Optimal surgical timing after post-infarction ventricular septal rupture
Background: Ventricular septal rupture (VSR) following acute myocardial infarction (AMI) is a dan-gerous condition. Surgical VSR closure is the definitive therapy, but there is controversy regarding the surgical timing and the bridging therapy between diagnosis and intervention. The objective of this study is to analyze the ideal time of surgical repair and to establish the contribution of mechanical circulatory support (MCS) devices on the prognosis. Methods: We designed an observational, retrospective, multicenter study, selecting all consecutive patients with post-AMI VSR between January 1, 2008 and December 31, 2018, with non-exclusion criteria. The main objective of this study was to analyze the optimal timing for surgical repair of post-AMI VSR. Second- ary endpoints were to determine which factors could influence mortality in the patients of the surgical group. Results: A total of 141 patients were included. We identified lower mortality rates with an odds ratio of 0.3 (0.1 & ndash;0.9) in patients operated on from day 4 compared with the surgical mortality in the first 24 hours after VSR diagnosis. The use of MCS was more frequent in patients treated with surgery, par- ticularly for intra-aortic balloon pump (IABP; 79.6% vs. 37.8%, p < 0.001), but also for veno-arterial extracorporeal membrane oxygenation (VA-ECMO; 18.2% vs. 6.4%, p = 0.134). Total mortality was 91.5% for conservative management and 52.3% with surgical repair (p < 0.001). Conclusions: In our study, we observed that the lowest mortality rates in patients with surgical repair of post-AMI VSR were observed in patients operated on from day 4 after diagnosis of VSR, compared to earlier interventions. Declared experiences of risky sexual behaviors in relation to alcohol consumption in the first year of college
Fundamentos: En universitarios, el consumo de alcohol
de mayor riesgo (borracheras y binge drinking (BD), tiene
consecuencias negativas sobre su desarrollo y probablemente
facilita conductas sexuales de riesgo. El objetivo de
este trabajo fue estudiar si las conductas sexuales de riesgo
al consumir alcohol (CSRA) se asocian a los consumos de
mayor riesgo.
Métodos: Estudio multicéntrico transversal con datos
del Proyecto uniHcos, de universitarios de 1er año de 11
universidades españolas, entre los cursos 2011-2012 y 2017-
2018. Datos recogidos mediante cuestionario autoadministrado.
Se realizó un análisis uni y bivariable, evaluando la significación
estadística de las diferencias de prevalencia con
chi-cuadrado. Se utilizó media y desviación típica para variables
cuantitativas y como estadístico de contraste t de Student.
Resultados: 9.862 participantes (72,2% mujeres). El
90,3% consumió alcohol y el 60,9% tuvo borracheras en último
año; el 49% tuvo BD en el último mes. El consumo en
el último mes y las borracheras fueron mayores en hombres
y < 21 años. Las CSRA fueron superiores entre los que se
emborracharon (15,7% sexo sin protección, 1,9% abuso sexual
y 0,7% aprovecharse sexualmente) y consumieron en
BD (17,1%, 1,9% y 0,7%). Las mujeres con ambos consumos
de riesgo presentaron más abusos sexuales (2,2%), y los
hombres fueron quienes más se aprovecharon sexualmente de
otros (borracheras:1,2%; BD: 1,3%).
Conclusiones: El consumo de alcohol está por encima
de grupos similares. El BD tiene un patrón similar por género
y edad. Las CSRA se asocian a los consumos de mayor riesgo,
no detectándose en este grupo diferencias por género en sexo
sin protección, sí en otras CSRA.Objective: In college students, higher risk alcohol consumption (drunkenness and binge drinking-BD) has negative consequences on their development and and probably facilitates risk sexual behaviors. The objective was to study if risky sexual behaviors when consuming alcohol (RSBA) are associated with higher risk consumption.
