    Post kidney transplant infections with special reference to cytomegalovirus

    Background: The burden of infectious disease is high among kidney transplant recipients because of concomitant immunosuppression. Methods: Study was a retrospective and prospective cohort study. The study was conducted for a period of 12 months and 30 transplant recipients were included. Results: Males comprised 86.66% of the study population. The mean age of the recipients was 37.96 years. UTI was the most common post-transplant infection observed in 15 patients (50%) of 30 patients. E. Coli infection was most predominant (50%) causing UTI. 5 patients (16.66%) in this study developed tuberculosis after renal transplantation. Two patients (6.66%) developed CMV infection in the study. Two patients (6.66%) developed COVID-19 infection in the study. Two patients (6.66%) developed herpes infection post-transplant. One patient (3.33%) in the study developed cryptococcal meningitis. One patient (3.33%) developed hepatitis C after 18 years of transplant. One patient (3.33%) developed hepatitis B after 10 years of transplant. Three patients (10%) developed lower respiratory tract infection and developed acute respiratory distress. Three patients (10%) had developed acute graft rejection within first year after transplantation. There are total of 13 deaths (43.33%) among 30 patients all of which were secondary to infections. Total of 4 patients among the 13 deaths had chronic graft rejection. Most common cause of death was tuberculosis (38.46%) followed by UTI (23%) and Lower respiratory tract infection (23%). Conclusions: The incidence of infections is relatively higher in kidney transplant recipients when compared to general populations due to immunosuppression

    Salvaging collateral damage by COVID-19 pandemic in form of exposed silicone ear framework in 33-year post reconstructed ear

    Total ear reconstruction is being practiced by different techniques. Ready to use Silicone ear framework (Silastic ear framework by Dow corning) was being used frequently by Plastic surgeons in the eighties and nineties of the twentieth century. Framework exposure, either due to skin necrosis or due to infection used to be the commonest complication in the early postoperative period. A follow-up case of a 50 year old male patient, our 33 years follow up case of Total ear reconstruction by silicone ear framework implantation presented to us with exposed silicone framework and infection. Due to constant use of facemask with elastic ear loop for support during COVID-19 pandemic. The exposed infected implant successfully salvaged using negative pressure wound therapy. In all cases of autologous or alloplastic ear reconstruction, we strictly recommend not to use facemasks with elastic ear loops. If a facemask has to be used it should have a head loop or to be used with an ear protector. 

    A charge reversal differentiates (p)ppGpp synthesis by monofunctional and bifunctional Rel proteins

    A major regulatory mechanism evolved by microorganisms to combat stress is the regulation mediated by (p)ppGpp (the stringent response molecule), synthesized and hydrolyzed by Rel proteins. These are divided into bifunctional and monofunctional proteins based on the presence or absence of the hydrolysis activity. Although these proteins require Mg2+ for (p)ppGpp synthesis, high Mg2+ was shown to inhibit this reaction in bifunctional Rel proteins from Mycobacterium tuberculosis and Streptococcus equisimilis. This is not a characteristic feature in enzymes that use a dual metal ion mechanism, such as DNA polymerases that are known to carry out a similar pyrophosphate transfer reaction. Comparison of polymerase Polβ and RelSeq structures that share a common fold led to the proposal that the latter would follow a single metal ion mechanism. Surprisingly, in contrast to bifunctional Rel, we did not find inhibition of guanosine 5′-triphosphate, 3′-diphosphate (pppGpp) synthesis at higher Mg2+ in the monofunctional RelA from Escherichia coli. We show that a charge reversal in a conserved motif in the synthesis domains explains this contrast; an RXKD motif in the bifunctional proteins is reversed to an EXDD motif. The differential response of these proteins to Mg2+ could also be noticed in fluorescent nucleotide binding and circular dichroism experiments. In mutants where the motifs were reversed, the differential effect could also be reversed. We infer that although a catalytic Mg2+ is common to both bifunctional and monofunctional proteins, the latter would utilize an additional metal binding site formed by EXDD. This work, for the first time, brings out differences in (p)ppGpp synthesis by the two classes of Rel proteins

    Background: Stress is very common in medical professionals. Stress begins in the first year of medical school and increases with subsequent years of medical life. Stress decreases overall performance and had a multitude of health-related adverse effect. Yoga has been tried as a stress reduction technique in different populations. In present study yoga was performed in the 1st year MBBS students and impact on stress reduction was studied using PSS-10 stress scale.Methods: Study groups, yoga and control contained 26 and 27 subjects respectively. The yoga group practiced selected yogic asana, pranayama, and yoga nidra 1hour daily 6days a week for 3months. Control group kept in touch and allowed their usual activity as before. The PSS-10 scale used to measure the level of stress in both groups pre and post study.Results: There was a highly significant reduction in the PSS-10 Score (stress level) in the yoga group (P Value <0.0001) but there was no significant change in the PSS-10 Score of control group (P Value = 0.2930).Conclusions: Yoga is an effective modality of stress reduction technique in 1st year medical students. Therefore, yoga should be introduced as a part of the curricula in the first year of medical school. This may be taken as the 1st step in implantation of healthy lifestyle in future health care providers

