    The economic impact of recreational trails: a systematic literature review

    Recreational trails are a type of nature-based tourism providing various activities such as hiking, biking, rafting or horseback riding. Increasing investment in infrastructure and touristic services development has resulted in higher visitor expenditure and thus contributed to economic development. This study aims to review the current economic impact assessment studies on recreational trail tourism and to extract the main economic impact determinants. A systematic literature review analysis was applied in a quantitative approach about economic models, study cases of nature-based tourism, comparison of economic impacts. A qualitative analysis was then applied with an inductive approach to compare the economic impacts of nature-based tourism forms and identify the main determinants of economic impact. This study suggests that I-O is the most suitable theoretical approach to study the economic impact of long-distance trails, while the Keynesian multiplier approach and Ad hoc model are the most suitable approaches to study the economic impact of short-distance trails.info:eu-repo/semantics/publishedVersio

    Profile and management of patients at a memory clinic

    Increasing longevity and a growing older population are being accompanied by a higher prevalence of dementia and concomitant demand for care. In this connection, the University of Cape Town/Groote Schuur Hospital (UCT/ GSH) Memory Clinic provides a valuable service to patients, families and health professionals. High levels of behavioural and psychological symptoms of dementia need expert tertiary level assessment and management. Public education on dementia, early referral for assessment by primary care health professionals, and advanced training of health professionals are needed to encourage early recognition and appropriate management. Community-based care services too are needed to support caregivers of cognitively impaired older individuals

    Interest and barriers to research in obstetric haematology – findings from a national survey in the United Kingdom

    Introduction: In 2021, the steering committee members of British Society of Haematology Obstetric Haematology Special Interest Group noted difficulties in opening research studies. This led to the development of a survey to further evaluate this issue. Method: An electronic survey was distributed to all members of the British Society of Haematology Obstetric Haematology Special Interest Group and to relevant specialty leads of the National Institute for Health and Care Research Clinical Research Network for further dissemination within these networks. Results: Responses were received from 65 participants (73% consultant grade); mainly haematologists (52%) or obstetricians (39%). Less than a third of participants reported dedicated time for research in their job plan, with only five participants reporting no challenges in opening research studies in obstetric haematology. Discussion: The survey confirmed significant interest in obstetric haematology research, with barriers to participation. We propose further actions to facilitate increased research

    Network analysis of host-virus communities in bats and rodents reveals determinants of cross-species transmission.

    Bats are natural reservoirs of several important emerging viruses. Cross-species transmission appears to be quite common among bats, which may contribute to their unique reservoir potential. Therefore, understanding the importance of bats as reservoirs requires examining them in a community context rather than concentrating on individual species. Here, we use a network approach to identify ecological and biological correlates of cross-species virus transmission in bats and rodents, another important host group. We show that given our current knowledge the bat viral sharing network is more connected than the rodent network, suggesting viruses may pass more easily between bat species. We identify host traits associated with important reservoir species: gregarious bats are more likely to share more viruses and bats which migrate regionally are important for spreading viruses through the network.     First-line support for assistance in breathing in children: statistical and health economic analysis plan for the FIRST-ABC trial

    BACKGROUND: The FIRST-ABC trial comprises of two pragmatic, multicentre, parallel groups, non-inferiority randomised clinical trials designed to evaluate the clinical non-inferiority of first-line use of high flow nasal cannula (HFNC) to continuous positive airway pressure (CPAP) in critically ill children who require non-invasive respiratory support (NRS). OBJECTIVES: To describe the pre-specified statistical and health economic analysis for the FIRST-ABC trial before completion of patient recruitment and data collection. METHODS: The statistical analysis plan was designed by the chief investigators and statisticians. We define the primary and secondary outcomes, summarise methods for data collection and safety monitoring, and present a detailed description of the planned statistical and health economic analysis. RESULTS: The primary clinical outcome is time to liberation from respiratory support. The primary effect estimate will be the adjusted hazard ratio, reported with a 95% confidence interval. As a sensitivity analysis, the primary analysis will be repeated using time to start weaning of NRS. Subgroup analyses will be performed to test for interactions between the effect of allocated treatment group and pre-specified baseline covariates. The health economic analysis will follow the intention-to-treat principle and report the mean (95% confidence interval) incremental costs, quality-adjusted life years (QALYs) and cost-effectiveness up to 6 months. All analyses will be performed separately for each of the two trials, and any results will not be combined. CONCLUSION: The FIRST-ABC trial will assess the non-inferiority of HFNC compared to CPAP in two parallel trials with shared infrastructure (step-up RCT and step-down RCT). We have developed a pre-specified statistical and health economics analysis plan for the FIRST-ABC study before trial completion to minimise analytical bias.     Population Physiologically Based Pharmacokinetic Modeling for the Human Lactational Transfer of PCB-153 with Consideration of Worldwide Human Biomonitoring Results