Methods: Cross-sectional multicenter study with UniHcos Project, 1st year university students from 11 universities in Spain, academic years 2011-2012 to 2017-2018 data. This data were collected by self-administered questionnaire. A uni and bivariate analysis was performed, evaluated the statistical significance of the differences in prevalence with chi-square. Mean and standard deviation were used for quantitative variables and Student's t test statistic was used.
Results: 9,862 subjects (72.2% women). 90.3% reported having consumed alcohol and 60.9% had drunk the last year, 49% BD in last month. It was deteded in men, significantly higher consumption in the last month and drunkenness. Last month consumption and drunkenness were significantly higher in men and in <21 years. The RSBA were significantly higher among who were drunk (15.7% unprotected sex, 1.9% sexual abuse and 0.7% taking sexual advantage) and had BD (17.1%, 1.9% and 0.7 %). Women with both risk consumptions had more sexual abuse (2.2%), and men had greater behaviors of taking sexual advantage of someone (drunk: 1.2%; BD: 1.3%).
Conclusions: Alcohol consumption was above similar groups. BD consumption was similar by gender and age. Risk sexual behaviors appear mainly in problematic consumption. Graph based study of allergen cross-reactivity of plant lipid transfer proteins (LTPs) using microarray in a multicenter study.
The study of cross-reactivity in allergy is key to both understanding. the allergic response of many patients and providing them with a rational treatment In the present study, protein microarrays and a co-sensitization graph approach were used in conjunction with an allergen microarray immunoassay. This enabled us to include a wide number of proteins and a large number of patients, and to study sensitization profiles among members of the LTP family. Fourteen LTPs from the most frequent plant food-induced allergies in the geographical area studied were printed into a microarray specifically designed for this research. 212 patients with fruit allergy and 117 food-tolerant pollen allergic subjects were recruited from seven regions of Spain with different pollen profiles, and their sera were tested with allergen microarray. This approach has proven itself to be a good tool to study cross-reactivity between members of LTP family, and could become a useful strategy to analyze other families of allergens
Declaración de Chinchón: decálogo sobre eldulcorantes sin y bajos en calorías (ESBC)
Multidisciplinary experts in the areas of nutrition and
health met in Chinchón, Madrid, on November 25-26, 2013
under the auspices of the Fundación para la Investigación
Nutricional (Nutrition Research Foundation) and with the
collaboration of the Madrid Regional Government’s Health
Ministry, the International Sweeteners Association and the
Carlos III Health Institute CIBER of Physiopathology of
Obesity and Nutrition. They analyzed the current status
of scientific knowledge on low- and no-calorie sweeteners
(LNCS) and developed a consensus Decalogue on their use;
this constitutes the Chinchón Declaration. Sweeteners,
including sugar, represent a subject of undeniable
interest and are currently a popular topic, although areas
relating to their safety and benefits remain unknown to
segments of academia and the general public. The nature
of LNCS makes them vulnerable to biased and even
contradictory information. They are food additives that
are broadly used as sugar substitutes to sweeten foods,
medicines and food supplements when non-nutritional or
non-caloric alternatives are needed. The Chinchón
Decalogue is the outcome of a meeting for reflection and
consensus by a group of experts with backgrounds in
different scientific disciplines (toxicology, clinical nutrition,
community nutrition, physiology, food science, public
health, pediatrics, endocrinology and nutrition, nursing,
pharmaceutical care and food legislation). The Decalogue
includes different aspects of LNCS related to regulation,
use, benefits and safety. In general, benefits of LNCS have
been traditionally neglected in comparison with the
tendency for emphasising unexisting or unproven possible
risks. The need to strengthen research on LNCS in Spain
was emphasized, as well as the need to educate both

actualidad, aunque no exento de desconocimiento por
algunos sectores tanto académicos como de la población en
general. La propia naturaleza de los ESBC los hace susceptibles
de informaciones tergiversadas e incluso contradictorias.
