Bem-estar spiritual dos enfermeiros das unidades de terapia intensiva em pandemia
Objective: Determine the spiritual well-being and its relationship with demographic variables of nursing professionals who work at the Intensive Care Unit (ICU) during the COVID-19 pandemic. Materials and methods: Quantitative, cross-sectional study that determined the levels of spiritual well-being through the Reed´s Spiritual Perspective Scale (SPS) in 116 nurses that worked in the ICU during the public health emergency of COVID-19, from June to July of 2020. The relationship of the variables was determined using the Fisher test, and the differences between the groups using the Mann-Whitney U test and the Analysis of Variance (ANOVA) test. Significant statistical differences, with p<0,05, were considered. Results: The participants were predominantly female, under 45 years of age, single and with a bachelor´s degree as their maximum level of education. High levels of practice, belief and spiritual well-being were observed regardless the public health situation. Conclusion: The spiritual perspective behaves differently depending on the age, gender, civil status, academic background, and income. A high well-being in the nursing professional who works at the ICU during the COVID-19 pandemic, is a strength that could help maintain their mental health, improving their capacity to respond to scenarios of crisis.Objetivo: Determinar el bienestar espiritual y su relación con variables demográficas y profesionales en enfermeros que se desempeñan en Unidades de Cuidado Intensivo (UCI) durante la pandemia del COVID-19. Materiales y métodos: Estudio cuantitativo, transversal que determinó los niveles de bienestar espiritual mediante la escala de perspectiva espiritual (SPS) de Pamela Reed, en 116 enfermeros que actúan en UCI durante la emergencia sanitaria del COVID-19 de junio a julio de 2020. La relación de las variables se determinó empleando el test de Fisher y las diferencias entre los grupos usando Mann-Withney y ANOVA. Fueron consideradas diferencias estadísticas significativas con p<0,05. Resultados: Los participantes fueron predominantemente del sexo femenino, con edades inferiores a 45 años, estado civil soltero y con pregrado como máximo nivel de formación. Se observaron altos niveles de práctica, creencia y bienestar espiritual independiente de la situación de salud pública. Conclusión: La perspectiva espiritual se comporta de manera diferenciada según la edad, género, estado civil, formación académica e ingresos económicos. Conclusão: A perspectiva espiritual comporta-se de maneira diferenciada segundo a idade, sexo, estado civil, formação acadêmica e ingressos financeiros. O alto bem-estar espiritual no enfermeiro que trabalha na UTI durante a pandemia da COVID-19, é uma fortaleza que pode apoiar lhe na manutenção da sua saúde mental e melhorar a sua capacidade de afrontamento em cenários de crise
Relación entre bienestar psicológico y la percepción de calidad de vida en pacientes en hemodiálisis
Introducción: La hemodiálisis es la alternativa terapéutica más empleada en los pacientes con enfermedad renal crónica; sin embargo, no se han establecido de forma multidimensional las condiciones de bienestar psicológico y la calidad de vida de estos pacientes.
Objetivo: Analizar el perfil de bienestar psicológico y percepción de calidad de vida de pacientes en hemodiálisis, así como diferenciar e identificar la relación entre estas variables.
Material y método: Estudio observacional descriptivo transversal realizado a 173 pacientes en hemodiálisis. Se emplearon los cuestionarios PGWBI para evaluar el bienestar psicológico y el WHOQOL-BREF para la calidad de vida.
Resultados: Pacientes con ERC en tratamiento con HD con un promedio de 57,59±13,31 años, predominantemente de género masculino (n=110; 63,58%). Se encontraron altos índices de bienestar psicológico (78,61%) y percepciones moderadas de calidad de vida (54,91%), acompañadas de sintomatología depresiva (97,11%) y ansiosa (97,11%). La edad, nivel educativo, condición laboral, ingresos económicos, tener Diabetes Mellitus y concentraciones sanguíneas inadecuadas de hemoglobina, potasio y fósforo, mostraron diferencias en los dominios y categorías (p<0,01). El análisis correlacional encontró una estrecha relación lineal entre las dos variables estudiadas (r=0,81; p=0,0001).
Conclusiones: Los participantes del estudio mostraron altas percepciones de bienestar psicológico y moderadas apreciaciones de calidad de vida a expensas de elevadas valoraciones de positividad, autocontrol, vitalidad y percepciones de salud física, psicológica y relaciones sociales. El bienestar psicológico desde sus diferentes categorías influencia directamente la percepción de calidad de vida de los pacientes en hemodiálisis
Plan de egreso: herramienta del cuidado - diada con enfermedad crónica

El programa plan de egreso para el paciente con enfermedad crónica no trasmisible ECNT y su cuidador familiar es evidenciada como una herramienta de apoyo para el cuidado continuo en el hogar.
