    Guide and Federate the Telemedicine Actors for the Development of Sustainable Projects Consistent With French Recommendations

    Telemedicine is facing many public health challenges which makes benchmarking essential. In this perspective, the association Agir Pour La Telemedicine wanted to develop a collaborative web platform for the provision of resources for telemedicine projects. The platform makes it possible through a dynamic inventory to find an existing project, access its identity card and compare it to other projects. This tool is intended for project holders, administrations of the healthcare system and companies who are working in the telehealth industry. By facilitating feedback from telemedicine stakeholders, this interface will enable them to federate and support them efficiently through the provision of project management tools. By creating a single database this platform could allow a harmonisation of telemedicine projects in France

    A highly conductive nanostructured PEDOT polymer confined into the mesoporous MIL-100(Fe)

    [EN] Despite the higher efficiency, larger color range and faster stimulus response of polymeric electrochromic materials, their poor cyclability strongly hampers their application in optoelectronics. As an original strategy to stabilize and further nanostructure these polymers, herein an efficient encapsulation and in situ polymerization inside highly porous metal-organic frameworks (MOFs) is reported. In particular, the successful accommodation of poly(3,4-ethylendioxythiophene) (PEDOT) and its partially oxidized polarons inside the mesopores of the nontoxic iron trimesate MIL-100(Fe) is convincingly proved by a large panel of experimental techniques. Remarkably, the polymer-MOF interaction occurring for entrapped PEDOT within the pores (deeply assessed by experimental and simulation methods) might be responsible for the enhanced electrical conductivity of the resulting PEDOT@MIL-100(Fe) composite when compared to the insulating MIL-100(Fe) and the conductive free PEDOT. Furthermore, it was possible to observe the electrochromic properties of the PEDOT@MIL-100(Fe) composite, achieving an improved stability and good cyclability as a consequence of the effective protection by the MOF matrix.This work was supported by a 2017 Leonardo Grant for Researchers and Cultural Creators, BBVA Foundation (IN[17]_CBB_QUI_0197).     µ-Calpain Conversion of Antiapoptotic Bfl-1 (BCL2A1) into a Prodeath Factor Reveals Two Distinct alpha-Helices Inducing Mitochondria-Mediated Apoptosis

    Anti-apoptotic Bfl-1 and pro-apoptotic Bax, two members of the Bcl-2 family sharing a similar structural fold, are classically viewed as antagonist regulators of apoptosis. However, both proteins were reported to be death inducers following cleavage by the cysteine protease µ-calpain. Here we demonstrate that calpain-mediated cleavage of full-length Bfl-1 induces the release of C-terminal membrane active α-helices that are responsible for its conversion into a pro-apoptotic factor. A careful comparison of the different membrane-active regions present in the Bfl-1 truncated fragments with homologous domains of Bax show that helix α5, but not α6, of Bfl-1 induces cell death and cytochrome c release from purified mitochondria through a Bax/Bak-dependent mechanism. In contrast, both helices α5 and α6 of Bax permeabilize mitochondria regardless of the presence of Bax or Bak. Moreover, we provide evidence that the α9 helix of Bfl-1 promotes cytochrome c release and apoptosis through a unique membrane-destabilizing action whereas Bax-α9 does not display such activities. Hence, despite a common 3D-structure, C-terminal toxic domains present on Bfl-1 and Bax function in a dissimilar manner to permeabilize mitochondria and induce apoptosis. These findings provide insights for designing therapeutic approaches that could exploit the cleavage of endogenous Bcl-2 family proteins or the use of Bfl-1/Bax-derived peptides to promote tumor cell clearance

    Management of anaphylaxis due to COVID-19 vaccines in the elderly

    Older adults, especially men and/or those with diabetes, hypertension, and/or obesity, are prone to severe COVID-19. In some countries, older adults, particularly those residing in nursing homes, have been prioritized to receive COVID-19 vaccines due to high risk of death. In very rare instances, the COVID-19 vaccines can induce anaphylaxis, and the management of anaphylaxis in older people should be considered carefully. An ARIA-EAACI-EuGMS (Allergic Rhinitis and its Impact on Asthma, European Academy of Allergy and Clinical Immunology, and European Geriatric Medicine Society) Working Group has proposed some recommendations for older adults receiving the COVID-19 vaccines. Anaphylaxis to COVID-19 vaccines is extremely rare (from 1 per 100,000 to 5 per million injections). Symptoms are similar in younger and older adults but they tend to be more severe in the older patients. Adrenaline is the mainstay treatment and should be readily available. A flowchart is proposed to manage anaphylaxis in the older patients.Peer reviewe

    Traitements par inhibiteurs de la pompe à protons en médecine gériatrique et leurs effets secondaires

    Les inhibiteurs de la pompe à protons sont une classe thérapeutique efficace bien tolérée, considérée comme sûre et donc largement prescrite en gériatrie. La survenue d'effets indésirables rares mais potentiellement graves est actuellement débattue. A l'échelle d'une population fortement exposée, ces effets secondaires peuvent devenir significatifs en termes de santé publique. Les patients inclus dans l'étude étaient hospitalisés dans un service de médecine interne et gériatrie aigue et nécessitaient tous la réalisation d'une gastroscopie. Chaque prescription d'IPP antérieure à l'hospitalisation a été répertoriée ainsi que les modifications thérapeutiques apportées au décours de l'exploration endoscopique. Pour chacun, le motif d'introduction a été comparé aux recommandations éditées par l'AFSSAPS. L'étude des effets secondaires a été réalisée chez les patients sous IPP au long cours. 269 patients (2/3 de femmes, âge moyen 85 ans) ont été inclus dans l'étude. Seulement 39 patients, soit 14,5 % avaient un IPP au long cours, parmi lesquels 75 % étaient inappropriés. A la sortie, 139 patients, soit 55 % étaient sous IPP dont 19 % seulement de prescriptions non conformes. Nous avons constaté une augmentation significative des prescriptions d'IPP lorsqu'un ulcère était diagnostiqué et une diminution significative des prescriptions lorsque la gastroscopie était normale. Nous n'avons pas établi d'association significative entre la prescription d'IPP et la présence d'une infection à Helicobacter pylori. Il n'a pas non plus été mis en évidence d'association significative entre les prescriptions d'IPP hors AMM et la prescription d'aspirine. Concernant les effets secondaires, nous avons relevé une diminution significative du VGM lors de la prise d'IPP au long cours et une tendance non significative à la diminution du fer sérique.
