    A Prospective Clinical Study on Blood Mercury Levels Following Endodontic Root-end Surgery with Amalgam

    Introduction: The purpose of this clinical study was to compare the blood mercury levels before and after endodontic surgery using amalgam as a root-end filling material. Materials and Methods: Fourteen patients requiring periradicular surgery participated in this prospective clinical study. A zinc-free amalgam was employed as root-end filling material. Blood samples were collected at three intervals: immediately before, immediately after and one week postoperatively. Mercury content of the blood was determined using gold amalgamation cold-vapor atomic absorption spectrometry. Obtained data were analyzed using analysis of variance for repeated measures and paired t-test. Results: The mean (SD) of blood mercury levels was 2.20 (0.24) ng/mL immediately before surgery, 2.24 (0.28) ng/mL immediately after surgery and 2.44 (0.17) ng/mL one week after the periradicular surgery. The blood mercury level one week post-operative was significantly higher than both blood mercury levels immediately before (P<0.001) and immediately after (P=0.005) the surgery. Conclusion: Placement of an amalgam retroseal during endodontic surgery can increase blood mercury levels after one week. The mercury levels however, are still lower than the toxic mercury levels. We suggest using more suitable and biocompatible root-end filling materials

    Genome size, morphological and palynological variations, and heterostyly in some species of the genus Linum L. (Linaceae) in Iran

    Heterostyly is the occurrence of flowers with different sexual organ arrangements in different plants of the same species. This floral polymorphism occurs in four sections of genus Linum. The present study compares the morphological, palynologycal and genome size (C-value content) characteristics in the long-styled and short-styled plants in three Linum species, that is, Linum austriacum L., Linum album Ky.ex Boiss. and L. glaucum Boiss. & Nöe . 15 qualitative and quantitative morphological characters from both vegetative and reproductive organs of these plants were studied. A higher mean value of the plant height, size of the basal leaves width, flower leaves width, calyx length, sepal length and petal length occurred in the long-styled plants, while the mean value of branch number, basal leaves length, flower leaves length, calyx width, pedicel length and sepal length was higher in the short-styled plant populations. T-test analysis of morphological characters showed significant difference (p<0.05) for some of the characters studied. Principal coordinate analysis (PCoA) plot of long-styled and short-styled plant populations based on all morphological characters also separated these two kinds of plants in the three species studied. The pollen obtained from the mature buds was prepared for light microscopy (LM) and scanning electron microscopy (SEM). The polar and equatorial views of the pollen grains were similar in the long-styled (Ls) and short-styled (Ss) plants but the aperture shape differed in these populations. C-values obtained by flow cytometry, differed in the long-styled and short- styled plants of the species studied. The analysis of variance (ANOVA) test performed among the three Linum species showed a significant difference in 2C-value content. Positive significant correlation was observed between 2C-value and northern distribution of the Linum species studied, while a negative significant correlation occurred with eastern distribution.Key words: Heterostyly, Linum, morphology, palynology, genome size

    Screening Characteristics of Bedside Ultrasonography in Confirming Endotracheal Tube Placement; a Diagnostic Accuracy Study

    Introduction: Confirmation of proper endotracheal tube placement is one of the most important and lifesaving issues of tracheal intubation. The present study was aimed to evaluate the accuracy of tracheal ultrasonography by emergency residents in this regard.  Method: This was a prospective, cross sectional study for evaluating the diagnostic accuracy of ultrasonography in endotracheal tube placement confirmation compared to a combination of 4 clinical confirmation methods of chest and epigastric auscultation, direct laryngoscopy, aspiration of the tube, and pulse oximetry (as reference test).Results: 150 patients with the mean age of 58.52 ± 1.73 years were included (56.6% male). Sensitivity, specificity, positive predictive value, negative predictive value, and positive and negative likelihood ratio of tracheal ultrasonography in endotracheal tube confirmation were 96 (95% CI: 92-99), 88 (95% CI: 62-97), 98 (95% CI: 94-99), 78 (95% CI: 53-93), 64 (95% CI: 16-255), and 0.2 (95% CI: 0.1-0.6), respectively.Conclusion: The present study showed that tracheal ultrasonography by trained emergency medicine residents had excellent sensitivity (>90%) and good specificity (80-90) for confirming endotracheal tube placement. Therefore, it seems that ultrasonography is a proper screening tool in determining endotracheal tube placement

    Combined and Singular Effects of Ethanolic Extract of Persian Shallot (Allium hirtifolium Boiss) and Synbiotic Biomin®IMBO on Growth Performance, Serum- and Mucus-Immune Parameters and Antioxidant Defense in Zebrafish (Danio rerio)

    This study was carried out to evaluate combined and singular effects of ethanolic extract of Persian shallot (Allium hirtifolium Boiss) and synbiotic Biomin®IMBO on growth performance, innate immune responses, and antioxidant defense in zebrafish (Danio rerio). Fish with initial weight of 151.90 ± 0.31 mg were allocated in 21 10-L glass aquariums. The experimental groups were as follows: T1, control (without any supplementation); T2, 1% synbiotic; T3, 3% synbiotic; T4, 1% Persian shallot (as a medical plant); T5, 3% Persian shallot; T6, 1% Persian shallot and 1% synbiotic; T7, 3% Persian shallot and 3% synbiotic. At the end of the experiment (60 days), all treatments significantly showed higher final weight (FW), weight gain (WG), WG (%), and specific growth rate (SGR) compared with the fish fed on control diet. Furthermore, both synbiotic Biomin®IMBO and Persian shallot significantly improved intestine immune parameters including lysozyme, alternative complement hemolytic activity (ACH50), total immunoglobulin (total Ig), and myeloperoxidase (MPO) of zebrafish compared to fish fed on control diet (p < 0.05). Also, in all experimental groups, hepatic catalase (CAT), superoxide dismutase (SOD), glutathione peroxidase (GPx), and glutathione reductase (GR) activities significantly increased compared to the control group. Whereas, the highest MDA level was observed in the control group compared to the treatments (p < 0.05). Moreover, skin mucus immune parameters of zebrafish have been noticeably improved with synbiotic Biomin®IMBO and Persian shallot compared to fish fed on the control diet (p < 0.05). The results indicate that synbiotic or Persian shallot supplemented diet could enhance the general health status of the zebrafish

