    Hormonal spawning induction and larval rearing of meagre, Argyrosomus regius (Pisces: Sciaenidae)

    The aim of the present study was to evaluate the culture potential of meagre, Argyrosomus regius (Asso, 1801). Trials were conducted in two different facilities in Spain, the LIMIA research centre (Mallorca) and the IFAPA research centre “El Toruño” (El Puerto de Santa Maria). In May 2006, males with free milt and females with vitellogenic oocytes bigger than 500 μm were injected with salmon gonadotropin releasing hormone analogues (sGnRHa). Spawning occurred approximately 38 hours after induction. Fecundity was high with 1.207.000 eggs collected from a 13 kg female at LIMIA in a single spawn. The larval development, growth and morphological changes were described from 0 to 30 days post hatching (DPH). The mean length of the newly hatched larvae ranged from 2.20 ± 0.02 mm (LIMIA) to 3.19 ±0.09 mm (IFAPA). Feeding began on 3 DPH, initial swim bladder inflation was observed on 5 DPH, and metamorphosis was completed on 30 DPH. Growth was very fast and the post-larvae reached 15.11 ± 3.49 mm (LIMIA) and 11.66 ± 0.96 mm (IFAPA) in 30 days Cannibalism was observed from 15 DPH onwards. These preliminary results indicate the meagre as a priority species for the diversification in aquaculture

    Validation of Spanish Language Evaluation Instruments for Body Dysmorphic Disorder and the Dysmorphic Concern Construct

    Dysmorphic concern (DC) refers to excessive preoccupation with a slight or imagined defect in physical appearance with social avoidance and behavior directed at controlling the defect in appearance. This study attempted to adapt the factor structure of two instruments that cover the DC construct, the Dysmorphic Concern Questionnaire (DCQ) and the Body Dysmorphic Disorder Examination Self-Report (BDDE-SR), to Spanish and establish their psychometric properties. A total of 920 subjects (62.7% women, Mage = 32.44 years) participated. Exploratory and Confirmatory Factor Analysis of both scales found adequate goodness of fit indices. A one-dimensional structure was found for the DCQ and two first-order factors (dissatisfaction/preoccupation with body image (BI) and BI avoidance behavior) were identified for the BDDE-SR. The psychometric test–retest reliability and validity properties (content, convergent, and discriminant) were satisfactory. It is suggested that the DC construct includes both cognitive and behavioral aspects and may represent a continuum of severity with Body Dysmorphic Disorder at the end.European Union’s Seventh Framework Programme for research, technological development and demonstration under [grant agreement no 31674

    Consensus on Criteria for Good Practices in Video Consultation: A Delphi Study

    The use of telemedicine has greatly increased, largely derived from the COVID-19 pandemic, which has created the need for a guide aimed towards the adequate management of a modality of health care: the video consultation. A Delphi study composed of three rounds was conducted with 16 experts in holding video consultations and managing non-technical skills from different specialties and nationalities to conceive a consensus on the criteria needed for properly managing video consultations by healthcare professionals. The consensus criteria were defined by three dimensions (preparation of video consultation, video consultation process, and post-video consultation) and their corresponding items. Excellent consensus data was obtained; therefore, use is recommended by any healthcare professional who is going to utilize a video consultation, in order to manage it effectively

    Psychological effects of a mindfulness intervention performed during the cool down in the Physical Education class: a quasi-experimental study

