    Congenital anomalies in low- and middle-income countries: the unborn child of global surgery.

    Surgically correctable congenital anomalies cause a substantial burden of global morbidity and mortality. These anomalies disproportionately affect children in low- and middle-income countries (LMICs) due to sociocultural, economic, and structural factors that limit the accessibility and quality of pediatric surgery. While data from LMICs are sparse, available evidence suggests that the true human and financial cost of congenital anomalies is grossly underestimated and that pediatric surgery is a cost-effective intervention with the potential to avert significant premature mortality and lifelong disability

    A cellular mechanism underlying enhanced capability for complex olfactory discrimination learning

    The biological mechanisms underlying complex forms of learning requiring the understanding of rules based on previous experience are not yet known. Previous studies have raised the intriguing possibility that improvement in complex learning tasks requires the long-term modulation of intrinsic neuronal excitability, induced by reducing the conductance of the slow calcium-dependent potassium current (sI(AHP)) simultaneously in most neurons in the relevant neuronal networks in several key brain areas. Such sIAHP reduction is expressed in attenuation of the postburst afterhyperpolarization (AHP) potential, and thus in enhanced repetitive action potential firing. Using complex olfactory discrimination (OD) learning as a model for complex learning, we show that brief activation of the GluK2 subtype glutamate receptor results in long-lasting enhancement of neuronal excitability in neurons from controls, but not from trained rats. Such an effect can be obtained by a brief tetanic synaptic stimulation or by direct application of kainate, both of which reduce the postburst AHP in pyramidal neurons. Induction of long-lasting enhancement of neuronal excitability is mediated via a metabotropic process that requires PKC and ERK activation. Intrinsic neuronal excitability cannot be modulated by synaptic activation in neurons from GluK2 knock-out mice. Accordingly, these mice are incapable of learning the complex OD task. Moreover, viral-induced overexpression of Gluk2 in piriform cortex pyramidal neurons results in remarkable enhancement of complex OD learning. Thus, signaling via kainate receptors has a central functional role in higher cognitive abilities

    The precise estimation of the global agricultural cropland- extents, areas, geographic locations, crop types, cropping intensities, and their watering methods (irrigated or rainfed; type of irrigation) provides a critical scientific basis for the development of water and food security policies (Thenkabail et al., 2012, 2011, 2010). By year 2100, the global human population is expected to grow to 10.4 billion under median fertility variants or higher under constant or higher fertility variants (Table 1) with over three quarters living in developing countries, in regions that already lack the capacity to produce enough food. With current agricultural practices, the increased demand for food and nutrition would require in about 2 billion hectares of additional cropland, about twice the equivalent to the land area of the United States, and lead to significant increases in greenhouse gas productions (Tillman et al., 2011). For example, during 1960-2010 world population more than doubled from 3 billion to 7 billion. The nutritional demand of the population also grew swiftly during this period from an average of about 2000 calories per day per person in 1960 to nearly 3000 calories per day per person in 2010..

    Rapidly progressive glomerulonephritis in a child with Henoch-Schönlein Vasculitis and familial Mediterranean fever

    Henoch-Schonlein Vasculitis (HSV) is systemic small vessel vasculitis involving the skin, kidney, joints, and gastrointestinal tract. The proportion of patients reported to have renal involvement varies between 20% and 80%. Rapidly progressive glomerulonephritis (RPGN)is rare syndrome in children, characterized by clinical features of glomerulonephritis (GN) and rapid loss of renal function. We present a severe kidney involvement in a 14 year old boy with HSV in who is carring MEFV mutation. A 14 year old boy had developed sudden onset of palpable purpuric rash on his extensor surfaces of lower extremities. He had elevated an erythrocyte sedimentation rate (ESR) (45 mm/h), C-reactive protein (3.74 mg/dl), serum urea 66 mg/dl, serum creatinine 1.8 mg/dl. Also, he had hypocomplementemia. Antinuclear antibody, anti ds DNA, antineutrophil cytoplasmic antibody, anticardiolipine antibodies were negative. Urinalysis revealed macroscopic hematuria and proteinuria with a 24-h urinary protein excretion of 55 mg/m2/h. The renal biopsy specimen showed crescentic and necrotizing glomerulonephritis. He had also M694V/E148Q compound heterozygote mutation. Clinical symptoms and renal failure resolved with intermittant hemodialysis and medical therapy