    Frequency of GJB2 mutations in families with autosomal recessive non-syndromic hearing loss in Khuzestan province

    Hearing loss is caused both by genetic and environmental factors. In this sense, more than half of the cases are genetic. Hereditary hearing loss is divided into syndromic and non-syndromic cases. Main pattern of inheritance (80%) in non-syndromic cases is autosomal recessive, which is known as autosomal recessive non-syndromic hearing loss (ARNSHL). Although the disease is very genetically heterogeneous, the GJB2 gene has highest effect. The aim of the present research is to determine the role of GJB2 mutations and DFNB1 locus in a group of families with ARNSHL in Khuzestan province of Iran. This study was conducted on 50 large families with ARNSHL (with the priority of 4 patients) in Khuzestan province, southwest Iran. First, in order to study GJB2 gene mutations, direct sequencing was conducted in all families. Besides, in the families with negative or heterozygous mutations in GJB2, linkage analysis was done using STR (Short Tandem Repeat) markers related to DFNB1 locus. Genotypes, related to each family, were determined using PCR-PAGE method. From 50 families with ARNSHL, after direct sequencing, six families (12%) showed homozygote mutations in the GJB2 gene. Accordingly, it was observed that c.35delG was the most frequent mutation (50%) in the population. Moreover, c.358-360delGAG, c.506G>A, c.71G>A mutations were identified in 3 different families. In addition, no linkage was observed in DFNB1 locus for the remaining families. Low frequency (12%) of GJB2 mutations in our study suggests that other loci and genes should be addressed to elucidate the etiology of ARNSHL in this province

    The impact of particulate matters on apoptosis in various organs: Mechanistic and therapeutic perspectives

    Ecological air contamination is the non-homogenous suspension of insoluble particles into gas or/and liquid fluids known as particulate matter (PM). It has been discovered that exposure to PM can cause serious cellular defects, followed by tissue damage known as cellular stress. Apoptosis is a homeostatic and regulated phenomenon associated with distinguished physiological actions inclusive of organ and tissue generation, aging, and development. Moreover, it has been proposed that the deregulation of apoptotic performs an active role in the occurrence of many disorders, such as autoimmune disease, neurodegenerative, and malignant, in the human population. Recent studies have shown that PMs mainly modulate multiple signaling pathways involved in apoptosis, including MAPK, PI3K/Akt, JAK/STAT, NFκB, Endoplasmic Stress, and ATM/P53, leading to apoptosis dysregulation and apoptosis-related pathological conditions. Here, the recently published data concerning the effect of PM on the apoptosis of various organs, with a particular focus on the importance of apoptosis as a component in PM-induced toxicity and human disease development, is carefully discussed. Moreover, the review also highlighted the various therapeutic approaches, including small molecules, miRNA replacement therapy, vitamins, and PDRN, for treating diseases caused by PM toxicity. Notably, researchers have considered medicinal herbs a potential treatment for PM-induced toxicity due to their fewer side effects. So, in the final section, we analyzed the performance of some natural products for inhibition and intervention of apoptosis arising from PM-induced toxicity

    The Iranian blood pressure measurement campaign, 2019: study protocol and preliminary results

    Purpose Hypertension is one of the most important risk factors for premature mortality and morbidity in Iran. The objective of the Iranian blood pressure (BP) measurement campaign was to identify individuals with raised blood pressure and providing appropriate care and increase the awareness among the public and policymakers of the importance of tackling hypertension. Methods The campaign was conducted in two phases. The first (communication) phase started on May 17th (International Hypertension Day). The second phase started on June 8th, 2019, and lasted up to July 7th during which, blood pressures were measured. The target population was Iranians aged >= 30 years. Participants voluntarily referred to health houses in rural and health posts and comprehensive health centers in urban areas in the setting of the Primary Health Care network. Additionally, over 13,700 temporary stations were set up in highly visited places in urban areas. Volunteer healthcare staff interviewed the participants, measured their BP, and provided them with lifestyle advice and knowledge of the risks and consequences of high blood pressure. They referred participants to physicians in case their BP was high. Participants immediately received a text message containing the relevant advice based on their measured BP and their past history. Results Blood pressure was measured for a total of 26,678,394 participants in the campaign. A total of 13,722,148 participants (51.4%) were female. The mean age was 46 +/- 14.1 years. Among total participants, 15,012,693 adults (56.3%) with no past history of hypertension had normal BP, 7,959,288 participants had BP in the prehypertension range (29.8%), and finally, 3,706,413 participants (13.9%) had either past medical history of hypertension, used medications, or had high BP measured in the campaign. Conclusion The campaign was feasible with the objective to increase the awareness among the public and policymakers of the importance of tackling hypertension in Iran