    [[abstract]]Background: One of the most serious human health concerns related to environmental contamination with polychlorinated biphenyls (PCBs) is the presence of these chemicals in breast milk. Objectives: We developed a physiologically based pharmacokinetic model of PCB-153 in women, and predict its transfer via lactation to infants. The model is the first human, population-scale lactational model for PCB-153. Data in the literature provided estimates for model development and for performance assessment. Methods: We used physiologic parameters from a cohort in Taiwan and reference values given in the literature to estimate partition coefficients based on chemical structure and the lipid content in various body tissues. Using exposure data from Japan, we predicted acquired body burden of PCB-153 at an average childbearing age of 25 years and compared predictions to measurements from studies in multiple countries. We attempted one example of reverse dosimetry modeling using our PBPK model for possible exposure scenarios in Canadian Inuits, the population with the highest breast milk PCB-153 level in the world. Results: Forward-model predictions agree well with human biomonitoring measurements, as represented by summary statistics and uncertainty estimates. Conclusion: The model successfully describes the range of possible PCB-153 dispositions in maternal milk, suggesting a promising option for back-estimating doses for various populations

    Exact Controllability of the Time Discrete Wave Equation: A Multiplier Approach

    In this paper we summarize our recent results on the exact boundary controllability of a trapezoidal time discrete wave equation in a bounded domain. It is shown that the projection of the solution in an appropriate space in which the high frequencies have been filtered is exactly controllable with uniformly bounded controls (with respect to the time-step). By classical duality arguments, the problem is reduced to a boundary observability inequality for a time-discrete wave equation. Using multiplier techniques the uniform observability property is proved in a class of filtered initial data. The optimality of the filtering parameter is also analyzed

    Multiple Stellar Fly-Bys Sculpting the Circumstellar Architecture in RW Aurigae

    We present high-resolution ALMA Band 6 and 7 observations of the tidally disrupted protoplanetary disks of the RW Aurigae binary. Our observations reveal the presence of additional tidal streams to the previously observed tidal arm around RW Aur A. The observed configuration of tidal streams surrounding RW Aur A and B is incompatible with a single star--disk tidal encounter, suggesting that the RW Aurigae system has undergone multiple fly-by interactions. We also resolve the circumstellar disks around RW Aur A and B, with CO radii of 58 au and 38 au consistent with tidal truncation, and 2.5 times smaller dust emission radii. The disks appear misaligned by 12^{\circ} or 57^{\circ}. Using new photometric observations from the American Association of Variable Star Observers (AAVSO) and All Sky Automated Survey for SuperNovae (ASAS-SN) archives, we have also identified an additional dimming event of the primary that began in late 2017 and is currently ongoing. With over a century of photometric observations, we are beginning to explore the same spatial scales as ALMA

    Assessment of burnout in veterinary medical students using the Maslach Burnout Inventory-Educational Survey: a survey during two semesters

    BACKGROUND: Burnout among veterinary students can result from known stressors in the absence of a support system. The objectives of this study were to evaluate use of the Maslach Burnout Inventory-Educator Survey (MBI-ES) to assess burnout in veterinary students and evaluate the factors that predict the MBI-ES scores. METHODS: The MBI-ES was administered to first (Class of 2016) and second year (Class of 2015) veterinary medical students during the 2012-2013 academic year in the fall and spring semesters. Factor analysis and test reliability for the survey were determined. Mean scores for the subscales determining burnout namely emotional exhaustion (EE), depersonalization (DP) and lack of personal accomplishment (PA) were calculated for both classes in the 2 semesters. Multiple regression analysis was performed to evaluate other factors that predict the MBI-ES scores. RESULTS: A non-probability sampling method was implemented consisting of a voluntary sample of 170 and 123 students in the fall and spring semesters, respectively. Scores for EE, DP and PA were not different between the 2 classes within the same semester. Mean ± SD scores for EE, DP and PA for the fall semester were 22.9 ± 9.6, 5.0 ± 4.8 and 32.3 ± 6.7, respectively. Mean ± SD scores for EE, DP and PA the spring semester were 27.8 ± 10.7, 6.5 ± 6.1and 31.7 ± 6.8, respectively. The EE score was higher in spring compared to fall while DP and PA scores were not different between the 2 semesters. Living arrangements specifically as to whether or not a student lived with another veterinary medical students was the only variable significantly associated with the MBI-ES scores. Students in this study had moderate levels of burnout based on the MBI-ES scores. CONCLUSIONS: The MBI-ES was an acceptable instrument for assessing burnout in veterinary medical students. The EE scores were higher in the spring semester as compared to the fall semester. Thus students in the first and second years of veterinary school under the current curriculum experience the greatest levels of emotional exhaustion during the spring semester. This has administrative implications for the school, when considering the allocation and use of resources for student support systems during each semester