    En los últimos años los niveles de estrés en los adolescentes han aumentado progresivamente. Ante esta problemática, la práctica del mindfulness se extiende en el ámbito escolar por sus numerosos beneficios cognitivos, sociales y emocionales. En este sentido, las principales instituciones sugieren la importancia del cuidado mental en los jóvenes, especialmente en los adolescentes. En el currículum escolar, la asignatura de Educación Física representa un escenario idóneo para la aplicación de las actividades de esta índole, particularmente en el bloque de contenidos que incluye la relajación y respiración, así como en la vuelta a la calma, contenido común de todos los bloques. El objetivo del presente estudio es valorar los efectos de una intervención de mindfulness desarrollado en la vuelta a la calma de las clases de Educación Física con 39 niños y niñas de 4.º de Educación Secundaria mediante dos cuestionarios validados: Child and Adolescent Mindfulness Measure y la Escala de Atención Plena. Los resultados muestran que los participantes del grupo experimental, comparados con el grupo control, obtuvieron mejoras en su atención plena, siendo estas significativas en el caso de la Child and Adolescent Mindfulness Measure. Por lo tanto, se concluye que una breve intervención de mindfulness en la clase de Educación Física puede contribuir al bienestar psicológico del alumnado.In recent years, stress levels in adolescents have increased progressively. Facing this problem, the practice of mindfulness is spreading in schools due to its numerous cognitive, social and emotional benefits. In this sense, the main world institutions suggest the importance of mindfulness in young people, especially in adolescents. Within the school curriculum, the subject of Physical Education represents an ideal scenario for activities of this nature, particularly in the section of contents that include relaxation and breathing, as well as in the cool down, common content of all the blocks. The aim of this study is to assess the effects of a mindfulness intervention developed within the cool down in Physical Education classes with 39 boys and girls in the 4th year of Secondary Education through two validated questionnaires: Child and Adolescent Mindfulness Measure and Mindfulness Scale. The results show that the experimental group, compared with the control group, obtained significant differences in the mindfulness skills, these being significant in the case of the Child and Adolescent Mindfulness Measure. Therefore, it is concluded that a brief mindfulness intervention in the physical education classroom can contribute to the psychological well-being of students

    Increase in Video Consultations During the COVID-19 Pandemic: Healthcare Professionals’ Perceptions about Their Implementation and Adequate Management

    In response to the COVID-19 pandemic, health care modalities such as video consultations have been rapidly developed to provide safe health care and to minimize the risk of spread. The purpose of our study is to explore Spanish healthcare professionals’ perceptions about the implementation of video consultations. Based on the testimonies of 53 professionals, different categories emerged related to the four identified themes: benefits of video consultations (for professionals, patients, and the health system, and compared to phone calls), negative aspects (inherent to new technologies and the risk of a perceived distancing from the professional), difficulties associated with the implementation of video consultations (technological difficulties, lack of technical skills and refusal to use video consultation among professionals and patients), and the need for training (technological, nontechnical, and social-emotional skills, and adaptation of technical skills). Additionally, the interviewees indicated that this new modality of health care may be extended to a broader variety of patients and clinical settings. Therefore, since video consultations are becoming more widespread, it would be advisable for health policies and systems to support this modality of health care, promoting their implementation and guaranteeing their operability, equal access and quality

    Predictive Model and Mortality Risk Score during Admission for Ischaemic Stroke with Conservative Treatment

The aim of this study is to develop a predictive model to estimate the risk of mortality in the admission of patients who have not received reperfusion treatment. Methods: A retrospective cohort study was conducted of a clinical–administrative database, reflecting all cases of non-reperfused ischaemic stroke admitted to Spanish hospitals during the period 2008–2012. A predictive model based on logistic regression was developed on a training cohort and later validated by the “hold-out” method. Complementary machine learning techniques were also explored. Results: The resulting model had the following nine variables, all readily obtainable during initial care. Age (OR 1.069), female sex (OR 1.202), readmission (OR 2.008), hypertension (OR 0.726), diabetes (OR 1.105), atrial fibrillation (OR 1.537), dyslipidaemia (0.638), heart failure (OR 1.518) and neurological symptoms suggestive of posterior fossa involvement (OR 2.639). The predictability was moderate (AUC 0.742, 95% CI: 0.737–0.747), with good visual calibration; Pearson’s chi-square test revealed non-significant calibration. An easily consulted risk score was prepared. Conclusions: It is possible to create a predictive model of mortality for patients with ischaemic stroke from which important advances can be made towards optimising the quality and efficiency of care. The model results are available within a few minutes of admission and would provide a valuable complementary resource for the neurologist.Fundacion Progreso y Salud AP-0013-2020-C1-F

