspirin anti-platelet therapy, complications like thromboembolism, continue to be encountered with PED. We studied the safety and the efficacy of prasugrel in the management of clopidogrel non-responders treated for intracranial aneurysms. Methods: 437 consecutive neurosurgery patients were identified between January 2011 and May 2016. Patients allergic or having \u3c30% platelet-inhibition with a daily 75mg of clopidogrel were dispensed 10mg of prasugrel daily (n=20) or 90mg of ticagrelor twice daily (n=2). The average follow-up was 15.8 months (SD=12.4 months). Patient clinical well being was evaluated with the modified Rankin Scale (mRS) registered before the discharge and at each follow-up visit. To control confounding we used multivariable mixed-effects logistic regression and propensity score conditioning. Results: 26 of 437(5.9%) patients (mean of age 56.3 years; 62 women [14,2%]) presented with a sub-arachnoid hemorrhage. 1 patient was allergic to clopidogrel and prasugrel simultaneously. All the patients receiving prasugrel (n=22) had a mRS\u3c2 on their latest follow-up visit (mean=0.67; SD=1.15). In a multivariate analysis, clopidogrel did not affect the mRS on last follow-up, p=0.14. Multivariable logistic regression showed that clopidogrel was not associated with an increased long-term recurrence rate (odds ratio[OR], 0.17; 95%Confidence Interval [CI95%], 0.01-2.70; p=0.21) neither with an increased thromboembolic accident rate (OR, 0.46; CI95%, 0.12-1.67; p=0.36) nor with an increased hemorrhagic event rate (OR, 0.39; CI95%,0.91-1.64; p=0.20). None of the patients receiving prasugrel deceased or had a long-term recurrence nor a hemorrhagic event, only 1 patient suffered from mild aphasia subsequent to a thromboembolic event. 3 patients on clopidogrel passed during the study: (2) from acute SAH and (1) from intra-parenchymal hemorrhage. Clopidogrel was not associated with an increased mortality rate (OR, 2.18; CI95%,0.11-43.27; p=0.61). The same associations were present in propensity score adjusted models. Conclusion: In a cohort of patients treated with PED for their intracranial aneurysms, prasugrel (10mg/day) is a safe alternative to clopidogrel resistant, allergic or non-responders

    Stochastic partial differential equation based modelling of large space-time data sets

    Increasingly larger data sets of processes in space and time ask for statistical models and methods that can cope with such data. We show that the solution of a stochastic advection-diffusion partial differential equation provides a flexible model class for spatio-temporal processes which is computationally feasible also for large data sets. The Gaussian process defined through the stochastic partial differential equation has in general a nonseparable covariance structure. Furthermore, its parameters can be physically interpreted as explicitly modeling phenomena such as transport and diffusion that occur in many natural processes in diverse fields ranging from environmental sciences to ecology. In order to obtain computationally efficient statistical algorithms we use spectral methods to solve the stochastic partial differential equation. This has the advantage that approximation errors do not accumulate over time, and that in the spectral space the computational cost grows linearly with the dimension, the total computational costs of Bayesian or frequentist inference being dominated by the fast Fourier transform. The proposed model is applied to postprocessing of precipitation forecasts from a numerical weather prediction model for northern Switzerland. In contrast to the raw forecasts from the numerical model, the postprocessed forecasts are calibrated and quantify prediction uncertainty. Moreover, they outperform the raw forecasts, in the sense that they have a lower mean absolute error

    The anatomical distance of functional connections predicts brain network topology in health and schizophrenia.

    The human brain is a topologically complex network embedded in anatomical space. Here, we systematically explored relationships between functional connectivity, complex network topology, and anatomical (Euclidean) distance between connected brain regions, in the resting-state functional magnetic resonance imaging brain networks of 20 healthy volunteers and 19 patients with childhood-onset schizophrenia (COS). Normal between-subject differences in average distance of connected edges in brain graphs were strongly associated with variation in topological properties of functional networks. In addition, a club or subset of connector hubs was identified, in lateral temporal, parietal, dorsal prefrontal, and medial prefrontal/cingulate cortical regions. In COS, there was reduced strength of functional connectivity over short distances especially, and therefore, global mean connection distance of thresholded graphs was significantly greater than normal. As predicted from relationships between spatial and topological properties of normal networks, this disorder-related proportional increase in connection distance was associated with reduced clustering and modularity and increased global efficiency of COS networks. Between-group differences in connection distance were localized specifically to connector hubs of multimodal association cortex. In relation to the neurodevelopmental pathogenesis of schizophrenia, we argue that the data are consistent with the interpretation that spatial and topological disturbances of functional network organization could arise from excessive "pruning" of short-distance functional connections in schizophrenia.PEV is supported by the Medical Research Council (grant number MR/K020706/1).     HAWAII, which is noted for its equable te the lack of great variations between often obscures the importance of day-nd geographic variations in weather. ii owes its relatively uniform climate to ion with respect to the sub-tropical h pressure cell and accompanying ocean cur-The rugged topography causes salient aphic rainfall patterns. The variations in ual rainfall through short horizontal dis-re common knowledge among residents ii. Areas in the mountains immediately d Honolulu receive up to 160 inches an-nually, while at the seashore, 5.5 miles away from the mountains, less than 15 inches is re-corded. On Kauai, the annual mean varies from 450 to 20 inches in a horizontal distance of 12 miles. The changes of greatest economic importance, surprisingly enough, are those that occur be-tween consecutive months and between con-secutive years. The growing of pineapples and of sugar cane represents the two most important industries in the Hawaiian Islands. These in-dustries, which utilize a large proportion of the arable land in the rainfall zone between 20 and 100 inches, have found that the wide variations in rainfall from the mean in consecutive months and consecutive years constitute one of the most critical risks in their operations. ‘Published with the approval of the Director a
