    On Certain Antinomies of Freedom: Divine Foreknowledge and Immutability

    The objective of this inquiry is to establish the compatibility of free operation in the divine essence given that God is omniscient, and immutable. As such, this inquiry differs from conventional philosophical debate surrounding the divine attributes and creaturely freedom. Chapter I will respond to the antinomy of God’s foreknowledge and divine freedom, and offers a theory for divine freedom and foreknowledge compatibilism from the theory of truthmaking. Chapter II will respond to the antinomy of divine freedom and immutability, and offers a Neo-Thomist account of freedom to explain free action in the divine essence

    Reconnaissance of Pharmaceutical Chemicals in Urban Streams of the Tualatin River Basin, Oregon, 2002

    A reconnaissance of pharmaceutical chemicals in urban streams of the Tualatin River basin was conducted in July 2002 in an effort to better understand the occurrence and distribution of such compounds, and to determine whether they might be useful indicators of human-related stream contamination. Of the 21 pharmaceutical chemicals and metabolites tested, only 6 (acetaminophen, caffeine, carbamazepine, codeine, cotinine, and sulfamethoxazole) were detected in filtered stream samples from 10 sites. The concentrations of most of the detected compounds were relatively low (less than 0.05 microgram per liter). The most frequently detected compounds were cotinine (a nicotine metabolite, 8 of 10 samples) and caffeine (a stimulant, 7 of 10 samples). More compounds were detected in urban stream samples than in samples from forested or agricultural drainages. Filtered water samples also were collected from four locations within an advanced wastewater treatment facility to quantify the relative amounts of these chemicals in a municipal waste stream and to determine the degree to which those chemicals are removed by treatment processes. Fifteen pharmaceutical chemicals or metabolites were detected in wastewater treatment facility influent, with concentrations far exceeding those measured in streams. Only five of those compounds, however, were detected in the treated effluent (carbamazepine, cotinine, ibuprofen, metformin, and sulfamethoxazole) and most of those were at concentrations less than 0.2 microgram per liter. The target pharmaceutical chemicals and metabolites showed limited potential for use as tracers of specific types of human-related contamination in Tualatin River basin streams because of widespread sources (caffeine, for example) or extremely low concentrations. Caffeine and cotinine are likely to be good indicators of sources that can occur in urban areas, such as sewage spills or leaks or the widespread use and careless disposal of tobacco products and caffeine-containing beverages. Neither compound, however, is likely to be a good tracer for a specific source unless that source is large. The presence of 1,7-dimethylxanthine (a caffeine metabolite) concurrently with caffeine might indicate the presence of untreated wastewater; in contrast, the absence of the metabolite might help rule out that source. Acetaminophen might make a good tracer for untreated wastewater because of its common usage, high concentration in raw wastewater, and effective removal via treatment. Carbamazepine and sulfamethoxazole have the potential to be good indicators of treated wastewater because of their incomplete removal in treatment facilities. Some of these pharmaceutical chemicals, either singly or in combination, might prove useful as tracers of contamination after further study

    Effect of α7 nicotinic acetylcholine receptor activation on cardiac fibroblasts: A mechanism underlying RV fibrosis associated with cigarette smoke exposure

    INTRODUCTION: Right ventricular dysfunction is associated with numerous smoking-related illnesses including chronic obstructive pulmonary disease (COPD) where it is present even in absence of pulmonary hypertension. It is unknown if exposure to cigarette smoke has direct effects on RV function and cardiac fibroblast proliferation or collagen synthesis. In this study, we evaluated cardiac function and fibrosis in mice exposed to cigarette smoke (CS) and determined mechanisms of smoke-induced changes in cardiac fibroblast signaling and fibrosis. METHODS: AKR mice were exposed to cigarette smoke for six weeks followed by echocardiography and evaluation of cardiac hypertrophy, collagen content, and pulmonary muscularization. Proliferation and collagen content were evaluated in primary isolated rat cardiac fibroblasts (CF) exposed to cigarette smoke extract (CSE) or nicotine. Markers of cell proliferation, fibrosis, and proliferative signaling were determined by immunoblot or Sircol collagen assay. RESULTS: Mice exposed to CS had significantly decreased RV function as determined by TAPSE. There were no changes in LV parameters. RV collagen content was significantly elevated but there was no change in RV hypertrophy or pulmonary vascular muscularization. CSE directly increased cardiac fibroblast proliferation and collagen content in CF. Nicotine alone reproduced these effects. CSE and nicotine-induced fibroblast proliferation and collagen content were mediated through α7 nicotinic acetylcholine receptors and were dependent on PKC-α, PKC-δ, and reduced p38-MAPK phosphorylation. CONCLUSION: CS and nicotine have direct effects on cardiac fibroblasts to induce proliferation and fibrosis which may negatively affect right heart function

