    Assessment of the Association of Health with the Liberalisation of Trade in Services under the World Trade Organisation

    Background: The liberalisation of trade in services which began in 1995 under the General Agreement on Trade in Services (GATS) of the World Trade Organisation (WTO) has generated arguments for and against its potential health effects. Our goal was to explore the relationship between the liberalisation of services under the GATS and three health indicators – life expectancy (LE), under-5 mortality (U5M) and maternal mortality (MM) - since the WTO was established. Methods and Findings: This was a cross-sectional ecological study that explored the association in 2010 and 1995 between liberalisation and health (LE, U5M and MM), and between liberalisation and progress in health in the period 1995–2010, considering variables related to economic and social policies such as per capita income (GDP pc), public expenditure on health (PEH), and income inequality (Gini index). The units of observation and analysis were WTO member countries with data available for 2010 (n = 116), 1995 (n = 114) and 1995–2010 (n = 114). We conducted bivariate and multivariate linear regression analyses adjusted for GDP pc, Gini and PEH. Increased global liberalisation in services under the WTO was associated with better health in 2010 (U5M: 20.358 p,0.001; MM: 20.338 p = 0.001; LE: 0.247 p = 0.008) and in 1995, after adjusting for economic and social policy variables. For the period 1995–2010, progress in health was associated with income equality, PEH and per capita income. No association was found with global liberalisation in services. Conclusions: The favourable association in 2010 between health and liberalisation in services under the WTO seems to reflect a pre-WTO association observed in the 1995 data. However, this liberalisation did not appear as a factor associated with progress in health during 1995–2010.     A search for pre- and proto-brown dwarfs in the dark cloud Barnard 30 with ALMA

    In this work we present ALMA continuum observations at 880 μ\mum of 30 sub-mm cores previously identified with APEX/LABOCA at 870μ\mum in the Barnard 30 cloud. The main goal is to characterize the youngest and lowest mass population in the cloud. As a result, we report the detection of five (out of 30) spatially unresolved sources with ALMA, with estimated masses between 0.9 and 67 MJup_{\rm Jup}. From these five sources, only two show gas emission. The analysis of multi-wavelength photometry from these two objects, namely B30-LB14 and B30-LB19, is consistent with one Class II- and one Class I low-mass stellar object, respectively. The gas emission is consistent with a rotating disk in the case of B30-LB14, and with an oblate rotating envelope with infall signatures in the case of LB19. The remaining three ALMA detections do not have infrared counterparts and can be classified as either deeply embedded objects or as starless cores if B30 members. In the former case, two of them (LB08 and LB31) show internal luminosity upper limits consistent with Very Low Luminosity objects, while we do not have enough information for LB10. In the starless core scenario, and taking into account the estimated masses from ALMA and the APEX/LABOCA cores, we estimate final masses for the central objects in the substellar domain, so they could be classified as pre-BD core candidates.Comment: Published in A&

    [RESUMEN] ¿Por qué no aprendemos en las escuelas los grandes logros que miles de mujeres han alcanzado para el avance de la humanidad? El currículum escolar, especialmente el formal, no refleja las conquistas y éxitos que las mujeres han conseguido a lo largo de la Historia. Su escasa presencia en los libros de texto, fundamentalmente en el campo de la Ciencia y de la Tecnología, genera una idea errónea, sesgada y parcial de quiénes fueron los protagonistas de los hechos y procesos históricos, científicos y tecnológicos más importantes para el desarrollo y el progreso de nuestra sociedad. Esta identificación del hombre como protagonista histórico, principalmente en estos campos, podría estar provocando una feminización y una masculinización de las carreras académicas y profesionales, propiciada desde la infancia por un orden simbólico minuciosamente establecido en el que impera la dominación masculina. La aplicación por tanto de la perspectiva de género en los contenidos escolares y en el propio currículum oculto, nos permite equilibrar las relaciones desiguales de poder y desbancar al androcentrismo del puesto privilegiado que la autoridad masculina le ha otorgado. En este texto se presenta un proyecto de investigación que pretende por un lado devolver a las mujeres pioneras, científicas y tecnólogas el espacio que les corresponde dentro de las escuelas y por tanto dentro del conocimiento global. Y por otro, visibilizar nuevos modelos femeninos referenciales para transformar las subjetividades individuales que se edifican con fuerza en la infancia y en la adolescencia

    A cross‑sectional study of Leishmania infantum infection in stray cats in the city of Zaragoza (Spain) using serology and PCR

