    Antibodies Against M-Type Phospholipase A2 Receptor (PLA2R) in Patients with Primary Membranous Nephropathy and Lupus Nephritis Class V: A Review

    Membranous nephropathy (MN) is a very common disease of male adults with nephrotic syndrome. The disease can be primary, when the cause is not known, or secondary associated with infections, drugs, neoplasias and autoimmune systemic diseases, such as systemic lupus erythematosus (SLE). The primary form accounts for 70–80% of the cases. SLE is a common cause of secondary MN affecting young women. The differential diagnosis from primary and lupus MN by clinical and morphological findings can be difficult. The search for autoantibodies against podocyte antigen M-type phospholipase A2 receptor (PLA2R) has demonstrated high positivity in the serum and renal biopsies in the primary MN and negativity in lupus MN (WHO class V). There is a large literature on the role of anti-PLA2R antibody in the diagnosis and follow-up of patients with membranous nephropathy. The aim of this review is to summarize the literature data on the etiopathogenesis of MN and the value of anti-PLA2R antibody screening for the diagnosis and management of patients

    Glycoprotein Hormone Receptor Knockdown Leads to Reduced Reproductive Success in Male Aedes aegypti

    Glycoprotein hormone receptors mediate a diverse range of physiological functions in vertebrate and invertebrate organisms. The heterodimeric glycoprotein hormone GPA2/GPB5 and its receptor LGR1, constitute a recently discovered invertebrate neuroendocrine signaling system that remains to be functionally characterized. We previously reported that LGR1 is expressed in the testes of adult Aedes aegypti mosquitoes, where its immunoreactivity is particularly regionalized. Here, we show that LGR1 immunoreactivity is associated with the centriole adjunct of spermatids and is observed transiently during spermatogenesis in mosquitoes, where it may act to mediate the regulation of flagellar development. RNA interference to downregulate LGR1 expression was accomplished by feeding mosquito larvae with bacteria that produced LGR1-specific dsRNA, which led to defects in spermatozoa, characterized with shortened flagella. LGR1 knockdown mosquitoes also retained ∼60% less spermatozoa in reproductive organs and demonstrated reduced fertility compared to controls. To date, the endocrine regulation of spermatogenesis in mosquitoes remains an understudied research area. The distribution of LGR1 and detrimental effects of its knockdown on spermatogenesis in A. aegypti indicates that this heterodimeric glycoprotein hormone signaling system contributes significantly to the regulation of male reproductive biology in this important disease-vector

    Síndrome de transfusão feto-fetal: neurodesenvolvimento de lactentes tratados com cirurgia a laser

    To assess the neurodevelopmental functions of survivors of twin-twin transfusion syndrome (TTTS) treated by fetoscopic laser coagulation (FLC), during the first year of life, comparing them to a control group; and to verify the influence of specific variables on neurodevelopment. Method This was a prospective, longitudinal study. The sample comprised 33 monochorionic diamniotic twins who underwent FLC for treatment of TTTS and 22 full-term infants of single-fetus pregnancies. Bayley Scales of Infant and Toddler Development Screening Test were used for evaluation. Prenatal, perinatal and postnatal information were obtained. Results There was an increased frequency of infants in the TTTS group with inadequate performance compared to the control group. The identified variables (fetal donor, low economic income and cardiorespiratory disease) negatively impacted expressive communication and fine motor skills. Resultados Houve maior número de lactentes com desempenho inadequado no grupo STFF do que no controle. As variáveis identificadas (feto doador, baixa renda econômica e doença cárdio-respiratória) influenciaram negativamente a comunicação expressiva e as habilidades motoras finas. Conclusão Embora o acompanhamento seja recomendado para todos lactentes com STFF, especial atenção deve ser dada àqueles que apresentam fatores de risc

    A localized outbreak of Chikungunya virus in Salvador, Bahia, Brazil

    A localized Chikungunya virus (CHIKV; East/Central/South African genotype) outbreak (50 cases, 70% laboratory-confirmed; attack rate: 5.3 confirmed cases/100 people) occurred in a Salvador, Brazil neighborhood, between Apr-Jun/2017. Highly clustered cases in space and time, mostly along a single street, highlight an increased risk of CHIKV transmission among pockets of susceptible populations.     Pt-Decorated TiO2 Materials Supported on Carbon : Increasing Activities and Stabilities toward the ORR by Tuning the Pt Loading

    Pt nanoparticles (Pt NPs) supported on carbon have been widely employed as electrocatalysts toward oxygen reduction reaction. The development of more efficient electrocatalysts that enable one to reduce or even not require the use of Pt is a central challenge. In addition to the control over the physical and chemical features of Pt NPs, metal support interactions can be employed to enhance activities via the generation and exposure of surface-active sites. In this context, we report herein the development of electrocatalysts composed of Pt NPs supported on TiO2 microspheres, that were subsequently impregnated onto carbon. We have found that, by optimizing the loading of Pt at the TiO2 surface, the electrocatalytic activity toward the ORR could be improved compared to that of the commercial Pt/C (E-TEK) material, even at lower Pt loadings. The enhancement in activities could be assigned to the balance between Pt loading and generation of reactive surface sites, such as adsorbed oxygenated species. Moreover, the utilization of TiO2 as support enabled improved stabilities relative to Pt/C (E-TEK). We believe that the results described herein may inspire the development of electrocatalysts for the ORR with improved activities and stabilities.Peer reviewe

