    Level of Information About Radiation Among Medical Staff Working in Operating Rooms With Fluoroscopy

    DergiPark: 379005tmsjAims: The aim of this study is to determine the level of information regarding radiation safety and the side effects related to the use of fluoroscopy among a group of teaching assistants, research assistants, medical students, nurses, technicians and caretakers working at Trakya University Health Center for Medical Research and Practice.Methods: A descriptive, cross-sectional survey study was conducted from December 2014 to April 2015 at Trakya University Health Center for Medical Research and Practice. The questionnaire consisted of 24 questions and was applied to 53 people. Responses regarding radiation safety and the side effects of fluoroscopy were analyzed in Statistical Package for the Social Sciences (SPSS) 20.0 using descriptive statistics including frequencies and percentages.Results: 98.08% of the participants responded “yes” to the question ”Do you think fluoroscopy is hazardous to health?”. On the other hand, the results showed that only 32.69% of the participants wore lead apron and 32.77% used thyroid shields in every operation. 88.46% of the participants reported that they would like to attend a training regarding fluoroscopy and radiation safety.Conclusion: The results indicated that the operating room staff of Trakya University Health Center for Medical Research and Practice does not possess adequate knowledge regarding radiation safet

    Eosinophilic lymph node abscesses following a COVID-19 vaccination: A case report

    In this paper, we reported a 37-year-old man who developed several lymphadenopathies after using the second dose of Pfizer-BioNtech vaccination against SARS-CoV-2. The excisional lymph node biopsy showed eosinophil-rich inflammation with micro-abscesses. Although eosinophilic dermatosis and eosinophilic myocarditis have been described previously following COVID-19 vaccinations, eosinophilic lymph node abscess was not reported in the literature. In our case, all lesions were completely recovered with steroid treatment. The patient has been doing well and no recurrence has been observed for six months

    Radiofrequency ablation therapy in renal clear cell carcinoma: a case report

    Berrak hücreli karsinom böbrek tümörlerinin en sik görülen histolojik alt tipidir ve 6. Ve 8. Dekadlar arasinda daha Çok görülmektedir. Bu yazimizda 70 yaşinda sağ lomber bölgede ağri ile kliniğimize başvuran ve biyopsi ile Berrak hücreli karsinom saptayip radyofrekans ablasyon tedavisi uyguladiğimiz hasta güncel literatür eşliğinde Tartişilacaktir. Hipertansiyon, diyabeti ve konjestif kalp yetmezliği olan 70 yaşinda bayan hasta sağ lomber Bölgede 15 gündür süren ağri nedeni ile kliniğimize başvurdu. Görüntüleme yöntemleri ile sağ böbrekte kitle Tespit edildi. Hastaya ultrason eşliğinde perkütan tru-cut biopsi yapildi. Histopatolojik olarak berrak hücreli Karsinom olarak değerlendirildi. Komorbiditeleri nedeni ile cerrahi yapilamayan hastaya us eşliğinde perkütan Yolla radyofrekans ablasyon uygulandi. Hastanin takibinde çekilen batin tomografilerinde lezyona uyan bölgede Skar dokusu izlendi ve yeni bir lezyon tespit edilmedi. Hastanin takibi devam etmektedir. Berrak hücreli karsinom Sikliği giderek artmakta ve insidental olarak yapilan görüntülemeler sayesinde hastalar asemptomatik ve böbreğe Sinirli evrede karşimiza çikmaktadir. Cerrahi açidan uygun olmayan soliter böbreği ve böbrek kanseri olan Hastalarda cerrahiye alternatif olarak radyofrekans ablasyon ve kriyoablasyon tedavileri düşünülmelidir.Clear cell carcinoma is the most common histological subtype of malignant kidney tumors and the incidence is higher in the 6th to 8th decades of life. We discuss here against the background of current literature a seventyyear- old patient who presented to our clinic with pain in the right lumbar region. A diagnosis of clear cell carcinoma was obtained by biopsy and the patient was given radiofrequency ablation treatment. The 70-year old female patient, who had hypertension, diabetes and congestive heart deficiency, came to our clinic with a complaint of pain in the right lumbar region for the last 15 days. By imaging methods, a mass in the right kidney was detected. An ultrasound-guided percutaneous trucut biopsy was performed. The case was evaluated histopathologically as clear cell carcinoma. Because surgery was contraindivated due to her comorbidities, the patient was given ultrasound-guided percutaneous radiofrequency ablation. In the abdominal tomographies performed during the follow-up scar tissue was seen in the region associated with the lesion and no new lesion was detected. The follow-up is continuing. Clear cell carcinoma incidence appears to be on the increase, and thanks to incidental imaging discoveries the condition is caught in an asymptomatic and kidney-limited stage. Radiofrequency ablation or cryoablation must be considered an alternative in patients with a solitary kidney or with renal carcinoma should there be a contraindication for surgery

    Pnömonili sığırlarda miyeloid hücrelerde eksprese edilen mid-regional pro-adrenomedullin ve soluble tetikleyici reseptörün serum düzeyleri

