    A New Natural Antimycotic Agent is Effective Against Oropharyngeal Candidiasis: The VIPROCAN Study.

    Background: The incidence of community and nosocomial candidiasis has dramatically increased in the last two decades. There are multiple treatments for this infection, but the toxicity of some and the induction of resistant strains require the development of new compounds. Objectives: With the aim of reducing the Candida population in the oropharyngeal cavity, we have formulated a toothpaste with VG-01 agent, composed of a mixture of carnosic acid (CA) and propolis (PP). Methods: We investigated the ability of VG-01 toothpaste to minimize and stabilize fungal presence in 21 patients diagnosed with clinical oropharyngeal candidiasis. Results: Our data indicate that VG-01 toothpaste showed an effect not only against the most frequent species of Candida, C. albicans, but also in the other species analyzed. 82% of patients stated that they would continue using it outside the study. Conclusion: Our data demonstrate that VG-01, composed of CA and PP is a potential antimycotic agent effective against the most common species that cause oropharyngeal candidiasis present in clinical practicepost-print988 K

    El siguiente informe muestra la problemática social que se presenta en el país a causa del conflicto armado. En este trabajo se pretende realizar un análisis, tomando como referencia, el contexto psicosocial y la forma en la que cada problemática puede ser abordada, haciendo uso de herramientas como la imagen y la narrativa que sirven para afrontar este tipo de situaciones desde una perspectiva más expresiva. La violencia, sin lugar a duda es uno de los acontecimientos que sufren a diario muchas personas, sin embargo, en esta oportunidad, se hará un análisis al relato de “Carlos Arturo”, un joven que desde sus 14 años fue víctima de la violencia, una violencia que le dejó secuelas de por vida, situación que lo coloca en un proceso de vulnerabilidad frente a la sociedad. De igual manera , se realiza el análisis del caso Peñas Coloradas, aquí se pretende identificar el impacto que asume la población al ser catalogada como cómplice de grupos armados, además, se dará a conocer al lector los emergentes psicosociales que se evidencian a causa del hostigamiento militar del que fue víctima la comunidad. Por otra parte, mediante un ejercicio llamado foto voz, se logra identificar en un contexto cercano, las diferentes dinámicas de violencia que se presentan en él, realizando un acercamiento al entorno seleccionado y recolectando un conjunto de fotografías que permitan expresar y evidenciar los factores de riesgo y los factores transformadores que emergen de la misma.The following report shows the social problems that arise in the country due to the armed conflict.     First report on sexually transmitted infections among trans (male to female transvestites, transsexuals, or transgender) and male sex workers in Argentina: High HIV, HPV, HBV, and syphilis prevalence

    Objectives: Due to the scarce data on the prevalence of sexually transmitted infections (STIs) among male-to-female trans-sex workers (TSW) and male sex workers (MSW) in Argentina, the present study aimed to estimate the incidence of human immunodeficiency virus (HIV), and the prevalence of HIV, hepatitis B virus (HBV), hepatitis C virus (HCV), and Treponema pallidum. Human papillomavirus (HPV) and Chlamydia trachomatis infections were tested among TSW. Methods: Two hundred and seventy-three TSW and 114 MSW were recruited by nongovernmental organizations. HIV incidence was estimated by STARHS (serologic testing algorithm for recent HIV seroconversion). HPV and C. trachomatis infections were tested in anal cells from TSW. Results: TSW showed significantly higher prevalences of HIV (34.1 vs. 11.4%), HBV (40.2 vs. 22.0%), and T. pallidum (50.4 vs. 20.4%) than MSW. TSW tested positive for HPV in 111/114 cases and for C. trachomatis in 4/80 cases. Investigation of HBV, HCV, HIV, and T. pallidum co-infections showed that 72% of TSW and 39% of MSW had at least one STI. T. pallidum was the most frequent mono-infection. The estimated HIV incidence was 10.7 per 100 person-years (95% confidence interval (CI) 3.8-17.7) for TSW and 2.3 per 100 person-years (95% CI 0-6.7) for MSW. Conclusions: The high prevalence of STIs and the high incidence of HIV demonstrate the great vulnerability of these high-risk populations and indicate the urgent need for preventive strategies on intervention and facilitation of access to healthcare programs.Fil: Dos Ramos Farías, María Sol.     Everolimus plus minimized tacrolimus on kidney function in liver transplantation: REDUCE, a prospective, randomized controlled study