    Impacts of Second-Generation Biofuel Feedstock Production in the Central U.S. on the Hydrologic Cycle and Global Warming Mitigation Potential

    Biofuel feedstocks provide a renewable energy source that can reduce fossil fuel emissions; however, if produced on a large scale they can also impact local to regional water and carbon budgets. Simulation results for 2005–2014 from a regional weather model adapted to simulate the growth of two perennial grass biofuel feedstocks suggest that replacing at least half the current annual cropland with these grasses would increase water use efficiency and drive greater rainfall downwind of perturbed grid cells, but increased evapotranspiration (ET) might switch the Mississippi River basin from having a net warm-season surplus of water (precipitation minus ET) to a net deficit. While this scenario reduces land required for biofuel feedstock production relative to current use for maize grain ethanol production, it only offsets approximately one decade of projected anthropogenic warming and increased water vapor results in greater atmospheric heat content

    The NASA Making Earth System Data Records for Use in Research Environments (MEaSUREs) Global Food Security Support Analysis Data (GFSAD) Crop Mask Global 1 kilometer (km) dataset was created using multiple input data including: remote sensing such as Landsat, Advanced Very High Resolution Radiometer (AVHRR), Satellite Probatoire d'Observation de la Terre (SPOT) vegetation and Moderate Resolution Imaging Spectrometer (MODIS); secondary elevation data; climate 50-year precipitation and 20-year temperature data; reference sub-meter to 5-meter resolution ground data and country statistics data. The GFSAD1KCM provides spatial distribution of a disaggregated five class global cropland extent map derived for nominal 2010 at 1-km based on four major studies: Thenkabail et al. (2009a, 2011), Pittman et al. (2010), Yu et al. (2013), and Friedl et al. (2010). The GFSAD1KCM nominal 2010 product is based on data ranging from years 2007 through 2012

    Implications of land use change on the national terrestrial carbon budget of Georgia

    <p>Abstract</p> <p>Background</p> <p>Globally, the loss of forests now contributes almost 20% of carbon dioxide emissions to the atmosphere. There is an immediate need to reduce the current rates of forest loss, and the associated release of carbon dioxide, but for many areas of the world these rates are largely unknown. The Soviet Union contained a substantial part of the world's forests and the fate of those forests and their effect on carbon dynamics remain unknown for many areas of the former Eastern Bloc. For Georgia, the political and economic transitions following independence in 1991 have been dramatic. In this paper we quantify rates of land use changes and their effect on the terrestrial carbon budget for Georgia. A carbon book-keeping model traces changes in carbon stocks using historical and current rates of land use change. Landsat satellite images acquired circa 1990 and 2000 were analyzed to detect changes in forest cover since 1990.</p> <p>Results</p> <p>The remote sensing analysis showed that a modest forest loss occurred, with approximately 0.8% of the forest cover having disappeared after 1990. Nevertheless, growth of Georgian forests still contribute a current national sink of about 0.3 Tg of carbon per year, which corresponds to 31% of the country anthropogenic carbon emissions.</p> <p>Conclusions</p> <p>We assume that the observed forest loss is mainly a result of illegal logging, but we have not found any evidence of large-scale clear-cutting. Instead local harvesting of timber for household use is likely to be the underlying driver of the observed logging. The Georgian forests are a currently a carbon sink and will remain as such until about 2040 if the current rate of deforestation persists. Forest protection efforts, combined with economic growth, are essential for reducing the rate of deforestation and protecting the carbon sink provided by Georgian forests.</p
