Laryngeal mask airway in prone position in ambulatory surgery: a prospective observational study
Introduction: The laryngeal mask airway (LMA) is a common airway device used for anesthesia in ambulatory
surgery, with a recently new described utilization in prone position. The aim of this study was to evaluate the safety and the effectiveness of the LMA, in prone position for anesthesia in ambulatory surgery, based on our new anesthesia department protocol.
Methods: Patients from February 2013 to July 2014 were included in this prospective study. After the patient selfpositioning in prone position, general anesthesia was induced and the LMA was placed. Second generation LMA types were used (Supreme™, iGel™ and Proseal™) and all patients were mechanically ventilated. At the end of surgery, LMA was removed in prone or supine position. Number of attempts of LMA insertion, volume leak, airway peak pressure and complications were registered until 2 hours after the procedure.
Results: The LMA placement was 85.1% effective in the first attempt and 100% in the second attempt. The need for a second attempt LMA placement was due to non-progression of the aspiration probe, a high leak and
impossible ventilation. Mechanical ventilation was considered effective and safe with maximum peak airway
pressure of 17.9 ± 5.5 cm H2O and maximum leak of 47.7 ± 31.2 ml. Complications were present in 6 patients (9%) with hypoventilation, bronchospasm and laryngospasm. Other “minor” events registered included the presence of blood in the LMA at the end of procedure and gum lesion with the rigid piece of the Supreme™ LMA.
Discussion and conclusion: Complications found in our study are similar to those described in literature in
supine position and are related to the anesthetist previous experience. Anesthetic depth adjustment improved all complications found. Effectiveness and overall safety of LMA use in prone position was observed, allowing further utilizations in selected patients according to the anesthesia department protocol.info:eu-repo/semantics/publishedVersio
Visual Grading Analysis of image quality in pediatric abdominal images acquired by Direct Digital Radiography and Computer Radiography Systems
The advent of digital technology allowed for great improvements in radiology and lead the way for digital radiology, leaving behind conventional x-ray techniques, [1]. Digital post-processing of image is the main advantage of digital image systems (e.g., computed radiology and direct digital radiology) over the conventional systems. Image quality can indeed be improved avoiding the increase of patient dose and the number of unnecessary exposures [1-3]. Image quality is directly linked to the dose of radiation applied to the patient. The literature points out the need for appropriate image evaluation in order to reduce the patient dose. Optimization and practice justification are of great relevance in diagnostic radiology. In pediatric patients a good practice justification is even more important as radiation exposure on the first 10 years of life increases the possibility of negative effects on the patient's health compared to an exposure between the ages of 30 to 40 years old [4, 5].
In the past, the main concern of radiology technicians was image quality, leaving radiation dose to a second plan. Nowadays, dose reduction and the cost/benefit ratio represent the main concerns in radiology. The radiographer is responsible for applying the ALARA (As Low As Reasonable Acceptable) concept in every study involving the use of ionizing radiation [4, 6]. According to the ALARA concept, the necessary level of diagnostic image quality should be attained with the lowest patient dose possible.
In Pediatric radiology, besides the ALARA principle, the International Commission on Radiological Protection (ICRP) adds a new concept - the SMART message, also related with optimization and radiation protection when applied to pediatric radiology (Figure 1) [6]

Effective recommendations towards healthy routines to preserve mental health during the COVID-19 pandemic
Objective: To assess the adherence to a set of evidence-based recommendations to support mental health during the coronavirus disease 2019 (COVID-19) pandemic and its association with depressive and anxiety symptoms. Methods: A team of health workers and researchers prepared the recommendations, formatted into three volumes (1: COVID-19 prevention; 2: Healthy habits; 3: Biological clock and sleep). Participants were randomized to receive only Volume 1 (control), Volumes 1 and 2, Volumes 1 and 3, or all volumes. We used a convenience sample of Portuguese-speaking participants over age 18 years. An online survey consisting of sociodemographic and behavioral questionnaires and mental health instruments (Patient Health Questionnaire-9 [PHQ-9] and Generalized Anxiety Disorder-7 [GAD-7]) was administered. At 14 and 28 days later, participants were invited to complete follow-up surveys, which also included questions regarding adherence to the recommendations. A total of 409 participants completed the study – mostly young adult women holding university degrees. Results: The set of recommendations contained in Volumes 2 and 3 was effective in protecting mental health, as suggested by significant associations of adherence with PHQ-9 and GAD-7 scores (reflecting anxiety and depression symptoms, respectively). Conclusion: The recommendations developed in this study could be useful to prevent negative mental health effects in the context of the pandemic and beyond
