    Avrupa Yönetişimi ve Ulusaltı Yönetimlerin Ulusötesi Ağ Faaliyetleri

    Literatürde ulusaltı yönetimlerin ulusötesi ağ faaliyetleri (transnational networking)- yerel ve bölgesel yönetimlerin muadilleri ile işbirliği ve ortaklık bağlamında çeşitli etkileşim süreçleri içine girmesi- AB yönetişiminin dikkat çeken unsurlarından biri olarak değerlendirilebilmektedir. Bu bağlamda söz konusu ağ faaliyetlerinin ulusaltı yönetimlerin AB yönetişim süreçlerine dahil olmalarında kolaylaştırıcı bir rol üstlendiği ifade edilebilmektedir. Bununla birlikte, ulusaltı yönetimlerin karşılıklı bilgi paylaşımı, işbirliği, ortak proje geliştirme, en iyi uygulama paylaşımı, lobi faaliyetleri yürütme, finansal kaynaklara erişim gibi amaçlarla bir araya geldikleri ve ağ faaliyeti gerçekleştirdikleri belirtilebilirken, bu yolla söz konusu idarelerin AB yönetişim süreçleri içinde hareketlilik kazandıkları, etkileşim ve görünürlüklerini arttırdıkları tartışılabilmektedir. Bu çalışma çerçevesinde ulusaltı yönetimler ve AB yönetişimine ilişkin bağlam yönetişim ağları perspektifinden tartışılmakta; farklı yaklaşımlara dair değerlendirmeler vurgulanırken, bu alanda yapılacak çalışmaların yerel yönetim literatürüne sağlayacağı katkı ele alınmaktadır

    This article aims to evaluate the changing notions of public services as expressed in NPM, and to place these in the context of the emergence of new political ideas which are associated with the New Right, New Left and Third Way. There have been different attempts to theorise and formulate the rapidly changing nature of political and managerial re-organisation in Britain. Additionally, there have been different ways of defining and describing these developments. However, similar themes have been emphasized and an agenda of common concerns are recognised that can be articulated as a modernisation movement seeking to enhance democratic governance and effective public service delivery. Among new ideas, most emphasis was placed on corporate management, strategic planning, performance measurement and fundamental corporate review, quality management and administrative accountability. There is also a new emphasis on central-local government relations

    Examination of Konya spoon tunes in terms of rhythmic pattern, scala and melodic pattern

    Türk Halk Müziği’nin bir türü olarak karşımıza çıkan kaşık havaları, Konya’da geleneksel bir muhafazakârlık ve Mevleviliğin de etkisiyle yaygın bir musiki anlayışı olmasına karşın, halkın eğlence aracı olarak sürekliliğini sürdürmüş ve günümüze kadar ulaşmıştır. Bu çalışmada, Konya’nın geleneksel müzik ve eğlence kültürü hakkında genel bilgiler verilmiş bu çerçevede geleneksel eğlence kültürünün öğelerinden biri olan kaşık havaları; usul, makam ve ayak yönünden incelenmiştir. Verilere, betimsel araştırma yöntemlerinden literatür taraması ile ulaşılmıştır. Çalışmanın kapsamını, TRT müzik dairesi bünyesindeki Türk Halk Müziği notalarından seçilen 10 adet Konya türküsü oluşturmuştur. Sonuç olarak, türkülerin çoğunun makam karşılıklarıyla uyuşmadıkları, sadece benzerlik gösterdikleri görülmüştür. Farklı dizi kullanan birçok türkünün tek bir ayak ismiyle ifade edildiği, mevcut ayakların türkülerin sınıflandırılmasında yetersiz ve belirsiz kaldığı saptanmıştır. Çalışmanın sonucunda, Konya Kaşık Havalarının dolayısıyla da Türk Halk Müziği teorisinin kendine özgü özellikleriyle tekrar ele alınıp araştırılması ve yapılandırılması gerekliliği ortaya çıkmıştır.Spoon Tune which is a form of Turkish Folk Music, although a widespread understanding of the music, has been maintained the continuity of the people as a means of entertainment and has reached up to the present day although conservatism and Mevlevi sect in Konya. This study provided a general information about the culture of Konya and spoon tunes are examinated in terms of rhythmic pattern, scala and melodic pattern. The data were obtained from in descriptive literature. 10 notes of Konya folk songs selected from the TRT music office. The most of the folk songs were determined that they were indiscrepancy for the scala. The most of the folk songs were specified as different scalas, however; they were named in a single melodic pattern. Current melodic pattern classification hasn’t been enough for folk songs. Further sudies about Spoon Tune have needed for classification in features of Turkish Folk Music

