    Impairment of Vowel Articulation as a Possible Marker of Disease Progression in Parkinson's Disease

    Purpose: The aim of the current study was to survey if vowel articulation in speakers with Parkinson’s disease (PD) shows specific changes in the course of the disease. Method: 67 patients with PD (42 male) and 40 healthy speakers (20 male) were tested and retested after an average time interval of 34 months. Participants had to read a given text as source for subsequent calculation of the triangular vowel space area (tVSA) and vowel articulation index (VAI). Measurement of tVSA and VAI were based upon analysis of the first and second formant of the vowels /a/, /i/and /u / extracted from defined words within the text. Results: At first visit, VAI values were reduced in male and female PD patients as compared to the control group, and showed a further decrease at the second visit. Only in female Parkinsonian speakers, VAI was correlated to overall speech impairment based upon perceptual impression. VAI and tVSA were correlated to gait impairment, but no correlations were seen between VAI and global motor impairment or overall disease duration. tVSA showed a similar reduction in the PD as compared to the control group and was also found to further decline between first and second examination in female, but not in male speakers with PD. Conclusions: Measurement of VAI seems to be superior to tVSA in the description of impaired vowel articulation and its further decline in the course of the disease in PD.     Articulatory network reorganization in Parkinson's disease as assessed by multimodal MRI and acoustic measures

    Introduction: Hypokinetic dysarthria (HD) is common in Parkinson's disease (PD). Our objective was to evaluate articulatory networks and their reorganization due to PD pathology in individuals without overt speech impairment using a multimodal MRI protocol and acoustic analysis of speech. Methods: A total of 34 PD patients with no subjective HD complaints and 25 age-matched healthy controls (HC) underwent speech task recordings, structural MRI, and reading task-induced and resting-state fMRI. Grey matter probability maps, task-induced activations, and resting-state functional connectivity within the regions engaged in speech production (ROIs) were assessed and compared between groups. Correlation with acoustic parameters was also performed. Results: PD patients as compared Tto HC displayed temporal decreases in speech loudness which were related to BOLD signal increases in the right-sided regions of the dorsal language pathway/articulatory network. Among those regions, activation of the right anterior cingulate was increased in PD as compared to HC. We also found bilateral posterior superior temporal gyrus (STG) GM loss in PD as compared to HC that was strongly associated with diadochokinetic (DDK) irregularity in the PD group. Task-induced activations of the left STG were increased in PD as compared to HC and were related to the DDK rate control. Conclusions: The results provide insight into the neural correlates of speech production control and distinct articulatory network reorganization in PD apparent already in patients without subjective speech impairment

    Retaining Expression on De-identified Faces

    © Springer International Publishing AG 2017The extensive use of video surveillance along with advances in face recognition has ignited concerns about the privacy of the people identifiable in the recorded documents. A face de-identification algorithm, named k-Same, has been proposed by prior research and guarantees to thwart face recognition software. However, like many previous attempts in face de-identification, kSame fails to preserve the utility such as gender and expression of the original data. To overcome this, a new algorithm is proposed here to preserve data utility as well as protect privacy. In terms of utility preservation, this new algorithm is capable of preserving not only the category of the facial expression (e.g., happy or sad) but also the intensity of the expression. This new algorithm for face de-identification possesses a great potential especially with real-world images and videos as each facial expression in real life is a continuous motion consisting of images of the same expression with various degrees of intensity.Peer reviewe

    Stressors and resources related to academic studies and improvements suggested by medical students: a qualitative study

    Background: Prior evidence suggests that medical students’ mental health is poor and deteriorates during the course of academic studies. This qualitative study therefore aims to improve our understanding of medical students’ perceptions of i) stressors related to their academic studies, ii) resources that may facilitate coping with those stressors and iii) suggestions to potentially reduce stress. Methods: Eight focus groups were conducted with medical students enrolled at a medical school in Germany until thematic saturation was reached. A topic guide was used to facilitate the discussion. Subsequently, focus group discussions were transcribed and content-analyzed using MaxQDA. Results: Organizational factors especially related to inadequate information flow as well as exams (e.g. repeat exams, scheduling, perceived unfair grading), poor theoretical and practical teaching quality, time and performance pressure, social interactions and individual characteristics (e.g. self-expectations, fear of failure) emerged as major contributors to stress. Resources perceived to facilitate coping with those stressors pertained to some other organizational aspects (e.g. flexibility, availability of contact persons), career prospects, practical training, social support, personal characteristics (e.g. knowledge base, past experience) and leisure time. Suggestions for improvement related primarily to organizational measures rather than individual-level measures. Conclusions: Besides well-known stressors (e.g. exams and high performance pressure), some new aspects emerged from our study including stress related to organizational factors and repeat exams. Accordingly, students’ wishes for organizational-level interventions, including better information systems and better interweaving of practical and theoretical education, could be first target areas for improvement

    Patient‐reported outcomes in Huntington’s disease: Quality of life in neurological disorders (Neuro‐QoL) and Huntington’s disease health‐related quality of life (HDQLIFE) physical function measures

    Background: There is a need for patient‐reported outcome measures that capture the impact that motor impairments have on health‐related quality of life in individuals with Huntington’s disease.Objectives: The objectives of this study were to establish the reliability and validity of new physical functioning patient‐reported outcome measures in Huntington’s disease.Methods: A total of 510 individuals with Huntington’s disease completed 2 Quality of Life in Neurological Disorders (Lower Extremity Function and Upper Extremity Function) and 3 Huntington’s Disease Health‐Related Quality of Life (Chorea, Speech Difficulties, and Swallowing Difficulties) measures. Clinician‐rated and generic self‐report measures were also administered.Results: Reliabilities for the new patient reported physical functioning measures were excellent (all Cronbach’s α > .92).     Prosodic Structure as a Parallel to Musical Structure

Although early speculation identified a wide variety of possibilities, the literature has largely focused on the parallels between musical structure and syntactic structure. Here, we argue that parallels between musical structure and prosodic structure deserve more attention. We review the evidence for a link between musical and prosodic structure and find it to be strong. In fact, certain elements of prosodic structure may provide a parsimonious comparison with musical structure without sacrificing empirical findings related to the parallels between language and music. We then develop several predictions related to such a hypothesis
