    Nature Tourism in Muar River Valley

    Sungai Muar or Muar River is special. Passing through three states of Peninsular Malaysia, it originates in Negeri Sembilan, passes briefly in Malacca, into Johor and ends into the Straits of Malacca. Though laden with history and presently undergoing much socio-economic development, the importance of this river remains inconspicuous, as it is simply known as Sungai Muar. This paper proposes that a geographical official entity be recognized for this river and its surrounding areas as Muar River Valley (MRV) or in Malay, Lembangan Sungai Muar to enhance any future planning and development which would be site-based, thus giving due recognition to the river. Essential data was researched to produce two maps. The first map is for geophysical information of MRV, which forms the first objective of the research. To realistically prove its importance as an official entity, the tourism development was chosen. This research examined previous and potentially new nature tourism research. These are then marked on the map and thus a simple visual appreciation to understand the potential of nature tourism of MRV is easily seen. Hence, the second map is for existing tourism activities and potential tourism destinations and products in MRV. This forms the second objective. To further enhance, that venture such as nature tourism, could increase the appreciation to the importance of MRV, a nature tourism product development was done and described, forming the third objective. The exemplary product development is based on a newly recognized oyster species, endemic to MRV named as Crassostrea (Magallana) saidii. All data for the three objectives were obtained through literature search using search engine like Google. Additional methods include interviewing through phone calls, face-to-face, and also Google Meet. The information related to land use, and drainage and irrigation system around Bandar Maharani are obtained, along with the topography around the extent of Muar District are obtained as well, as basic requirement for the proposal of MRV. Finally the newly recognized oyster is developed into a tourism product of which the 3 hours package include activities such as observing traditional harvesting of the oysters and tasting of oyster dishes. To complete the nature tourism product development, prototypes of souvenirs are made from shells of the new oysters. For conservation purpose, souvenirs were also made from common species. This recommendation to officiate the designation of MRV would be made to the state government of Johore, through the Muar District Office

    Efficacy of telemedicine for the management of cardiovascular disease: a systematic review and meta-analysis

    BACKGROUND: Telemedicine has been increasingly integrated into chronic disease management through remote patient monitoring and consultation, particularly during the COVID-19 pandemic. We did a systematic review and meta-analysis of studies reporting effectiveness of telemedicine interventions for the management of patients with cardiovascular conditions. METHODS: In this systematic review and meta-analysis, we searched PubMed, Scopus, and Cochrane Library from database inception to Jan 18, 2021. We included randomised controlled trials and observational or cohort studies that evaluated the effects of a telemedicine intervention on cardiovascular outcomes for people either at risk (primary prevention) of cardiovascular disease or with established (secondary prevention) cardiovascular disease, and, for the meta-analysis, we included studies that evaluated the effects of a telemedicine intervention on cardiovascular outcomes and risk factors. We excluded studies if there was no clear telemedicine intervention described or if cardiovascular or risk factor outcomes were not clearly reported in relation to the intervention. Two reviewers independently assessed and extracted data from trials and observational and cohort studies using a standardised template. Our primary outcome was cardiovascular-related mortality. We evaluated study quality using Cochrane risk-of-bias and Newcastle-Ottawa scales. The systematic review and the meta-analysis protocol was registered with PROSPERO (CRD42021221010) and the Malaysian National Medical Research Register (NMRR-20–2471–57236). FINDINGS: 72 studies, including 127 869 participants, met eligibility criteria, with 34 studies included in meta-analysis (n=13 269 with 6620 [50%] receiving telemedicine). Combined remote monitoring and consultation for patients with heart failure was associated with a reduced risk of cardiovascular-related mortality (risk ratio [RR] 0·83 [95% CI 0·70 to 0·99]; p=0·036) and hospitalisation for a cardiovascular cause (0·71 [0·58 to 0·87]; p=0·0002), mostly in studies with short-term follow-up. There was no effect of telemedicine on all-cause hospitalisation (1·02 [0·94 to 1·10]; p=0·71) or mortality (0·90 [0·77 to 1·06]; p=0·23) in these groups, and no benefits were observed with remote consultation in isolation. Small reductions were observed for systolic blood pressure (mean difference –3·59 [95% CI –5·35 to –1·83] mm Hg; p<0·0001) by remote monitoring and consultation in secondary prevention populations. Small reductions were also observed in body-mass index (mean difference –0·38 [–0·66 to –0·11] kg/m(2); p=0·0064) by remote consultation in primary prevention settings. INTERPRETATION: Telemedicine including both remote disease monitoring and consultation might reduce short-term cardiovascular-related hospitalisation and mortality risk among patients with heart failure. Future research should evaluate the sustained effects of telemedicine interventions. FUNDING: The British Heart Foundation

    Does The Smart Tourism Experience in Malaysia Increase Local Tourists' Happiness and Revisit Intentions?

