    Microbiology and atmospheric processes: Biological, physical and chemical characterization of aerosol particles

    The interest in bioaerosols has traditionally been linked to health hazards for humans, animals and plants. However, several components of bioaerosols exhibit physical properties of great significance for cloud processes, such as ice nucleation and cloud condensation. To gain a better understanding of their influence on climate, it is therefore important to determine the composition, concentration, seasonal fluctuation, regional diversity and evolution of bioaerosols. In this paper, we will review briefly the existing techniques for detection, quantification, physical and chemical analysis of biological particles, attempting to bridge physical, chemical and biological methods for analysis of biological particles and integrate them with aerosol sampling techniques. We will also explore some emerging spectroscopy techniques for bulk and single-particle analysis that have potential for in-situ physical and chemical analysis. Lastly, we will outline open questions and further desired capabilities (e. g., in-situ, sensitive, both broad and selective, on-line, time-resolved, rapid, versatile, cost-effective techniques) required prior to comprehensive understanding of chemical and physical characterization of bioaerosols

    Which patients should be transported to the emergency department? A perpetual prehospital dilemma

    Objective: To examine the ability of paramedics to identify patients who could be managed in the community and to identify predictors that could be used to accurately identify patients who should be transported to EDs. Methods: Lower acuity patients who were assessed by paramedics in the Perth metropolitan area in 2013 were studied. Paramedics prospectively indicated on the patient care record if they considered that the patient could be treated in the community. The paramedic decisions were compared with actual disposition from the ED (discharge and admission), and the occurrence of subsequent events (ambulance request, ED visit, admission and death) for discharged patients at the scene was investigated. Decision tree analysis was used to identify predictors that were associated with hospital admission. Results: In total, 57183 patients were transported to the ED, and 10204 patients were discharged at the scene by paramedics. Paramedics identified 2717 patients who could potentially be treated in the community among those who were transported to the ED. Of these, 1455 patients (53.6%) were admitted to hospital. For patients discharged at the scene, those who were indicated as suitable for community care were more likely to experience subsequent events than those who were not. The decision tree found that two predictors (age and aetiology) were associated with hospital admission. Overall discriminative power of the decision tree was poor; the area under the receiver operating characteristic curve was 0.686. Conclusion: Lower acuity patients who could be treated in the community were not accurately identified by paramedics.     Mechanisms by which sialylated milk oligosaccharides impact bone biology in a gnotobiotic mouse model of infant undernutrition

    Undernutrition in children is a pressing global health problem, manifested in part by impaired linear growth (stunting). Current nutritional interventions have been largely ineffective in overcoming stunting, emphasizing the need to obtain better understanding of its underlying causes. Treating Bangladeshi children with severe acute malnutrition with therapeutic foods reduced plasma levels of a biomarker of osteoclastic activity without affecting biomarkers of osteoblastic activity or improving their severe stunting. To characterize interactions among the gut microbiota, human milk oligosaccharides (HMOs), and osteoclast and osteoblast biology, young germ-free mice were colonized with cultured bacterial strains from a 6-mo-old stunted infant and fed a diet mimicking that consumed by the donor population. Adding purified bovine sialylated milk oligosaccharides (S-BMO) with structures similar to those in human milk to this diet increased femoral trabecular bone volume and cortical thickness, reduced osteoclasts and their bone marrow progenitors, and altered regulators of osteoclastogenesis and mediators of Th2 responses. Comparisons of germ-free and colonized mice revealed S-BMO-dependent and microbiota-dependent increases in cecal levels of succinate, increased numbers of small intestinal tuft cells, and evidence for activation of a succinate-induced tuft cell signaling pathway linked to Th2 immune responses. A prominent fucosylated HMO, 2'-fucosyllactose, failed to elicit these changes in bone biology, highlighting the structural specificity of the S-BMO effects. These results underscore the need to further characterize the balance between, and determinants of, osteoclastic and osteoblastic activity in stunted infants/children, and suggest that certain milk oligosaccharides may have therapeutic utility in this setting

