    Aplicación de estrategias Flipped-learning para la enseñanza de Física y Química en Secundaria y Bachillerato

    El presente Trabajo de Fin de Máster se centra en abordar una de las metodologías activas más en auge los últimos años, la metodología flipped-learning, desarrollada ante la necesidad de cambiar el sistema tradicional de aprendizaje para adaptarlo a las necesidades actuales. Se presentarán sus características y elementos más importantes, así como estudios realizados en la etapa de Educación Secundaria Obligatoria y Bachillerato que muestran resultados muy esperanzadores. Esta metodología, donde el estudiante es el protagonista, permite atender a la diversidad de formas de aprender que pueda haber en una clase y apoya todas las fases del ciclo de enseñanza-aprendizaje del alumno. Con el fin de acercar todos sus beneficios a la realidad de este trabajo, se ha diseñado una unidad didáctica para la materia de Física y Química de 4º de la ESO, ‘’El movimiento rectilíneo’’, basada en esta metodología activa. Dar la vuelta a la enseñanza de la cinemática puede tener muchas ventajas, ya que permite la realización de numerosos ejercicios y actividades dinámicas, que no serían posibles en la metodología tradicional. A pesar de que esta unidad no se ha podido poner en práctica, ha sido diseñada con el fin de que pueda servir para aplicaciones futuras.This Master's Thesis is focused on addressing one of the most booming active methodologies in recent years, the flipped-learning methodology, developed in response to the need to change the traditional learning system to adapt it to current needs. Its most important characteristics and elements will be presented, as well as studies carried out in Compulsory Secondary Education and Baccalaureate that show very encouraging results. This methodology, where the student is the main character, allows to attend the diversity of ways of learning that may exist in a class and supports all phases of the student's teaching-learning cycle. In order to bring all of its benefits closer to the reality of this work, a didactic unit has been designed for the subject Physics and Chemistry of the 4th year of Secondary Education, ''The rectilinear movement'', based on this active methodology. Flipping the teaching of kinematics can have many advantages, as it allows to carry out numerous exercises and dynamic activities, which would not be possible in the traditional methodology.     A New application of Social Impact in Social Media for overcoming fake news in health

    One of the challenges today is to face fake news (false information) in health due to its potential impact on people's lives. This article contributes to a new application of social impact in social media (SISM) methodology. This study focuses on the social impact of the research to identify what type of health information is false and what type of information is evidence of the social impact shared in social media. The analysis of social media includes Reddit, Facebook, and Twitter. This analysis contributes to identifying how interactions in these forms of social media depend on the type of information shared. The results indicate that messages focused on fake health information are mostly aggressive, those based on evidence of social impact are respectful and transformative, and finally, deliberation contexts promoted in social media overcome false information about health. These results contribute to advancing knowledge in overcoming fake health-related news shared in social media

    Pozzolanic materials to reduce CO2 emissions: local solutions for a global issue.

    Abstract de una keynote invitada.In recent decades, the cement sector has been looking for solutions to reduce the carbon footprint, being one of the most promising strategies, the replacement of clinker with supplementary cementitious materials, SCMs. However, the main limitation of this approach is the availability of suitable SCMs. This work presents the study of three families of pozzolanic materials, Spanish calcined clays (CC), Natural Pozzolans (NP) and fly ashes (FA). The characterization of each family will be presented, with emphasis on the kaolinite content of the original clays and the amorphous contents of the natural pozzolans and fly ashes. The results of the pozzolanic prediction tests will be compared: strength activity index, SAI, and R3 test according to ASTM C1897-20. The SAI test has two important limitations: i) it gives false positives at 28 days, as does the addition of quartz (Qz) and ii) a minimum of 28 days is required to obtain the pozzolanic activity results. In addition, the R3 test has proved to be useful in ruling out inert additions, such as quartz. Moreover, it presents a very good correlation between the heat emitted and the combined water at 7 days and the amount of kaolinite in clays or amorphous in ashes. However, the absolute values of heat and combined water cannot be compared between different families. That is, in the calcined clay family, it can be inferred that if the kaolinite content is higher than 50 wt%, the heat released should be between 500-700 J/g, whereas a fly ash with an amorphous content of around 70 wt% will release between 200-250 J/g.PID2020-114650RB-I00 grant from Spanish government, Master Builders Solutions Deutschland GmbH (Germany) and Buzzi Unicem SpA (Italy) are thanked for the funding.     Oral fosfomycin for the treatment of lower urinary tract infections among kidney transplant recipients—Results of a Spanish multicenter cohort