Acrobatic training prevents learning impairments and astrocyte remodeling in the hippocampus of rats undergoing chronic cerebral hypoperfusion: sex-specific benefits
BackgroundChronic cerebral hypoperfusion (CCH) leads to memory and learning impairments associated with degeneration and gliosis in the hippocampus. Treatment with physical exercise carries different therapeutic benefits for each sex. We investigated the effects of acrobatic training on astrocyte remodeling in the CA1 and CA3 subfields of the hippocampus and spatial memory impairment in male and female rats at different stages of the two-vessel occlusion (2VO) model.MethodsWistar rats were randomly allocated into four groups of males and females: 2VO acrobatic, 2VO sedentary, sham acrobatic, and sham sedentary. The acrobatic training was performed for 4 weeks prior to the 2VO procedure. Brain samples were collected for morphological and biochemical analysis at 3 and 7 days after 2VO. The dorsal hippocampi were removed and prepared for Western blot quantification of Akt, p-Akt, COX IV, cleaved caspase-3, PARP, and GFAP. GFAP immunofluorescence was performed on slices of the hippocampus to count astrocytes and apply the Sholl's circle technique. The Morris water maze was run after 45 days of 2VO.ResultsAcutely, the trained female rats showed increased PARP expression, and the 2VO-trained rats of both sexes presented increased GFAP levels in Western blot. Training, mainly in males, induced an increase in the number of astrocytes in the CA1 subfield. The 2VO rats presented branched astrocytes, while acrobatic training prevented branching. However, the 2VO-induced spatial memory impairment was partially prevented by the acrobatic training.ConclusionAcrobatic training restricted the astrocytic remodeling caused by 2VO in the CA1 and CA3 subfields of the hippocampus. The improvement in spatial memory was associated with more organized glial scarring in the trained rats and better cell viability observed in females
SARS-CoV-2 introductions and early dynamics of the epidemic in Portugal
Genomic surveillance of SARS-CoV-2 in Portugal was rapidly implemented by
the National Institute of Health in the early stages of the COVID-19 epidemic, in collaboration
with more than 50 laboratories distributed nationwide.
Methods By applying recent phylodynamic models that allow integration of individual-based
travel history, we reconstructed and characterized the spatio-temporal dynamics of SARSCoV-2 introductions and early dissemination in Portugal.
Results We detected at least 277 independent SARS-CoV-2 introductions, mostly from
European countries (namely the United Kingdom, Spain, France, Italy, and Switzerland),
which were consistent with the countries with the highest connectivity with Portugal.
Although most introductions were estimated to have occurred during early March 2020, it is
likely that SARS-CoV-2 was silently circulating in Portugal throughout February, before the
first cases were confirmed.
Conclusions Here we conclude that the earlier implementation of measures could have
minimized the number of introductions and subsequent virus expansion in Portugal. This
study lays the foundation for genomic epidemiology of SARS-CoV-2 in Portugal, and highlights the need for systematic and geographically-representative genomic surveillance.We gratefully acknowledge to Sara Hill and Nuno Faria (University of Oxford) and

MAMMALS IN PORTUGAL : A data set of terrestrial, volant, and marine mammal occurrences in P ortugal
Mammals are threatened worldwide, with 26% of all species being includedin the IUCN threatened categories. This overall pattern is primarily associatedwith habitat loss or degradation, and human persecution for terrestrial mam-mals, and pollution, open net fishing, climate change, and prey depletion formarine mammals. Mammals play a key role in maintaining ecosystems func-tionality and resilience, and therefore information on their distribution is cru-cial to delineate and support conservation actions. MAMMALS INPORTUGAL is a publicly available data set compiling unpublishedgeoreferenced occurrence records of 92 terrestrial, volant, and marine mam-mals in mainland Portugal and archipelagos of the Azores and Madeira thatincludes 105,026 data entries between 1873 and 2021 (72% of the data occur-ring in 2000 and 2021). The methods used to collect the data were: live obser-vations/captures (43%), sign surveys (35%), camera trapping (16%),bioacoustics surveys (4%) and radiotracking, and inquiries that represent lessthan 1% of the records.