    Evidence-based paramedic models of care to reduce unnecessary emergency department attendance – feasibility and safety

    Background: As demand for Emergency Department (ED) services continues to exceed increases explained by population growth, strategies to reduce ED presentations are being explored. The concept of ambulance paramedics providing an alternative model of care to the current default ‘see and transport to ED’ has intuitive appeal and has been implemented in several locations around the world. The premise is that for certain noncritical ill patients, the Extended Care Paramedic (ECP) can either ‘see and treat’ or ‘see and refer’ to another primary or community care practitioner, rather than transport to hospital. However, there has been little rigorous investigation of which types of patients can be safely identified and managed in the community, or the impact of ECPs on ED attendance.Methods/Design: St John Ambulance Western Australia paramedics will indicate on the electronic patient care record (e-PCR) of patients attended in the Perth metropolitan area if they consider them to be suitable to be managed in the community. ‘Follow-up’ will examine these patients using ED data to determine the patient’s disposition from the ED. A clinical panel will then develop a protocol to identify those patients who can be safely managed in the community. Paramedics will then assess patients against the derived ECP protocols and identify those deemed suitable to ‘see and treat’ or ‘see and refer’. The ED disposition (and other clinical outcomes) of these ‘ECP protocol identified’ patients will enable us to assess whether it would have been appropriate to manage these patients in the community. We will also ‘track’ re-presentations to EDs within seven days of the initial presentation. This is a ‘virtual experiment’ with no direct involvement of patients or changes in clinical practice. A systems modelling approach will be used to assess the likely impact on ED crowding.Discussion: To date the efficacy, cost-effectiveness and safety of alternative community-based models of emergency care have not been rigorously investigated. This study will inform the development of ECP protocols through the identification of types of patient presentation that can be considered both safe and appropriate for paramedics to manage in the community

    Controlled trial of an mHealth intervention to promote healthy behaviours in adolescence (TeenPower): Effectiveness analysis

    Aim To evaluate the effectiveness on lifestyle change of an mHealth intervention to promote healthy behaviours in adolescence (TeenPower) and to analyse the predictors of the mHealth intervention effectiveness. Design This study is designed as a non‐randomized controlled trial with a two‐arm structure. Methods Adolescents of 12–16‐year old were recruited from three school districts, with access to the Internet and smartphone/tablet devices. The intervention group was invited to engage in the mHealth intervention (TeenPower) for 6 months in addition to a school‐based intervention. The control group only followed the school‐based intervention. A repeated measures factorial ANOVA was used and the main effectiveness outcome was the lifestyle change measured by the adolescent lifestyle profile. Results The outcomes of the mHealth intervention (TeenPower) show a significant effect on nutrition (ƞ2p = 0.03, p = .03), positive life perspective (ƞ2p = 0.04, p = .01), and global lifestyle (ƞ2p = 0.02, p = .05), with a dropout rate of 62.1%. The analysis of the effectiveness predictors of the mHealth intervention suggested that older adolescents tended to show a significant increase in the rates of stress management (r = .40; p < .05). Conclusions Although the considerable dropout rate, the mHealth intervention presented significant impact on multiple lifestyle domains, providing support for the effectiveness of mHealth interventions for health promotion as an add‐on to standard interdisciplinary interventions. Impact Adolescents must have the necessary and appropriate knowledge for the correct and responsible decision‐making regarding their health and lifestyle. Innovative strategies (mHealth intervention) were used to promote healthy behaviours. This study evaluates the effectiveness of an mHealth intervention (TeenPower) specifically designed for adolescents.     Transmission spectroscopy of the inflated exoplanet WASP-52b, and evidence for a bright region on the stellar surface