    Preliminary results of this study were presented at the 29th European Congress of Clinical Microbiology and Infectious Diseases (ECCMID), held in Amsterdam, The Netherlands, from 13 to 16 April, 2019 (oral communication O‐0699).Oral fosfomycin may constitute an alternative for the treatment of lower urinary tract infections (UTIs) in kidney transplant recipients (KTRs), particularly in view of recent safety concerns with fluroquinolones. Specific data on the efficacy and safety of fosfomycin in KTR are scarce. We performed a retrospective study in 14 Spanish hospitals including KTRs treated with oral fosfomycin (calcium and trometamol salts) for posttransplant cystitis between January 2005 and December 2017. A total of 133 KTRs developed 143 episodes of cystitis. Most episodes (131 [91.6%]) were produced by gram‐negative bacilli (GNB), and 78 (54.5%) were categorized as multidrug resistant (including extended‐spectrum β‐lactamase‐producing Enterobacteriaceae [14%] or carbapenem‐resistant GNB [3.5%]). A median daily dose of 1.5 g of fosfomycin (interquartile range [IQR]: 1.5‐2) was administered for a median of 7 days (IQR: 3‐10). Clinical cure (remission of UTI‐attributable symptoms at the end of therapy) was achieved in 83.9% (120/143) episodes. Among those episodes with follow‐up urine culture, microbiological cure at month 1 was achieved in 70.2% (59/84) episodes. Percutaneous nephrostomy was associated with a lower probability of clinical cure (adjusted odds ratio: 10.50; 95% confidence interval: 0.98‐112.29; P = 0.052).     Efficacy and Safety of Oral Fosfomycin for Asymptomatic Bacteriuria in Kidney Transplant Recipients: Results from a Spanish Multicenter Cohort

    Current guidelines recommend against systematic screening for or treating asymptomatic bacteriuria (AB) among kidney transplant (KT) recipients, although the evidence regarding episodes occurring early after transplantation or in the presence of anatomical abnormalities is inconclusive. Oral fosfomycin may constitute a good option for the treatment of posttransplant AB, particularly due to the emergence of multidrug-resistant (MDR) uropathogens. Available clinical evidence supporting its use in this specific setting, however, remains scarce. We performed a retrospective study in 14 Spanish institutions from January 2005 to December 2017. Overall, 137 episodes of AB diagnosed in 133 KT recipients treated with oral fosfomycin (calcium and trometamol salts) with a test-of-cure urine culture within the first 30 days were included. Median time from transplantation to diagnosis was 3.1 months (interquartile range [IQR], 1.1 to 10.5). Most episodes (96.4% [132/137]) were caused by Gram-negative bacteria (GNB), and 56.9% (78/137) were categorized as MDR (extended‐spectrum β‐lactamase‐producing Enterobacterales [20.4%] and carbapenem‐resistant GNB [2.9%]). Rate of microbiological failure at month 1 was 40.1% (95% confidence interval [CI], 31.9% to 48.9%) for the whole cohort and 42.3% (95% CI, 31.2% to 54.0%) for episodes due to MDR pathogens. Previous urinary tract infection (odds ratio [OR], 2.42; 95% CI, 1.11 to 5.29; P value = 0.027) and use of fosfomycin as salvage therapy (OR, 8.31; 95% CI, 1.67 to 41.35; P value = 0.010) were predictors of microbiological failure. No severe treatment-related adverse events were detected.     Effectiveness of an intervention for improving drug prescription in primary care patients with multimorbidity and polypharmacy:Study protocol of a cluster randomized clinical trial (Multi-PAP project)