    This study aimed to determine the serum levels of two inflammatory biomarkers, named, mid-regional pro-adrenomedullin (MR-Pro ADM) and soluble triggering receptor expressed on myeloid cells (sTREM-1), in cattle diagnosed with pneumonia. For this purpose, 40 patient female cattle, which were aged 2-7 years and displayed coughing, dyspnea, nasal discharge, anorexia and abdominal respiration, and 15 healthy female cattle within the same age range, were evaluated. The diseased cattle underwent clinical and radiological examinations and were sampled for blood prior to receiving treatment. The healthy subjects also underwent clinical examination and were sampled for blood once. Blood samples were used for biochemical and hematological measurements. While the diseased group had higher serum levels of MR-Pro ADM (86.38±6.33), compared to the healthy control group (61.81±4.96); the pneumonic cattle had lower levels of sTREM-1 (75.93±1.86), in comparison to the healthy group (96.55±9.13). In conclusion: MR-Pro ADM and sTREM-1 levels are very important diagnostically in cattle with pneumonia.Bu çalışma, pnömoni tanısı konulan sığırlarda mid-regional pro-adrenomedullin (MR-Pro ADM) ve miyeloid hücrelerde eksprese soluble tetikleyici reseptör olmak üzere iki enflamatuvar biyobelirteç ile bazı biyokimyasal ve hematolojik parametrelerin serum düzeylerinin belirlenmesini amaçladı. Bu amaçla 2-7 yaş arası öksürük, nefes darlığı, burun akıntısı, iştahsızlık ve abdominal solunumu gösteren 40 hasta dişi sığır ve aynı yaş aralığında 15 sağlıklı dişi sığır değerlendirildi. Hasta sığırlar tedavi edilmeden önce klinik ve radyolojik muayenelere tabi tutuldu ve kan örnekleri alındı. Sağlıklı sığırlar ayrıca klinik muayeneye tabi tutuldu ve bir kez kan örnekleri alındı. Kan örnekleri biyokimyasal ve hematolojik ölçümler için kullanıldı. Hasta grupta MR-Pro ADM serum düzeyleri (86,38±6,33) kontrol grubuna (61,81±4,96) göre daha yüksek bulundu. Hasta sığırların sTREM-1 seviyeleri (75,93±1,86) sağlıklı gruba kıyasla (96,55±9,13) karşılaştırıldığında istatistiksel olarak anlamlı düşük bulundu. Sonuç olarak: MR-Pro ADM ve sTREM-1 seviyeleri pnömonili sığırlarda diagnostik açıdan oldukça önemlidir

    Children in 2077: Designing Children’s Technologies in the Age of Transhumanism

    What for and how will we design children’s technologies in the transhumanism age, and what stance will we take as designers? This paper aims to answer this question with 13 fictional abstracts from sixteen authors of different countries, institutions and disciplines. Transhumanist thinking envisions enhancing human body and mind by blending human biology with technological augmentations. Fundamentally, it seeks to improve the human species, yet the impacts of such movement are unknown and the implications on children’s lives and technologies were not explored deeply. In an age, where technologies can clearly be defined as transhumanist, such as under-skin chips or brain-machine interfaces, our aim is to reveal probable pitfalls and benefits of those technologies on children’s’ lives by using the power of design fiction

    Impact of opioid-free analgesia on pain severity and patient satisfaction after discharge from surgery: multispecialty, prospective cohort study in 25 countries

    Background: Balancing opioid stewardship and the need for adequate analgesia following discharge after surgery is challenging. This study aimed to compare the outcomes for patients discharged with opioid versus opioid-free analgesia after common surgical procedures.Methods: This international, multicentre, prospective cohort study collected data from patients undergoing common acute and elective general surgical, urological, gynaecological, and orthopaedic procedures. The primary outcomes were patient-reported time in severe pain measured on a numerical analogue scale from 0 to 100% and patient-reported satisfaction with pain relief during the first week following discharge. Data were collected by in-hospital chart review and patient telephone interview 1 week after discharge.Results: The study recruited 4273 patients from 144 centres in 25 countries; 1311 patients (30.7%) were prescribed opioid analgesia at discharge. Patients reported being in severe pain for 10 (i.q.r. 1-30)% of the first week after discharge and rated satisfaction with analgesia as 90 (i.q.r. 80-100) of 100. After adjustment for confounders, opioid analgesia on discharge was independently associated with increased pain severity (risk ratio 1.52, 95% c.i. 1.31 to 1.76; P &lt; 0.001) and re-presentation to healthcare providers owing to side-effects of medication (OR 2.38, 95% c.i. 1.36 to 4.17; P = 0.004), but not with satisfaction with analgesia (beta coefficient 0.92, 95% c.i. -1.52 to 3.36; P = 0.468) compared with opioid-free analgesia. Although opioid prescribing varied greatly between high-income and low- and middle-income countries, patient-reported outcomes did not.Conclusion: Opioid analgesia prescription on surgical discharge is associated with a higher risk of re-presentation owing to side-effects of medication and increased patient-reported pain, but not with changes in patient-reported satisfaction. Opioid-free discharge analgesia should be adopted routinely
