    Repercusiones del temor al contagio por covid-19 en la salud mental de mujeres trabajadoras embarazadas en ecuador

    El temor al contagio por COVID-19 en mujeres embarazadas y sus posibles consecuencias en las emociones, conductas y temores propios del embarazo ha sido un tema poco abordado desde la perspectiva laboral. La presente investigación tuvo como finalidad describir la relación y establecer la inferencia que ha tenido el temor al contagio sobre la salud mental de las mujeres trabajadoras embarazadas en Ecuador. El estudio fue transversal con alcance correlacional. Se desarrolló un cuestionario de 25 preguntas que fue aplicado durante los controles prenatales a 48 mujeres. Los resultados muestran que existe relación entre el temor al contagio y la salud mental, la cual fue medida a través de la presencia de desórdenes emocionales, de la cantidad de sueño, en la percepción de mayor seguridad del parto en casa, su preferencia como lugar de parto y en el incremento de los temores propios del embarazo, especialmente aquellos relacionados con enfermedades o malformaciones en los neonatos y a no poder alimentarlos. Se ha confirmado lo encontrado en otros países como consecuencias del temor al contagio en las mujeres gestantes. Los resultados sugieren la necesidad de incluir a psicólogos en los servicios médicos ocupacionales que aborden estas manifestaciones con un enfoque salutogénico y preventivo

    Estudio comparativo del sindrome de burnout en una muestra multiocupacional ecuatoriana

    Desde que se iniciaron los estudios sobre el síndrome de burnout hasta la actualidad se ha visto que cualquier profesión puede padecerlo ya sea en sus tres manifestaciones o iniciar con alguna de ellas, su inclusión como enfermedad relacionada con el trabajo pone de manifiesto la necesidad de su abordaje de forma integral, toda vez que tiene nexos con múltiples patologías mentales y biológicas subsecuentes.  El objetivo de esta investigación fue comparar su forma de presentación en diferentes ocupaciones para diseñar un conjunto de intervenciones personalizadas que se aplicaron durante un año y medir el impacto inicial de estas, por lo que tuvo un diseño longitudinal de dos años con alcance descriptivo. Se utilizó el instrumento Maslach Burnout Inventory (MBI) y se encontró una prevalencia de entre 3,99 y 48,12. En las comparaciones realizadas se evidenció significancia estadística entre las diferentes manifestaciones y las cohortes estudiadas. Las acciones de promoción de la salud alcanzaron a un 82,48% de personas, de quienes acudieron a tratamiento psicológico, el 52,91% recibió el alta y el 93,38% de los diagnósticos médicos que se otorgaron a los que necesitaron este tipo de atención, confirman lo encontrado en la literatura. Sobre el impacto del programa aplicado, se observó disminución significativa del síndrome, sobre todo en las manifestaciones realización profesional y despersonalización. Se requiere complementar este estudio con otras variables de tipo ambiental y sociodemográfico, pero se sugiere la permanencia de las intervenciones sobre factores relacionados con el burnout y el trabajo personalizado en sus manifestaciones

    Ecological factors driving avian influenza virus dynamics in Spanish wetland ecosystems

Most work has focused on viral survival and transmission under laboratory conditions and through mathematical modelling. However, more integrated studies performed under field conditions are required to validate these results. In this study, we combined information on bird community, environmental factors and viral epidemiology to assess the contribution of biotic and abiotic factors in the occurrence of low pathogenic AIV in Spanish wetlands. For that purpose, seven locations in five different wetlands were studied during two years (2007-2009), including seven sampling visits by location. In each survey, fresh faeces (n = 4578) of wild birds and water samples were collected for viral detection. Also, the vegetation structure, water physical properties of wetlands, climatic conditions and wild bird community composition were determined. An overall AIV prevalence of 1.7%±0.4 was detected in faecal samples with important fluctuations among seasons and locations. Twenty-six AIV were isolated from the 78 RRT-PCR positive samples and eight different haemagglutinines and five neuraminidases were identified, being the combination H3N8 the most frequent. Variation partitioning procedures identified the combination of space and time variables as the most important pure factor - independently to other factors - explaining the variation in AIV prevalence (36.8%), followed by meteorological factor (21.5%) and wild bird community composition/vegetation structure (21.1%).     Midlife Vascular Factors and Prevalence of Mild Cognitive Impairment in Late-Life in Mexico

    Objective: To estimate the prevalence of mild cognitive impairment (MCI) and its subtypes and investigate the impact of midlife cardiovascular risk factors on late-life MCI among the aging Mexican population. Method: Analyses included a sample of non-demented adults over the age of 55 living in both urban and rural areas of Mexico (N = 1807). MCI diagnosis was assigned based on a comprehensive cognitive assessment assessing the domains of memory, executive functioning, language, and visuospatial ability. The normative sample was selected by means of the robust norms approach. Cognitive impairment was defined by a 1.5-SD cut-off per cognitive domain using normative corrections for age, years of education, and sex. Risk factors included age, education, sex, rurality, depression, insurance status, workforce status, hypertension, diabetes, stroke, and heart disease. Results: The prevalence of amnestic MCI was 5.9%. Other MCI subtypes ranged from 4.2% to 7.7%. MCI with and without memory impairment was associated with older age (OR = 1.01 [1.01, 1.05]; OR = 1.03 [1.01, 1.04], respectively) and residing in rural areas (OR = 1.49 [1.08, 2.06]; OR = 1.35 [1.03, 1.77], respectively). Depression (OR = 1.07 [1.02, 1.12]), diabetes (OR = 1.37 [1.03, 1.82]), and years of education (OR = 0.94 [0.91, 0.97]) were associated with MCI without memory impairment. Midlife CVD increased the odds of MCI in late-life (OR = 1.76 [1.19, 2.59], which was driven by both midlife hypertension and diabetes (OR = 1.70 [1.18, 2.44]; OR = 1.88 [1.19, 2.97], respectively). Conclusions: Older age, depression, low education, rurality, and midlife hypertension and diabetes were associated with higher risk of late-life MCI among older adults in Mexico. Our findings suggest that the causes of cognitive impairment are multifactorial and vary by MCI subtype

