    Influence of the surface properties of nanocapsules on their interaction with intestinal barriers

    Despite the convenience of the oral route for drug administration, the existence of different physiological barriers associated with the intestinal tract greatly lowers the bioavailability of many active compounds. We have previously suggested the potential polymeric nanocapsules, consisting of an oily core surrounded by a polymer shell, as oral drug delivery carriers. Here we present a systematic study of the influence of the surface properties of these nanocapsules on their interaction with the intestinal barriers. Two different surfactants, Pluronic®F68 (PF68) and F127 (PF127), and two polymeric shells, chitosan (CS) and polyarginine (PARG) were chosen for the formulation of the nanocapsules. We analyzed nine different combinations of these polymers and surfactants, and studied the effect of each specific combination on their colloidal stability, enzymatic degradation, and mucoadhesion/mucodiffusion. Our results indicate that both, the polymer shell and the surfactants located at the oil/water interface, influence the interaction of the nanocapsules with the intestinal barriers.     Los Poyos del Molinillo (Frigiliana): new site of the Bronze Age in the East Axarquía (Málaga, Andalucía)

    En este trabajo damos a conocer un nuevo yacimiento descubierto recientemente en el municipio de Frigiliana. La delimitación del área denominada Los Poyos del Molinillo permitió definir la existencia de un poblado y una covacha con restos de una inhumación. El estudio de los materiales arqueológicos, que incluye cerámica, piezas metálicas o elementos de molturación, o la datación AMS obtenida, permiten adscribir el yacimiento a la Edad del Bronce y ampliar el conocimiento de esta etapa en la Axarquía oriental (Málaga, Andalucía).In this work we present a new site recently discovered in the Frigiliana municipality (Málaga). The spatial delimitation of the so called Los Poyos del Molinillo area let us define a Bronze age settlement and an inhumation burial cave. The archaeological record includes ceramic, metallic items or grinding elements, all of them belonging to the Bronze Age, as well as an AMS Radiocarbon data. This site extends the knowledge about this period in the Eastern Axarquía (Málaga, Andalucía)

    High drug resistance prevalence among vertically HIV-Infected patients transferred from pediatric care to adult units in Spain

    BACKGROUND: Antiretroviral treatment (ART) has contributed to increased life expectancy of HIV-1 infected children. In developed countries, an increasing number of children reaching adulthood are transferred to adult units. The objectives were to describe the demographic and clinical features, ART history, antiviral drug resistance and drug susceptibility in HIV-1 perinatally infected adolescents transferred to adult care units in Spain from the Madrid Cohort of HIV-1 infected children. METHODS: Clinical, virological and immunological features of HIV-1 vertically infected patients in the Madrid Cohort of HIV-infected children were analyzed at the time of transfer. Pol sequences from each patient were recovered before transfer. Resistance mutations according to the InternationaI AIDS Society 2011 list were identified and interpreted using the Stanford algorithm. Results were compared to the non-transferred HIV-1 infected pediatric cohort from Madrid. RESULTS: One hundred twelve infected patients were transferred to adult units between 1997 and 2011. They were mainly perinatally infected (93.7%), with a mean nadir CD4+-T-cells count of 10% and presented moderate or severe clinical symptoms (75%). By the time of transfer, the mean age was 18.9 years, the mean CD4+T-cells count was 627.5 cells/ml, 64.2% presented more than 350 CD4+T-cells/ml and 47.3% had ≤200 RNA-copies/ml. Most (97.3%) were ART experienced receiving Highly Active ART (HAART) (84.8%). Resistance prevalence among pretreated was 50.9%, 76.9% and 36.5% for Protease Inhibitors (PI), Nucleoside Reverse Transcriptase Inhibitors (NRTI) and Non-NRTI (NNRTI), respectively. Resistance mutations were significantly higher among transferred patients compared to non-transferred for the PI+NRTI combination (19% vs. 8.4%). Triple resistance was similar to non-transferred pediatric patients (17.3% vs. 17.6%). CONCLUSION: Despite a good immunological and virological control before transfer, we found high levels of resistance to PI, NRTI and triple drug resistance in HIV-1 infected adolescents transferred to adult units