    Meanings attributed by family and patients to family presence in emergency rooms

    Objective: to understand the process that leads adult and family patients to support family presence in emergency care. Method: a qualitative study that adopted Symbolic Interactionism as a theoretical reference and the Grounded Theory as a methodological framework. The theoretical sample consisted of 15 relatives and 15 patients assisted at two emergency units in the South of Brazil. Data were analyzed using open, axial and selective coding. Results: the central category ¿Convergence of ideas: family members and patients supporting family presence in emergency care? is supported by the categories: Affectionate relationship among family members?; Tacit obligation to care for the sick relative?; Benefits for the family?; Benefits for the patient?; and Benefits for the health team?. Conclusion: family presence in emergency care provides maintenance and strengthening of affectionate bonds among relatives and the experience of more sensitive and qualified care.Objetivo: comprender el proceso que lleva a pacientes adultos y familiares a apoyar la presencia de la familia en la atención de emergencia. Método: estudio cualitativo que adoptó el Interaccionismo Simbólico como referencial teórico y la Teoría Fundamentada en los Datos como referencial metodológico. El muestreo teórico fue compuesta por 15 familiares y 15 pacientes atendidos en dos unidades de emergencia en el sur de Brasil. Los datos fueron analizados por medio de codificación abierta, axial y selectiva. Resultados: la categoría central “Convergencia de ideas: familiares y pacientes apoyando la presencia de la familia en la atención de emergencia” es sostenida por las categorías: “Afetuosa relación entre los miembros de la familia”; “Obligación tácita de cuidar del familiar enfermo “; “Beneficios para la familia”; “Beneficios para el paciente”; y “Beneficios para el equipo de salud”. Conclusión: la presencia familiar en la atención de emergencia proporciona el mantenimiento y el fortalecimiento de los lazos afectivos entre los familiares y la vivencia de cuidados más sensibles y calificados.Objetivo: compreender o processo que leva pacientes adultos e familiares a apoiarem a presença da família no atendimento emergencial.     Assessing the intestinal permeability and anti-inflammatory potential of sesquiterpene lactones from chicory

Chicory is the main dietary source of SLs, which have underexplored bioactive potential. In this study, we assessed the capacity of SLs to permeate the intestinal barrier to become physiologically available, using in silico predictions and in vitro studies with the well-established cell model of the human intestinal mucosa (differentiated Caco-2 cells). The potential of SLs to modulate inflammatory responses through modulation of the nuclear factor of activated T-cells (NFAT) pathway was also evaluated, using a yeast reporter system. Lactucopicrin was revealed as the most permeable chicory SL in the intestinal barrier model, but it had low anti-inflammatory potential. The SL with the highest anti-inflammatory potential was 11β,13-dihydrolactucin, which inhibited up to 54% of Calcineurin-responsive zinc finger (Crz1) activation, concomitantly with the impairment of the nuclear accumulation of Crz1, the yeast orthologue of human NFAT.publishersversionpublishe

    Intercerter comparison of different maxillary expansion modalities in patients with complete cleft lip and palate

    Introduction: The maxillary dental arch of patients with unilateral complete cleft lip and palate is generally constricted showing a transversal deficiency that very frequently requires maxillary expansion. Some centers prefer the rapid orthopedic expansion, using the Hyrax appliance while others prefer the slow maxillary expansion and use the quadhelix appliance. The literature is not clear about each expander promotes the best expansion. Purpose: to compare the occlusal effects of rapid and slow maxillary expansion, in patients with unilateral complete cleft lip and palate, using digital models. Methodology: 34 patients in the mixed dentition composed the sample. The rapid maxillary expansion using hyrax appliance was performed in 17 patients at the Hospital for Rehabilitation of Craniofacial Anomalies, University of São Paulo (HRCA-USP) and the slow maxillary expansion was performed in 17 patients, using quad-helix appliance, at the Los Angeles Childrens Hospital, University of South California (LOCH-USC). Conventional dental casts was performed before expansion (T1) and 6 months after the expansion (T2), when the appliance was removed. After digitalization of the dental casts (3Shape R700 3D), the maxillary dental arch transverse dimensions, buccolingual inclination of posterior teeth, dental arch perimeter and palate volume was measured. Dependent tests was used to evaluate the changes between phases (p<0.05) and Independent test was used to evaluate the changes between the groups (p<0.05). Conclusions: Both orthopedic expander and the quad-helix produced significant changes in the maxillary arch widths and perimeter. The palate volume increased in the rapid maxillary group and posterior maxillary tooth tipping was bigger in the quad-helix group.FAPES

    Disruption of the inositol phosphorylceramide synthase gene affects Trypanosoma cruzi differentiation and infection capacity

    Sphingolipids (SLs) are essential components of all eukaryotic cellular membranes. In fungi, plants and many protozoa, the primary SL is inositol-phosphorylceramide (IPC). Trypanosoma cruzi is a protozoan parasite that causes Chagas disease (CD), a chronic illness for which no vaccines or effective treatments are available. IPC synthase (IPCS) has been considered an ideal target enzyme for drug development because phosphoinositol-containing SL is absent in mammalian cells and the enzyme activity has been described in all parasite forms of T. cruzi. Furthermore, IPCS is an integral membrane protein conserved amongst other kinetoplastids, including Leishmania major, for which specific inhibitors have been identified. Using a CRISPR-Cas9 protocol, we generated T. cruzi knockout (KO) mutants in which both alleles of the IPCS gene were disrupted. We demonstrated that the lack of IPCS activity does not affect epimastigote proliferation or its susceptibility to compounds that have been identified as inhibitors of the L. major IPCS. However, disruption of the T. cruzi IPCS gene negatively affected epimastigote differentiation into metacyclic trypomastigotes as well as proliferation of intracellular amastigotes and differentiation of amastigotes into tissue culture-derived trypomastigotes. In accordance with previous studies suggesting that IPC is a membrane component essential for parasite survival in the mammalian host, we showed that T. cruzi IPCS null mutants are unable to establish an infection in vivo, even in immune deficient mice
