    Modelling Pathways for Outbreaks in Field Occupational Epidemiology

    Background: The investigation of an occupational outbreak, once the index case has been identified, triggers a stress situation to epidemiologists. Modelling occupational outbreaks will be useful to guide the field investigation. Objective: To identify standard pathways for occupational epidemic outbreaks. Methods: In-depth critical appraisal of 57 occupational outbreaks. Standard pathways of occupational outbreaks were identified by analysing the similarity between out-breaks. The model’s accuracy and homogeneity were established through Fisher’s exact test and the Kappa Index. Results: The analysis allowed synthesizing the occupational outbreaks variability in 4 pathways. 92.98% of the analysed outbreaks could be allocated to one of those 4 types. The theoretical patterns showed a good adjustment with the analysed out-breaks: Type I (Kappa = 0.94 - 0.60), Type II (Kappa = 1.00), Type III (Kappa = 1.00 - 0.68) and Type IV (Kappa = 0.94 - 0.87). The probability of a given outbreak fitting with its three components in any of the theoretical pathways was 0.83. Conclusions: The incorporation of those pathways to the field occupational epidemiology will allow: 1) to provide early guidance to epidemiological, clinical and environmental studies focused on specific hypothesis of causality; 2) to anticipate preventive measures; 3) to contribute to an earlier and more efficient outbreak resolution.S

    Systematics, biogeography and conservation of Bald Uakaris (Cacajao Lesson, 1840)

    Uakaris, genus Cacajao, possess adaptations to their unripe seed-focused diet and habitat preference for Amazonian flooded forest habitats. These include large procumbent canines (shared by the other 2 Pitheciinae, Chiropotes and Pithecia, large home ranges, and large group sizes). The diversity, evolution and phylogenetics of Pitheciinae are limited to few studies and their geographic distribution to a few records and localities. The overall goal of this study is to investigate the phylogenetic relationship of bald uakaris, genus Cacajao, using molecular data, and to get new information on its genetic diversity and geographic distribution to assess their taxonomic classification. The molecular analysis of the mitochondrial DNA supports the origin of the ancestral of Cacajao in the Western Amazon with the sister genus Chiropotes expanding to the Guiana and Brazilian shields during the Pleistocene. Therefore, the genus Cacajao had its diversification influenced by the formation of the flooded forests of Western Amazon during the process of drainage of the Pebas Lake. The ddRAD analysis supported the reciprocal monophyly of bald uakaris, with all clades including only individuals with exclusive diagnostic characters. Therefore, bald uakaris can be classified as follow: Cacajao calvus (I. Geoffroy, 1847), C. rubicundus (I. Geoffroy and Deville, 1848), C. ucayalii (Thomas, 1928), and C. novaesi (Hershkovitz, 1987). I provide an update of the geographic distribution of each species of bald uakari and how the predicted scenarios of deforestation and climate change can affect the four species by 2050. Cacajao novaesi has the more restricted geographic distribution occurring only between Gregorio and Tarauacá rivers. All other bald uakaris occur in a patchy distribution. The synergistic effects of climate change and deforestation will imply in habitat loss in the future scenarios, and along with hunting of C. ucayalii and C. novaesi, will result in a population decrease. Studies on the feeding ecology in different field sites and the population status for each taxon are the priorities for the research of bald uakaris

    Brazilian gamma detection device for sentinel lymph node biopsy

    OBJECTIVE: To test the effectiveness of an intra-operative gamma detection Brazilian device (IPEN) on sentinel lymph node biopsy (SLNB) procedures. METHODS: Forty melanoma or breast cancer patients with indication for undergoing SLNB were studied. Lymphoscintigraphy was done 2 to 24 hours prior to surgery. Lymphatic mapping with vital dye and gamma detection were performed intraoperatively. For gamma detection Neoprobe ® 1500 was used followed by IPEN (equipment under test) in the first 20 patients and for the remaining half IPEN was used first to verify its ability to locate the sentinel node (SN). Measurements were taken from the radiopharmaceutical product injection site, from SN (in vivo and ex vivo) and from background. It was recorded if the SN was stained or not and if it was found easily by surgeon. RESULTS: There were 33 (82.5%) breast cancer and 7 (17.5%) melanoma patients. Ages varied from 21 to 68 year-old (median age of 46). Sex distribution was 35 (87.5%) women and 5 (12.5%) men. Sentinel node was found in all but one patient. There was no statistical difference between the reasons ex vivo/ background obtained with the measures of both equipments (p=0, 2583-ns). The SN was easily found by the surgeon with both devices. CONCLUSION: The SLNB was successfully performed using either equipment. It was possible to do SLNB with the Brazilian device developed by IPEN without prejudice for the patient.OBJETIVO: Testar a eficácia de equipamento de detecção gama intra-operatória (DGI) desenvolvido pelo IPEN (Brasil), em procedimentos de biópsia de linfonodo sentinela (BLS) no melanoma e no câncer de mama. MÉTODOS: Foram estudados 40 pacientes portadores de melanoma ou câncer de mama com indicação para realização de BLS.Todos pacientes foram submetidos à linfocintilografia e a BLS ocorreu entre 2 a 24 horas após a mesma. Concomitantemente à DGI, realizou-se o mapeamento linfático com corante vital. Foram feitas leituras com o equipamento convencional Neoprobe® 1500 e com o equipamento em teste (IPEN) dos valores de captação do sítio de injeção do radiofármaco, do LS in vivo e ex vivo e da captação de fundo. Foi registrado se o LS estava corado e se o cirurgião teve facilidade para encontrá-lo. Nos primeiros 20 pacientes utilizou-se o equipamento convencional e depois o de teste; nos outros 20, utilizou-se primeiro o equipamento em teste, com objetivo de verificar se o mesmo identificava primariamente o LS. RESULTADOS: Dos quarenta pacientes, 33 eram portadores de tumor de mama e sete de melanoma cutâneo; variação da idade: 21 a 68 anos (mediana= 46 anos); 35 mulheres e 5 homens. Em apenas um paciente o LS não foi encontrado, nem pela DGI nem pelo corante vital. Não houve diferença estatística entre as razões ex vivo/fundo obtidas com os dois equipamentos (p=0, 2583-ns).     The Roosevelt – Rondon expedition marmoset (Mico marcai) : unveiling the conservation status of a data deficient species

