    Precise Measures of Orbital Period, Before and After Nova Eruption for QZ Aurigae

    For the ordinary classical nova QZ Aurigae (which erupted in 1964), we report 1317 magnitudes from 1912--2016, including four eclipses detected on archival photographic plates from long before the eruption. We have accurate and robust measures of the orbital period both pre-eruption and post-eruption, and we find that the orbital period decreased, with a fractional change of -290.71+-0.28 parts-per-million across the eruption, with the orbit necessarily getting smaller. Further, we find that the light curve outside of eclipses and eruption is flat at near B=17.14 from 1912--1981, whereupon the average light curve starts fading down to B=17.49 with large variability. QZ Aur is a robust counter-example against the Hibernation model for the evolution of cataclysmic variables, where the model requires that all novae have their period increase across eruptions. Large period decreases across eruptions can easily arise from mass imbalances in the ejecta, as are commonly seen in asymmetric nova shells.Comment: MNRAS in press, 24 pages, 5 tables, 6 figure

    XMM-Newton observation of the long-period polar V1309 Ori: The case for pure blobby accretion

    Using XMM-Newton we have obtained the first X-ray observation covering a complete orbit of the longest period polar, V1309 Ori. The X-ray light curve is dominated by a short, bright phase interval with EPIC pn count rates reaching up to 15 cts/sec per 30 sec resolution bin. The bright phase emission is well described by a single blackbody component with kT_bb = (45 +- 3) eV. The absence of a bremsstrahlung component at photon energies above 1 keV yields a flux ratio F_bb/F_br > 6700. This represents the most extreme case of a soft X-ray excess yet observed in an AM Herculis star. The bright, soft X-ray emission is subdivided into a series of individual flare events supporting the hypothesis that the soft X-ray excess in V1309 is caused by accretion of dense blobs. In addition to the bright phase emission, a faint, hard X-ray component is visible throughout the binary orbit with an almost constant count rate of 0.01 cts/sec. Spectral modelling indicates that this emission originates from a complex multi-temperature plasma. At least three components of an optically thin plasma with temperatures kT= 0.065, 0.7, and 2.9 keV are required to fit the observed flux distribution. The faint phase emission is occulted during the optical eclipse. Eclipse ingress lasts about 15--20 min and is substantially prolonged beyond nominal ingress of the white dwarf. This and the comparatively low plasma temperature provide strong evidence that the faint-phase emission is not thermal bremsstrahlung from a post-shock accretion column above the white dwarf. A large fraction of the softer faint-phase emission could be explained by scattering of photons from the blackbody component in the infalling material above the accretion region.     The MATISSE study: a randomised trial of group art therapy for people with schizophrenia.

Group Art Therapy has been widely used as an adjunctive treatment for people with schizophrenia but there have been few attempts to examine its effects and cost effectiveness has not been examined. The MATISSE study aims to evaluate the clinical and cost effectiveness of group Art Therapy for people with schizophrenia. METHOD/DESIGN: The MATISSE study is a three-arm, parallel group, pragmatic, randomised, controlled trial of referral to group Art Therapy plus standard care, referral to an attention control 'activity' group plus standard care, or standard care alone. Study participants were recruited from inpatient and community-based mental health and social care services at four centres in England and Northern Ireland. Participants were aged over 18 years with a clinical diagnosis of schizophrenia, confirmed by an examination of case notes using operationalised criteria. Participants were then randomised via an independent and remote telephone randomisation service using permuted stacked blocks, stratified by site. Art Therapy and activity groups were made available to participants once a week for up to 12 months. Outcome measures were assessed by researchers masked to allocation status at 12 and 24 months after randomisation. Participants and care givers were aware which arm of the trial participants were allocated to. The primary outcomes for the study are global functioning (measured using the Global Assessment of Functioning scale) and mental health symptoms (measured using the Positive and Negative Syndrome Scale) assessed at 24 months. Secondary outcomes were assessed at 12 and 24 months and comprise levels of group attendance, social function, satisfaction with care, mental wellbeing, and costs.     Recommendations for the detection and diagnosis of Niemann-Pick disease type C: An update.

