    Long-term exposure to outdoor air pollution and the incidence of chronic obstructive pulmonary disease in a national English cohort.

    OBJECTIVES: The role of outdoor air pollution in the incidence of chronic obstructive pulmonary disease (COPD) remains unclear. We investigated this question using a large, nationally representative cohort based on primary care records linked to hospital admissions. METHODS: A cohort of 812 063 patients aged 40-89 years registered with 205 English general practices in 2002 without a COPD diagnosis was followed from 2003 to 2007. First COPD diagnoses recorded either by a general practitioner (GP) or on admission to hospital were identified. Annual average concentrations in 2002 for particulate matter with an aerodynamic diameter <10 µm (PM10) and <2.5 µm (PM2.5), nitrogen dioxide (NO2), ozone and sulfur dioxide (SO2) at 1 km(2) resolution were estimated from emission-based dispersion models. Hazard ratios (HRs) per interquartile range change were estimated from Cox models adjusting for age, sex, smoking, body mass index and area-level deprivation. RESULTS: 16 034 participants (1.92%) received a COPD diagnosis from their GP and 2910 participants (0.35%) were admitted to hospital for COPD. After adjustment, HRs for GP recorded COPD and PM10, PM2.5 and NO2 were close to unity, positive for SO2 (HR=1.07 (95% CI 1.03 to 1.11) per 2.2 µg/m(3)) and negative for ozone (HR=0.94 (0.89 to 1.00) per 3 µg/m(3)). For admissions HRs for PM2.5 and NO2 remained positive (HRs=1.05 (0.98 to 1.13) and 1.06 (0.98 to 1.15) per 1.9 µg/m(3) and 10.7 µg/m(3), respectively). CONCLUSIONS: This large population-based cohort study found limited, inconclusive evidence for associations between air pollution and COPD incidence. Further work, utilising improved estimates of air pollution over time and enhanced socioeconomic indicators, is required to clarify the association between air pollution and COPD incidence

    A PTAS for Scheduling Unrelated Machines of Few Different Types

    Scheduling on Unrelated Machines is a classical optimization problem where nn jobs have to be distributed to mm machines. Each of the jobs j{1,,n}j \in \{1, \ldots, n\} has on machine i{1,,m}i \in \{1, \ldots, m\} a processing time pij0p_{ij} \geq 0. The goal is to minimize the makespan, i.e.\ the maximum completion time of the longest-running machine. Unless P=NPP = NP, this problem does not allow for a polynomial-time approximation algorithm with a ratio better than 32\frac{3}{2}. A natural scenario is however that many machines are of the same type, like a CPU and GPU cluster: for each of the KK machine types, the machines iii \neq i' of the same type kk satisfy pij=pijp_{ij} = p_{i'j} for all jobs jj. For the case where the number KK of machine types is constant, this paper presents an approximation scheme, i.e.\ an algorithm of approximation ratio 1+ε1+\varepsilon for ε>0\varepsilon > 0, with an improved running time only single exponential in 1ε\frac{1}{\varepsilon}

    We discuss the aspect of synchronisation in the language design and implementation of the asynchronous data flow language S-Net. Synchronisation is a crucial aspect of any coordination approach. S-Net provides a particularly simple construct, the synchrocell. As a primitive S-Net language construct synchrocell implements a one-off synchronisation of two data items of different type on a stream of such data items. We believe this semantics captures the essence of synchronisation, and no simpler design is possible. While the exact built-in behaviour as such is typically not what is required by S-Net application programmers, we show that in conjunction with other language features S-Net synchrocells meet typical demands for synchronisation in streaming networks quite well. Moreover, we argue that their simplistic design, in fact, is a necessary prerequisite to implement an even more interesting scenario: modelling state in streaming networks of stateless components. We finish with the outline of an efficient implementation by the S-Net runtime system

    Long-term air pollution exposure and living close to busy roads are associated with COPD in women

    BACKGROUND: Lung function and exacerbations of chronic obstructive pulmonary disease (COPD) have been associated with short-term exposure to air pollution. However, the effect of long-term exposure to particulate matter from industry and traffic on COPD as defined by lung function has not been evaluated so far. Our study was designed to investigate the influence of long-term exposure to air pollution on respiratory symptoms and pulmonary function in 55-year-old women. We especially focused on COPD as defined by GOLD criteria and additionally compared the effects of air pollution on respiratory symptoms by questionnaire data and by lung function measurements. METHODS: In consecutive cross sectional studies conducted between 1985–1994, we investigated 4757 women living in the Rhine-Ruhr Basin of Germany. NO(2 )and PM(10 )exposure was assessed by measurements done in an 8 km grid, and traffic exposure by distance from the residential address to the nearest major road using Geographic Information System data. Lung function was determined and COPD was defined by using the GOLD criteria. Chronic respiratory symptoms and possible confounders were defined by questionnaire data. Linear and logistic regressions, including random effects were used to account for confounding and clustering on city level. RESULTS: The prevalence of COPD (GOLD stages 1–4) was 4.5%. COPD and pulmonary function were strongest affected by PM(10 )and traffic related exposure. A 7 μg/m(3 )increase in five year means of PM(10 )(interquartile range) was associated with a 5.1% (95% CI 2.5%–7.7%) decrease in FEV(1), a 3.7% (95% CI 1.8%–5.5%) decrease in FVC and an odds ratio (OR) of 1.33 (95% CI 1.03–1.72) for COPD. Women living less than 100 m from a busy road also had a significantly decreased lung function and COPD was 1.79 times more likely (95% CI 1.06–3.02) than for those living farther away. Chronic symptoms as based on questionnaire information showed effects in the same direction, but less pronounced. CONCLUSION: Chronic exposure to PM(10), NO(2 )and living near a major road might increase the risk of developing COPD and can have a detrimental effect on lung function

    Outdoor Air Pollution and Arterial Hypertension

    Air pollution is a major environmental risk factor. There is accumulating evidence that air pollution could induce elevated blood pressure and potentiate hypertension. Acute elevations in the outdoor air pollution levels can trigger immediate or shortly delayed increases in arterial blood pressure. Moreover, few studies suggest that short-term increases in the levels of particulate and gaseous pollutants could lead to an acute onset of hypertension. Prolonged exposure to outdoor air pollution is associated with elevated blood pressure. Furthermore, some longitudinal studies have linked long-term exposure to air pollution with the incidence of hypertension. Various components of air pollution, such as inhalable particulate matter (PM2.5, PM10), nitrogen oxides, sulfur dioxide, and ozone, have shown associations with blood pressure in some studies. The hypothesized underlying mechanisms include inflammatory reactions and oxidative stress in lungs and in systemic circulation, imbalance of autonomous nervous system, and pathologic changes in vascular endothelium. In addition to “traditional” susceptible groups such as elderly individuals or patients with chronic diseases, children and pregnant women could be especially susceptible to the adverse effects of air pollution. The interplay of air pollution with the related environmental exposures, such as traffic noise and climate change, should be investigated further