    The Roosevelt-Rondon Expedition marmoset, Mico marcai, was collected in 1914 and to date, all information on this species comes from three skins brought back by the Expedition and two additional skins collected in the 1990s. It is no surprise then that M.marcai has been classified as Data Deficient (DD). Given that Mico marcai’s suspected range sits on the path of the advancing Brazilian “Arc-of-Deforestation”, it is urgent that relevant data be collected to assess this taxon. Here we present the first comprehensive field data on the distribution, population size and threats on M. marcai with the goal of removing the species from the DD category. From 2012 to 2015, we surveyed for the species in 11 localities, in and around the Marmelos-Aripuanã interfluve, and estimated density using distance sampling on 10 transects. We also used spatial predictive modelling to project the amount of habitat that will be lost within its range in 18 years under different deforestation scenarios. We found marmosets in 14 localities and calculated its Extent of Occurrence to be 31,073 km2. We walked 271 km and detected 30 marmoset groups, allowing us to estimate their density to be 8.31 individuals/km2 and a total population of 258,217.71 individuals. By a “Business as usual” scenario, 20,181 km2 of habitat will be lost in three marmoset generations (~18 years), compromising 33% of the species’ range. Accordingly, M. marcai should be classified as globally Vulnerable under category A3c. Following our study, we propose the Amazonian marmosets, genus Mico, should undergo similar re-assessment as their ranges all fall in the path of the Arc-of-Deforestation.     L-[1-(11)C]leucine PET can be used to measure in vivo protein synthesis in the brain. However, the relationship between regional protein synthesis and on-going neural dynamics is unclear. We use a graph theoretical approach to examine the relationship between cerebral protein synthesis (rCPS) and both static and dynamical measures of functional connectivity (measured using resting state functional MRI, R-fMRI). Our graph theoretical analysis demonstrates a significant positive relationship between protein turnover and static measures of functional connectivity. We compared these results to simple measures of metabolism in the cortex using [(18)F]FDG PET). Whilst some relationships between [(18)F]FDG binding and graph theoretical measures was present, there remained a significant relationship between protein turnover and graph theoretical measures, which were more robustly explained by L-[1-(11)C]Leucine than [(18)F]FDG PET. This relationship was stronger in dynamics at a faster temporal resolution relative to dynamics measured over a longer epoch. Using a Dynamic connectivity approach, we also demonstrate that broad-band dynamic measures of Functional Connectivity (FC), are inversely correlated with protein turnover, suggesting greater stability of FC in highly interconnected hub regions is supported by protein synthesis. Overall, we demonstrate that cerebral protein synthesis has a strong relationship independent of tissue metabolism to neural dynamics at the macroscopic scale

    A Single Argonaute Gene Participates in Exogenous and Endogenous RNAi and Controls Cellular Functions in the Basal Fungus Mucor circinelloides

    The mechanism of RNAi is well described in metazoans where it plays a role in diverse cellular functions. However, although different classes of endogenous small RNAs (esRNAs) have been identified in fungi, their biological roles are poorly described due, in part, to the lack of phenotype of mutants affected in the biogenesis of these esRNAs. Argonaute proteins are one of the key components of the RNAi pathways, in which different members of this protein family participate in the biogenesis of a wide repertoire of esRNAs molecules. Here we identified three argonaute genes of the fungus Mucor circinelloides and investigated their participation in exogenous and endogenous RNAi. We found that only one of the ago genes, ago-1, is involved in RNAi during vegetative growth and is required for both transgene-induced RNA silencing and the accumulation of distinct classes of esRNAs derived from exons (ex-siRNAs). Classes I and II ex-siRNAs bind to Ago-1 to control mRNA accumulation of the target protein coding genes. Class III ex-siRNAs do not specifically bind to Ago-1, but requires this protein for their production, revealing the complexity of the biogenesis pathways of ex-siRNAs. We also show that ago-1 is involved in the response to environmental signals, since vegetative development and autolysis induced by nutritional stress are affected in ago-1(-) M. circinelloides mutants. Our results demonstrate that a single Ago protein participates in the production of different classes of esRNAs that are generated through different pathways. They also highlight the role of ex-siRNAs in the regulation of endogenous genes in fungi and expand the range of biological functions modulated by RNAi