    PURPOSE OF REVIEW: Niemann-Pick disease type C (NP-C) is a neurovisceral disorder that may be more prevalent than earlier estimates. Diagnosis of NP-C is often delayed; a key aim for clinical practice is to reduce this delay. Recently, substantial progress has been made in the field of NP-C screening and diagnosis, justifying an update to the existing recommendations for clinical practice. RECENT FINDINGS: New biomarker profiling and genetic analysis technologies are included as first-line diagnostic tests for NP-C. Most diagnoses can now be confirmed by combination of biomarker and genetic analyses. Filipin staining may facilitate diagnosis in uncertain cases. Recommendations are provided for psychiatrists, neuro-ophthalmologists, and radiologists, and on screening within specific at-risk patient cohorts. The NP-C diagnostic algorithm has been updated and simplified. SUMMARY: This publication provides expert recommendations for clinicians who may see patients presenting with the signs and symptoms of NP-C, including general practitioners, pediatricians, neurologists, and psychiatrists

    A comparison of tuberculous and bacterial native joint septic arthritis infections in a retrospective cohort: presentation characteristics, outcomes and long term follow up

    Objectives This retrospective observational cohort study aimed to characterise and compare the demographics, initial laboratory tests and outcomes between patients with large-joint bacterial septic arthritis (BSA) and tuberculous septic arthritis (TBSA). Methods All patients with a culture from a large, native joint growing either non-mycobacterial bacteria or Mycobacterium tuberculosis between 1 January 2012 and 1 October 2018 in our institution were included. Clinical details and admission laboratory values were obtained from patient records. Comparisons were made by Mann-Whitney U, chi-squared tests, and logistic regression analysis. Results We identified 64 BSA and 29 TBSA. On average, the BSA cases were older, had higher CRP levels and neutrophil counts and lower albumin levels. The odds ratio for having a BSA was 46 in cases with a CRP greater than 100 mg/L (95% confidence interval (CI) 8.5–850, p < 0.001) and 24 with a neutrophil count greater than 7.5x109 (95% CI 6.1–160, p < 0.001). 51% of BSA were asymptomatic at last follow up compared to 72% of TBSA. 14% of the BSA cases died during admission; there were no deaths in the TBSA group. Conclusions Significant differences exist between patients with BSA and TBSA. Whilst no test is sufficient to exclude BSA, a raised neutrophil count or a CRP greater than 100 mg/L significantly increases the odds of a bacterial aetiology. Patients with BSA had worse long-term outcomes and higher incidence of inpatient mortality

    Group art therapy as an adjunctive treatment for people with schizophrenia: a randomised controlled trial (MATISSE).