    Non-O157 Shiga Toxin–producing Escherichia coli Associated with Venison

    News reports of “E. coli outbreaks” usually refer to Shiga toxin–producing E. coli O157. But there are other types of Shiga toxin–producing E. coli, often called STEC,about which less is known. For these other types of STEC, what is the source? What are the risk factors? An outbreak among 29 high school students in Minnesota provided some answers. The source of this outbreak was a white-tailed deer that had been butchered and eaten at the school. The risk factors for infection were handling raw or eating undercooked venison. To prevent this type of STECinfection, people should handle and cook venison with the same caution recommended for other meats

    A physically-based model of global freshwater surface temperature

    Temperature determines a range of physical properties of water and exerts a strong control on surface water biogeochemistry. Thus, in freshwater ecosystems the thermal regime directly affects the geographical distribution of aquatic species through their growth and metabolism and indirectly through their tolerance to parasites and diseases. Models used to predict surface water temperature range between physically based deterministic models and statistical approaches. Here we present the initial results of a physically based deterministic model of global freshwater surface temperature. The model adds a surface water energy balance to river discharge modeled by the global hydrological model PCR-GLOBWB. In addition to advection of energy from direct precipitation, runoff, and lateral exchange along the drainage network, energy is exchanged between the water body and the atmosphere by shortwave and longwave radiation and sensible and latent heat fluxes. Also included are ice formation and its effect on heat storage and river hydraulics. We use the coupled surface water and energy balance model to simulate global freshwater surface temperature at daily time steps with a spatial resolution of 0.5 degrees on a regular grid for the period 1976-2000. We opt to parameterize the model with globally available data and apply it without calibration in order to preserve its physical basis with the outlook of evaluating the effects of atmospheric warming on freshwater surface temperature. We validate our simulation results with daily temperature data from rivers and lakes (U.S. Geological Survey (USGS), limited to the USA) and compare mean monthly temperatures with those recorded in the Global Environment Monitoring System (GEMS) data set. Results show that the model is able to capture the mean monthly surface temperature for the majority of the GEMS stations, while the interannual variability as derived from the USGS and NOAA data was captured reasonably well. Results are poorest for the Arctic rivers because the timing of ice breakup is predicted too late in the year due to the lack of including a mechanical breakup mechanism. Moreover, surface water temperatures for tropical rivers were overestimated, most likely due to an overestimation of rainfall temperature and incoming shortwave radiation. The spatiotemporal variation of water temperature reveals large temperature differences between water and atmosphere for the higher latitudes, while considerable lateral transport of heat can be observed for rivers crossing hydroclimatic zones, such as the Nile, the Mississippi, and the large rivers flowing to the Arctic. Overall, our model results show promise for future projection of global surface freshwater temperature under global change

    Metastatic breast carcinoma of the coracoid process: two case reports

    <p>Abstract</p> <p>Background</p> <p>The coracoid process of the scapula is a rare site of involvement for metastatic disease or for primary tumors. We are unaware of any reports in the literature of pathologic coracoid process fractures and only one report of metastatic disease to the coracoid.</p> <p>Methods and Results</p> <p>In this case report, we present two cases with metastatic breast carcinoma of the coracoid process, one of which presented with a pathologic fracture of the coracoid.</p> <p>Conclusions</p> <p>An orthopaedic surgeon must be aware of the potential for metastatic disease to the coracoid as they may be the first medical provider to encounter evidence of malignant disease.</p

