TSUNAMI HAZARD IN NORTHERN VENEZUELA
Based on LANDSAT ETM and Digital Elevation Model (DEM) data derived by the Shuttle Radar Topography Mission (SRTM, 2000) of the coastal areas of Northern Venezuela were investigated in order to detect traces of earlier tsunami events. Digital image processing methods used to enhance LANDSAT ETM imageries and to produce morphometric maps (such as hillshade, slope, minimum and maximum curvature maps) based on the SRTM DEM data contribute to the detection of morphologic traces that might be related to catastrophic tsunami events. These maps combined with various geodata such as seismotectonic data in a GIS environment allow the delineation of coastal regions with potential tsunami risk. The LANDSAT ETM imageries merged with digitally processed and enhanced SRTM data clearly indicate areas that might be prone by flooding in case of catastrophic tsunami events
Association of Adverse Pregnancy Outcomes With Hypertension 2 to 7 Years Postpartum
Background Identifying pregnancy-associated risk factors before the development of major cardiovascular disease events could provide opportunities for prevention. The objective of this study was to determine the association between outcomes in first pregnancies and subsequent cardiovascular health. Methods and Results The Nulliparous Pregnancy Outcomes Study Monitoring Mothers-to-be Heart Health Study is a prospective observational cohort that followed 4484 women 2 to 7 years (mean 3.2 years) after their first pregnancy. Adverse pregnancy outcomes (defined as hypertensive disorders of pregnancy, small-for-gestational-age birth, preterm birth, and stillbirth) were identified prospectively in 1017 of the women (22.7%) during this pregnancy. The primary outcome was incident hypertension (HTN). Women without adverse pregnancy outcomes served as controls. Risk ratios (RR) and 95% CIs were adjusted for age, smoking, body mass index, insurance type, and race/ethnicity at enrollment during pregnancy. The overall incidence of HTN was 5.4% (95% CI 4.7% to 6.1%). Women with adverse pregnancy outcomes had higher adjusted risk of HTN at follow-up compared with controls (RR 2.4, 95% CI 1.8-3.1). The association held for individual adverse pregnancy outcomes: any hypertensive disorders of pregnancy (RR 2.7, 95% CI 2.0-3.6), preeclampsia (RR 2.8, 95% CI 2.0-4.0), and preterm birth (RR 2.7, 95% CI 1.9-3.8). Women who had an indicated preterm birth and hypertensive disorders of pregnancy had the highest risk of HTN (RR 4.3, 95% CI 2.7-6.7). Conclusions Several pregnancy complications in the first pregnancy are associated with development of HTN 2 to 7 years later. Preventive care for women should include a detailed pregnancy history to aid in counseling about HTN risk
Generating and repairing genetically programmed DNA breaks during immunoglobulin class switch recombination
Adaptive immune responses require the generation of a diverse repertoire of immunoglobulins (Igs) that can recognize and neutralize a seemingly infinite number of antigens. V(D)J recombination creates the primary Ig repertoire, which subsequently is modified by somatic hypermutation (SHM) and class switch recombination (CSR). SHM promotes Ig affinity maturation whereas CSR alters the effector function of the Ig. Both SHM and CSR require activation-induced cytidine deaminase (AID) to produce dU:dG mismatches in the Ig locus that are transformed into untemplated mutations in variable coding segments during SHM or DNA double-strand breaks (DSBs) in switch regions during CSR. Within the Ig locus, DNA repair pathways are diverted from their canonical role in maintaining genomic integrity to permit AID-directed mutation and deletion of gene coding segments. Recently identified proteins, genes, and regulatory networks have provided new insights into the temporally and spatially coordinated molecular interactions that control the formation and repair of DSBs within the Ig locus. Unravelling the genetic program that allows B cells to selectively alter the Ig coding regions while protecting non-Ig genes from DNA damage advances our understanding of the molecular processes that maintain genomic integrity as well as humoral immunity
PDGF and PDGF receptors in glioma
