    A lo largo del artículo dedicado al proyecto final del Diplomado de Profundización en Linux se muestra el funcionamiento de diferentes servicios que ofrece la distribución Zentyal Server ideal para PYMES. Se comienza con la instalación de una red local compuesta de un servidor Zentyal y una o más maquinas clientes con Ubuntu Desktop y Windows, enseguida de hacer una configuración inicial, se procede a instalar y configurar los módulos necesarios para realizar conexión por DHCP, autenticación por DNS, asignar y controlar dominios, acceso mediante Proxy no transparente, restricción y apertura de sitios web mediante reglas de Firewall, controlar servicios de archivos compartidos e impresoras mediante LDAP, y por ultimo implementar un túnel privado entre maquinas clientes y el servidor mediante una VPN.Throughout the article dedicated to the final project of the Diploma of Deepening in Linux, the operation of different services offered by the ideal Zentyal Server distribution for SMEs is shown. It begins with the installation of a local network composed of a Zentyal server and one or more client machines with Ubuntu Desktop and Windows, immediately after making an initial configuration, we proceed to install and configure the necessary modules to make connection by DHCP, authentication by DNS, assign and control domains, access through non-transparent Proxy, restriction and opening of websites through Firewall rules, control of shared file and printer services through LDAP, and finally implement a private tunnel between client machines and the server through a VPN

    Análisis cinemático y diseño de un mecanismo de cuatro barras para falange proximal de dedo antropomórfico

    This paper shows the kinematic analysis and a parametric design methodology of the proximal phalanx for an anthropomorphic finger using a one degree-of-freedom. The mechanism design is done by taking ten positions from its output bar. A dynamic simulation is given using a CAD model to verify the feasibility of the mechanism results. Finally, we show the comparative analysis between kinematics numerically obtained and other experimental results.En este artículo, se presenta el análisis cinemático y una metodología de diseño paramétrico de la falange proximal para un dedo antropomórfico1 de un grado de libertad. El diseño del mecanismo se lleva a cabo, tomando diez posiciones de la barra de salida del mismo. Se presenta la simulación dinámica, utilizando un modelo de CAD para comprobar la factibilidad de los resultados obtenidos por el mecanismo.     The Use of P63 Immunohistochemistry for the Identification of Squamous Cell Carcinoma of the Lung

    6 páginas, 2 figuras, 3 tablas.Introduction While some targeted agents should not be used in squamous cell carcinomas (SCCs), other agents might preferably target SCCs. In a previous microarray study, one of the top differentially expressed genes between adenocarcinomas (ACs) and SCCs is P63. It is a well-known marker of squamous differentiation, but surprisingly, its expression is not widely used for this purpose. Our goals in this study were (1) to further confirm our microarray data, (2) to analize the value of P63 immunohistochemistry (IHC) in reducing the number of large cell carcinoma (LCC) diagnoses in surgical specimens, and (3) to investigate the potential of P63 IHC to minimize the proportion of “carcinoma NOS (not otherwise specified)” in a prospective series of small tumor samples. Methods With these goals in mind, we studied (1) a tissue-microarray comprising 33 ACs and 99 SCCs on which we performed P63 IHC, (2) a series of 20 surgically resected LCCs studied for P63 and TTF-1 IHC, and (3) a prospective cohort of 66 small thoracic samples, including 32 carcinoma NOS, that were further classified by the result of P63 and TTF-1 IHC. Results The results in the three independent cohorts were as follows: (1) P63 IHC was differentially expressed in SCCs when compared to ACs (p<0.0001); (2) half of the 20 (50%) LCCs were positive for P63 and were reclassified as SCCs; and (3) all P63 positive cases (34%) were diagnosed as SCCs.     Effects of intubation timing in patients with COVID-19 throughout the four waves of the pandemic : a matched analysis