El programa plan de egreso, se connota como una herramienta que puede ayudar a disminuir los reingresos prevenibles de los pacientes con ECNT, en las instituciones.
Introducción: La OMS reporta incremento de las enfermedades crónicas no trasmisibles, dada su complejidad, los pacientes presentan constantes hospitalizaciones, por lo cual es necesario fortalecer la competencia del cuidado de la enfermedad tanto del paciente como del cuidador en el hogar a través del programa plan de egreso. Objetivo: determinar el cambio en la competencia para el cuidado posterior a la implementación de herramienta plan de egreso para el cuidado en el hogar de la diada persona-cuidador familiar en situación de cronicidad en una institución de segundo nivel de atención. Materiales y métodos: Estudio de abordaje cuantitativo, seudo-experimental, con diseño de pruebas pre-test y pos-test, aplicando los instrumentos de “caracterización diada paciente-cuidador” y “Cuidar”. La población correspondió a 62 diadas. Muestreo no probabilístico a conveniencia. Se calculó del delta pre-post y se utilizó t-Student pareada para comparar. Resultados: el programa plan de egreso aumentó en el paciente las dimensiones de Conocimiento con un delta de 3.2 y Unicidad de 3,0 y competencia de cuidado global en 10,3; mientras que las competencias del cuidador familiar mejoraron en las dimensiones de Conocimiento con un delta de 3.34, Unicidad con delta 1.5, la dimensión de Disfrute con 1.7 y la de Reciprocidad –Relación con un delta de 1.61 para un total en la competencia de 9.2. Discusión y conclusiones: Estos hallazgos sugieren fortalecer el programa plan de egreso, durante la hospitalización y alta con el propósito de mejorar la competencia para cuidar en el hogar tanto del paciente como su cuidador familiar en situación de cronicidad.

Defining a novel leptin–melanocortin–kisspeptin pathway involved in the metabolic control of puberty
Objective
Puberty is a key developmental phenomenon highly sensitive to metabolic modulation. Worrying trends of changes in the timing of puberty have been reported in humans. These might be linked to the escalating prevalence of childhood obesity and could have deleterious impacts on later (cardio-metabolic) health, but their underlying mechanisms remain unsolved. The neuropeptide α-MSH, made by POMC neurons, plays a key role in energy homeostasis by mediating the actions of leptin and likely participates in the control of reproduction. However, its role in the metabolic regulation of puberty and interplay with kisspeptin, an essential puberty-regulating neuropeptide encoded by Kiss1, remain largely unknown. We aim here to unveil the potential contribution of central α-MSH signaling in the metabolic control of puberty by addressing its role in mediating the pubertal effects of leptin and its potential interaction with kisspeptin.
Methods
Using wild type and genetically modified rodent models, we implemented pharmacological studies, expression analyses, electrophysiological recordings, and virogenetic approaches involving DREADD technology to selectively inhibit Kiss1 neurons, in order to interrogate the physiological role of a putative leptin→α-MSH→kisspeptin pathway in the metabolic control of puberty.
Results
Stimulation of central α-MSH signaling robustly activated the reproductive axis in pubertal rats, whereas chronic inhibition of melanocortin receptors MC3/4R, delayed puberty, and prevented the permissive effect of leptin on puberty onset. Central blockade of MC3/4R or genetic elimination of kisspeptin receptors from POMC neurons did not affect kisspeptin effects. Conversely, congenital ablation of kisspeptin receptors or inducible, DREADD-mediated inhibition of arcuate nucleus (ARC) Kiss1 neurons resulted in markedly attenuated gonadotropic responses to MC3/4R activation. Furthermore, close appositions were observed between POMC fibers and ARC Kiss1 neurons while blockade of α-MSH signaling suppressed Kiss1 expression in the ARC of pubertal rats.
