Grey-matter texture abnormalities and reduced hippocampal volume are distinguishing features of schizophrenia
Neurodevelopmental processes are widely believed to underlie schizophrenia. Analysis of
brain texture from conventional magnetic resonance imaging (MRI) can detect disturbance in
brain cytoarchitecture. We tested the hypothesis that patients with schizophrenia manifest
quantitative differences in brain texture that, alongside discrete volumetric changes, may
serve as an endophenotypic biomarker. Texture analysis (TA) of grey matter distribution and
voxel-based morphometry (VBM) of regional brain volumes were applied to MRI scans of 27
patients with schizophrenia and 24 controls. Texture parameters (uniformity and entropy)
were also used as covariates in VBM analyses to test for correspondence with regional brain
volume. Linear discriminant analysis tested if texture and volumetric data predicted
diagnostic group membership (schizophrenia or control). We found that uniformity and
entropy of grey matter differed significantly between individuals with schizophrenia and
controls at the fine spatial scale (filter width below 2 mm). Within the schizophrenia group,
these texture parameters correlated with volumes of the left hippocampus, right amygdala
and cerebellum. The best predictor of diagnostic group membership was the combination of
fine texture heterogeneity and left hippocampal size. This study highlights the presence of
distributed grey-matter abnormalities in schizophrenia, and their relation to focal structural
abnormality of the hippocampus. The conjunction of these features has potential as a
neuroimaging endophenotype of schizophrenia
Balanced translocation linked to psychiatric disorder, glutamate and cortical structure/function
Rare genetic variants of large effect can help elucidate the pathophysiology of brain disorders. Here we expand the clinical and genetic analyses of a family with a (1;11)(q42;q14.3) translocation multiply affected by major psychiatric illness and test the effect of the translocation on the structure and function of prefrontal, and temporal brain regions. The translocation showed significant linkage (LOD score 6.1) with a clinical phenotype that included schizophrenia, schizoaffective disorder, bipolar disorder, and recurrent major depressive disorder. Translocation carriers showed reduced cortical thickness in the left temporal lobe, which correlated with general psychopathology and positive psychotic symptom severity. They showed reduced gyrification in prefrontal cortex, which correlated with general psychopathology severity. Translocation carriers also showed significantly increased activation in the caudate nucleus on increasing verbal working memory load, as well as statistically significant reductions in the right dorsolateral prefrontal cortex glutamate concentrations. These findings confirm that the t(1;11) translocation is associated with a significantly increased risk of major psychiatric disorder and suggest a general vulnerability to psychopathology through altered cortical structure and function, and decreased glutamate levels
Reduced Gray to White Matter Tissue Intensity Contrast in Schizophrenia
BACKGROUND: While numerous structural magnetic resonance imaging (MRI) studies revealed changes of brain volume or density, cortical thickness and fibre integrity in schizophrenia, the effect of tissue alterations on the contrast properties of neural structures has so far remained mostly unexplored. METHODS: Whole brain high-resolution MRI at 3 Tesla was used to investigate tissue contrast and cortical thickness in patients with schizophrenia and healthy controls. RESULTS: Patients showed significantly decreased gray to white matter contrast in large portions throughout the cortical mantle with preponderance in inferior, middle, superior and medial temporal areas as well as in lateral and medial frontal regions. The extent of these intensity contrast changes exceeded the extent of cortical thinning. Further, contrast changes remained significant after controlling for cortical thickness measurements. CONCLUSIONS: Our findings clearly emphasize the presence of schizophrenia related brain tissue changes that alter the imaging properties of brain structures. Intensity contrast measurements might not only serve as a highly sensitive metric but also as a potential indicator of a distinct pathological process that might be independent from volume or thickness alterations

Psychometric Characteristics of the Drug Use Disorders Identification Test (DUDIT) and the Drug Use Disorders Identification Test-Extended (DUDIT-E) Among Young Drug Users in Hungary.
BACKGROUND: The Drug Use Disorders Identification Test (DUDIT) was developed for problematic substance use screening, and for a more detailed assessment of problematic use, the Drug Use Disorders Identification Test-Extended (DUDIT-E) was additionally developed. PURPOSE: Examining the psychometric properties of DUDIT and DUIT-E across diverse settings in populations of young drug users. METHODS: We examined the psychometric characteristics of these instruments across various settings in populations of young substance users differing in substance use severity and treatment status. Data were collected from three clinically relevant groups (n = 259) as well as a control sample of college students (n = 109). RESULTS: Reliability analyses indicated good internal consistency for both instruments; high intraclass correlations further indicated good test-retest reliability. Differences among study groups were significant on the DUDIT scale and all DUDIT-E subscales (p < 0.01), with the target groups exhibiting higher scores compared to controls. A two-factor solution was identified for the factor structure of DUDIT. CONCLUSION: The Hungarian version of DUDIT and DUDIT-E can effectively identify substance use problems among young users