    EconoCINE: Una experiencia de cine y economía

    [ES] El objetivo del grupo de innovación, y de esta iniciativa, es utilizar distintas formas de arte como soporte para la enseñanza de la economía. En este caso, hemos propuesto el cine como vehículo educativo. La experiencia se desarrolla a través de ciclos de cine y economía en los que el alumnado profundiza, mediante el visionado de películas, en conceptos económicos concretos. Hasta el momento, se han puesto en marcha tres ciclos y los resultados revelan que el alumnado ha logrado un conocimiento más profundo de las implicaciones que provocan las decisiones económicas y su forma de llevarlas a cabo. El cine sirve al profesorado como laboratorio de ideas y resultados, y permite mostrar interpretaciones imposibles de representar en el aula. El alumnado ha valorado de forma muy positiva la experiencia y reconoce que el sistema propuesto le ha permitido un aprendizaje más completo y reflexivo

    A personalized intervention to prevent depression in primary care: cost-effectiveness study nested into a clustered randomized trial

    Abstract Background: Depression is viewed as a major and increasing public health issue, as it causes high distress in the people experiencing it and considerable financial costs to society. Efforts are being made to reduce this burden by preventing depression. A critical component of this strategy is the ability to assess the individual level and profile of risk for the development of major depression. This paper presents the cost-effectiveness of a personalized intervention based on the risk of developing depression carried out in primary care, compared with usual care. Methods: Cost-effectiveness analyses are nested within a multicentre, clustered, randomized controlled trial of a personalized intervention to prevent depression. The study was carried out in 70 primary care centres from seven cities in Spain. Two general practitioners (GPs) were randomly sampled from those prepared to participate in each centre (i.e. 140 GPs), and 3326 participants consented and were eligible to participate. The intervention included the GP communicating to the patient his/her individual risk for depression and personal risk factors and the construction by both GPs and patients of a psychosocial programme tailored to prevent depression. In addition, GPs carried out measures to activate and empower the patients, who also received a leaflet about preventing depression. GPs were trained in a 10- to 15-h workshop. Costs were measured from a societal and National Health care perspective. Qualityadjustedlife years were assessed using the EuroQOL five dimensions questionnaire.     Effectiveness of an intervention for improving drug prescription in primary care patients with multimorbidity and polypharmacy:Study protocol of a cluster randomized clinical trial (Multi-PAP project)

A key problem linked to multimorbidity is polypharmacy, which in turn is associated with increased risk of partly preventable adverse effects, including mortality. The Ariadne principles describe a model of care based on a thorough assessment of diseases, treatments (and potential interactions), clinical status, context and preferences of patients with multimorbidity, with the aim of prioritizing and sharing realistic treatment goals that guide an individualized management. The aim of this study is to evaluate the effectiveness of a complex intervention that implements the Ariadne principles in a population of young-old patients with multimorbidity and polypharmacy. The intervention seeks to improve the appropriateness of prescribing in primary care (PC), as measured by the medication appropriateness index (MAI) score at 6 and 12months, as compared with usual care. Methods/Design: Design:pragmatic cluster randomized clinical trial. Unit of randomization: family physician (FP). Unit of analysis: patient. Scope: PC health centres in three autonomous communities: Aragon, Madrid, and Andalusia (Spain). Population: patients aged 65-74years with multimorbidity (≥3 chronic diseases) and polypharmacy (≥5 drugs prescribed in ≥3months). Sample size: n=400 (200 per study arm). Intervention: complex intervention based on the implementation of the Ariadne principles with two components: (1) FP training and (2) FP-patient interview. Outcomes: MAI score, health services use, quality of life (Euroqol 5D-5L), pharmacotherapy and adherence to treatment (Morisky-Green, Haynes-Sackett), and clinical and socio-demographic variables. Statistical analysis: primary outcome is the difference in MAI score between T0 and T1 and corresponding 95% confidence interval. Adjustment for confounding factors will be performed by multilevel analysis. All analyses will be carried out in accordance with the intention-to-treat principle. Discussion: It is essential to provide evidence concerning interventions on PC patients with polypharmacy and multimorbidity, conducted in the context of routine clinical practice, and involving young-old patients with significant potential for preventing negative health outcomes.     Safety and immunogenicity of the protein-based PHH-1V compared to BNT162b2 as a heterologous SARS-CoV-2 booster vaccine in adults vaccinated against COVID-19 : a multicentre, randomised, double-blind, non-inferiority phase IIb trial