Conclusions

Connecting nutritional deprivation and pubertal inhibition via GRK2-mediated repression of kisspeptin actions in GnRH neurons
Background: Perturbations in the timing of puberty, with potential adverse consequences in later health, are increasingly common. The underlying neurohormonal mechanisms are unfolded, but nutritional alterations are key contributors. Efforts to unveil the basis of normal puberty and its metabolic control have focused on mechanisms controlling expression of Kiss1, the gene encoding the puberty-activating neuropeptide, kisspeptin. However, other regulatory phenomena remain ill-defined. Here, we address the putative role of the G protein-coupled-receptor kinase-2, GRK2, in GnRH neurons, as modulator of pubertal timing via repression of the actions of kisspeptin, in normal maturation and conditions of nutritional deficiency. Methods: Hypothalamic RNA and protein expression analyses were conducted in maturing female rats. Pharmacological studies involved central administration of GRK2 inhibitor, βARK1-I, and assessment of gonadotropin responses to kisspeptin or phenotypic and hormonal markers of puberty, under normal nutrition or early subnutrition in female rats. In addition, a mouse line with selective ablation of GRK2 in GnRH neurons, aka G-GRKO, was generated, in which hormonal responses to kisspeptin and puberty onset were monitored, in normal conditions and after nutritional deprivation. Results: Hypothalamic GRK2 expression increased along postnatal maturation in female rats, especially in the preoptic area, where most GnRH neurons reside, but decreased during the juvenile-to-pubertal transition. Blockade of GRK2 activity enhanced Ca responses to kisspeptin in vitro, while central inhibition of GRK2 in vivo augmented gonadotropin responses to kisspeptin and advanced puberty onset. Postnatal undernutrition increased hypothalamic GRK2 expression and delayed puberty onset, the latter being partially reversed by central GRK2 inhibition. Conditional ablation of GRK2 in GnRH neurons enhanced gonadotropin responses to kisspeptin, accelerated puberty onset, and increased LH pulse frequency, while partially prevented the negative impact of subnutrition on pubertal timing and LH pulsatility in mice. Treatment with tocilizumab or corticosteroids for COVID-19 patients with hyperinflammatory state: a multicentre cohort study (SAM-COVID-19)
Objectives: The objective of this study was to estimate the association between tocilizumab or corticosteroids and the risk of intubation or death in patients with coronavirus disease 19 (COVID-19) with a hyperinflammatory state according to clinical and laboratory parameters.
Methods: A cohort study was performed in 60 Spanish hospitals including 778 patients with COVID-19 and clinical and laboratory data indicative of a hyperinflammatory state. Treatment was mainly with tocilizumab, an intermediate-high dose of corticosteroids (IHDC), a pulse dose of corticosteroids (PDC), combination therapy, or no treatment. Primary outcome was intubation or death; follow-up was 21 days. Propensity score-adjusted estimations using Cox regression (logistic regression if needed) were calculated. Propensity scores were used as confounders, matching variables and for the inverse probability of treatment weights (IPTWs).
Results: In all, 88, 117, 78 and 151 patients treated with tocilizumab, IHDC, PDC, and combination therapy, respectively, were compared with 344 untreated patients. The primary endpoint occurred in 10 (11.4%), 27 (23.1%), 12 (15.4%), 40 (25.6%) and 69 (21.1%), respectively. The IPTW-based hazard ratios (odds ratio for combination therapy) for the primary endpoint were 0.32 (95%CI 0.22-0.47; p < 0.001) for tocilizumab, 0.82 (0.71-1.30; p 0.82) for IHDC, 0.61 (0.43-0.86; p 0.006) for PDC, and 1.17 (0.86-1.58; p 0.30) for combination therapy. Other applications of the propensity score provided similar results, but were not significant for PDC. Tocilizumab was also associated with lower hazard of death alone in IPTW analysis (0.07; 0.02-0.17; p < 0.001).
Conclusions: Tocilizumab might be useful in COVID-19 patients with a hyperinflammatory state and should be prioritized for randomized trials in this situatio
Clonal chromosomal mosaicism and loss of chromosome Y in elderly men increase vulnerability for SARS-CoV-2
The pandemic caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2, COVID-19) had an estimated overall case fatality ratio of 1.38% (pre-vaccination), being 53% higher in males and increasing exponentially with age. Among 9578 individuals diagnosed with COVID-19 in the SCOURGE study, we found 133 cases (1.42%) with detectable clonal mosaicism for chromosome alterations (mCA) and 226 males (5.08%) with acquired loss of chromosome Y (LOY). Individuals with clonal mosaic events (mCA and/or LOY) showed a 54% increase in the risk of COVID-19 lethality. LOY is associated with transcriptomic biomarkers of immune dysfunction, pro-coagulation activity and cardiovascular risk. Interferon-induced genes involved in the initial immune response to SARS-CoV-2 are also down-regulated in LOY. Thus, mCA and LOY underlie at least part of the sex-biased severity and mortality of COVID-19 in aging patients. Given its potential therapeutic and prognostic relevance, evaluation of clonal mosaicism should be implemented as biomarker of COVID-19 severity in elderly people. Worldwide trends in underweight and obesity from 1990 to 2022: a pooled analysis of 3663 population-representative studies with 222 million children, adolescents, and adults
Background Underweight and obesity are associated with adverse health outcomes throughout the life course. We
estimated the individual and combined prevalence of underweight or thinness and obesity, and their changes, from
1990 to 2022 for adults and school-aged children and adolescents in 200 countries and territories.