    This study was funded by the Fondo de Investigaciones Sanitarias ISCIII (Grant Numbers PI15/00276, PI15/00572, PI15/00996), REDISSEC (Project Numbers RD12/0001/0012, RD16/0001/0005), and the European Regional Development Fund ("A way to build Europe").Background: Multimorbidity is associated with negative effects both on people's health and on healthcare systems. A key problem linked to multimorbidity is polypharmacy, which in turn is associated with increased risk of partly preventable adverse effects, including mortality. The Ariadne principles describe a model of care based on a thorough assessment of diseases, treatments (and potential interactions), clinical status, context and preferences of patients with multimorbidity, with the aim of prioritizing and sharing realistic treatment goals that guide an individualized management. The aim of this study is to evaluate the effectiveness of a complex intervention that implements the Ariadne principles in a population of young-old patients with multimorbidity and polypharmacy. The intervention seeks to improve the appropriateness of prescribing in primary care (PC), as measured by the medication appropriateness index (MAI) score at 6 and 12months, as compared with usual care. Methods/Design: Design:pragmatic cluster randomized clinical trial. Unit of randomization: family physician (FP). Unit of analysis: patient. Scope: PC health centres in three autonomous communities: Aragon, Madrid, and Andalusia (Spain). Population: patients aged 65-74years with multimorbidity (≥3 chronic diseases) and polypharmacy (≥5 drugs prescribed in ≥3months). Sample size: n=400 (200 per study arm). Intervention: complex intervention based on the implementation of the Ariadne principles with two components: (1) FP training and (2) FP-patient interview. Outcomes: MAI score, health services use, quality of life (Euroqol 5D-5L), pharmacotherapy and adherence to treatment (Morisky-Green, Haynes-Sackett), and clinical and socio-demographic variables. Statistical analysis: primary outcome is the difference in MAI score between T0 and T1 and corresponding 95% confidence interval. Adjustment for confounding factors will be performed by multilevel analysis. All analyses will be carried out in accordance with the intention-to-treat principle. Discussion: It is essential to provide evidence concerning interventions on PC patients with polypharmacy and multimorbidity, conducted in the context of routine clinical practice, and involving young-old patients with significant potential for preventing negative health outcomes.     CIBERER : Spanish national network for research on rare diseases: A highly productive collaborative initiative

    Altres ajuts: Instituto de Salud Carlos III (ISCIII); Ministerio de Ciencia e Innovación.CIBER (Center for Biomedical Network Research; Centro de Investigación Biomédica En Red) is a public national consortium created in 2006 under the umbrella of the Spanish National Institute of Health Carlos III (ISCIII). This innovative research structure comprises 11 different specific areas dedicated to the main public health priorities in the National Health System. CIBERER, the thematic area of CIBER focused on rare diseases (RDs) currently consists of 75 research groups belonging to universities, research centers, and hospitals of the entire country. CIBERER's mission is to be a center prioritizing and favoring collaboration and cooperation between biomedical and clinical research groups, with special emphasis on the aspects of genetic, molecular, biochemical, and cellular research of RDs. This research is the basis for providing new tools for the diagnosis and therapy of low-prevalence diseases, in line with the International Rare Diseases Research Consortium (IRDiRC) objectives, thus favoring translational research between the scientific environment of the laboratory and the clinical setting of health centers. In this article, we intend to review CIBERER's 15-year journey and summarize the main results obtained in terms of internationalization, scientific production, contributions toward the discovery of new therapies and novel genes associated to diseases, cooperation with patients' associations and many other topics related to RD research
