    En nuestra sociedad el empleo constituye el elemento clave para la integración social de la persona. Este trabajo aborda la situación sociolaboral de las personas con discapacidad en España. Incluye un análisis de los datos estadísticos sobre empleo atendiendo a las variables edad, sexo, tipo de actividad, nivel de estudios entre otras. Ofrecemos una visión general de los diversos factores que inciden directamente en la incorporación de las personas con discapacidad en el trabajo. Se recogen a su vez las medidas de Fomento de Empleo aprobadas para personas con discapacidad en España.In our society employment constitutes the key element for the individual social mainstreaming. This article deals with the working and social conditions of the people with disabilities in Spain. It provides an analysis of employability data according to gender, age, activity, qualifications among others. It provides a general description of the different aspects that are involved directly in the incorporation of people with disabilities into work. It also includes the measures taken to foster employment of people with disabilities in Spain

    Sobre la educación inclusiva en España: políticas y prácticas

    En este artículo se elabora una visión del proceso de educación inclusiva en España a lo largo de los últimos cuarenta años, desde una doble perspectiva: la que corresponde a la vertiente política y la que atiende a la práctica cotidiana en los centros educativos. Con relación a la primera de ellas se presenta el desarrollo de la evolución de la legislación sobre educación para las personas con discapacidad a lo largo del periodo de tiempo mencionado, en el que se transita desde un modelo educativo de segregación hacia uno de integración, que con gran esfuerzo trata de configurarse actualmente como un modelo de inclusión. Desde el punto de vista de la práctica acudiremos a los datos más recientes sobre educación inclusiva y atención al alumnado con necesidades educativas especiales, con el fin de elaborar una exposición de las principales barreras que afronta en la actualidad el proceso de educación inclusiva en España

    ConCiencia Histórica: la arqueología comunitaria como medio para la coconstrucción de conocimiento en un entorno rural

    En la última década la arqueología comunitaria ha experimentado un importante crecimiento en España. Este crecimiento se explica, en algunos casos, por la necesaria reinvención de los arqueólogos tras el colapso de la profesión ligada a la crisis económica que se inicia en el 2009; en otros casos está relacionado con la cada vez más demandada divulgación científica dentro del marco de la transferencia de conocimiento. Esto ha supuesto la realización de programas didácticos paralelos a los proyectos de investigación arqueológica. En este artículo presentamos la experiencia de arqueología comunitaria realizada por el grupo de investigación Llabor que lleva a cabo los trabajos arqueológicos en el yacimiento de Vigaña, el colegio público y el Ayuntamiento de Balmonte de Miranda. Un ensayo que puede ser un buen ejemplo para desarrollar experiencias similares en ámbitos rurales.Peer reviewe

    Serum 25-hydroxyvitamin D and breast cancer risk by pathological subtype (MCC-Spain)

    Epidemiologic evidence on the association between vitamin D and breast cancer is still inconclusive. This study analyzes the association between serum 25-hydroxyvitamin D (25(OH)D) and breast cancer risk by pathologic subtype, stage at diagnosis and specific breast cancer risk factors. We conducted a population-based multicase-control study where 546 histologically-confirmed breast cancer cases and 558 population controls, frequently matched by geographic area, age and body mass index, were recruited in 12 Spanish provinces (MCC-Spain). Information was collected by a questionnaire and plasma 25(OH)D was measured by solid-phase extraction on-line coupled to liquid chromatography-tandem mass spectrometry (SPE-LC-MS/MS). Odds ratios and 95% confidence intervals were calculated using logistic and multinomial mixed regression models. We found a clear protective effect between 25(OH)D levels and breast cancer risk, with a significant dose-response trend (OR per 10?nmol/L?=?0.88; 95%CI?=?0.82-0.94). While no differences were observed between pre and postmenopausal women, stage at diagnosis, or across strata of the main breast cancer risk factors, the protection was more pronounced for triple negative tumors (OR per 10?nmol/L?=?0.64; p-heterogeneity?=?0.038). Similar results were observed when only cases sampled in the first month after diagnosis were considered.     Smoking history and breast cancer risk by pathological subtype: MCC-Spain study

    INTRODUCTION The role of cigarette smoking on breast cancer risk remains controversial, due to its dual carcinogenic-antiestrogenic action.METHODS In the population-based multi-case-control study (MCC-Spain), we collected epidemiological and clinical information for 1733 breast cancer cases and 1903 controls, including smoking exposure. The association with breast cancer, overall, by pathological subtype and menopausal status, was assessed using logistic and multinomial regression models.RESULTS Smokers had higher risk of premenopausal breast cancer, particularly if they had smoked >= 30 years (AOR=1.75; 95% CI: 1.04-2.94), although most estimates did not achieve statistical significance. In contrast, among postmenopausal women, smoking was associated with lower risk of breast cancer, mainly in overweight and obese women. The strongest risk reductions were observed among postmenopausal women who had stopped smoking >= 10 years before cancer diagnosis, particularly for HER2+ tumors (AOR=0.28; 95% CI: 0.11-0.68); p for heterogeneity = 0.040). Also, those who had smoked <10 pack-years (AOR=0.68; 95% CI: 0.47-0.98) or 10-25 pack-years (AOR=0.62; 95% CI: 0.42-0.92) during their lifetime were at a reduced risk of all breast cancer subtypes (p for heterogeneity: 0.405 and 0.475, respectively); however, women who had smoked more than 25 pack-years showed no reduced risk.CONCLUSIONS Menopausal status plays a key role in the relationship between tobacco and breast cancer for all cancer subtypes. While smoking seems to increase the risk in premenopausal woman, it might be associated to lower risk of breast cancer among postmenopausal women with excess weight