    Background and aim: reduction in calcineurin inhibitor levels is considered crucial to decrease the incidence of kidney dysfunction in liver transplant (LT) recipients. The aim of this study was to evaluate the safety and impact of everolimus plus reduced tacrolimus (EVR + rTAC) vs. mycophenolate mofetil plus tacrolimus (MMF + TAC) on kidney function in LT recipients from Spain. Methods: the REDUCE study was a 52-week, multicenter, randomized, controlled, open-label, phase 3b study in de novo LT recipients. Eligible patients were randomized (1:1) 28 days post-transplantation to receive EVR + rTAC (TAC levels <_ 5 ng/mL) or to continue with MMF + TAC (TAC levels = 6-10 ng/mL). Mean estimated glomerular filtration rate (eGFR), clinical benefit in renal function, and safety were evaluated. Results: in the EVR + rTAC group (n = 105), eGFR increased from randomization to week 52 (82.2 [28.5] mL/min/1.73 m2 to 86.1 [27.9] mL/min/1.73 m2) whereas it decreased in the MMF + TAC (n = 106) group (88.4 [34.3] mL/min/1.73 m2 to 83.2 [25.2] mL/min/1.73 m2), with significant (p < 0.05) differences in eGFR throughout the study. However, both groups had a similar clinical benefit regarding renal function (improvement in 18.6 % vs. 19.1 %, and stabilization in 81.4 % vs. 80.9 % of patients in the EVR + rTAC vs. MMF + TAC groups, respectively). There were no significant differences in the incidence of acute rejection (5.7 % vs. 3.8 %), deaths (5.7 % vs. 2.8 %), and serious adverse events (51.9 % vs. 44.0 %) between the 2 groups. Conclusion: EVR + rTAC allows a safe reduction in tacrolimus exposure in de novo liver transplant recipients, with a significant improvement in eGFR but without significant differences in renal clinical benefit 1 year after liver transplantation

    Lineamientos para una metodología de identificación de estilos de aprendizaje aplicables al sector agropecuario colombiano

    In Colombia, one of the deficiencies of technology transfer processes has been the lack of strategies that allow identifying the way producers learn, which, in turn, is reflected in the low implementation of the practices suggested in training processes. For this reason, the aim of this study was to analyze the research carried out on the identification of learning styles to generate a methodological proposal suitable to be implemented in the agricultural sector, which contributes to improving the effectiveness of the transfer processes. Models that have been studied at a global level were identified and used as input to build a methodology with four dimensions (motivational, perceptive, strategic, and social) that respond to the characteristics of the rural context and the training processes of producers. These results highlight the importance of identifying learning styles before carrying out a training process to achieve the implementation of new technologies by agricultural producers.En Colombia, una de las deficiencias en los procedimientos de transferencia de tecnología ha sido la falta de estrategias que permitan identificar la forma en que aprenden los productores, lo que se ha reflejado en la baja implementación de prácticas sugeridas en los procesos de capacitación. El objetivo de este estudio fue analizar la literatura existente sobre identificación de estilos de aprendizaje para generar una propuesta metodológica aplicable al sector agropecuario que contribuya a mejorar la efectividad de los procesos de transferencia. Se identificaron modelos estudiados a nivel global, los cuales se usaron como insumo para construir una metodología con cuatro dimensiones (motivacional, perceptiva, estratégica y social) que responden a las características del contexto rural y los procesos de capacitación de productores. Los resultados permiten resaltar la importancia de identificar estilos de aprendizaje antes de llevar a cabo un proceso de formación para aumentar la implementación de nuevas tecnologías por parte de productores agropecuarios

    Compromising between European and US allergen immunotherapy schools: Discussions from GUIMIT, the Mexican immunotherapy guidelines

    Background: Allergen immunotherapy (AIT) has a longstanding history and still remains the only disease-changing treatment for allergic rhinitis and asthma. Over the years 2 different schools have developed their strategies: the United States (US) and the European. Allergen extracts available in these regions are adapted to local practice. In other parts of the world, extracts from both regions and local ones are commercialized, as in Mexico. Here, local experts developed a national AIT guideline (GUIMIT 2019) searching for compromises between both schools. Methods: Using ADAPTE methodology for transculturizing guidelines and AGREE-II for evaluating guideline quality, GUIMIT selected 3 high-quality Main Reference Guidelines (MRGs): the European Academy of Allergy, Asthma and Immunology (EAACI) guideines, the S2k guideline of various German-speaking medical societies (2014), and the US Practice Parameters on Allergen Immunotherapy 2011. We formulated clinical questions and based responses on the fused evidence available in the MRGs, combined with local possibilities, patient's preference, and costs. We came across several issues on which the MRGs disagreed. These are presented here along with arguments of GUIMIT members to resolve them. GUIMIT (for a complete English version, see Supplementary data) concluded the following: Results: Related to the diagnosis of IgE-mediated respiratory allergy, apart from skin prick testing complementary tests (challenges, in vitro testing and molecular such as species-specific allergens) might be useful in selected cases to inform AIT composition. AIT is indicated in allergic rhinitis and suggested in allergic asthma (once controlled) and IgE-mediated atopic dermatitis. Concerning the correct subcutaneous AIT dose for compounding vials according to the US school: dosing tables and formula are given; up to 4 non-related allergens can be mixed, refraining from mixing high with low protease extracts. When using European extracts: the manufacturer's indications should be followed; in multi-allergic patients 2 simultaneous injections can be given (100% consensus); mixing is discouraged. In Mexico only allergoid tablets are available; based on doses used in all sublingual immunotherapy (SLIT) publications referenced in MRGs, GUIMIT suggests a probable effective dose related to subcutaneous immunotherapy (SCIT) might be: 50–200% of the monthly SCIT dose given daily, maximum mixing 4 allergens. Also, a table with practical suggestions on non-evidence-existing issues, developed with a simplified Delphi method, is added. Finally, dissemination and implementation of guidelines is briefly discussed, explaining how we used online tools for this in Mexico. Conclusions: Countries where European and American AIT extracts are available should adjust AIT according to which school is followed
