Double coin in esophagus at same location and same alignment - a rare occurrence: a case report
Coin is the most common foreign body swallowed by pediatric age group. The multiple coin swallowing is extremely rare and very few cases had been reported in English literature. Most of them were present at different site and had different alignment in the esophagus. The location of the coin (trachea vs. esophagus) is commonly determined by the alignment of the coin on radiographic studies. A 4-year-girl was presented to us with history of coin ingestions one day back without any respiratory distress. On radiological study there was suspicion of two coins on same location and alignment. The diagnosis was confirmed after removal. The both coin was removed successfully by esophagoscopy. Unexpected second foreign bodies in pediatric esophageal coin ingestions are rare and it is mandatory to do post operative radiography after removal to exclude duplex coin or tracheal coin. We are presenting this case because of its rarity, difficulty in diagnosis especially when proper history is not available
Decadal variations in NDVI and food production in India
In this study we use long-term satellite, climate, and crop observations to document the spatial distribution of the recent stagnation in food grain production affecting the water-limited tropics (WLT), a region where 1.5 billion people live and depend on local agriculture that is constrained by chronic water shortages. Overall, our analysis shows that the recent stagnation in food production is corroborated by satellite data. The growth rate in annually integrated vegetation greenness, a measure of crop growth, has declined significantly (p < 0.10) in 23 of the WLT cropland area during the last decade, while statistically significant increases in the growth rates account for less than 2. Inmost countries, the decade-long declines appear to be primarily due to unsustainable crop management practices rather than climate alone. One quarter of the statistically significant declines are observed in India, which with the world's largest population of food-insecure people and largest WLT croplands, is a leading example of the observed declines. Here we show geographically matching patterns of enhanced crop production and irrigation expansion with groundwater that have leveled off in the past decade. We estimate that, in the absence of irrigation, the enhancement in dry-season food grain production in India, during 1982-2002, would have required an increase in annual rainfall of at least 30 over almost half of the cropland area. This suggests that the past expansion of use of irrigation has not been sustainable. We expect that improved surface and groundwater management practices will be required to reverse the recent food grain production declines. © 2010 by the authors; licensee Molecular Diversity Preservation International, Basel, Switzerland
Single-Stage Repair versus Traditional Repair of High Anorectal Malformations, Functional Results’ Correlation with Kelly’s Score and Postoperative Magnetic Resonance Imaging Findings
Introduction: Posterior sagittal anorectoplasty (PSARP) is the standard treatment for anorectal malformations. In the present study, the clinical evaluation of anal continence was carried out using Kelly’s scoring system and the results of primary PSARP or abdomino-PSARP were compared with the traditional three-stage procedure and the functional outcome was correlated with the findings of MRI, which was used as an objective method of evaluation.Patients and methods: A total of 40 patients with intermediate and high anorectal malformations were studied and were divided into two groups on the basis of a random number table. The patients in group A were treated with a single-stage operation, whereas the patients in group B were treated with a standard staged operation (either PSARP or abdominoperineal pull-through). After clinical evaluation using the Kelly score, patients were divided into three clinical groups irrespective of whether they were operated in one stage or in three stages. All patients were subjected to MRI at the age of 3 years and the findings were correlated with the clinical scoring system.Result: Patients were categorized according to their Kelly’s scores as follows: group 1: clinically good (score 5–6); group 2: clinically fair (score 3–4); and group 3: clinically poor (score 0–2). The proportions of good development of the muscles (puborectalis, external sphincter muscle, and levator muscle hammock) were 78.9% in group 1, 40% in group 2, and none in group 3. Development of muscles was found to be a significant factor for anal continence. Other significant factors for anal continence are rectal diameter and anorectal angle.Conclusion: Clinical assessment using the Kelly score was similar for the single-stage operation and the staged procedure, and this was supported by MRI findings. Therefore, we recommend the single-stage procedure to achieve a better outcome in intermediate and high anorectal malformation.Keywords: Anorectal Malformations, MRI, Posterior Sagittal Anorectoplast
On the exponential transform of lemniscates
It is known that the exponential transform of a quadrature domain is a
rational function for which the denominator has a certain separable form. In
the present paper we show that the exponential transform of lemniscate domains
in general are not rational functions, of any form. Several examples are given
to illustrate the general picture. The main tool used is that of polynomial and

Phase-space consistency of stellar dynamical models determined by separable augmented densities
Assuming the separable augmented density, it is always possible to construct
a distribution function of a spherical population with any given density and
anisotropy. We consider under what conditions the distribution constructed as
such is in fact non-negative everywhere in the accessible phase-space. We first
generalize known necessary conditions on the augmented density using fractional
calculus. The condition on the radius part R(r^2) (whose logarithmic derivative
is the anisotropy parameter) is equivalent to the complete monotonicity of
R(1/w)/w. The condition on the potential part on the other hand is given by its
derivative up to any order not greater than (3/2-beta) being non-negative where
beta is the central anisotropy parameter. We also derive a specialized
inversion formula for the distribution from the separable augmented density,
which leads to sufficient conditions on separable augmented densities for the
non-negativity of the distribution. The last generalizes the similar condition
derived earlier for the generalized Cuddeford system to arbitrary separable