    Nivolumab for relapsed or refractory Hodgkin lymphoma: Real-life experience

    Background: Reed-Sternberg cells of classical Hodgkin's lymphoma (cHL) are characterized by genetic alterations at the 9p24.1 locus, leading to over-expression of programmed death-ligand 1 and 2. In a phase 1b study, nivolumab, a PD-1-blocking antibody, produced a high response in patients with relapsed or refractory cHL, with an acceptable safety profile. Patients and methods: We present a retrospective analysis of 82 patients (median age: 30 years; range: 18-75) with relapsed/refractory HL treated with nivolumab in a named patient program from 24 centers throughout Turkey. The median follow-up was 7 months, and the patients had a median of 5 (2-11) previous lines of therapy. Fifty-seven (70%) and 63 (77%) had been treated by stem-cell transplantation and brentuximab vedotin, respectively. Results: Among 75 patients evaluated after 12 weeks of nivolumab treatment, the objective response rate was 64%, with 16 complete responses (CR; 22%); after 16 weeks, it was 60%, with 16 (26%) patients achieving CR. Twenty patients underwent subsequent transplantation. Among 11 patients receiving allogeneic stem-cell transplantation, 5 had CR at the time of transplantation and are currently alive with ongoing response. At the time of analysis, 41 patients remained on nivolumab treatment. Among the patients who discontinued nivolumab, the main reason was disease progression (n = 19). The safety profile was acceptable, with only four patients requiring cessation of nivolumab due to serious adverse events (autoimmune encephalitis, pulmonary adverse event, and two cases of graft-versus-host disease aggravation). The 6-month overall and progression-free survival rates were 91.2% (95% confidence interval: 0.83-0.96) and 77.3% (0.66-0.85), respectively. Ten patients died during the follow-up; one of these was judged to be treatment-related. Conclusions: Nivolumab represents a novel option for patients with cHL refractory to brentuximab vedotin, and may serve as a bridge to transplantation; however, it may be associated with increased toxicity

The Medication Adherence Scale in FMF Patients (MASIF) is a 18-item questionnaire that evaluates adherence to medication in four domains. Validation of the instrument was accomplished in paediatric FMF patients (aged 2-18 years) under medication at least for 6 months. The first step was to build up the scale through qualitative approach (with interviews using semi-structured questions). Validation analyses included assessment of feasibility, face and content validity; construct validity, internal consistency and test-retest reliability. Results. One hundred and fifty patients with FMF were enrolled in the study. The mean age of the patients was 11.11±4.02 years and 48.7% of them were male. The MASIF was found to be feasible and valid for both face and content. It correlated with the Morisky Medication Adherence Scale as a gold standard thereby demonstrating good construct validity (r=0.515, p < 0.001). Assessment of content validity identified four subscales. The internal consistency, Cronbach's alpha was 0.728. There was a positive and significant correlation between test and retest scores (r=0.843; p < 0.001). Also, a significant correlation between parents' and children's reports (r=0.781, p < 0.001). Conclusion. Based on these results, the use of this scale to assess and follow up the adherence to treatment in paediatric FMF patients under medical treatment is recommended. © Clinical and Experimental Rheumatology 2015