    Nowadays, the term "smart" has become popular in reflecting technological advancements in social and economic improvement, as well as knowledge and information exchange. The advent of smart technology has empowered everything to be more connected, informed, and conveniently involved with clients, especially in the global tourism industry. Malaysia, on the other hand, has less research on the use of smart tourism since it is still in the early stages of development, and tourism locations are still seeking their own rhythm to thrive. In addition, there is a lack of a specific framework, numerous global-scale crises with negative impacts on tourist arrivals and the environment, and poor digitalization, which has resulted in a drop-off in tourist experiences and tourism development. This study was carried out to investigate whether local tourists are satisfied with the smart tourism technology experience in Malaysia and to explore whether smart tourism technology has a positive effect on tourists’ happiness. An online questionnaire was created using Google Form, distributed through WhatsApp, Telegram, Facebook, Instagram from May 1, 2021, to August 1, 2021, and completed by 529 participants. The results suggested the majority (&gt; 90%) of local tourists in Malaysia found difficulty searching for information about the local attractions in each state of Malaysia during travel, which caused a greater decline in their excitement to do local traveling. With reliable centralized mobile apps that exist for all tourism sectors in Malaysia, research finding showed that it could increase tourist’s revisit intention and happiness, which consequently boosts the revenue of the economy. &nbsp

    Genome-Wide Association Study in East Asians Identifies Novel Susceptibility Loci for Breast Cancer

    Genetic factors play an important role in the etiology of both sporadic and familial breast cancer. We aimed to discover novel genetic susceptibility loci for breast cancer. We conducted a four-stage genome-wide association study (GWAS) in 19,091 cases and 20,606 controls of East-Asian descent including Chinese, Korean, and Japanese women. After analyzing 690,947 SNPs in 2,918 cases and 2,324 controls, we evaluated 5,365 SNPs for replication in 3,972 cases and 3,852 controls. Ninety-four SNPs were further evaluated in 5,203 cases and 5,138 controls, and finally the top 22 SNPs were investigated in up to 17,423 additional subjects (7,489 cases and 9,934 controls). SNP rs9485372, near the TGF-β activated kinase (TAB2) gene in chromosome 6q25.1, showed a consistent association with breast cancer risk across all four stages, with a P-value of 3.8×10−12 in the combined analysis of all samples. Adjusted odds ratios (95% confidence intervals) were 0.89 (0.85–0.94) and 0.80 (0.75–0.86) for the A/G and A/A genotypes, respectively, compared with the genotype G/G. SNP rs9383951 (P = 1.9×10−6 from the combined analysis of all samples), located in intron 5 of the ESR1 gene, and SNP rs7107217 (P = 4.6×10−7), located at 11q24.3, also showed a consistent association in each of the four stages. This study provides strong evidence for a novel breast cancer susceptibility locus represented by rs9485372, near the TAB2 gene (6q25.1), and identifies two possible susceptibility loci located in the ESR1 gene and 11q24.3, respectively

    Impact of opioid-free analgesia on pain severity and patient satisfaction after discharge from surgery: multispecialty, prospective cohort study in 25 countries

    Background: Balancing opioid stewardship and the need for adequate analgesia following discharge after surgery is challenging. This study aimed to compare the outcomes for patients discharged with opioid versus opioid-free analgesia after common surgical procedures.Methods: This international, multicentre, prospective cohort study collected data from patients undergoing common acute and elective general surgical, urological, gynaecological, and orthopaedic procedures. The primary outcomes were patient-reported time in severe pain measured on a numerical analogue scale from 0 to 100% and patient-reported satisfaction with pain relief during the first week following discharge. Data were collected by in-hospital chart review and patient telephone interview 1 week after discharge.Results: The study recruited 4273 patients from 144 centres in 25 countries; 1311 patients (30.7%) were prescribed opioid analgesia at discharge. Patients reported being in severe pain for 10 (i.q.r. 1-30)% of the first week after discharge and rated satisfaction with analgesia as 90 (i.q.r. 80-100) of 100. After adjustment for confounders, opioid analgesia on discharge was independently associated with increased pain severity (risk ratio 1.52, 95% c.i. 1.31 to 1.76; P &lt; 0.001) and re-presentation to healthcare providers owing to side-effects of medication (OR 2.38, 95% c.i. 1.36 to 4.17; P = 0.004), but not with satisfaction with analgesia (beta coefficient 0.92, 95% c.i. -1.52 to 3.36; P = 0.468) compared with opioid-free analgesia. Although opioid prescribing varied greatly between high-income and low- and middle-income countries, patient-reported outcomes did not.Conclusion: Opioid analgesia prescription on surgical discharge is associated with a higher risk of re-presentation owing to side-effects of medication and increased patient-reported pain, but not with changes in patient-reported satisfaction. Opioid-free discharge analgesia should be adopted routinely