    Alguna vez, Jean Cocteau dijo que "El tiempo de los hombres era una eternidad plegada". Si halamos del hilo de este ovillo bien podríamos decir que nuestras cotidianidades son historia que se acomoda en pequeños pliegues. El devenir histórico, aquel de cuya explicación se han ocupado los hombres, es el que se compone de las grandes hazañas y los beneméritos logros de los pueblos. Más, qué decir de la existencia de cada uno de aquellos hombres cuyo existir transcurrió en tiempos que aparentaban ser carentes de sentido, vacíos, de los que tantas veces se ha dicho ''No pasó nada". Hasta hace poco, todos estos hechos habían sido desdeñados por el quehacer histórico. Y sin embargo, para cada uno de aquellos hombres cuyos nombres ya no existen, esos acontecimientos que cupieron en sus vidas fueron, verdaderamente, historia, tan fuerte y contundente, como aquella que se narró a sí misma a través de la pluma de Michelet. La historia es como un espejo en el que el hombre se mira. Busca en él las imágenes de lo que fue en otro tiempo para con base en ellas acicalarse y vivir un presente que se engarza de manera incuestionable con el futuro. El hombre del presente se compone y se recompone teniendo siempre gran cuenta de lo que fue; porque al fin, lo que fue en el pasado perdura y es de algún modo en el presente. La historia de las mentalidades constituye en este sentido un espejo de cualidades ciertamente peculiares. En él, el hombre que somos se mira, no sólo como miembro de una sociedad, de una nación, sino también como un individuo inmerso en una cotidianidad. Será acaso por esta característica que dicha forma de historia nos resulta tan cercana, tan familiar, pues es, a fin de cuentas el espejo, no donde miramos como colectividad, sino aquel que nos refleja como individuos. Inscrita en esta historia de las mentalidades se encuentra la obra que comento. Se trata del libro llamado Amor y desamor - vivencias de parejas en las sociedad novohispana.     Real-world persistence of initial targeted therapy strategy in monotherapy versus combination therapy in patients with chronic inflammatory arthritis

    Objective: The persistence of biologic (b) and targeted synthetic (ts) disease-modifying antirheumatic drugs(DMARDs) in monotherapy versus in combination with conventional synthetic (cs) DMARDs is still a controversial topic in rheumatic diseases. To clarify this issue, the retention of the initial treatment strategy of b/tsDMARD in combination with csDMARD versus monotherapy in rheumatoid arthritis (RA), psoriatic arthritis (PsA) and ankylosing spondylitis (AS) patients under real-life conditions was evaluated. Factors associated with maintenance of the initial strategy were analysed. Methods: Nested cohort study within the Spanish BIOBADASER III registry. Bivariate comparisons and multivariate Cox proportional hazards models were used for the analyses. Results: A total of 2521 patients were included in the study. In the multivariate model, the initial strategy of combination therapy was associated with shorter persistence in patients with RA (hazard ratio [HR] 1.58;95% confidence interval [CI] 1.00-2.50; p = .049), PsA (HR 2.48; 95% CI 1.65-3.72) and AS (HR 16.77; 95% CI 7.37-38.16; p < .001), regardless of sex, time of disease progression, baseline disease activity, glucocorticoid use or type of b/tsDMARD. Overall, the combination strategy was associated with an increased incidence of adverse events (incidence rate ratio [IRR] 1.13; 95% CI 1.05-1.21). Conclusions: In this real-life study, the strategy of combining a b/tsDMARD with a csDMARD is associated with lower persistence and worse safety profile compared to monotherapy in RA and especially in PsA and AS, suggesting that combination therapy should be rethought as first choice in RA patients, but especially in PsA and AS patients.This research is supported by the Research Unit of the Spanish Society of Rheumatology.     Relationship between olive oil consumption and ankle-brachial pressure index in a population at high cardiovascular risk

    Background and aims: The aim of this study was to ascertain the association between the consumption of different categories of edible olive oils (virgin olive oils and olive oil) and olive pomace oil and ankle-brachial pressure index (ABI) in participants in the PREDIMED-Plus study, a trial of lifestyle modification for weight and cardiovascular event reduction in individuals with overweight/obesity harboring the metabolic syndrome. Methods: We performed a cross-sectional analysis of the PREDIMED-Plus trial. Consumption of any category of olive oil and olive pomace oil was assessed through a validated food-frequency questionnaire. Multivariable linear regression models were fitted to assess associations between olive oil consumption and ABI. Additionally, ABI ≤1 was considered as the outcome in logistic models with different categories of olive oil and olive pomace oil as exposure. Results: Among 4330 participants, the highest quintile of total olive oil consumption (sum of all categories of olive oil and olive pomace oil) was associated with higher mean values of ABI (beta coefficient: 0.014, 95% confidence interval [CI]: 0.002, 0.027) (p for trend = 0.010). Logistic models comparing the consumption of different categories of olive oils, olive pomace oil and ABI ≤1 values revealed an inverse association between virgin olive oils consumption and the likelihood of a low ABI (odds ratio [OR] 0.73, 95% CI [0.56, 0.97]), while consumption of olive pomace oil was positively associated with a low ABI (OR 1.22 95% CI [1.00, 1.48]). Conclusions: In a Mediterranean population at high cardiovascular risk, total olive oil consumption was associated with a higher mean ABI. These results suggest that olive oil consumption may be beneficial for peripheral artery disease prevention, but longitudinal studies are needed

