    Impact of interstitial lung disease on the survival of systemic sclerosis with pulmonary arterial hypertension

    To assess severity markers and outcomes of patients with systemic sclerosis (SSc) with or without pulmonary arterial hypertension (PAH-SSc/non-PAH-SSc), and the impact of interstitial lung disease (ILD) on PAH-SSc. Non-PAH-SSc patients from the Spanish SSc registry and PAH-SSc patients from the Spanish PAH registry were included. A total of 364 PAH-SSc and 1589 non-PAH-SSc patients were included. PAH-SSc patients had worse NYHA-functional class (NYHA-FC), worse forced vital capacity (FVC) (81.2 ± 20.6% vs 93.6 ± 20.6%, P < 0.001), worse tricuspid annular plane systolic excursion (TAPSE) (17.4 ± 5.2 mm vs 19.9 ± 6.7 mm, P < 0.001), higher incidence of pericardial effusion (30% vs 5.2%, P < 0.001) and similar prevalence of ILD (41.8% vs. 44.9%). In individuals with PAH-SSc, ILD was associated with worse hemodynamics and pulmonary function tests (PFT). Up-front combination therapy was used in 59.8% and 61.7% of patients with and without ILD, respectively. Five-year transplant-free survival rate was 41.1% in PAH-SSc patients and 93.9% in non-PAH-SSc patients (P < 0.001). Global survival of PAH-SSc patients was not affected by ILD regardless its severity. The multivariate survival analysis in PAH-SSc patients confirmed age at diagnosis, worse NYHA-FC, increased PVR, reduced DLCO, and lower management with up-front combination therapy as major risk factors. In conclusion, in PAH-SSc cohort risk of death was greatly increased by clinical, PFT, and hemodynamic factors, whereas it was decreased by up-front combination therapy. Concomitant ILD worsened hemodynamics and PFT in PAH-SSc but not survival regardless of FVC impairment

    Standardized incidence ratios and risk factors for cancer in patients with systemic sclerosis: Data from the Spanish Scleroderma Registry (RESCLE)

    Aim: Patients with systemic sclerosis (SSc) are at increased risk of cancer, a growing cause of non-SSc-related death among these patients. We analyzed the increased cancer risk among Spanish patients with SSc using standardized incidence ratios (SIRs) and identified independent cancer risk factors in this population. Material and methods: Spanish Scleroderma Registry data were analyzed to determine the demographic characteristics of patients with SSc, and logistic regression was used to identify cancer risk factors. SIRs with 95% confidence intervals (CIs) relative to the general Spanish population were calculated. Results: Of 1930 patients with SSc, 206 had cancer, most commonly breast, lung, hematological, and colorectal cancers. Patients with SSc had increased risks of overall cancer (SIR 1.48, 95% CI 1.36-1.60; P < 0.001), and of lung (SIR 2.22, 95% CI 1.77-2.73; P < 0.001), breast (SIR 1.31, 95% CI 1.10-1.54; P = 0.003), and hematological (SIR 2.03, 95% CI 1.52-2.62; P < 0.001) cancers. Cancer was associated with older age at SSc onset (odds ratio [OR] 1.22, 95% CI 1.01-1.03; P < 0.001), the presence of primary biliary cholangitis (OR 2.35, 95% CI 1.18-4.68; P = 0.015) and forced vital capacity <70% (OR 1.8, 95% CI 1.24-2.70; P = 0.002). The presence of anticentromere antibodies lowered the risk of cancer (OR 0.66, 95% CI 0.45-0.97; P = 0.036). Conclusions: Spanish patients with SSc had an increased cancer risk compared with the general population. Some characteristics, including specific autoantibodies, may be related to this increased risk

    Population-based multicase-control study in common tumors in Spain (MCC-Spain): rationale and study design

    Introduction: We present the protocol of a large population-based case-control study of 5 common tumors in Spain (MCC-Spain) that evaluates environmental exposures and genetic factors. Methods: Between 2008-2013, 10,183 persons aged 20-85 years were enrolled in 23 hospitals and primary care centres in 12 Spanish provinces including 1,115 cases of a new diagnosis of prostate cancer, 1,750 of breast cancer, 2,171 of colorectal cancer, 492 of gastro-oesophageal cancer, 554 cases of chronic lymphocytic leukaemia (CLL) and 4,101 population-based controls matched by frequency to cases by age, sex and region of residence. Participation rates ranged from 57% (stomach cancer) to 87% (CLL cases) and from 30% to 77% in controls. Participants completed a face-to-face computerized interview on sociodemographic factors, environmental exposures, occupation, medication, lifestyle, and personal and family medical history. In addition, participants completed a self-administered food-frequency questionnaire and telephone interviews. Blood samples were collected from 76% of participants while saliva samples were collected in CLL cases and participants refusing blood extractions. Clinical information was recorded for cases and paraffin blocks and/or fresh tumor samples are available in most collaborating hospitals. Genotyping was done through an exome array enriched with genetic markers in specific pathways. Multiple analyses are planned to assess the association of environmental, personal and genetic risk factors for each tumor and to identify pleiotropic effects.     Complement component C4 structural variation and quantitative traits contribute to sex-biased vulnerability in systemic sclerosis

