    Reopening Johnson County Hospital: The Beginning of Interprofessional Healthcare Education at ETSU

    Johnson County Hospital was a hospital located in Mountain City, TN that existed from 1972 until its final closure in 1998. The hospital faced multiple financial challenges during its existence and was forced to close three times. In 1988 it closed for the penultimate time, and as it had in the past, it counted on the community, local business and local government to reopen the hospital. Three local businessmen were particularly dedicated to reopening the county hospital and reached out to East Tennessee State University (ETSU) and their newly formed College of Medicine. A partnership was formed between the community and ETSU to open a rural campus to educate medical, nursing, public health and allied health students. This rural campus was made a reality through the Kellogg Grant awarded to ETSU in 1991 and was expanded to include graduate medical education in 1996. This collaboration between the community, local businesses, local government and ETSU was able to reopen the hospital in 1992 and jump-started interprofessional education at ETSU which is continued today in the form of the Academic Health Sciences Center and the Interprofessional Education Program

    Promoting Health Education and Literacy in Rural Tennessee: The Go-Packs Pilot Project

    Little Milligan Elementary School in rural Carter County, Tennessee was provided with health literacy resources in the form of health education Go-Packs––easily implementable lessons contained in a small storage tote––as part of the community project requirement of the James H. Quillen College of Medicine Rural Primary Care Track Curriculum. These Go-Packs included detailed lesson plans and accompanying materials that were designed to facilitate health education in the classrooms by providing easily deployable lessons for the teachers to utilize. Four Go-Packs were provided for hygiene, oral health, tobacco use, and nutrition that teachers used to augment instruction during teachable moments that arose in their classrooms. Our objective was to determine whether the development and implementation of these Go-Packs increased the amount of health education delivered to the students and determine what barriers persist to provide health education in the classroom. Participants were randomly assigned a number which they placed on their pre and post surveys. A focus group was also conducted to better understand the faculty’s experience utilizing the Go-Packs and where improvements could be made. A paired sample t-test showed no significant differences in pre and post attitudes of teachers at the school. The focus group and survey questions identified the need to improve the usability of specific Go-Packs, map the Go-Packs to state mandated curriculums and target Go-Pack usage towards non-core instructors

    Effects of antiplatelet therapy on stroke risk by brain imaging features of intracerebral haemorrhage and cerebral small vessel diseases: subgroup analyses of the RESTART randomised, open-label trial

    Background Findings from the RESTART trial suggest that starting antiplatelet therapy might reduce the risk of recurrent symptomatic intracerebral haemorrhage compared with avoiding antiplatelet therapy. Brain imaging features of intracerebral haemorrhage and cerebral small vessel diseases (such as cerebral microbleeds) are associated with greater risks of recurrent intracerebral haemorrhage. We did subgroup analyses of the RESTART trial to explore whether these brain imaging features modify the effects of antiplatelet therapy

    The Gillings Sampler – An electrostatic air sampler as an alternative method for aerosol in vitro exposure studies

    There is growing interest in studying the toxicity and health risk of exposure to multi-pollutant mixtures found in ambient air, and the U.S. Environmental Protection Agency (EPA) is moving towards setting standards for these types of mixtures. Additionally, the Health Effects Institute's strategic plan aims to develop and apply next-generation multi-pollutant approaches to understanding the health effects of air pollutants. There's increasing concern that conventional in vitro exposure methods are not adequate to meet EPA's strategic plan to demonstrate a direct link between air pollution and health effects. To meet the demand for new in vitro technology that better represents direct air-to-cell inhalation exposures, a new system that exposes cells at the air-liquid interface was developed. This new system, named the Gillings Sampler, is a modified two-stage electrostatic precipitator that provides a viable environment for cultured cells. Polystyrene latex spheres were used to determine deposition efficiencies (38-45%), while microscopy and imaging techniques were used to confirm uniform particle deposition. Negative control A549 cell exposures indicated the sampler can be operated for up to 4 hours without inducing any significant toxic effects on cells, as measured by lactate dehydrogenase (LDH) and interleukin-8 (IL-8). A novel positive aerosol control exposure method, consisting of a p-tolualdehyde (TOLALD) impregnated mineral oil aerosol (MOA), was developed to test this system. Exposures to the toxic MOA at a 1 ng/cm(2) dose of TOLALD yielded a reproducible 1.4 and 2 fold increase in LDH and IL-8 mRNA levels over controls. This new system is intended to be used as an alternative research tool for aerosol in vitro exposure studies. While further testing and optimization is still required to produce a “commercially ready” system, it serves as a stepping-stone in the development of cost-effective in vitro technology that can be made accessible to researchers in the near future