    A case of serendipity*

    An account is given of how a sensitive bioassay system for measurement of the neurotransmitter acetylcholine serendipitously led to the identification of adenosine triphosphate (ATP) released in vitro from active skeletal muscle. Subsequent application of the identification procedures to exercising human muscle in vivo, cardiac muscle cells in vitro, and human erythrocytes exposed to hypoxia gave rise to the general concept of ATP as a molecule that could influence cell function from the extracellular direction. Mechanisms of ATP release from cells in terms of “trigger” events such as mechanical distortion of the membrane, depolarization of the membrane, and exposure to hypoxia are discussed. Potential therapeutic uses of extracellular ATP in cancer therapy, radiation therapy, and a possible influence upon aging are discussed. Possible roles (distant and local) of extracellular ATP released from muscle during whole body exercise are discussed

    Recruiting men from across the socioeconomic spectrum via GP registers and community outreach to a weight management feasibility randomised controlled trial

    Background Men, particularly those living in disadvantaged areas, are less likely to participate in weight management programmes than women despite similar levels of excess weight. Little is known about how best to recruit men to weight management interventions. This paper describes patient and public involvement in pre-trial decisions relevant to recruitment and aims to report on recruitment to the subsequent men-only weight management feasibility trial, including the: i) acceptability and feasibility of recruitment; and ii) baseline sample characteristics by recruitment strategy. Methods Men with BMI ≥30 kg/m2 and/or waist circumference ≥ 40 in. were recruited to the feasibility trial via two strategies; community outreach (venue information stands and word of mouth) and GP letters, targeting disadvantaged areas. Recruitment activities (e.g. letters sent, researcher venue hours) were recorded systematically, and baseline characteristics questionnaire data collated. Qualitative interviews (n = 50) were conducted three months post-recruitment. Analyses and reporting followed a complementary mixed methods approach. Results 105 men were recruited within four months (community n = 60, GP letter n = 45). Community outreach took 2.3 recruiter hours per participant and GP letters had an opt-in rate of 10.2% (n = 90/879). More men were interested than could be accommodated. Most participants (60%) lived in more disadvantaged areas. Compared to community outreach, men recruited via GP letters were older (mean = 57 vs 48 years); more likely to report an obesity-related co-morbidity (87% vs 44%); and less educated (no formal qualifications, 32% vs 10%, degree educated 11% vs 41%). Recruitment strategies were acceptable, a sensitive approach and trusting relationships with recruiters valued, and the ‘catchy’ study name drew attention. Conclusions Targeted community outreach and GP letters were acceptable strategies that successfully recruited participants to a men-only weight management feasibility trial. Both strategies engaged men from disadvantaged areas, a typically underserved population. Using two recruitment strategies produced samples with different health risk profiles, which could add value to research where either primary or secondary prevention is of interest. Further work is required to examine how these strategies could be implemented and sustained in practice

    The production of nominal and verbal inflection in an agglutinative language: evidence from Hungarian

    The contrast between regular and irregular inflectional morphology has been useful in investigating the functional and neural architecture of language. However, most studies have examined the regular/irregular distinction in non-agglutinative Indo-European languages (primarily English) with relatively simple morphology. Additionally, the majority of research has focused on verbal rather than nominal inflectional morphology. The present study attempts to address these gaps by introducing both plural and past tense production tasks in Hungarian, an agglutinative non-Indo-European language with complex morphology. Here we report results on these tasks from healthy Hungarian native-speaking adults, in whom we examine regular and irregular nominal and verbal inflection in a within-subjects design. Regular and irregular nouns and verbs were stem on frequency, word length and phonological structure, and both accuracy and response times were acquired. The results revealed that the regular/irregular contrast yields similar patterns in Hungarian, for both nominal and verbal inflection, as in previous studies of non-agglutinative Indo-European languages: the production of irregular inflected forms was both less accurate and slower than of regular forms, both for plural and past-tense inflection. The results replicate and extend previous findings to an agglutinative language with complex morphology. Together with previous studies, the evidence suggests that the regular/irregular distinction yields a basic behavioral pattern that holds across language families and linguistic typologies. Finally, the study sets the stage for further research examining the neurocognitive substrates of regular and irregular morphology in an agglutinative non-Indo-European language