    A SARS-CoV-2 protein-based heterodimer vaccine, PHH-1V, has been shown to be safe and well-tolerated in healthy young adults in a first-in-human, Phase I/IIa study dose-escalation trial. Here, we report the interim results of the Phase IIb HH-2, where the immunogenicity and safety of a heterologous booster with PHH-1V is assessed versus a homologous booster with BNT162b2 at 14, 28 and 98 days after vaccine administration. The HH-2 study is an ongoing multicentre, randomised, active-controlled, double-blind, non-inferiority Phase IIb trial, where participants 18 years or older who had received two doses of BNT162b2 were randomly assigned in a 2:1 ratio to receive a booster dose of vaccine-either heterologous (PHH-1V group) or homologous (BNT162b2 group)-in 10 centres in Spain. Eligible subjects were allocated to treatment stratified by age group (18-64 versus ≥65 years) with approximately 10% of the sample enrolled in the older age group. The primary endpoints were humoral immunogenicity measured by changes in levels of neutralizing antibodies (PBNA) against the ancestral Wuhan-Hu-1 strain after the PHH-1V or the BNT162b2 boost, and the safety and tolerability of PHH-1V as a boost. The secondary endpoints were to compare changes in levels of neutralizing antibodies against different variants of SARS-CoV-2 and the T-cell responses towards the SARS-CoV-2 spike glycoprotein peptides. The exploratory endpoint was to assess the number of subjects with SARS-CoV-2 infections ≥14 days after PHH-1V booster. This study is ongoing and is registered with , . From 15 November 2021, 782 adults were randomly assigned to PHH-1V (n = 522) or BNT162b2 (n = 260) boost vaccine groups. The geometric mean titre (GMT) ratio of neutralizing antibodies on days 14, 28 and 98, shown as BNT162b2 active control versus PHH-1V, was, respectively, 1.68 (p < 0.0001), 1.31 (p = 0.0007) and 0.86 (p = 0.40) for the ancestral Wuhan-Hu-1 strain; 0.62 (p < 0.0001), 0.65 (p < 0.0001) and 0.56 (p = 0.003) for the Beta variant; 1.01 (p = 0.92), 0.88 (p = 0.11) and 0.52 (p = 0.0003) for the Delta variant; and 0.59 (p ≤ 0.0001), 0.66 (p < 0.0001) and 0.57 (p = 0.0028) for the Omicron BA.1 variant. Additionally, PHH-1V as a booster dose induced a significant increase of CD4 + and CD8 + T-cells expressing IFN-γ on day 14. There were 458 participants who experienced at least one adverse event (89.3%) in the PHH-1V and 238 (94.4%) in the BNT162b2 group. The most frequent adverse events were injection site pain (79.7% and 89.3%), fatigue (27.5% and 42.1%) and headache (31.2 and 40.1%) for the PHH-1V and the BNT162b2 groups, respectively. A total of 52 COVID-19 cases occurred from day 14 post-vaccination (10.14%) for the PHH-1V group and 30 (11.90%) for the BNT162b2 group (p = 0.45), and none of the subjects developed severe COVID-19. Our interim results from the Phase IIb HH-2 trial show that PHH-1V as a heterologous booster vaccine, when compared to BNT162b2, although it does not reach a non-inferior neutralizing antibody response against the Wuhan-Hu-1 strain at days 14 and 28 after vaccination, it does so at day 98. PHH-1V as a heterologous booster elicits a superior neutralizing antibody response against the previous circulating Beta and the currently circulating Omicron BA.1 SARS-CoV-2 variants in all time points assessed, and for the Delta variant on day 98 as well. Moreover, the PHH-1V boost also induces a strong and balanced T-cell response. Concerning the safety profile, subjects in the PHH-1V group report significantly fewer adverse events than those in the BNT162b2 group, most of mild intensity, and both vaccine groups present comparable COVID-19 breakthrough cases, none of them severe.