    Background: Feline leishmaniosis is a vector-borne parasitic disease caused by Leishmania spp. Leishmania infection in dogs is prevalent in the Mediterranean basin, but in other animals, such as cats, it could also play a role in the epidemiology of the disease. Information on the geographical distribution and epidemiological features of L. infantum infection in cats is scarce, particularly in urban stray cats living in regions where canine leishmaniosis is endemic. As diagnosis can be challenging, combining different serological and molecular methods is a useful approach. Our aim was to investigate the prevalence of infection of L. infantum in apparently healthy stray cats in an endemic region of Spain (Zaragoza city) using serological and molecular methods, and to compare the results of the different techniques. Methods: The prevalence of Leishmania infection was studied in stray cats captured in urban and peri-urban areas of Zaragoza. Blood was collected from each animal for serology and molecular analysis. Three serological methods, namely the immunofluorescent antibody test (IFAT), enzyme-linked immunosorbent assay (ELISA) and western blot (WB), were used to detect L. infantum antibodies and a real-time PCR (qPCR) assay was used to detect L. infantum DNA. The results were analyzed by Fisher's exact test and Cohen's kappa statistic (κ) to assess the level of agreement between the diagnostic techniques. Results: Serological analysis of blood samples from 180 stray cats revealed 2.2% (4/179) Leishmania infection positivity by IFAT, 2.8% (5/179) by ELISA and 14.5% (26/179) by WB. Leishmania DNA was detected by qPCR in 5.6% (10/179) of the cats. Sixteen cats (8.9%) tested positive by only one serological technique and four tested positive by all three serological methods used. The overall rate of infected cats (calculated as the number of cats seropositive and/or qPCR positive) was 15.6%, and only two cats tested positive by all the diagnostic methods. A significant association was found between male cats and a positive qPCR result. Comparison of the techniques revealed a fair agreement in seropositivity between blood qPCR and IFAT (κ = 0.26), blood qPCR and ELISA (κ = 0.24), WB and ELISA (κ = 0.37) and WB and IFAT (κ = 0.40). The highest agreement between seropositive results was between IFAT and ELISA (κ = 0.89), and the lowest was between blood qPCR and WB (κ = 0.19). The prevalence of the feline leukemia virus antigen was 4.49% (8/178 cats) and that of the feline immunodeficiency virus (FIV) antibody was 6.74% (12/178), while co-infection with both retroviruses was observed in one female cat (1/178). Leishmania ELISA and IFAT seropositivity were statistically associated with FIV status by the chi-square test. Conclusions: The results obtained in this study, using serological tests and qPCR, indicate the existence of L. infantum asymptomatic infection in apparently healthy stray cats in the city of Zaragoza, an endemic area in Spain

    Diagnostic performance of CerTest and Panbio antigen rapid diagnostic tests to diagnose SARS-CoV-2 infection

    Objectives Antigen rapid diagnostic tests (Ag-RDT) have been developed as reliable tools to control the SARS-CoV-2 pandemic. The objective of our study was to evaluate the diagnostic performance of two Ag-RDTs. Methods We evaluated CerTest SARS-CoV-2 Ag One Step Card Test and Panbio COVID-19 Ag Rapid Test Device Ag-RDTs. We included 320 nasopharyngeal samples: 150 PCR negative samples to assess the specificity and 170 PCR positive samples to evaluate the sensitivity. We also evaluated their sensitivity according to cycle threshold (Ct) values and the time from the onset of symptoms. Tests were compared using the McNemar?s test and agreement was evaluated using the kappa score (k). Results Both Ag-RDTs showed a specificity of 100 %. Overall sensitivity was 53.5 % for CerTest and 60.0 % for Panbio. For samples with 25, sensitivity was 94.0 % for CerTest and 96.4 % for Panbio (p = 0.500). Regarding samples with Ct>25, sensitivity was 14.0 % for CerTest and 24.4 % for Panbio (p = 0.004). Sensitivity for samples within the first 5 days after the onset of symptoms were 84.8 % for CerTest and 91.3 % for Panbio (p = 0.250) and notably decreased for samples taken after the fifth day. Both Ag-RDTs showed an excellent agreement between them (agreement = 96.7 %, k = 0.920). Agreement with PCR was also excellent for high viral load samples (Ct<25) for CerTest (98.0 %, k = 0.954) and Panbio (98.8 %, k = 0.973). Conclusions CerTest SARS-CoV-2 and Panbio COVID-19 Ag showed excellent performance and agreement results for samples with high viral loads (Ct 25) or samples taken within the first 5 days after the onset of symptoms