Son aditivos alimentarios ampliamente utilizados
como sustitutivos del azúcar para endulzar alimentos,
medicamentos y complementos alimenticios cuando se
persiguen fines no nutritivos. El Decálogo de Chinchón es
fruto de una reunión de reflexión y consenso por parte de
un grupo de expertos procedentes de distintas disciplinas
científicas (toxicología, nutrición clínica, nutrición comunitaria,
fisiología, bromatología, salud pública, atención
primaria, pediatría, endocrinología y nutrición, enfermería,
atención farmacéutica y legislación alimentaria). El
decálogo incluye diferentes aspectos de los EBSC relacionados
con la legislación, uso, beneficios y seguridad. En
general, los beneficios de los EBSC han sido tradicionalmente
desatendidos en comparación con la tendencia de
destacar posibles riesgos inexistentes o que no han sido
probados. Hace especial hincapié en la necesidad de fortalecer
la investigación de los EBSC en España, así como la
necesidad de formar en este ámbito a los profesionales y a
los consumidores en genera
Psychological Distress, Family Support and Employment Status in First-Year University Students in Spain
[EN] Mental disorders are consistently and closely related to psychological distress. At the start of the university period, the relationship between a student’s psychological distress, family support, and employment status is not well-known. The aims of this study were: To determine the prevalence of psychological distress in first-year university students and to analyze its relationship with family support and the student’s employment status. Data from 4166 first-year university students from nine universities across Spain were considered. The prevalence of psychological distress was obtained using the GHQ-12, a valid and reliable screening tool to detect poor mental health. To analyze the relationship between psychological distress, family support, and employment status, logistic regression models were fitted. Regarding the prevalence found, 46.9% of men and 54.2% of women had psychological distress. In both genders, psychological distress levels increased as family support decreased. Among women, psychological distress was associated with their employment status. The prevalence of psychological distress among first-year university students in Spain is high. In addition, family support, and employment status for women, could be factors to take into account when developing psychological distress prevention strategies at the beginning of the university period.S
The involvement of thaumatin-like proteins in plant food cross-reactivity: a multicenter study using a specific protein microarray.
Cross-reactivity of plant foods is an important phenomenon in allergy, with geographical variations with respect to the number and prevalence of the allergens involved in this process, whose complexity requires detailed studies. We have addressed the role of thaumatin-like proteins (TLPs) in cross-reactivity between fruit and pollen allergies. A representative panel of 16 purified TLPs was printed onto an allergen microarray. The proteins selected belonged to the sources most frequently associated with peach allergy in representative regions of Spain. Sera from two groups of well characterized patients, one with allergy to Rosaceae fruit (FAG) and another against pollens but tolerant to food-plant allergens (PAG), were obtained from seven geographical areas with different environmental pollen profiles. Cross-reactivity between members of this family was demonstrated by inhibition assays. Only 6 out of 16 purified TLPs showed noticeable allergenic activity in the studied populations. Pru p 2.0201, the peach TLP (41%), chestnut TLP (24%) and plane pollen TLP (22%) proved to be allergens of probable relevance to fruit allergy, being mainly associated with pollen sensitization, and strongly linked to specific geographical areas such as Barcelona, Bilbao, the Canary Islands and Madrid. The patients exhibited mayor que50% positive response to Pru p 2.0201 and to chestnut TLP in these specific areas. Therefore, their recognition patterns were associated with the geographical area, suggesting a role for pollen in the sensitization of these allergens. Finally, the co-sensitizations of patients considering pairs of TLP allergens were analyzed by using the co-sensitization graph associated with an allergen microarray immunoassay. Our data indicate that TLPs are significant allergens in plant food allergy and should be considered when diagnosing and treating pollen-food allergy

I.amAble: la ciencia (química) al alcance de toda la sociedad
En este proyecto de innovación, que nace con vocación de continuar en años sucesivos, se persigue mejorar la calidad de la formación de los estudiantes de la Facultad de Ciencias Químicas (F. CC.QQ.) en el ámbito de la docencia teórico-práctica y de la divulgación científica. El trabajo ha consistido en la preparación de unos experimentos prácticos para llevarlos a cabo en centros educativos no universitarios en los que se ha tenido en cuenta la participación conjunta de personas con y sin diversidad funcional, desde una perspectiva inclusiva colaborativa. Estas actividades las han realizado los estudiantes bajo la supervisión de profesores (PDI) y personal de administración y servicios (PAS). Los experimentos se han recogido en fichas didácticas para facilitar su desarrollo y aplicación por parte de otros usuarios. En estas fichas se explica detalladamente cómo realizar las experiencias en formato de taller.
