    The Impact of New EUV Diagnostics on CME-Related Kinematics

    We present the application of novel diagnostics to the spectroscopic observation of a Coronal Mass Ejection (CME) on disk by the Extreme Ultraviolet Imaging Spectrometer (EIS) on the Hinode spacecraft. We apply a recently developed line profile asymmetry analysis to the spectroscopic observation of NOAA AR 10930 on 14-15 December 2006 to three raster observations before and during the eruption of a 1000km/s CME. We see the impact that the observer's line-of-sight and magnetic field geometry have on the diagnostics used. Further, and more importantly, we identify the on-disk signature of a high-speed outflow behind the CME in the dimming region arising as a result of the eruption. Supported by recent coronal observations of the STEREO spacecraft, we speculate about the momentum flux resulting from this outflow as a secondary momentum source to the CME. The results presented highlight the importance of spectroscopic measurements in relation to CME kinematics, and the need for full-disk synoptic spectroscopic observations of the coronal and chromospheric plasmas to capture the signature of such explosive energy release as a way of providing better constraints of CME propagation times to L1, or any other point of interest in the heliosphere.Comment: Accepted to appear in Solar Physics Topical Issue titled "Remote Sensing of the Inner Heliosphere".     The treatment of hepatitis C: history, presence and future

    The treatment of chronic hepatitis C has made remarkable progress over the past two decades. For interferon-alpha monotherapy, sustained virological response rates were between 2 and 9% in genotype 1 and between 16 and 23% in genotypes 2 and 3. By adjusting treatment duration up to 48 weeks for genotype 1 and combining regular interferon-alpha with ribavirin, sustained response rates could be improved to 28 to 31% in genotype 1 and around 65% in genotypes 2 and 3. Attempts to further increase efficacy included the addition of amantadine without conclusive evidence up till now. With the recent introduction of long-acting pegylated interferon-alpha in combination with ribavirin, sustained virological response rates of 8o% can be obtained in genotypes 2 and 3. However, sustained virological response rates for patients with either genotype 1, nonresponse to prior treatment, cirrhosis or a combination of these characteristics are still less than 50%. In view of results with daily high-dose interferon-alpha induction in combination with prolongation of treatment duration up to 18 months, such patients might benefit from induction and prolonged PEG-IFN-alpha treatment and should be treated in an experimental setting

    BACKGROUND: Evidence based medicine depends on unbiased selection of completed randomized controlled trials. For completeness it is important to publish all trials. This report describes the first large randomised controlled trial where combination therapy was compared to placebo therapy and to ribavirin monotherapy, which has not been published untill now. METHODS: One hundred and twenty one patients with chronic hepatitis C and elevated transaminases who did not respond to previous treatment with standard interferon monotherapy, were included from 16 centers in Belgium, the Netherlands and Luxembourg between 1992 and 1996. Patient poor-response characteristics were: genotype 1 (69%), HCV RNA above 2 × 10(6 )copies/ml (55%) and cirrhosis (38%). Patients were randomized to 6 months combination therapy with interferon alpha-2b (3 MU tiw) and ribavirin (1000–1200 mg / day), 6 months ribavirin monotherapy (1000–1200 mg / day) or 6 months ribavirin placebo. The study was double blinded for the ribavirin / placebo component. One patient did not fit the entry criteria, and 3 did not start. All 117 patients who received at least one dose of treatment were included in the intention to treat analysis. RESULTS: At the end of treatment, HCV RNA was undetectable in 35% of patients on combination therapy and in none of the patients treated with ribavirin monotherapy or placebo. The sustained virological response rate at 6 months after therapy was 15% for patients treated with interferon and ribavirin. During the 6 months treatment period 13% of patients on interferon ribavirin combination therapy, 13% of patients on ribavirin monotherapy and 11% of patients on placebo withdrew due to side effects or noncompliance. At 24 weeks of treatment the mean Hb level was 85% of the baseline value, which means a mean decrease from 9.1 mmol/l to 7.8 mmol/l. The Hb levels at the end of treatment were not significantly different from patients treated with ribavirin monotherapy (p = 0.76). End of treatment WBC was significantly lower in patients treated with combination therapy, compared to ribavirin (p < 0.01) as well as for patients treated with ribavirin monotherapy compared to placebo (p < 0.01). DISCUSSION: This belated report on the only placebo controlled study of interferon ribavirin combination therapy in non responders to standard doses of interferon monotherapy documents the effectiveness, be it limited, of this approach as well as the dynamics of the effects on blood counts