    Analysis of two SMC HII Regions Considering Thermal Inhomogeneities: Implications for the Determinations of Extragalactic Chemical Abundances

    We present long slit spectrophotometry considering the presence of thermal inhomogeneities (t^2) of two HII regions in the Small Magellanic Cloud (SMC): NGC 456 and NGC 460. Physical conditions and chemical abundances were determined for three positions in NGC 456 and one position in NGC 460, first under the assumption of uniform temperature and then allowing for the possibility of thermal inhomogeneities. We determined t^2 values based on three different methods: i) by comparing the temperature derived using oxygen forbidden lines with the temperature derived using helium recombination lines, ii) by comparing the abundances derived from oxygen forbidden lines with those derived from oxygen recombination lines, and iii) by comparing the abundances derived from ultraviolet carbon forbidden lines with those derived from optical carbon recombination lines. The first two methods averaged t^2=0.067+-0.013 for NGC 456 and t^2=0.036+-0.027 for NGC 460. These values of t^2 imply that when gaseous abundances are determined with collisionally excited lines they are underestimated by a factor of nearly 2. From these objects and others in the literature, we find that in order to account for thermal inhomogeneities and dust depletion, the O/H ratio in low metallicity HII regions should be corrected by 0.25-0.45 dex depending on the thermal structure of the nebula, or by 0.35 dex if such information is not available.Comment: Accepted for publication in The Astrophysical Journal. 41 pages in pre-print format. 3 figure

    Electrodeposited magnetic nanowires with radial modulation of composition

    In the last few years, magnetic nanowires have gained attention due to their potential implementation as building blocks in spintronics applications and, in particular, in domain-wall- based devices. In these devices, the control of the magnetic properties is a must. Cylindrical magnetic nanowires can be synthesized rather easily by electrodeposition and the control of their magnetic properties can be achieved by modulating the composition of the nanowire along the axial direction. In this work, we report the possibility of introducing changes in the composition along the radial direction, increasing the degrees of freedom to harness the magnetization. In particular, we report the synthesis, using template-assisted deposition, of FeNi (or Co) magnetic nanowires, coated with a Au/Co (Au/FeNi) bilayer. The diameter of the nanowire as well as the thickness of both layers can be tuned at will. In addition to a detailed structural characterization, we report a preliminary study on the magnetic properties, establishing the role of each layer in the global collective behavior of the system

    Short-term prognosis of polypharmacy in elderly patients treated in emergency departments: results from the EDEN project

    Background: Polypharmacy is a growing phenomenon among elderly individuals. However, there is little information about the frequency of polypharmacy among the elderly population treated in emergency departments (EDs) and its prognostic effect. This study aims to determine the prevalence and short-term prognostic effect of polypharmacy in elderly patients treated in EDs. Methods: A retrospective analysis of the Emergency Department Elderly in Needs (EDEN) project's cohort was performed. This registry included all elderly patients who attended 52 Spanish EDs for any condition. Mild and severe polypharmacy was defined as the use of 5-9 drugs and >10 drugs, respectively. The assessed outcomes were ED revisits, hospital readmissions, and mortality 30 days after discharge. Crude and adjusted logistic regression analyses, including the patient's comorbidities, were performed. Results: A total of 25,557 patients were evaluated [mean age: 78 (IQR: 71-84) years]; 10,534 (41.2%) and 5678 (22.2%) patients presented with mild and severe polypharmacy, respectively. In the adjusted analysis, mild polypharmacy and severe polypharmacy were associated with an increase in ED revisits [odds ratio (OR) 1.13 (95% confidence interval (CI): 1.04-1.23) and 1.38 (95% CI: 1.24-1.51)] and hospital readmissions [OR 1.18 (95% CI: 1.04-1.35) and 1.36 (95% CI: 1.16-1.60)], respectively, compared to non-polypharmacy. Mild and severe polypharmacy were not associated with increased 30-day mortality [OR 1.05 (95% CI: 0.89-2.26) and OR 0.89 (95% CI: 0.72-1.12)], respectively. Conclusion: Polypharmacy was common among the elderly treated in EDs and associated with increased risks of ED revisits and hospital readmissions 10 drugs. In these patients comorbidities were associated with an increase in the number of drugs. In the patients with severe polypharmacy (>10 drugs), diuretics were the most frequently drugs prescribed, followed by antihypertensives and statins. The results obtained indicate that polypharmacy is a frequent phenomenon among the elderly population treated in Emergency departments, being antihypertensives the most frequently used drugs in this population. Those patients who takes >10 drugs have a higher risk of new visits to the emergency room and hospital readmissions in short term period
