Revisions of Extensive Acetabular Defects with Impaction Grafting and a Cement Cup
Item does not contain fulltextBACKGROUND: Loosening of acetabular components often leads to bony defects. Management of extensive acetabular bone loss in hip revision arthroplasty can be a tremendous challenge. QUESTIONS/PURPOSES: We asked whether a reconstruction with impacted bone grafts will provide a durable and pain-free function in extensive acetabular defects. We specifically determined the (1) survival rates with the end point of revision for any reason, aseptic revision, and radiographic loosening; (2) visual analog scale (VAS) pain score, Harris hip score (HHS), and the Oxford Hip Questionnaire score (OHQS); (3) number of repeat revisions; (4) complications; and (5) radiographic loosening, wear, and radiolucencies. PATIENTS AND METHODS: We retrospectively followed 25 patients (27 hips) with extensive acetabular defects. No patient was lost to followup. Two patients died during followup. Minimum followup was 3 years (mean, 8.8 years; range, 3-14.1 years). RESULTS: Three patients (three hips) underwent repeat revision surgery and another two patients (two hips) had radiographically loose hips. The 10-year survival rate was 88% (95% confidence interval, 74.2%-100%) with the end point acetabular revision for any reason and 95% (95% confidence interval, 86.0%-100%) with the end point acetabular revision for aseptic loosening. The mean HHSs were 55 points before surgery and 72 points postoperatively. CONCLUSIONS: Acetabular reconstruction with impaction bone grafting and a cemented cup is a reliable technique with a 10-year survival rate of 88% in patients with extensive acetabular deficiencies. Whatās in a word? Using construal-level theory to predict voice endorsement
Voice endorsement is a pivotal means by which employees can influence leadership processes and the organization at large. Up till now, research on voice endorsement has lacked unified theoretical frameworks that can shed light on antecedents of voice endorsement in a more integrative way as well as help identify new and relevant antecedents in a systematic and theory-based manner. We propose that construal-level theory can serve as one such unifying framework and showcase this potential by applying it to voice endorsement. Drawing on construal-level theory we propose that when an employee frames his/her voice messages in a manner that is compatible with the psychological distance between the employee and the supervisor, the supervisor will find the employeeās voice messages easier to process and, consequently, will be more likely to endorse them. Three experiments using different manipulations of voice message frame and psychological distance, and a mini meta-analysis of the three experiments, provide support for our construal compatibility hypothesis and initial evidence for the experienced ease-of-processing logic. We discuss how our construal-level approach to voice endorsement can shed light on previous findings as well as open up new avenues for future research
Long-term follow-up, quality of life, and survival of patients with Lambert-Eaton myasthenic syndrome
ObjectiveTo study survival and to characterize long-term functional impairments and health-related quality of life (HRQOL) of patients with Lambert-Eaton myasthenic syndrome (LEMS).MethodsIn this observational study, survival of patients with LEMS, separately for nontumor (NT) and small cell lung cancer (SCLC), was compared to that of the Dutch general population and patients with SCLC. Disease course in patients with LEMS was recorded retrospectively. Several scales for functional impairments and health-related quality of life were assessed.ResultsWe included 150 patients with LEMS. Survival was similar to that of the general population in 65 patients with NT-LEMS. Tumor survival was significantly longer in 81 patients with SCLC-LEMS compared to patients with non-LEMS SCLC (overall median survival 17 vs 7.0 months, p <0.0001). At diagnosis, 39 (62%) of 63 patients with complete follow-up data were independent for activities of daily living, improving to 85% at the 1-year follow-up. The physical HRQOL composite score (55.9) was significantly lower than in the general population (76.3, p <0.0001) and comparable to that of patients with myasthenia gravis (60.5). The mental HRQOL composite score was 71.8 in patients with LEMS, comparable to that of the general population (77.9, p = 0.19) and patients with myasthenia gravis (70.3).ConclusionsThis study shows that patients with NT-LEMS have normal survival. Patients with SCLC-LEMS have an improved tumor survival, even after correction for tumor stage. A majority of patients with LEMS report a stable disease course and remain or become independent for self-care after treatment
Total hip arthroplasties in the Dutch Arthroplasty Register (LROI) and the Nordic Arthroplasty Register Association (NARA): comparison of patient and procedure characteristics in 475,685 cases
Background and purpose ā Collaborations between arthroplasty registries are important in order to create the possibility of detecting inferior implants early and improve our understanding of differences between nations in terms of indications and outcomes. In this registry study we compared patient and procedure characteristics, and revision rates in the Nordic Arthroplasty Register Association (NARA) database and the Dutch Arthroplasty Register (LROI).
Patients and methods ā All total hip arthroplasties (THAs) performed in 2010ā2016 were included from the LROI (n = 184,862) and the NARA database (n = 290,823), which contains data from Denmark, Norway, Sweden, and Finland. Descriptive statistics and KaplanāMeier survival analyses based on all reasons for revision and stratified by fixation were performed and compared between countries.
Results ā In the Netherlands, the proportion of patients aged < 55 years (9%) and male patients (34%) was lower than in Nordic countries (< 55 years 11ā13%; males 35ā43%); the proportion of osteoarthritis (OA) (87%) was higher compared with Sweden (81%), Norway (77%), and Denmark (81%) but comparable to Finland (86%). Uncemented fixation was used in 62% of patients in the Netherlands, in 70% of patients in Denmark and Finland, and in 28% and 19% in Norway and Sweden, respectively. The 5-year revision rate for THAs for OA was lower in Sweden (2.3%, 95% CI 2.1ā2.5) than in the Netherlands (3.0%, CI 2.9ā3.1), Norway (3.8%, CI 3.6ā4.0), Denmark (4.6%, CI 4.4ā4.8), and Finland (4.4%, CI 4.3ā4.5). Revision rates in Denmark, Norway, and Finland were higher for all fixation groups.