    Association of time of breakfast and nighttime fasting duration with breast cancer risk in the multicase-control study in Spain

    Circadian nutritional behaviors, defined by the daily eating/fasting cycle, have been linked with breast cancer. This study aimed to further disentangle the association of nighttime fasting duration and time of breakfast with breast cancer risk. We analyzed data from 1,181 breast cancer cases and 1,326 population controls from the Spanish multicase-control study (MCC-Spain), 2008-2013. We collected circadian nutritional behaviors at mid-age via a telephonic interview. We applied logistic regression to estimate odds ratios (OR) and 95% confidence intervals (CIs) for the association of nighttime fasting duration and time of breakfast with breast cancer risk in all women and stratified by menopausal status. Models were adjusted for age, center, education, family history of breast cancer, age at menarche, number of children, breastfeeding, age at first child, body mass index (BMI), contraceptive use, and hormonal replacement therapy (HRT). A later time of breakfast was associated with a non-significant increased risk of breast cancer (OR = 1.05, 95% CI: 0.95-1.16, per hour increase). This association was stronger among premenopausal women, among whom each hour later, the time of breakfast was associated with an 18% increase in breast cancer risk (OR = 1.18, 95% CI: 1.01-1.40). The association was not observed in postmenopausal women. We did not observe an association between nighttime fasting duration and breast cancer risk after adjusting for the time of breakfast. In this study, late breakfast was associated with increased breast cancer risk, especially among premenopausal women, compared with early breakfast. Aside from nutritional quality, circadian nutritional behaviors should be further studied in relation to cancer

We assessed whether timing of meals is associated with breast and prostate cancer risk taking into account lifestyle and chronotype, a characteristic correlating with preference for morning or evening activity. We conducted a population-based case-control study in Spain, 2008-2013. In this analysis we included 621 cases of prostate and 1,205 of breast cancer and 872 male and 1,321 female population controls who had never worked night shift. Subjects were interviewed on timing of meals, sleep and chronotype and completed a Food Frequency Questionaire. Adherence to the World Cancer Research Fund/American Institute of Cancer Research recommendations for cancer prevention was examined. Compared with subjects sleeping immediately after supper, those sleeping two or more hours after supper had a 20% reduction in cancer risk for breast and prostate cancer combined (adjusted Odds Ratio [OR] = 0.80, 95%CI 0.67-0.96) and in each cancer individually (prostate cancer OR = 0.74, 0.55-0.99; breast cancer OR = 0.84, 0.67-1.06). A similar protection was observed in subjects having supper before 9 pm compared with supper after 10 pm. The effect of longer supper-sleep interval was more pronounced among subjects adhering to cancer prevention recommendations (OR both cancers= 0.65, 0.44-0.97) and in morning types (OR both cancers = 0.66, 0.49-0.90). Adherence to diurnal eating patterns and specifically a long interval between last meal and sleep are associated with a lower cancer risk, stressing the importance of evaluating timing in studies on diet and cancer. What's new? Evidence shows that long-term disruption of endogenous circadian rhythms may be associated with cancer. The effects of mistimed sleeping and eating patterns that come with modern life are however less clear. This large Spanish population-based study examined whether meal timing and sleep patterns are associated with the two most common nightshift-related cancers. Adherence to a more diurnal eating pattern, and specifically an early supper and a long interval between last meal and sleep were associated with a lower breast and prostate cancer risk, stressing the importance of evaluating circadian rhythms in diet and cancer studies and revisiting recommendations for prevention

    Cryopreservation of unrelated donor hematopoietic stem cells: the right answer for transplantations during the COVID-19 pandemic?

    Cryopreservation was recommended to ensure continuity of unrelated donor (UD) hematopoietic stem cell transplantation (HSCT) during COVID-19 pandemic. However, its impact on clinical outcomes and feasibility was not well known. We compared 32 patients who underwent UD HSCT using cryopreserved peripheral blood stem cells (PBSC) during the COVID-19 pandemic with 32 patients who underwent UD HSCT using fresh PBSC in the previous period. Median neutrophil engraftment was 17.5 and 17.0 days with cryopreserved and fresh grafts, respectively. Non-significant delays were found in platelet recovery days (25.5 versus 19.0; P = 0.192) and full donor chimerism days (35.0 and 31.5; P = 0.872) using cryopreserved PBSC. The rate of acute graft-versus-host disease at 100 days was 41% (95% CI [21-55%]) in cryopreserved group versus 31% (95% CI [13-46%]) in fresh group (P = 0.380). One-hundred days progression-relapse free survival and overall survival did not differ significantly. During COVID-19 pandemic, six frozen UD donations were not transfused and logistical and clinical issues regarding cryopreservation procedure, packaging, and transporting appeared. In summary, UD HSCT with cryopreserved PBSC was safe during this challenging time. More efforts are needed to ensure that all frozen grafts are transplanted and cryopreservation requirements are harmonized
