    Scenario-Based Model for the Location of Multiple Uncapacitated Facilities: Case Study in an Agro-Food Supply Chain

    Supply chains for agricultural products have taken on great importance in recent times due to the need that exists for consumers to acquire fresh products in excellent quality conditions. In the design of supply networks for this type of product, the location of facilities and allocation decisions are among the most relevant decisions in operations management, which in many cases involve aspects of uncertainty. This research proposes to plan the distribution of multiple agro-food products taking into account the location and capacities of producers, potential location of facilities and variations in crop yields. A scenario-based optimization model for the location of multiple uncapacitated facilities is developed. The model is tested in the cassava agro-food chain in Sucre, Colombia. First, the description for the construction of the scenarios for the uncertainty associated with the yield per hectare of cassava crops is presented. Next, the mixed integer programming model (MIP) for the location of uncapacitated facilities (UFLP) is presented. The aim of the model is to minimize operational distribution costs. The results of the case study were obtained with the help of the CPLEX Solver integrated in GAMS in low computational time. A reduction of costs by almost 60% of the distribution costs is obtained

El primer artículo parte desde el campo de la investigación de diseño. En él se aborda a la profesión en el contexto de la dinámica de cambio organizacional, cuyas tendencias exigen de la misma una evolución en algunos de sus conceptos esenciales para mantenerse como herramienta competitiva del proceso comercial y productivo de manufacturas nacionales. Propone finalmente una tarea concreta para comenzar esta construcción de información a nivel nacional. Los siguientes artículos se centran en el valor principal de este proyecto. Fueron escritos por profesionales experimentados en alguna especialidad del diseño industrial en el país, con una intención evidente por verter al lector sus conocimientos. En sus conceptos, en ocasiones empíricos, se reflexiona sobre interrogantes concretas de los escenarios actuales y futuros de la especialidad de diseño respectiva, realizadas a partir de un conocimiento agudo del tema. Las posturas son diversas, se habla desde el contexto de la empresa transnacional hasta el de la pequeña y mediana empresa nacional; desde el producto para el mercado mundial, al destinado al mercado local; desde la tecnología de punta, a la mediana tecnología; desde las altas producciones, a las piezas únicas; desde los productos que marcan pauta en mercado, a los que siguen el concepto del líder. Es decir, se aborda la realidad amplia, cambiante y diversa que caracteriza en la actualidad al diseño industrial del país

Avengers: Endgame” hace un estudio de caso de la campaña de marketing en torno al filme que cierra la saga de los superhéroes Los Vengadores, de Marvel y Disney.     Evidence for classification of c.1852_1853AA>GC in MLH1 as a neutral variant for Lynch syndrome