The family of platelet-derived growth factors (PDGFs) plays a number of critical roles in normal embryonic development, cellular differentiation, and response to tissue damage. Not surprisingly, as it is a multi-faceted regulatory system, numerous pathological conditions are associated with aberrant activity of the PDGFs and their receptors. As we and others have shown, human gliomas, especially glioblastoma, express all PDGF ligands and both the two cell surface receptors, PDGFR-α and -β. The cellular distribution of these proteins in tumors indicates that glial tumor cells are stimulated via PDGF/PDGFR-α autocrine and paracrine loops, while tumor vessels are stimulated via the PDGFR-β. Here we summarize the initial discoveries on the role of PDGF and PDGF receptors in gliomas and provide a brief overview of what is known in this field
WSES Jerusalem guidelines for diagnosis and treatment of acute appendicitis
Acute appendicitis (AA) is among the most common cause of acute abdominal pain. Diagnosis of AA is challenging; a variable combination of clinical signs and symptoms has been used together with laboratory findings in several scoring systems proposed for suggesting the probability of AA and the possible subsequent management pathway. The role of imaging in the diagnosis of AA is still debated, with variable use of US, CT and MRI in different settings worldwide. Up to date, comprehensive clinical guidelines for diagnosis and management of AA have never been issued. In July 2015, during the 3rd World Congress of the WSES, held in Jerusalem (Israel), a panel of experts including an Organizational Committee and Scientific Committee and Scientific Secretariat, participated to a Consensus Conference where eight panelists presented a number of statements developed for each of the eight main questions about diagnosis and management of AA. The statements were then voted, eventually modified and finally approved by the participants to The Consensus Conference and lately by the board of co-authors. The current paper is reporting the definitive Guidelines Statements on each of the following topics: 1) Diagnostic efficiency of clinical scoring systems, 2) Role of Imaging, 3) Non-operative treatment for uncomplicated appendicitis, 4) Timing of appendectomy and in-hospital delay, 5) Surgical treatment 6) Scoring systems for intra-operative grading of appendicitis and their clinical usefulness 7) Non-surgical treatment for complicated appendicitis: abscess or phlegmon 8) Pre-operative and post-operative antibiotics.Peer reviewe
COASTAL RISK IN THE ALGIERS REGION (ALGERIA): INSIGHTS FROM TSUNAMI VELOCITIES, SEISMIC GROUND MOTION AND REMOTE SENSING
The Algiers region (Algeria) is exposed to destructive earthquakes that sometimes trigger tsunamis. In this paper, we present an interdisciplinary approach that identifies the locations prone to related induced damage for a worst-case scenario off Algiers. Firstly, a tsunami modeling for a 7.6 earthquake in the Khair Al Din Bank is computed with the Geoclaw package. The simulation indicates that the maximum values for the surface heights are about 1.5 meters and 40 to 60 m/s for the flow velocities. Seismic shaking maps are computed as well using the OpenSHA application for the same earthquake scenario. The results show that the peak ground accelerations and peak ground velocities are the highest in the Algiers massif, which means that this area is the most exposed to a high level of infrastructural damage. Finally, the use of remote sensing and GIS applications helped to generate a susceptibility to flooding hazard map for the bay of Algiers. This approach showed that the central and the eastern part of the bay have the higher susceptibility degree to flooding
Functional recovery despite prolonged bilateral loss of somatosensory evoked potentials: report on two patients
A bilateral loss of short latency somatosensory evoked potentials (SSEPs) after head trauma or non-traumatic brain damage is normally associated with a deleterious neurological outcome. An adequate recovery in reported in two deeply comatose patients with head trauma or severe hypertensive encephalopathy despite prolonged bilateral loss of SSEPs over days, found in repeated recordings. Hence, a bilateral loss of SSEPs should not be considered alone for prediction of outcome in cerebral injury.