A procedure for the change point problem in parametric models based on phi-divergence test-statistics
This paper studies the change point problem for a general parametric,
univariate or multivariate family of distributions. An information theoretic
procedure is developed which is based on general divergence measures for
testing the hypothesis of the existence of a change. For comparing the accuracy
of the new test-statistic a simulation study is performed for the special case
of a univariate discrete model. Finally, the procedure proposed in this paper
is illustrated through a classical change-point example
Inclusive and Direct Photons in S + Au Central Collisions at 200A GeV/c
A hadron and string cascade model, JPCIAE, which is based on LUND string
model, PYTHIA event generator especially, is used to study both inclusive
photon production and direct photon production in 200A GeV S + Au central
collisions. The model takes into account the photon production from the
partonic QCD scattering process, the hadronic final-state interaction, and the
hadronic decay and deals with them consistently. The results of JPCIAE model
reproduce successfully both the WA93 data of low p_T inclusive photon
distribution and the WA80 data of transverse momentum dependent upper limit of
direct photon. The photon production from different decay channels is
investigated for both direct and inclusive photons. We have discussed the
effects of the partonic QCD scattering and the hadronic final-state interaction

Small inhibitor of Bcl-2, HA14-1, selectively enhanced the apoptotic effect of cisplatin by modulating Bcl-2 family members in MDA-MB-231 breast cancer cells
Inhibition or downregulation of Bcl-2 represents a new therapeutic approach to by-pass chemoresistance in cancer cells. Therefore, we explored the potential of this approach in breast cancer cells. Cisplatin and paclitaxel induced apoptosis in a dose-dependent manner in MCF-7 (drug-sensitive) and MDA-MB-231 (drug-insensitive) cells. Furthermore, when we transiently silenced Bcl-2, both cisplatin and paclitaxel induced apoptosis more than parental cells. Dose dependent induction of apoptosis by drugs was enhanced by the pre-treatment of these cells with HA14-1, a Bcl-2 inhibitor. Although the effect of cisplatin was significant on both cell lines, the effect of paclitaxel was much less potent only in MDA-MB-231 cells. To further understand the distinct role of drugs in MDA-MB-231 cells pretreated with HA14-1, caspases and Bcl-2 family proteins were studied. The apoptotic effect of cisplatin with or without HA14-1 pre-treatment is shown to be caspase-dependent. Among pro-apoptotic Bcl-2 proteins, Bax and Puma were found to be up-regulated whereas Bcl-2 and Bcl-x(L) were down-regulated when cells were pretreated with HA14-1 followed by paclitaxel or cisplatin. Enforced Bcl-2 expression in MDA-MB-231 cells abrogated the sensitizing effect of HA14-1 in cisplatin induced apoptosis. These results suggest that the potentiating effect of HA14-1 is drug and cell type specific and may not only depend on the inhibition of Bcl-2. Importantly, alteration of other pro-apoptotic or anti-apoptotic Bcl-2 family members may dictate the apoptotic response when HA14-1 is combined with chemotherapeutic drugs
Building robust prediction models for defective sensor data using Artificial Neural Networks
Predicting the health of components in complex dynamic systems such as an
automobile poses numerous challenges. The primary aim of such predictive
systems is to use the high-dimensional data acquired from different sensors and
predict the state-of-health of a particular component, e.g., brake pad. The
classical approach involves selecting a smaller set of relevant sensor signals
using feature selection and using them to train a machine learning algorithm.
However, this fails to address two prominent problems: (1) sensors are
susceptible to failure when exposed to extreme conditions over a long periods
of time; (2) sensors are electrical devices that can be affected by noise or
electrical interference. Using the failed and noisy sensor signals as inputs
largely reduce the prediction accuracy. To tackle this problem, it is
advantageous to use the information from all sensor signals, so that the
failure of one sensor can be compensated by another. In this work, we propose
an Artificial Neural Network (ANN) based framework to exploit the information
from a large number of signals. Secondly, our framework introduces a data
augmentation approach to perform accurate predictions in spite of noisy
signals. The plausibility of our framework is validated on real life industrial

Where were the monsoon regions and arid zones in Asia prior to the Tibetan Plateau uplift?
The impact of the Tibetan Plateau uplift on the Asian monsoons and inland arid climates is an important but also controversial question in studies of paleoenvironmental change during the Cenozoic. In order to achieve a good understanding of the background for the formation of the Asian monsoons and arid environments, it is necessary to know the characteristics of the distribution of monsoon regions and arid zones in Asia before the plateau uplift. In this study, we discuss in detail the patterns of distribution of the Asian monsoon and arid regions before the plateau uplift on the basis of modeling results without topography from a global coupled atmosphere–ocean general circulation model, compare our results with previous simulation studies and available biogeological data, and review the uncertainties in the current knowledge. Based on what we know at the moment, tropical monsoon climates existed south of 20°N in South and Southeast Asia before the plateau uplift, while the East Asian monsoon was entirely absent in the extratropics. These tropical monsoons mainly resulted from the seasonal shifts of the Inter-Tropical Convergence Zone. There may have been a quasi-monsoon region in central-southern Siberia. Most of the arid regions in the Asian continent were limited to the latitudes of 20–40°N, corresponding to the range of the subtropical high pressure year-around. In the meantime, the present-day arid regions located in the relatively high latitudes in Central Asia were most likely absent before the plateau uplift. The main results from the above modeling analyses are qualitatively consistent with the available biogeological data. These results highlight the importance of the uplift of the Tibetan Plateau in the Cenozoic evolution of the Asian climate pattern of dry–wet conditions. Future studies should be focused on effects of the changes in land–sea distribution and atmospheric CO2 concentrations before and after the plateau uplift, and also on cross-comparisons between numerical simulations and geological evidence, so that a comprehensive understanding of the evolution of the Cenozoic paleoenvironments in Asia can be achieved