    Background: Lynch syndrome (LS) is an autosomal dominant inherited cancer syndrome characterized by early onset cancers of the colorectum, endometrium and other tumours. A significant proportion of DNA variants in LS patients are unclassified. Reports on the pathogenicity of the c.1852_1853AA>GC (p.Lys618Ala) variant of the MLH1 gene are conflicting. In this study, we provide new evidence indicating that this variant has no significant implications for LS. Methods: The following approach was used to assess the clinical significance of the p.Lys618Ala variant: frequency in a control population, case-control comparison, co-occurrence of the p.Lys618Ala variant with a pathogenic mutation, co-segregation with the disease and microsatellite instability in tumours from carriers of the variant. We genotyped p.Lys618Ala in 1034 individuals (373 sporadic colorectal cancer [CRC] patients, 250 index subjects from families suspected of having LS [revised Bethesda guidelines] and 411 controls). Three well-characterized LS families that fulfilled the Amsterdam II Criteria and consisted of members with the p.Lys618Ala variant were included to assess co-occurrence and co-segregation. A subset of colorectal tumour DNA samples from 17 patients carrying the p.Lys618Ala variant was screened for microsatellite instability using five mononucleotide markers. Results: Twenty-seven individuals were heterozygous for the p.Lys618Ala variant; nine had sporadic CRC (2.41%), seven were suspected of having hereditary CRC (2.8%) and 11 were controls (2.68%). There were no significant associations in the case-control and case-case studies. The p.Lys618Ala variant was co-existent with pathogenic mutations in two unrelated LS families. In one family, the allele distribution of the pathogenic and unclassified variant was in trans, in the other family the pathogenic variant was detected in the MSH6 gene and only the deleterious variant co-segregated with the disease in both families. Only two positive cases of microsatellite instability (2/17, 11.8%) were detected in tumours from p.Lys618Ala carriers, indicating that this variant does not play a role in functional inactivation of MLH1 in CRC patients. Conclusions: The p.Lys618Ala variant should be considered a neutral variant for LS.     Las enfermedades cardiovasculares (ECV) siguen siendo la principal causa de muerte en nuestro país. El control adecuado de las alteraciones del metabolismo lipídico es un reto clave en prevención cardiovascular que está lejos de alcanzarse en la práctica clínica real. Existe una gran heterogeneidad en los informes del metabolismo lipídico de los laboratorios clínicos españoles, lo que puede contribuir al mal control del mismo. Por ello, un grupo de trabajo de las principales sociedades científicas implicadas en la atención de los pacientes de riesgo vascular hemos elaborado este documento con una propuesta básica de consenso sobre la determinación del perfil lipídico básico en prevención cardiovascular, recomendaciones para su realización y unificación de criterios para incorporar los objetivos de control lipídico adecuados al riesgo vascular de los pacientes en los informes de laboratorio.Cardiovascular diseases (CVD) continue to be the main cause of death in our country. Adequate control of lipid metabolism disorders is a key challenge in cardiovascular prevention that is far from being achieved in real clinical practice. There is a great heterogeneity in the reports of lipid metabolism from Spanish clinical laboratories, which may contribute to its poor control. For this reason, a working group of the main scientific societies involved in the care of patients at vascular risk, has prepared this document with a consensus proposal on the determination of the basic lipid profile in cardiovascular prevention, recommendations for its realization and unification of criteria to incorporate the lipid control goals appropriate to the vascular risk of the patients in the laboratory reports

    Management of acute diverticulitis with pericolic free gas (ADIFAS). an international multicenter observational study

    Background: There are no specific recommendations regarding the optimal management of this group of patients. The World Society of Emergency Surgery suggested a nonoperative strategy with antibiotic therapy, but this was a weak recommendation. This study aims to identify the optimal management of patients with acute diverticulitis (AD) presenting with pericolic free air with or without pericolic fluid. Methods: A multicenter, prospective, international study of patients diagnosed with AD and pericolic-free air with or without pericolic free fluid at a computed tomography (CT) scan between May 2020 and June 2021 was included. Patients were excluded if they had intra-abdominal distant free air, an abscess, generalized peritonitis, or less than a 1-year follow-up. The primary outcome was the rate of failure of nonoperative management within the index admission. Secondary outcomes included the rate of failure of nonoperative management within the first year and risk factors for failure. Results: A total of 810 patients were recruited across 69 European and South American centers; 744 patients (92%) were treated nonoperatively, and 66 (8%) underwent immediate surgery. Baseline characteristics were similar between groups. Hinchey II-IV on diagnostic imaging was the only independent risk factor for surgical intervention during index admission (odds ratios: 12.5, 95% CI: 2.4-64, P =0.003). Among patients treated nonoperatively, at index admission, 697 (94%) patients were discharged without any complications, 35 (4.7%) required emergency surgery, and 12 (1.6%) percutaneous drainage. Free pericolic fluid on CT scan was associated with a higher risk of failure of nonoperative management (odds ratios: 4.9, 95% CI: 1.2-19.9, P =0.023), with 88% of success compared to 96% without free fluid ( P <0.001). The rate of treatment failure with nonoperative management during the first year of follow-up was 16.5%. Conclusion: Patients with AD presenting with pericolic free gas can be successfully managed nonoperatively in the vast majority of cases. Patients with both free pericolic gas and free pericolic fluid on a CT scan are at a higher risk of failing nonoperative management and require closer observation
