    POSITIVE RELATIONS OF PHYSICAL FITNESS AND EXERCISE INTERVENTION PROGRAMS WITH MOTOR COMPETENCE AND HEALTH-RELATED QUALITY OF LIFE IN DEVELOPMENTAL COORDINATION DISORDER: A SYSTEMATIC REVIEW

    Developmental Coordination Disorder (DCD) is an impairment in the development of motor coordination creating varied problems and difficulties in children’s and adolescent’s daily life activities. As a result, the avoidance of participating in physical activity leads in low levels of fitness and also in secondary social and emotional problems. Fitness levels in children and adolescents with DCD have been recognized as an important factor which influences their performance in daily activities and has a positive impact on their health-related quality of life (HRQOL). Surveys showed that every domain in HRQOL (motor, cognitive, emotional, social) is significantly lower in children with DCD compared to their peers. These data showed that intervention is very important for improving motor skill performance and HRQOL, too, in children and adolescents with DCD. The present study aimed to systematically review the literature published in peer reviewed journals and to summarize information about possible relationships between intervention approaches focused on physical fitness and exercise, participation in physical activity and HRQOL, in children and adolescents with DCD. Studies which examined the effect of fitness and exercise intervention programs on motor competence and HRQOL in children and adolescents with DCD were also, included. The review of the literature has shown that several intervention programs developed for DCD population, focusing to improve motor ability, derived from occupational therapy, physiotherapy, medicine, dietetics and education scientific areas. Generally, the present review focused on two basic approaches in clinical practice regarding intervention programs for DCD individuals: task-oriented and process-oriented approach. The task-oriented approach aims to improve the performance of a specific skill and on the other hand the process-oriented approach aims to identify the underlying processes or dysfunctions which the individual has not developed adequately according to his/her age, although they are considered to be necessary for successful performance and acquisition of motor skills. Children and adolescents with DCD seemed to report poorer ΗRQOL than their typically developing peers. However, HRQOL in children and adolescents with DCD needs further investigation. In addition, there is a need of research in interventions focused on fitness and exercise programs with an ultimate goal to improve motor ability and HRQOL too; through participation in such interventions children and adolescents with DCD, may be possible to break the negative cycle of physical activity avoidance, reversing it to a positive one. It is concluded that, there are possible positive relationships between fitness and exercise intervention programs, motor competence and HRQOL in children and adolescents with DCD. Future research should focus on examining whether and how such interventions may eliminate functional constraints leading to an engagement in the positive cycle of physical activity, with a further improvement in HRQOL in DCD population.  Article visualizations

    Averaged null energy condition in a classical curved background

    The Averaged Null Energy Condition (ANEC) states that the integral along a complete null geodesic of the projection of the stress-energy tensor onto the tangent vector to the geodesic cannot be negative. Exotic spacetimes, such as those allow wormholes or the construction of time machines are possible in general relativity only if ANEC is violated along achronal geodesics. Starting from a conjecture that flat-space quantum inequalities apply with small corrections in spacetimes with small curvature, we prove that ANEC is obeyed by a minimally-coupled, free quantum scalar field on any achronal null geodesic surrounded by a tubular neighborhood whose curvature is produced by a classical source

    A scoping review of psychoeducational interventions for people after transient ischemic attack and minor stroke

    Background: Psychoeducation can provide information and support to cope with the physical and emotional effects of a health condition. This scoping review aimed to identify the evidence regarding psychoeducational interventions for people after a Transient Ischaemic Attack (TIA) and minor stroke. Methods: This review was conducted in accordance with the PRISMA Extension for Scoping Reviews. Three electronic databases (MEDLINE, Embase, PsycINFO) were searched for articles on interventions related to psychoeducational support post TIA and minor/mild stroke. Search retrieved 3722 articles. Three reviewers independently screened titles, abstracts, full-texts, and then extracted data for included studies. Study quality was assessed using the JADAD scale. TIDieR checklist was used to describe interventions. Results: Fifteen RCTs were included. Twelve studies were of high quality (JADAD score ≥2), two were low quality. A total of 1500 participants were recruited across the studies. Definition of TIA and minor stroke were unclear, leading to the exclusion of several studies. Various interventions were included, including education/psychoeducation (n=4); exercise and lifestyle advice (n=3); telephone-based education/counselling (n=3); secondary prevention education (n=1); motivational interviewing (n=2); self-management (n=2). Interventions were inconsistently described, with information missing about who delivered it and tailoring. Conclusions: Definitions of stroke severity are not adequately reported. There are variety of interventions including education about a range of stroke-specific topics. Many interventions are not adequately defined, thus making it difficult to determine if the aim was to provide information or support to promote self-management and wellbeing post TIA/minor stroke. There is a need for a more in-depth systematic review to develop a clear definition of psychoeducation

    GNE Is Involved in the Early Development of Skeletal and Cardiac Muscle

    UDP-N-acetylglucosamine 2 epimerase/N-acetylmannosamime kinase (GNE) is a bifunctional enzyme which catalyzes the two key sequential steps in the biosynthetic pathway of sialic acid, the most abundant terminal monosaccharide on glycoconjugates of eukaryotic cells. GNE knock out (GNE KO) mice are embryonically lethal at day E8.5. Although the role of GNE in the sialic pathway has been well established as well as the importance of sialylation in many diverse biological pathways, less is known about the involvement of GNE in muscle development. To address this issue we have studied the role of GNE during in vitro embryogenesis by comparing the developmental profile in culture of embryonic stem cells (ES) from wild type and from GNE KO E3.5 mice embryos, during 45 days. Neuronal cells appeared rarely in GNE KO ES cultures and did not reach an advanced differentiated stage. Although primary cardiac cells appeared at the same time in both normal and GNE KO ES cultures, GNE KO cardiac cells degraded very soon and their beating capacity decayed rapidly. Furthermore very rare skeletal muscle committed cells were detected in the GNE KO ES cultures at any stage of differentiation, as assessed by analysis of the expression of either Pax7, MyoD and MyHC markers. Beyond the supporting evidence that GNE plays an important role in neuronal cell and brain development, these results show that GNE is strongly involved in cardiac tissue and skeletal muscle early survival and organization. These findings could open new avenues in the understanding of muscle function mechanisms in health and in disease