    We have measured the transmission spectrum of the extremely inflated hot Jupiter WASP-52b using simultaneous photometric observations in Sloan Digital Sky Survey u΄, g΄ and a filter centred on the sodium doublet (Na i) with the ULTRACAM instrument mounted on the 4.2-m William Herschel Telescope. We find that Rayleigh scattering is not the dominant source of opacity within the planetary atmosphere and find a transmission spectrum more consistent with wavelength-independent opacity such as from clouds. We detect an in-transit anomaly that we attribute to the presence of stellar activity and find that this feature can be more simply modelled as a bright region on the stellar surface akin to solar faculae rather than spots. A spot model requires a significantly larger planet/star radius ratio than that found in previous studies. Our results highlight the precision that can be achieved by ground-based photometry with errors in the scaled planetary radii of less than one atmospheric scale height, comparable to Hubble Space Telescope observations

    The Unintended Consequences of a Ban on the Humane Slaughter (Processing) of Horses in the United States

    Federal legislation has been proposed to amend the Horse Protection Act to prohibit the shipping, transporting, moving, delivering, receiving, possessing, purchasing, selling, or donation of horses and other equines to be humanely slaughtered (processed) for human consumption, and for other purposes. The intent of the legislation is to enact a ban in the United States on processing horses for human consumption. The legislation does not provide fiscal support that would likely be needed to respond to an ever increasing number of unwanted, neglected, and abused horses. Often times horse neglect and abuse cases originate from a lack of economic resources needed to adequately maintain a horse’s health. While everyone fully supports and is committed to the humane treatment of all horses, there are unintended consequences of banning horse processing. The purpose of this paper is to identify and review the unintended consequences of a ban in the United States on the processing of horses for human consumption: 1. The potential for a large number of abandoned or unwanted horses is substantial. 2. Public animal rescue facilities are currently saturated with unwanted horses. No funding has been allocated to manage a large increase in horses that will likely become the responsibility of these facilities. 3. Cost of maintaining unwanted horses accumulates over time: A conservative estimate of the total cost of caring for unwanted horses, based upon 2005 statistics, is 220million;Cumulativeannualmaintenancecostsofotherwiseprocessedhorses,sincetheyear2000,wouldhaveexceededmorethan220 million; Cumulative annual maintenance costs of otherwise processed horses, since the year 2000, would have exceeded more than 513 million in 2005. 4. The export value of horse meat for human consumption was approximately $26 million. A ban on processing would eliminate these annual revenues. 5. The option of rendering equine carcasses is decreasing. Private-land burial and disposal in landfills have a negative impact on the environment. 6. The Bureau of Land Management’s Wild Horse and Burro Adoption Program may be negatively impacted by a ban on horse processing. BLM horses and an increasing number of unwanted horses will be competing for adoption homes. Horse processing facilities offer a humane end-of-life option for approximately 1% of the United States horse population. Tens of thousands of horses could be neglected or abandoned if a processing ban were imposed. The direct economic impact and future unintended–and currently unaccounted for–economic impact of banning horse processing for human consumption are substantial. Proponents have not addressed the inevitable costs of such a ban. Horse owners will realize a direct impact from lower horse sale prices. Local and state governments will be adversely impacted by increased costs of regulation and care of unwanted or neglected horses

    Large Extra Dimensions and Decaying KK Recurrences

    We suggest the possibility that in ADD type brane-world scenarios, the higher KK excitations of the graviton may decay to lower ones owing to a breakdown of the conservation of extra dimensional ``momenta'' and study its implications for astrophysics and cosmology. We give an explicit realization of this idea with a bulk scalar field Φ\Phi, whose nonzero KK modes acquire vacuum expectation values. This scenario helps to avoid constraints on large extra dimensions that come from gamma ray flux bounds in the direction of nearby supernovae as well as those coming from diffuse cosmological gamma ray background. It also relaxes the very stringent limits on reheat temperature of the universe in ADD models.Comment: 16 pages, late