Esta obra presenta diversas investigaciones y comunicaciones, con varias temáticas que pueden desglosarse en cuatro bloques: 1) En un primer grupo podemos considerar todos los capítulos que abordan la historia de los colegios farmacéuticos así como los avatares de la profesión. Se da cuenta en la provincia de Sevilla de las dificultades del Colegio de Farmacéuticos en el periodo de la Guerra Civil y la Posguerra (1936-1949), del proceso de colegiación obligatoria a partir de 1916, pinceladas históricas sobre los farmacéuticos cántabros del siglo XIX, del Colegio de Farmacéuticos de Filipinas a finales del XIX, de los conflictos de los farmacéuticos en las reuniones sanitarias de mitad del XX, y del papel de los farmacéuticos titulares en la potabilización de las aguas de consumo en Plentzia (Vizcaya). 2) Podemos destacar también todos los trabajos que giran en torno a diferentes medicamentos y productos farmacéuticos, entre ellos estudios históricos sobre piedras preciosas, medicamentos para tratar heridas, quina contra las tercianas, opio, alexifármacos, medicamentos homeopáticos, talidomida o curiosos productos como el Licor del Polo. 3) El papel de los laboratorios farmacéuticos como la Casa Nestlé durante la Guerra Civil española y el franquismo, diferentes laboratorios onubenses durante este mismo periodo, el papel del Instituto de Higiene Militar y la experimentación con insecticidas clorados sintéticos en la posguerra española, aglutinan el tercer cuerpo temático. 4) Finalmente, podemos destacar los trabajos que tienen una componente publicitaria, divulgadora y social entre los que cabe destacar el estudio del NO-DO y los diferentes noticieros y documentales sobre temas farmacéuticos que resultan muy ilustrativos. La propaganda farmacéutica desarrollada en la revista Matronas, el inventario del patrimonio farmacéutico catalán, junto a la percepción social de la farmacia a través de las fallas valencianas conforma este último grupo

    HTLV-1 infection in solid organ transplant donors and recipients in Spain

    HTLV-1 infection is a neglected disease, despite infecting 10-15 million people worldwide and severe illnesses develop in 10% of carriers lifelong. Acknowledging a greater risk for developing HTLV-1 associated illnesses due to immunosuppression, screening is being widely considered in the transplantation setting. Herein, we report the experience with universal HTLV testing of donors and recipients of solid organ transplants in a survey conducted in Spain. All hospitals belonging to the Spanish HTLV network were invited to participate in the study. Briefly, HTLV antibody screening was performed retrospectively in all specimens collected from solid organ donors and recipients attended since the year 2008. A total of 5751 individuals were tested for HTLV antibodies at 8 sites. Donors represented 2312 (42.2%), of whom 17 (0.3%) were living kidney donors. The remaining 3439 (59.8%) were recipients. Spaniards represented nearly 80%. Overall, 9 individuals (0.16%) were initially reactive for HTLV antibodies. Six were donors and 3 were recipients. Using confirmatory tests, HTLV-1 could be confirmed in only two donors, one Spaniard and another from Colombia. Both kidneys of the Spaniard were inadvertently transplanted. Subacute myelopathy developed within 1 year in one recipient. The second recipient seroconverted for HTLV-1 but the kidney had to be removed soon due to rejection. Immunosuppression was stopped and 3 years later the patient remains in dialysis but otherwise asymptomatic. The rate of HTLV-1 is low but not negligible in donors/recipients of solid organ transplants in Spain. Universal HTLV screening should be recommended in all donor and recipients of solid organ transplantation in Spain. Evidence is overwhelming for very high virus transmission and increased risk along with the rapid development of subacute myelopathy

    Spread of a SARS-CoV-2 variant through Europe in the summer of 2020

    [EN] Following its emergence in late 2019, the spread of SARS-CoV-21,2 has been tracked by phylogenetic analysis of viral genome sequences in unprecedented detail3,4,5. Although the virus spread globally in early 2020 before borders closed, intercontinental travel has since been greatly reduced. However, travel within Europe resumed in the summer of 2020. Here we report on a SARS-CoV-2 variant, 20E (EU1), that was identified in Spain in early summer 2020 and subsequently spread across Europe. We find no evidence that this variant has increased transmissibility, but instead demonstrate how rising incidence in Spain, resumption of travel, and lack of effective screening and containment may explain the variant’s success. Despite travel restrictions, we estimate that 20E (EU1) was introduced hundreds of times to European countries by summertime travellers, which is likely to have undermined local efforts to minimize infection with SARS-CoV-2. Our results illustrate how a variant can rapidly become dominant even in the absence of a substantial transmission advantage in favourable epidemiological settings. Genomic surveillance is critical for understanding how travel can affect transmission of SARS-CoV-2, and thus for informing future containment strategies as travel resumes.S