    RNA-Seq Data-Mining Allows the Discovery of Two Long Non-Coding RNA Biomarkers of Viral Infection in Humans

    There is a growing interest in unraveling gene expression mechanisms leading to viral host invasion and infection progression. Current findings reveal that long non-coding RNAs (lncRNAs) are implicated in the regulation of the immune system by influencing gene expression through a wide range of mechanisms. By mining whole-transcriptome shotgun sequencing (RNA-seq) data using machine learning approaches, we detected two lncRNAs (ENSG00000254680 and ENSG00000273149) that are downregulated in a wide range of viral infections and different cell types, including blood monocluclear cells, umbilical vein endothelial cells, and dermal fibroblasts. The efficiency of these two lncRNAs was positively validated in different viral phenotypic scenarios. These two lncRNAs showed a strong downregulation in virus-infected patients when compared to healthy control transcriptomes, indicating that these biomarkers are promising targets for infection diagnosis.     The Treatment With the SGLT2 Inhibitor Empagliflozin Modifies the Hepatic Metabolome of Male Zucker Diabetic Fatty Rats Towards a Protective Profile

    [Abstract] The EMPA-REG OUTCOME (Empagliflozin, Cardiovascular Outcome Event Trial in patients with Type 2 Diabetes Mellitus (T2DM)) trial evidenced the potential of sodium-glucose cotransporter 2 (SGLT2) inhibitors for the treatment of patients with diabetes and cardiovascular disease. Recent evidences have shown the benefits of the SGLT2 inhibitor empagliflozin on improving liver steatosis and fibrosis in patients with T2DM. Metabolomic studies have been shown to be very useful to improve the understanding of liver pathophysiology during the development and progression of metabolic hepatic diseases, and because the effects of empagliflozin and of other SGLT2 inhibitors on the complete metabolic profile of the liver has never been analysed before, we decided to study the impact on the liver of male Zucker diabetic fatty (ZDF) rats of a treatment for 6 weeks with empagliflozin using an untargeted metabolomics approach, with the purpose to help to clarify the benefits of the use of empagliflozin at hepatic level. We found that empagliflozin is able to change the hepatic lipidome towards a protective profile, through an increase of monounsaturated and polyunsaturated glycerides, phosphatidylcholines, phosphatidylethanolamines, lysophosphatidylinositols and lysophosphatidylcholines. Empagliflozin also induces a decrease in the levels of the markers of inflammation IL-6, chemerin and chemerin receptor in the liver.     Candidate Sequence Variants and Fetal Hemoglobin in Children with Sickle Cell Disease Treated with Hydroxyurea

    Fetal hemoglobin level is a heritable complex trait that strongly correlates with the clinical severity of sickle cell disease. Only few genetic loci have been identified as robustly associated with fetal hemoglobin in patients with sickle cell disease, primarily adults. The sole approved pharmacologic therapy for this disease is hydroxyurea, with effects largely attributable to induction of fetal hemoglobin. In a multi-site observational analysis of children with sickle cell disease, candidate single nucleotide polymorphisms associated with baseline fetal hemoglobin levels in adult sickle cell disease were examined in children at baseline and induced by hydroxyurea therapy. For baseline levels, single marker analysis demonstrated significant association with BCL11A and the beta and epsilon globin loci (HBB and HBE, respectively), with an additive attributable variance from these loci of 23%. Among a subset of children on hydroxyurea, baseline fetal hemoglobin levels explained 33% of the variance in induced levels. The variant in HBE accounted for an additional 13% of the variance in induced levels, while variants in the HBB and BCL11A loci did not contribute beyond baseline levels. These findings clarify the overlap between baseline and hydroxyurea-induced fetal hemoglobin levels in pediatric disease. Studies assessing influences of specific sequence variants in these and other genetic loci in larger populations and in unusual hydroxyurea responders are needed to further understand the maintenance and therapeutic induction of fetal hemoglobin in pediatric sickle cell disease

    Health and function of participants in the Long Life Family Study: A comparison with other cohorts

    Individuals from families recruited for the Long Life Family Study (LLFS) (n= 4559) were examined and compared to individuals from other cohorts to determine whether the recruitment targeting longevity resulted in a cohort of individuals with better health and function. Other cohorts with similar data included the Cardiovascular Health Study, the Framingham Heart Study, and the New England Centenarian Study. Diabetes, chronic pulmonary disease and peripheral artery disease tended to be less common in LLFS probands and offspring compared to similar aged persons in the other cohorts. Pulse pressure and triglycerides were lower, high density lipids were higher, and a perceptual speed task and gait speed were better in LLFS. Age-specific comparisons showed differences that would be consistent with a higher peak, later onset of decline or slower rate of change across age in LLFS participants. These findings suggest several priority phenotypes for inclusion in future genetic analysis to identify loci contributing to exceptional survival