    A 49-year-old diabetic woman who had end stage renal disease associated with membranous glomerulonephritis, was undergoing dialysis through a left brachiocephalic arteriovenous fistula. After sometime, fistula became infected and stopped functioning. It was closed and arterial segment was resected and repaired with reverse saphenous vein graft. After 6 months interval, she presented with painful, expansile swelling with intermittent bleeding. Arteriogram showed ruptured aneurysm of reverse saphenous vein graft with a proximal venous stenosis. It was successfully closed with 6 × 60 mm Fluency Plus self-expanding vascular stent graft (Bard, Tempe, AZ) with resolution of her symptoms.Kobieta w wieku 49 lat, chora na cukrzycę, w schyłkowym stadium niewydolności nerek związanej z błoniastym zapaleniem kłębuszków nerkowych, była dializowana z dostępu przez przetokę tętniczo-żylną ramienno-odpromieniową po lewej stronie. Po pewnym czasie przetoka przestała działać z powodu zakażenia. Przetokę zamknięto, a segment tętniczy wycięto i zastąpiono przeszczepem z odwróconej żyły odpiszczelowej. Po 6 miesiącach chora zgłosiła się z powodu bólu, znacznego obrzęku i okresowego krwawienia. Arteriogram ukazał pęknięty tętniak przeszczepu z żyły odpiszczelowej ze zwężeniem żyły w odcinku proksymalnym.     Analysis of genetic and epigenetic variation in in vitro propagated potato somatic hybrid by AFLP and MSAP marker

    Background: Genetic and epigenetic changes (DNA methylation) were examined in the tissue-culture propagated interspecific potato somatic hybrids between dihaploid Solanum tuberosum and S. pinnatisectum . Amplified fragment length polymorphism (AFLP) and methylation-sensitive amplified polymorphism (MSAP) were applied to detect the genetic and epigenetic changes, respectively in the somatic hybrids mother plants (1st cycle) and their regenerants (30th cycles sub-cultured). Results: To detect genetic changes, eight AFLP primer combinations yielded a total of 329 scorable bands of which 49 bands were polymorphic in both mother plants and regenerants. None of the scorable bands were observed in term of loss of original band of mother plant or gain of novel band in their regenerants. AFLP profiles and their cluster analysis based on the Jaccard\u2019s similarity coefficient revealed 100% genetic similarity among the mother plant and their regenerants. On the other hand, to analyze epigenetic changes, eight MSAP primer pair combinations detected a few DNA methylation patterns in the mother plants (0 to 3.4%) and their regenerants (3.2 to 8.5%). Out of total 2320 MSAP sites in the mother plants, 2287 (98.6%) unmethylated, 21 (0.9%) fully methylated and 12 (0.5%) hemi-methylated, and out of total 2494 MSAP sites in their regenerants, 2357 (94.5%) unmethylated, 79 (3.1%) fully methylated and 58 (2.3%) hemi-methylated sites were amplified. Conclusion: The study concluded that no genetic variations were observed among the somatic hybrids mother plants and their regenerants by eight AFLP markers. However, minimum epigenetic variations among the samples were detected ranged from 0 to 3.4% (mother plants) and 3.2 to 8.5% (regenerants) during the tissue culture process

    Physics Potential of the ICAL detector at the India-based Neutrino Observatory (INO)

    The upcoming 50 kt magnetized iron calorimeter (ICAL) detector at the India-based Neutrino Observatory (INO) is designed to study the atmospheric neutrinos and antineutrinos separately over a wide range of energies and path lengths. The primary focus of this experiment is to explore the Earth matter effects by observing the energy and zenith angle dependence of the atmospheric neutrinos in the multi-GeV range. This study will be crucial to address some of the outstanding issues in neutrino oscillation physics, including the fundamental issue of neutrino mass hierarchy. In this document, we present the physics potential of the detector as obtained from realistic detector simulations. We describe the simulation framework, the neutrino interactions in the detector, and the expected response of the detector to particles traversing it. The ICAL detector can determine the energy and direction of the muons to a high precision, and in addition, its sensitivity to multi-GeV hadrons increases its physics reach substantially. Its charge identification capability, and hence its ability to distinguish neutrinos from antineutrinos, makes it an efficient detector for determining the neutrino mass hierarchy. In this report, we outline the analyses carried out for the determination of neutrino mass hierarchy and precision measurements of atmospheric neutrino mixing parameters at ICAL, and give the expected physics reach of the detector with 10 years of runtime.     Measurement of the top quark forward-backward production asymmetry and the anomalous chromoelectric and chromomagnetic moments in pp collisions at √s = 13 TeV

    Abstract The parton-level top quark (t) forward-backward asymmetry and the anomalous chromoelectric (d̂ t) and chromomagnetic (μ̂ t) moments have been measured using LHC pp collisions at a center-of-mass energy of 13 TeV, collected in the CMS detector in a data sample corresponding to an integrated luminosity of 35.9 fb−1. The linearized variable AFB(1) is used to approximate the asymmetry. Candidate t t ¯ events decaying to a muon or electron and jets in final states with low and high Lorentz boosts are selected and reconstructed using a fit of the kinematic distributions of the decay products to those expected for t t ¯ final states. The values found for the parameters are AFB(1)=0.048−0.087+0.095(stat)−0.029+0.020(syst),μ̂t=−0.024−0.009+0.013(stat)−0.011+0.016(syst), and a limit is placed on the magnitude of | d̂ t| &lt; 0.03 at 95% confidence level.