    OBJECTIVE: To examine the clinical effectiveness and cost-effectiveness of referral to group art therapy plus standard care, compared with referral to an activity group plus standard care and standard care alone, among people with schizophrenia. DESIGN: A three-arm, parallel group, single-blind, pragmatic, randomised controlled trial. Participants were randomised via an independent and remote telephone randomisation service using permuted blocks, stratified by study centre. SETTING: Study participants were recruited from secondary care mental health and social services in four UK centres. PARTICIPANTS: Potential participants were aged 18 years or over, had a clinical diagnosis of schizophrenia, confirmed by an examination of case notes, and provided written informed consent. We excluded those who were unable to speak sufficient English to complete the baseline assessment, those with severe cognitive impairment and those already receiving arts therapy. INTERVENTIONS: Group art therapy was delivered by registered art therapists according to nationally agreed standards. Groups had up to eight members, lasted for 90 minutes and ran for 12 months. Members were given access to a range of art materials and encouraged to use these to express themselves freely. Activity groups were designed to control for the non-specific effects of group art therapy. Group facilitators offered various activities and encouraged participants to collectively select those they wanted to pursue. Standard care involved follow-up from secondary care mental health services and the option of referral to other services, except arts therapies, as required. MAIN OUTCOME MEASURES: Our co-primary outcomes were global functioning (measured using the Global Assessment of Functioning Scale - GAF) and mental health symptoms (measured using the Positive and Negative Syndrome Scale - PANSS) at 24 months. The main secondary outcomes were level of group attendance, social functioning, well-being, health-related quality of life, service utilisation and other costs measured 12 and 24 months after randomisation. RESULTS: Four hundred and seventeen people were recruited, of whom 355 (85%) were followed up at 2 years. Eighty-six (61%) of those randomised to art therapy and 73 (52%) of those randomised to activity groups attended at least one group. No differences in primary outcomes were found between the three study arms. The adjusted mean difference between art therapy and standard care at 24 months was -0.9 [95% confidence interval (CI) -3.8 to 2.1] on the GAF Scale and 0.7 (95% CI -3.1 to 4.6) on the PANSS Scale. Differences in secondary outcomes were not found, except that those referred to an activity group had fewer positive symptoms of schizophrenia at 24 months than those randomised to art therapy. Secondary analysis indicated that attendance at art therapy groups was not associated with improvements in global functioning or mental health. Although the total cost of the art therapy group was lower than the cost of the two comparison groups, referral to group art therapy did not appear to provide a cost-effective use of resources. CONCLUSIONS: Referring people with established schizophrenia to group art therapy as delivered in this randomised trial does not appear to improve global functioning or mental health of patients or provide a more cost-effective use of resources than standard care alone.     Red and green loops help uncover missing feedbacks in a coral reef social–ecological system

    Social–ecological systems (SES) exhibit complex cause‐and‐effect relationships. Capturing, interpreting, and responding to signals that indicate changes in ecosystems is key for sustainable management in SES. Breaks in this signal–response chain, when feedbacks are missing, will allow change to continue until a point when abrupt ecological surprises may occur. In these situations, societies and local ecosystems can often become uncoupled. In this paper, we demonstrate how the red loop–green loop (RL–GL) concept can be used to uncover missing feedbacks and to better understand past social–ecological dynamics. Reinstating these feedbacks in order to recouple the SES may ultimately create more sustainable systems on local scales. The RL–GL concept can uncover missing feedbacks through the characterization of SES dynamics along a spectrum of human resource dependence. Drawing on diverse qualitative and quantitative data sources, we classify SES dynamics throughout the history of Jamaican coral reefs along the RL–GL spectrum. We uncover missing feedbacks in red‐loop and red‐trap scenarios from around the year 600 until now. The Jamaican coral reef SES dynamics have moved between all four dynamic states described in the RL–GL concept: green loop, green trap, red loop and red trap. We then propose mechanisms to guide the current unsustainable red traps back to more sustainable green loops, involving mechanisms of seafood trade and ecological monitoring. By gradually moving away from seafood exports, Jamaica may be able to return to green‐loop dynamics between the local society and their locally sourced seafood. We discuss the potential benefits and drawbacks of this proposed intervention and give indications of why an export ban may insure against future missing feedbacks and could prolong the sustainability of the Jamaican coral reef ecosystem. Our approach demonstrates how the RL–GL approach can uncover missing feedbacks in a coral reef SES, a way the concept has not been used before. We advocate for how the RL–GL concept in a feedback setting can be used to synthesize various types of data and to gain an understanding of past, present and future sustainability that can be applied in diverse social–ecological settings

    Circumstellar Disks at White Dwarfs: Observations

    The current picture painted by the observations of circumstellar dust at white dwarfs, and the consequent atmospheric pollution, is of a surviving planetary system. This chapter recounts in detail both the discovery and empirical characterization of dust disks at single white dwarfs, including all observational data available up to early 2011. Observations of the disks themselves and the accreted heavy elements within the stellar photospheres are consistent with tidally disrupted asteroid analogs. The observed chemistry supports objects that condensed within the inner regions of main-sequence stars and thus far appear similar in constitution to the terrestrial planets and their building blocks.Comment: Chapter from the book "White Dwarf Atmospheres and Circumstellar Environments", ed.