    Prevalence, associated factors and outcomes of pressure injuries in adult intensive care unit patients: the DecubICUs study

Epidemiologic data is however unavailable. We aimed to provide an international picture of the extent of pressure injuries and factors associated with ICU-acquired pressure injuries in adult ICU patients. Methods: International 1-day point-prevalence study; follow-up for outcome assessment until hospital discharge (maximum 12 weeks). Factors associated with ICU-acquired pressure injury and hospital mortality were assessed by generalised linear mixed-effects regression analysis. Results: Data from 13,254 patients in 1117 ICUs (90 countries) revealed 6747 pressure injuries; 3997 (59.2%) were ICU-acquired. Overall prevalence was 26.6% (95% confidence interval [CI] 25.9–27.3). ICU-acquired prevalence was 16.2% (95% CI 15.6–16.8). Sacrum (37%) and heels (19.5%) were most affected. Factors independently associated with ICU-acquired pressure injuries were older age, male sex, being underweight, emergency surgery, higher Simplified Acute Physiology Score II, Braden score 3 days, comorbidities (chronic obstructive pulmonary disease, immunodeficiency), organ support (renal replacement, mechanical ventilation on ICU admission), and being in a low or lower-middle income-economy. Gradually increasing associations with mortality were identified for increasing severity of pressure injury: stage I (odds ratio [OR] 1.5; 95% CI 1.2–1.8), stage II (OR 1.6; 95% CI 1.4–1.9), and stage III or worse (OR 2.8; 95% CI 2.3–3.3). Conclusion: Pressure injuries are common in adult ICU patients. ICU-acquired pressure injuries are associated with mainly intrinsic factors and mortality. Optimal care standards, increased awareness, appropriate resource allocation, and further research into optimal prevention are pivotal to tackle this important patient safety threat

    Retrospective evaluation of whole exome and genome mutation calls in 746 cancer samples

    Funder: NCI U24CA211006Abstract: The Cancer Genome Atlas (TCGA) and International Cancer Genome Consortium (ICGC) curated consensus somatic mutation calls using whole exome sequencing (WES) and whole genome sequencing (WGS), respectively. Here, as part of the ICGC/TCGA Pan-Cancer Analysis of Whole Genomes (PCAWG) Consortium, which aggregated whole genome sequencing data from 2,658 cancers across 38 tumour types, we compare WES and WGS side-by-side from 746 TCGA samples, finding that ~80% of mutations overlap in covered exonic regions. We estimate that low variant allele fraction (VAF < 15%) and clonal heterogeneity contribute up to 68% of private WGS mutations and 71% of private WES mutations. We observe that ~30% of private WGS mutations trace to mutations identified by a single variant caller in WES consensus efforts. WGS captures both ~50% more variation in exonic regions and un-observed mutations in loci with variable GC-content. Together, our analysis highlights technological divergences between two reproducible somatic variant detection efforts

    Save Our Soles SG (SOS SG) was a campaign aimed at empowering women with the knowledge and awareness of the short term and long term health consequences of wearing high heels, and what they could do to care for their feet. Four final-year students of the Wee Kim Wee School of Communication and Information at the Nanyang Technological University conceptualised, planned, and ran this campaign, with the hopes of motivating more women to save their soles. SOS SG was targeted at working women aged 21 to 45 years old. Wearing high heels is a common preference, or even workplace requirement for some. The impact that wearing high heels has on women’s health had been well-documented, but this culture seemed to persist both globally and locally. This had led to outcry in some parts of the world where they had taken their companies to court to dispute painful workplace rules that forced women to wear high heels. While we recognised the world would take time to catch up and adjust to caring for women, we could at least empower women to care for themselves first. This report looked into the thoughts, perceptions, and habits of women in Singapore on the issue of wearing high heels. It also covered the planning, execution, and evaluation of the campaign to spread awareness of the health issues and good habits that could be adopted.
