    Education in Health care: a challenge to change in nursing care praxis

    O artigo apresenta a trajetória de implementação de uma proposta de educação em serviço do Hospital de Clínicas de Porto Ale-gre (RS) para os profissionais de enfermagem. Aborda os caminhos percorridos pelas educadoras em serviço da Escola Técnica de Enfermagem no sentido de promover mudanças nas práticas educativas que vinham sendo realizadas com base no paradigma da edu-cação continuada. A proposta está fundamentada na Política Nacional de Educação Permanente para os trabalhadores do Sistema Único de Saúde e na Política de Humanização em Saúde do Ministério da Saúde e visa à criação de espaços educativos que permitam a reflexão dialógica sobre os processos de trabalho das equipes de enfermagem. Nessa perspectiva, as mudanças na organização e no exercício da atenção são construídas no cotidiano do trabalho de maneira articulada entre gestão, atenção à saúde, ensino e pesquisa, buscando a superação das concepções tradicionais e o fortalecimento de uma nova visão de educação em saúde. Unitermos: Educação em saúde, educadores em saúde, enfermagem.This paper presents the implementation of an education proposal in health care services for nursing professionals at Hospital de Clínicas in Porto Alegre, Brazil. It approaches the routes followed by educators in service from the Nursing Technical School in the sense of promoting changes in educational practices that had been performed based on the paradigm of continuing education. The proposal is based on the National Policy for Permanent Education for those working for the Brazilian Unified Health System and on the Health Hu-manization Policy of the Brazilian Health Ministry. This experiment aims at the creation of educational spaces that can bring about dis-cussions and some reflection upon work processes by nursing teams. Under such perspective, changes in organization and attention focusing are built throughout everyday work, articulating between management, health care, teaching and research, aiming at overcom-ing the traditional concepts of education in health care and at consolidating a new paradigm in health education

    Objetivo: Descrever a construção de cenários e desenvolvimento da técnica de simulação realística em saúde sobre administração segura de medicamentos pela enfermagem. Método: Relato de experiência envolvendo as etapas de formação dos facilitadores, construção de cenários, desenvolvimento da simulação realística com a enfermagem de um hospital de ensino na Região Sul do Brasil fundamentada pelas diretrizes das melhores práticas em simulação da International Nursing Association for Clinical Simulation and Learning. A atividade foi desenvolvida de outubro de 2017 a maio de 2018. Resultados: Foram construídos quatro cenários baseados em eventos adversos ocorridos na instituição hospitalar. As enfermeiras educadoras realizaram formação como facilitadoras. Houve preocupação com a fidelidade dos cenários e com a execução das etapas de briefing e debriefing. Conclusões: A simulação permitiu a reflexão do "modo de fazer" das equipes, ajustando o processo de preparo e administração de medicamentos às recomendações institucionais com foco na segurança do processo.Objective: To describe the construction of the scenarios and the development of the realistic simulation technique in health on safe administration of drugs for nursing professionals. Method: Report of experience developed from October 2017 to May 2018 involving the steps of training of the facilitators, con- struction of the scenarios, development of the realistic simulation with nursing staff of a teaching hospital in the southern region of Brazil. The best simulation practice guidelines used from the International Nursing Association for Clinical Simulation and Learning. Results: Four practice-based scenarios were constructed based on adverse events. The nurses educators trained the facilitators partici- pating in all stages. There was concern about the fidelity of the scenarios, as well as the execution of the briefing and debriefing stages. Conclusions: The simulation allowed the reflection of the "way of doing" of the teams adjusting the process of preparation and administration of medicines to the institutional recommendations and, thus, promoting the safety of the process.Objetivo: Describir la construcción de los escenarios y el desarrollo de la técnica de simulación realista en salud sobre administración segura de medicamentos para profesionales de enfermería. Método: Relato de experiencia, desarrollado de octubre de 2017 a mayo de 2018, que involucra las etapas de formación de los facilitadores, la construcción de los escenarios, el desarrollo de la simulación realista con un equipo de enfermería de un hospital de enseñanza en la región sur de Brasil. Las directrices de las mejores prácticas en simulación utilizadas fueron de la International Nursing Association for Clinical Simulation and Learning. Resultados: Se construyeron cuatro escenarios alineados con la práctica, basados en eventos adversos. Las enfermeras educadoras realizaron formación como facilitadoras. Hubo preocupación por la fidelidad de los escenarios y, también, con la ejecución de las etapas de briefing y debriefing. Conclusiones: La simulación permitió reflexionar sobre el "modo de hacer" de los equipos, ajustando el proceso de preparación y administración de medicamentos a las recomendaciones institucionales y, de esta forma, promoviendo la seguridad del proceso