Las fichas de los talleres realizados están disponibles en una página web vinculada a la Universidad Complutense bajo el título I.amAble (iamable.ucm.es). Está página ha sido construida por un estudiante de la Facultad de Informática , bajo la supervisión de profesionales, tanto de esa facultad como del Instituto de Tecnología del Conocimiento, y está abierta a contribuciones similares de otras facultades y otras instituciones. La página web está diseñada de manera que resulte lo más intuitiva y accesible posible para todo tipo de público. Entre todos los experimentos se han elegido cuatro para llevarlos a la práctica en centros educativos como actividades inclusivas en las que han participado conjuntamente personas con y sin discapacidad.
Con este proyecto se pretende mejorar la calidad docente al ofrecer a los estudiantes la posibilidad de aprender enseñando mediante una actividad semipresencial. El desarrollo por parte de los estudiantes de competencias transversales en educación y en divulgación de la ciencia facilitarán algunas salidas profesionales en el ámbito educativo formal (centros de enseñanza) o informal (museos, animación sociocultural). Otro aspecto importante a resaltar es la potenciación de la colaboración entre todos los miembros de la institución universitaria. Este proyecto pretende contribuir a la mejora de la cultura científica, así como al establecimiento de puentes entre la UCM y la sociedad a la que debe servir. Inhibition of ATG3 ameliorates liver steatosis by increasing mitochondrial function
Non-alcoholic fatty liver disease (NAFLD) is a major health threat in both developed and developing countries and is a precursor of the more advanced liver diseases, including non-alcoholic steatohepatitis (NASH), cirrhosis, and liver cancer. Currently, understanding the multiple and complex molecular pathways implicated in NAFLD onset and progression is a major priority.
The transcription factor p63, which belongs to a family comprising p53, p63, and p73,1 is one of many factors that contributes to the development of liver steatosis. The role of p63 as a tumor suppressor and in cell maintenance and renewal is well studied, but we have recently reported that it is also relevant in the control of lipid metabolism.2 p63 encodes multiple isoforms that can be grouped into 2 categories; isoforms with an acidic transactivation domain (TA) and those without this domain (domain negative). The TAp63α isoform is elevated in the liver of animal models of NAFLD as well as in liver biopsies from obese patients with NAFLD. Furthermore, downregulation of p63α in the liver attenuates liver steatosis in diet-induced obese (DIO) mice, while the activation of TAp63α increases hepatic fat content, mediated by the activation of IKKβ and endoplasmic reticulum stress.2
A specialized form of autophagy that degrades lipid droplets, termed “lipophagy”, is a major pathway of lipid mobilization in hepatocytes. Lipophagy is elevated in hepatoma cells upon exposure to free fatty acids,3 and reduces the fatty acid load in mouse hepatocytes.4 Its impairment has been associated with the development of fatty liver and insulin resistance3,5; in contrast, the autophagic flux is increased during the activation of hepatic stellate cells.6
In the present study, we used an unbiased proteomics approach to gain insight into novel proteins modulating lipid metabolism in the liver of mice with genetic knockdown or overexpression of TAp63α. We found that autophagy-related gene 3 (ATG3) was upregulated by TAp63α activation and downregulated after p63α inhibition. ATG3 is elevated in several animal models of NAFLD and in the liver of patients with NAFLD. Genetic overexpression of ATG3 increased the lipid load in hepatocytes, while its repression alleviated TAp63α- and diet-induced steatosis. ATG3 exerted its role in lipid metabolism by regulating SIRT1 and mitochondrial function.