Interpretation ā Patient and THA procedure characteristics as well as revision rates evinced some differences between the Netherlands and the Nordic countries. The Netherlands compared best with Denmark in terms of patient and procedure characteristics, but resembled Sweden more in terms of short-term revision risk. Combining data from registries like LROI and the NARA collaboration is feasible and might possibly enable tracking of potential outlier implants.publishedVersio

Uterine bathing with sonography gel prior to IVF/ICSI-treatment in patients with endometriosis, a multicentre randomised controlled trial
STUDY QUESTION
What is the effect of uterine bathing with sonography gel prior to IVF/ICSI-treatment on live birth rates after fresh embryo transfer in patients with endometriosis?
SUMMARY ANSWER
After formal interim analysis and premature ending of the trial, no significant difference between uterine bathing using a pharmacologically neutral sonography gel compared to a sham procedure on live birth rate after fresh embryo transfer in endometriosis patients (26.7% vs. 15.4%, relative risk (RR) 1.73, 95% confidence interval (CI) 0.81ā3.72; P-value 0.147) could be found, although the trial was underpowered to draw definite conclusions.
WHAT IS KNOWN ALREADY
Impaired implantation receptivity contributes to reduced clinical pregnancy rates after IVF/ICSI-treatment in endometriosis patients. Previous studies have suggested a favourable effect of tubal flushing with LipiodolĀ® on natural conceptions. This benefit might also be explained by enhancing implantation through endometrial immunomodulation. Although recent studies showed no beneficial effect of endometrial scratching, the effect of mechanical stress by intrauterine infusion on the endometrium in endometriosis patients undergoing IVF/ICSI-treatment has not been investigated yet.
STUDY DESIGN, SIZE, DURATION
We performed a multicentre, patient-blinded, randomised controlled trial in which women were randomly allocated to either a Gel Infusion Sonography (GIS, intervention group) or a sham procedure (control group) prior to IVF/ICSI-treatment. Since recruitment was slow and completion of the study was considered unfeasible, the study was halted after inclusion of 112 of the planned 184 women.
PARTICIPANTS/MATERIALS, SETTING, METHODS
We included infertile women with surgically confirmed endometriosis ASRM stage IāIV undergoing IVF/ICSI-treatment. After informed consent, women were randomised to GIS with intrauterine instillation of ExEm-gelĀ® or sonography with gel into the vagina (sham). This was performed in the cycle preceding the embryo transfer, on the day GnRH analogue treatment was started. The primary endpoint was live birth rate after fresh embryo transfer. Analysis was performed by both intention-to-treat and per-protocol.
MAIN RESULTS AND THE ROLE OF CHANCE
Between July 2014 to September 2018, we randomly allocated 112 women to GIS (nā=ā60) or sham procedure (nā=ā52). The live birth rate after fresh embryo transfer was 16/60 (26.7%) after GIS versus 8/52 (15.4%) after the sham (RR 1.73, 95% CI 0.81ā3.72; P-value 0.147). Ongoing pregnancy rate was 16/60 (26.7%) after GIS versus 9/52 (17.3%) in the controls (RR 1.54, 95% CI 0.74ā3.18). Miscarriage occurred in 1/60 (1.7%) after GIS versus 5/52 (9.6%) in the controls (RR 0.17, 95% CI 0.02ā1.44) women. Uterine bathing resulted in a higher pain score compared with a sham procedure (visual analogue scale score 2.7 [1.3ā3.5] vs. 1.0 [0.0ā2.0], Pā<ā0.001). There were two adverse events after GIS compared with none after sham procedures.
LIMITATIONS, REASONS FOR CAUTION
The study was terminated prematurely due to slow recruitment and trial fatigue. Therefore, the trial is underpowered to draw definite conclusions regarding the effect of uterine bathing with sonography gel on live birth rate after fresh embryo transfer in endometriosis patients undergoing IVF/ICSI-treatment.
WIDER IMPLICATIONS OF THE FINDINGS
We could not demonstrate a favourable effect of uterine bathing procedures with sonography gel prior to IVF/ICSI-treatment in patients with endometriosis.

Learning intrinsic excitability in medium spiny neurons
We present an unsupervised, local activation-dependent learning rule for
intrinsic plasticity (IP) which affects the composition of ion channel
conductances for single neurons in a use-dependent way. We use a
single-compartment conductance-based model for medium spiny striatal neurons in
order to show the effects of parametrization of individual ion channels on the
neuronal activation function. We show that parameter changes within the
physiological ranges are sufficient to create an ensemble of neurons with
significantly different activation functions. We emphasize that the effects of
intrinsic neuronal variability on spiking behavior require a distributed mode
of synaptic input and can be eliminated by strongly correlated input. We show
how variability and adaptivity in ion channel conductances can be utilized to
store patterns without an additional contribution by synaptic plasticity (SP).
The adaptation of the spike response may result in either "positive" or
"negative" pattern learning. However, read-out of stored information depends on
a distributed pattern of synaptic activity to let intrinsic variability
determine spike response. We briefly discuss the implications of this