    SINEUP non-coding RNAs rescue defective frataxin expression and activity in a cellular model of Friedreich's Ataxia

    Friedreich's ataxia (FRDA) is an untreatable disorder with neuro- and cardio-degenerative progression. This monogenic disease is caused by the hyper-expansion of naturally occurring GAA repeats in the first intron of the FXN gene, encoding for frataxin, a protein implicated in the biogenesis of iron-sulfur clusters. As the genetic defect interferes with FXN transcription, FRDA patients express a normal frataxin protein but at insufficient levels. Thus, current therapeutic strategies are mostly aimed to restore physiological FXN expression. We have previously described SINEUPs, natural and synthetic antisense long non-coding RNAs, which promote translation of partially overlapping mRNAs through the activity of an embedded SINEB2 domain. Here, by in vitro screening, we have identified a number of SINEUPs targeting human FXN mRNA and capable to up-regulate frataxin protein to physiological amounts acting at the post-transcriptional level. Furthermore, FXN-specific SINEUPs promote the recovery of disease-associated mitochondrial aconitase defects in FRDA-derived cells. In summary, we provide evidence that SINEUPs may be the first gene-specific therapeutic approach to activate FXN translation in FRDA and, more broadly, a novel scalable platform to develop new RNA-based therapies for haploinsufficient diseases

    Generation and characterisation of Friedreich ataxia YG8R mouse fibroblast and neural stem cell models

    This article has been made available through the Brunel Open Access Publishing Fund.Background: Friedreich ataxia (FRDA) is an autosomal recessive neurodegenerative disease caused by GAA repeat expansion in the first intron of the FXN gene, which encodes frataxin, an essential mitochondrial protein. To further characterise the molecular abnormalities associated with FRDA pathogenesis and to hasten drug screening, the development and use of animal and cellular models is considered essential. Studies of lower organisms have already contributed to understanding FRDA disease pathology, but mammalian cells are more related to FRDA patient cells in physiological terms. Methodology/Principal Findings: We have generated fibroblast cells and neural stem cells (NSCs) from control Y47R mice (9 GAA repeats) and GAA repeat expansion YG8R mice (190+120 GAA repeats). We then differentiated the NSCs in to neurons, oligodendrocytes and astrocytes as confirmed by immunocytochemical analysis of cell specific markers. The three YG8R mouse cell types (fibroblasts, NSCs and differentiated NSCs) exhibit GAA repeat stability, together with reduced expression of frataxin and reduced aconitase activity compared to control Y47R cells. Furthermore, YG8R cells also show increased sensitivity to oxidative stress and downregulation of Pgc-1α and antioxidant gene expression levels, especially Sod2. We also analysed various DNA mismatch repair (MMR) gene expression levels and found that YG8R cells displayed significant reduction in expression of several MMR genes, which may contribute to the GAA repeat stability. Conclusions/Significance: We describe the first fibroblast and NSC models from YG8R FRDA mice and we confirm that the NSCs can be differentiated into neurons and glia. These novel FRDA mouse cell models, which exhibit a FRDA-like cellular and molecular phenotype, will be valuable resources to further study FRDA molecular pathogenesis. They will also provide very useful tools for preclinical testing of frataxin-increasing compounds for FRDA drug therapy, for gene therapy, and as a source of cells for cell therapy testing in FRDA mice. © 2014 Sandi et al

    Frataxin-deficient neurons and mice models of Friedreich ataxia are improved by TAT-MTScs-FXN treatment.

    Friedreich ataxia (FA) is a rare disease caused by deficiency of frataxin, a mitochondrial protein. As there is no cure available for this disease, many strategies have been developed to reduce the deleterious effects of such deficiency. One of these approaches is based on delivering frataxin to the tissues by coupling the protein to trans-activator of transcription (TAT) peptides, which enables cell membranes crossing. In this study, we tested the efficiency of TAT-MTScs-FXN fusion protein to decrease neurodegeneration markers on frataxin-depleted neurons obtained from dorsal root ganglia (DRG), one of the most affected tissues. In mice models of the disease, we tested the ability of TAT-MTScs-FXN to penetrate the mitochondria and its effect on lifespan. In DRG neurons, treatment with TAT-MTScs-FXN increased cell survival, decreased neurite degeneration and reduced apoptotic markers, such as α-fodrin cleavage and caspase 9 activation. Also, we show that heat-shock protein 60 (HSP60), a molecular chaperone targeted to mitochondria, suffered an impaired processing in frataxin-deficient neurons that was relieved by TAT-MTScs-FXN addition. In mice models of the disease, administration of TAT-MTScs-FXN was able to reach muscle mitochondria, restore the activity of the succinate dehydrogenase and produce a significant lifespan increase.