    A Coherence-Based Approach for Tracking Waves in the Solar Corona

    We consider the problem of automatically (and robustly) isolating and extracting information about waves and oscillations observed in EUV image sequences of the solar corona with a view to near real-time application to data from the Atmospheric Imaging Array (AIA) on the Solar Dynamics Observatory (SDO). We find that a simple coherence / travel-time based approach detects and provides a wealth of information on transverse and longitudinal wave phenomena in the test sequences provided by the Transition Region and Coronal Explorer (TRACE). The results of the search are "pruned" (based on diagnostic errors) to minimize false-detections such that the remainder provides robust measurements of waves in the solar corona, with the calculated propagation speed allowing automated distinction between various wave modes. In this paper we discuss the technique, present results on the TRACE test sequences, and describe how our method can be used to automatically process the enormous flow of data (~1Tb/day) that will be provided by SDO/AIA after launch in late 2008.Comment: 34 pages, 16 figures - in press Solar Physic

    Expressed emotion in families of children with and without Autism Spectrum Disorder, Cerebral Palsy and Down Syndrome: Relations with parenting stress and parenting behaviors

    This study examined the family emotional climate as assessed by Five Minute Speech Samples and the relation with parenting stress and parenting behaviors among parents of children (6–17 years, 64.7% boys) with autism spectrum disorder, cerebral palsy, Down syndrome, and without any known disability (n = 447). The large majority of parents (79%) showed low levels of Expressed Emotion, an indicator of a positive family climate. In all groups, more Emotional Over-involvement, more Criticism and fewer expressions of Warmth were associated with higher levels of parenting stress. Across groups, Emotional Over-involvement was related to more autonomy-supportive parenting, Criticism to more psychologically controlling and overreactive parenting, and Warmth was associated with more responsive and less psychologically controlling and overreactive parenting

    A DETAILED COMPARISON BETWEEN THE OBSERVED AND SYNTHESIZED PROPERTIES OF A SIMULATED TYPE II SPICULE

    We have performed a three-dimensional radiative MHD simulation of the solar atmosphere. This simulation shows a jet-like feature that shows similarities to the type II spicules observed for the first time with Hinode's Solar Optical Telescope. Rapid blueshifted events (RBEs) on the solar disk are associated with these spicules. Observational results suggest they may contribute significantly in supplying the corona with hot plasma. We perform a detailed comparison of the properties of the simulated jet with those of type II spicules (observed with Hinode) and RBEs (with ground-based instruments). We analyze a wide variety of synthetic emission and absorption lines from the simulations including chromospheric (Ca II 8542 Å, Ca II H, and Hα) to transition region and coronal temperatures (10,000 K to several million K). We compare their synthetic intensities, line profiles, Doppler shifts, line widths, and asymmetries with observations from Hinode/SOT and EIS, SOHO/SUMER, the Swedish 1 m Solar Telescope, and SDO/AIA. Many properties of the synthetic observables resemble the observations, and we describe in detail the physical processes that lead to these observables. Detailed analysis of the synthetic observables provides insight into how observations should be analyzed to derive information about physical variables in such a dynamic event. For example, we find that line-of-sight superposition in the optically thin atmosphere requires the combination of Doppler shifts and spectral line asymmetry to determine the velocity in the jet. In our simulated type II spicule, the lifetime of the asymmetry of the transition region lines is shorter than that of the coronal lines. Other properties differ from the observations, especially in the chromospheric lines. The mass density of the part of the spicule with a chromospheric temperature is too low to produce significant opacity in chromospheric lines. The synthetic Ca II 8542 Å and Hα profiles therefore do not show signal resembling RBEs. These and other discrepancies are described in detail, and we discuss which mechanisms and physical processes may need to be included in the MHD simulations to mimic the thermodynamic processes of the chromosphere and corona, in particular to reproduce type II spicules.