    Optimising Psychoeducation for Transient Ischaemic Attack and Minor Stroke Management (OPTIMISM): Protocol for a feasibility randomised controlled trial

    Background: A transient ischaemic attack (TIA) and minor stroke are medical emergencies and often a warning sign of future strokes if remain untreated. Few studies have investigated the long-term psychosocial effects of TIA and minor stroke. Secondary prevention and medical management are often the primary focus with limited access offered for further psychosocial support. Psychoeducational interventions can provide education and advice to people with physical health conditions and, with suitable tailoring, could be appropriate for people after TIA and minor stroke. This study aims to develop a group psychoeducational intervention for people after TIA and minor stroke and to test whether it is acceptable and feasible. Methods: This mixed-methodology study involves two phases: Phase 1) A qualitative study to determine the content of a suitable intervention; Phase 2) A single-centre feasibility randomised controlled trial to evaluate the acceptability of this intervention. The overall study has ethical approval. Stroke survivors have been involved in designing and monitoring the trial. The aim is to recruit 30-40 participants from a Stroke/TIA Service, within 6 months following their diagnosis. Participants will be randomly allocated to either the usual care control group or the intervention group (psychoeducational programme). The programme will consist of six group sessions based on providing education, psychological and social support. The primary outcomes will relate to the feasibility aims of the study. Outcomes will be collected at 3 and 6 months to assess mood, quality of life, knowledge and satisfaction, and resource use. Discussion: There is a need to develop and evaluate effective interventions that enhance the education provided to people after TIA and minor stroke and to promote their psychosocial wellbeing. Findings will indicate the acceptability of the intervention and parameters needed to conduct a definitive trial

    Resolving Structure and Mechanical Properties at the Nanoscale of Viruses with Frequency Modulation Atomic Force Microscopy

    Structural Biology (SB) techniques are particularly successful in solving virus structures. Taking advantage of the symmetries, a heavy averaging on the data of a large number of specimens, results in an accurate determination of the structure of the sample. However, these techniques do not provide true single molecule information of viruses in physiological conditions. To answer many fundamental questions about the quickly expanding physical virology it is important to develop techniques with the capability to reach nanometer scale resolution on both structure and physical properties of individual molecules in physiological conditions. Atomic force microscopy (AFM) fulfills these requirements providing images of individual virus particles under physiological conditions, along with the characterization of a variety of properties including local adhesion and elasticity. Using conventional AFM modes is easy to obtain molecular resolved images on flat samples, such as the purple membrane, or large viruses as the Giant Mimivirus. On the contrary, small virus particles (25–50 nm) cannot be easily imaged. In this work we present Frequency Modulation atomic force microscopy (FM-AFM) working in physiological conditions as an accurate and powerful technique to study virus particles. Our interpretation of the so called “dissipation channel” in terms of mechanical properties allows us to provide maps where the local stiffness of the virus particles are resolved with nanometer resolution. FM-AFM can be considered as a non invasive technique since, as we demonstrate in our experiments, we are able to sense forces down to 20 pN. The methodology reported here is of general interest since it can be applied to a large number of biological samples. In particular, the importance of mechanical interactions is a hot topic in different aspects of biotechnology ranging from protein folding to stem cells differentiation where conventional AFM modes are already being used

    Mediterranean diet and colorectal cancer risk : A pooled analysis of three Italian case-control studies

    Background: Adherence to the Mediterranean diet (MD) is associated with a reduced risk of several cancers. However, studies conducted in Mediterranean regions are scanty. Methods: To investigate the relation between MD and colorectal cancer risk in Italy, we pooled data from three case-control studies, including a total of 3745 colorectal cancer cases and 6804 hospital controls. Adherence to the MD was assessed using an a priori Mediterranean Diet Score (MDS), based on nine components. Results: Compared with the lowest adherence to the MD (0-2 MDS), the odds ratio (OR) was 0.52 (95% confidence interval (CI) 0.43-0.62) for the highest adherence (7-9 MDS), with a significant inverse trend in risk (P<0.0001). The OR for a 1-point increment in the MDS was 0.89 (95% CI 0.86-0.91). The inverse association was consistent across studies, cancer anatomical subsites and strata of selected covariates. Conclusions: This Italian study confirms a favourable role of MD on colorectal cancer risk

    Urticaria and infections

    Urticaria is a group of diseases that share a distinct skin reaction pattern. Triggering of urticaria by infections has been discussed for many years but the exact role and pathogenesis of mast cell activation by infectious processes is unclear. In spontaneous acute urticaria there is no doubt for a causal relationship to infections and all chronic urticaria must have started as acute. Whereas in physical or distinct urticaria subtypes the evidence for infections is sparse, remission of annoying spontaneous chronic urticaria has been reported after successful treatment of persistent infections. Current summarizing available studies that evaluated the course of the chronic urticaria after proven Helicobacter eradication demonstrate a statistically significant benefit compared to untreated patients or Helicobacter-negative controls without urticaria (p < 0.001). Since infections can be easily treated some diagnostic procedures should be included in the routine work-up, especially the search for Helicobacter pylori. This review will update the reader regarding the role of infections in different urticaria subtypes