We investigated the association of C4 CN with systemic sclerosis (SSc) risk. Imputed total C4, C4A, C4B, and HERV-K CN were analyzed in 26,633 individuals and validated in an independent cohort. Our results showed that higher C4 CN confers protection to SSc, and deviations from CN parity of C4A and C4B augmented risk. The protection contributed per copy of C4A and C4B differed by sex. Stronger protection was afforded by C4A in men and by C4B in women. C4 CN correlated well with its gene expression and serum protein levels, and less C4 was detected for both in SSc patients. Conditioned analysis suggests that C4 genetics strongly contributes to the SSc association within the major histocompatibility complex locus and highlights classical alleles and amino acid variants of HLA-DRB1 and HLA-DPB1 as C4-independent signals

    Multiancestry analysis of the HLA locus in Alzheimer’s and Parkinson’s diseases uncovers a shared adaptive immune response mediated by HLA-DRB1*04 subtypes

    Across multiancestry groups, we analyzed Human Leukocyte Antigen (HLA) associations in over 176,000 individuals with Parkinson’s disease (PD) and Alzheimer’s disease (AD) versus controls. We demonstrate that the two diseases share the same protective association at the HLA locus. HLA-specific fine-mapping showed that hierarchical protective effects of HLA-DRB1*04 subtypes best accounted for the association, strongest with HLA-DRB1*04:04 and HLA-DRB1*04:07, and intermediary with HLA-DRB1*04:01 and HLA-DRB1*04:03. The same signal was associated with decreased neurofibrillary tangles in postmortem brains and was associated with reduced tau levels in cerebrospinal fluid and to a lower extent with increased Aβ42. Protective HLA-DRB1*04 subtypes strongly bound the aggregation-prone tau PHF6 sequence, however only when acetylated at a lysine (K311), a common posttranslational modification central to tau aggregation. An HLA-DRB1*04-mediated adaptive immune response decreases PD and AD risks, potentially by acting against tau, offering the possibility of therapeutic avenues

    Resumen de la comunicacion oral presentada al XVII National Meeting of SEQT: "Advances in Drug Discovery: Successes, Trends and Future Challenges" celebrado en Madrid (España) del 2 al 4 de octubre de 2013.-- et al.The neurohormone melatonin and the natural occurring ß-carboline pinoline are produced in the pineal gland, and both have been found in various human tissues. Melatonin has been widely investigated; especially its effects mediated via MT1 and MT2 GPCRs and antioxidant properties. Its role supporting mitochondrial function and its likely additional actions mediated via nuclear receptors are gaining weight in the recent years, opening a vast field of study beyond MT1 and MT2 receptors. Pinoline is a much potent antioxidant than melatonin found in similar concentrations to melatonin in different tissues, but its pharmacology remains much less studied. Melatonin's chronobiological role as circadiam rhythm regulator is well known while the main role of pinoline in the organism remains unknown. It has been speculated that it may play an important role in sleep pattern and the dreaming process. Besides its antioxidant properties, pinoline also inhibits monoamine oxidase (MAO) and serotonin transporter (SERT). Studies conducted with [H3]pinoline have revealed high specific binding to nuclei of cells in the cerebral cortex and adrenal gland, suggesting a plausible effect at this organelle level. In order to gain more knowledge about these intriguing molecules we have based our approach in the systematic study of their pharmacological profiles (5-HTR, SERT, MAO, MT1/MT2, ORAC, AChE). Furthermore we have extended our efforts to the synthesis and evaluation of additional structures resulting from the merging of both pineal neurochemicals. So far, we have been able to determine that pinoline is a rather potent (EC50 = 33 nM) full agonist at the 5-HT2C which is in agreement with the role this receptor subtype displays in the sleeping homeostasis. Moreover we have been able to identify compound 1, as a potent in vitro neurogenic compound (Tuj+) able to promote neural maturity (MAP+) to a higher extent than melatonin. Being totally devoid of any affinity for the neurogenic 5-HTRs and unlikely to have any for MT1/MT2 receptors we are currently conducting studies to establish whether nuclear receptors could be implied in its neurogenic effect.MINECO (SAF2012-31035), Fundación Mutua Madrileña Automovilística (AP103952012), CSIC (PIE-201280E074), and PhD Brian L.     Usando la visión de Margalef para entender la ecología microbiana actual

    Ramon Margalef was a pioneering scientist who introduced an interdisciplinary approach to ecological studies. His studies were among the first to incorporate various concepts in the literature of aquatic ecology, covering topics such as organisms, ecosystem interactions and evolution. To bring Margalef’s approach into current scientific studies, in this review we explore his vision of aquatic ecology within four interrelated fields of study: ecological theory, microbial diversity, biogeochemical cycles and global environmental changes. Taking inspiration from his studies, we analyse current scientific challenges and propose an integrated approach, considering the unifying concept of Margalef’s Mandala with the aim of improving future studies on aquatic microbial ecology.Ramon Margalef fue un científico pionero que introdujo un enfoque interdisciplinario a los estudios ecológicos. Sus estudios fueron de los primeros en incorporar diferentes conceptos en la literatura de la ecología acuática, desde los organismos y las interacciones de los ecosistemas hasta la evolución. Para llevar el enfoque de Margalef a los estudios científicos actuales, en este articulo exploramos su visión de la ecología acuática dentro de cuatro campos de estudio interrelacionados: teoría ecológica, diversidad microbiana, ciclos biogeoquímicos y cambios ambientales globales. Inspirándonos en sus estudios pasados, analizamos en este texto los desafíos científicos actuales y proponemos un enfoque integrado considerando el concepto unificador del Mandala de Margalef con el objetivo de mejorar los estudios futuros sobre la ecología microbiana acuática