    The primary aim of our study was to investigate the association between intubation timing and hospital mortality in critically ill patients with COVID-19-associated respiratory failure. We also analysed both the impact of such timing throughout the first four pandemic waves and the influence of prior non-invasive respiratory support on outcomes. This is a secondary analysis of a multicentre, observational and prospective cohort study that included all consecutive patients undergoing invasive mechanical ventilation due to COVID-19 from across 58 Spanish intensive care units (ICU) participating in the CIBERESUCICOVID project. The study period was between 29 February 2020 and 31 August 2021. Early intubation was defined as that occurring within the first 24 h of intensive care unit (ICU) admission. Propensity score (PS) matching was used to achieve balance across baseline variables between the early intubation cohort and those patients who were intubated after the first 24 h of ICU admission. Differences in outcomes between early and delayed intubation were also assessed. We performed sensitivity analyses to consider a different timepoint (48 h from ICU admission) for early and delayed intubation. Of the 2725 patients who received invasive mechanical ventilation, a total of 614 matched patients were included in the analysis (307 for each group). In the unmatched population, there were no differences in mortality between the early and delayed groups. After PS matching, patients with delayed intubation presented higher hospital mortality (27.3% versus 37.1%, p =0.01), ICU mortality (25.7% versus 36.1%, p=0.007) and 90-day mortality (30.9% versus 40.2%, p=0.02) when compared to the early intubation group. Very similar findings were observed when we used a 48-hour timepoint for early or delayed intubation. The use of early intubation decreased after the first wave of the pandemic (72%, 49%, 46% and 45% in the first, second, third and fourth wave, respectively; first versus second, third and fourth waves p<0.001). In both the main and sensitivity analyses, hospital mortality was lower in patients receiving high-flow nasal cannula (n=294) who were intubated earlier. The subgroup of patients undergoing NIV (n=214) before intubation showed higher mortality when delayed intubation was set as that occurring after 48 h from ICU admission, but not when after 24 h. In patients with COVID-19 requiring invasive mechanical ventilation, delayed intubation was associated with a higher risk of hospital mortality. The use of early intubation significantly decreased throughout the course of the pandemic. Benefits of such an approach occurred more notably in patients who had received high-flow nasal cannul

    Management of acute diverticulitis with pericolic free gas (ADIFAS). an international multicenter observational study

    Background: There are no specific recommendations regarding the optimal management of this group of patients. The World Society of Emergency Surgery suggested a nonoperative strategy with antibiotic therapy, but this was a weak recommendation. This study aims to identify the optimal management of patients with acute diverticulitis (AD) presenting with pericolic free air with or without pericolic fluid. Methods: A multicenter, prospective, international study of patients diagnosed with AD and pericolic-free air with or without pericolic free fluid at a computed tomography (CT) scan between May 2020 and June 2021 was included. Patients were excluded if they had intra-abdominal distant free air, an abscess, generalized peritonitis, or less than a 1-year follow-up. The primary outcome was the rate of failure of nonoperative management within the index admission. Secondary outcomes included the rate of failure of nonoperative management within the first year and risk factors for failure. Results: A total of 810 patients were recruited across 69 European and South American centers; 744 patients (92%) were treated nonoperatively, and 66 (8%) underwent immediate surgery. Baseline characteristics were similar between groups. Hinchey II-IV on diagnostic imaging was the only independent risk factor for surgical intervention during index admission (odds ratios: 12.5, 95% CI: 2.4-64, P =0.003). Among patients treated nonoperatively, at index admission, 697 (94%) patients were discharged without any complications, 35 (4.7%) required emergency surgery, and 12 (1.6%) percutaneous drainage. Free pericolic fluid on CT scan was associated with a higher risk of failure of nonoperative management (odds ratios: 4.9, 95% CI: 1.2-19.9, P =0.023), with 88% of success compared to 96% without free fluid ( P &lt;0.001). The rate of treatment failure with nonoperative management during the first year of follow-up was 16.5%. Conclusion: Patients with AD presenting with pericolic free gas can be successfully managed nonoperatively in the vast majority of cases. Patients with both free pericolic gas and free pericolic fluid on a CT scan are at a higher risk of failing nonoperative management and require closer observation