Methods We used data from 3663 population-based studies with 222 million participants that measured height and
weight in representative samples of the general population. We used a Bayesian hierarchical model to estimate
trends in the prevalence of different BMI categories, separately for adults (age ≥20 years) and school-aged children
and adolescents (age 5–19 years), from 1990 to 2022 for 200 countries and territories. For adults, we report the
individual and combined prevalence of underweight (BMI <18·5 kg/m2) and obesity (BMI ≥30 kg/m2). For schoolaged children and adolescents, we report thinness (BMI <2 SD below the median of the WHO growth reference)
and obesity (BMI >2 SD above the median).
Findings From 1990 to 2022, the combined prevalence of underweight and obesity in adults decreased in
11 countries (6%) for women and 17 (9%) for men with a posterior probability of at least 0·80 that the observed
changes were true decreases. The combined prevalence increased in 162 countries (81%) for women and
140 countries (70%) for men with a posterior probability of at least 0·80. In 2022, the combined prevalence of
underweight and obesity was highest in island nations in the Caribbean and Polynesia and Micronesia, and
countries in the Middle East and north Africa. Obesity prevalence was higher than underweight with posterior
probability of at least 0·80 in 177 countries (89%) for women and 145 (73%) for men in 2022, whereas the converse
was true in 16 countries (8%) for women, and 39 (20%) for men. From 1990 to 2022, the combined prevalence of
thinness and obesity decreased among girls in five countries (3%) and among boys in 15 countries (8%) with a
posterior probability of at least 0·80, and increased among girls in 140 countries (70%) and boys in 137 countries (69%)
with a posterior probability of at least 0·80. The countries with highest combined prevalence of thinness and
obesity in school-aged children and adolescents in 2022 were in Polynesia and Micronesia and the Caribbean for
both sexes, and Chile and Qatar for boys. Combined prevalence was also high in some countries in south Asia, such
as India and Pakistan, where thinness remained prevalent despite having declined. In 2022, obesity in school-aged
children and adolescents was more prevalent than thinness with a posterior probability of at least 0·80 among girls
in 133 countries (67%) and boys in 125 countries (63%), whereas the converse was true in 35 countries (18%) and
42 countries (21%), respectively. In almost all countries for both adults and school-aged children and adolescents,
the increases in double burden were driven by increases in obesity, and decreases in double burden by declining
underweight or thinness.
Interpretation The combined burden of underweight and obesity has increased in most countries, driven by an
increase in obesity, while underweight and thinness remain prevalent in south Asia and parts of Africa. A healthy
nutrition transition that enhances access to nutritious foods is needed to address the remaining burden of
underweight while curbing and reversing the increase in obesit
Measurement of CP asymmetry in B-s(0) -> D-s(-/+) K--/+ decays
We report on measurements of the time-dependent CP violating observables in decays using a dataset corresponding to 1.0 fb of pp collisions recorded with the LHCb detector. We find the CP violating observables , , , , , where the uncertainties are statistical and systematic, respectively. We use these observables to make the first measurement of the CKM angle in decays, finding = (115) modulo 180 at 68% CL, where the error contains both statistical and systematic uncertainties.We report on measurements of the time-dependent CP violating observables in B → D K decays using a dataset corresponding to 1.0 fb of pp collisions recorded with the LHCb detector. We find the CP violating observables C = 0.53±0.25±0.04, A = 0.37 ± 0.42 ± 0.20, , S = −1.09±0.33±0.08, , where the uncertainties are statistical and systematic, respectively. Using these observables together with a recent measurement of the B mixing phase −2β leads to the first extraction of the CKM angle γ from B → D K decays, finding γ = (115 )° modulo 180° at 68% CL, where the error contains both statistical and systematic uncertainties.We report on measurements of the time-dependent CP violating observables in decays using a dataset corresponding to 1.0 fb of pp collisions recorded with the LHCb detector.
