    Evaluación de patología y vulnerabilidad en edificios patrimoniales de mampostería ubicados en zona de alta sismicidad: un caso de estudio

    La Secretaría de Turismo del Gobierno de Mendoza, Argentina, funciona en un edificio de mampostería monumental que data de 1913 ubicado en el microcentro de la Ciudad de Mendoza, que fue originariamente el Jockey Club Mendoza, con detalles arquitectónicos en herrería, vidrio y madera, que aún se conservan, pero hoy presenta patologías estructurales por eventos sísmicos y de mantenimiento por problemas de suelo y pérdidas en instalaciones. Las etapas del trabajo son: un seguimiento de antecedentes históricos, la evaluación de patologías y el estudio de los materiales utilizados, incluido el suelo, y el análisis de las condiciones de seguridad del edificio, previo a la definición definitiva del proyecto arquitectónico que se realiza por concurso público. La metodología utilizada incluye las siguientes etapas: relevamiento, decisiones de emergencia, análisis de las condiciones de conservación del edificio, diagnóstico y criterios sismorresistentes para la propuesta de rehabilitación La capacidad sismorresistente se evalúa de acuerdo a la normativa vigente en la Ciudad de Mendoza de 1987 que establece las pautas de calidad, resistencia y seguridad que deben alcanzar las construcciones que no cumplimentan los reglamentos sismorresistentes y que se deben aplicar obligatoriamente en edificios de uso público. Se analizan distintas propuestas de rehabilitación con técnicas de rehabilitación verificadas en laboratorio para la región, según distintos grados de valoración patrimonial de lo existente. En la propuesta del nuevo “Centro de Interpretación de Mendoza” se deben considerar como determinantes las condiciones del sitio y del suelo en relación a la vulnerabilidad edilicia y a la seguridad estructural, tener especial cuidado en el diseño de instalaciones sanitarias en relación a la vida útil que quiera otorgarse a la construcción y considerar la utilización de técnicas de reparación asociadas a las técnicas constructivas originales de la construcción en mampostería y hormigón armado para optimizar costos y alcanzar los requerimientos normativos.Tópico 6: Patrimonio Urbano de los siglos XVIII al XX.     Vesicoureteral reflux (VUR) is the abnormal retrograde flow of urine from the bladder into the upper urinary tract – ureters and kidneys. According to the extent of the abnormal flow through the urinary tract and the severity of the lesions in the renal collecting system (caused by the reflux), both assessed by voiding cystourethrogram, this pathology may be classified in 5 different grades (I to V).VUR can be suspected prenatally by ultrasound findings of fetal hydronephrosis. Postnatally, on the other hand, this condition is usually diagnosed either after an investigation in an infant presenting with recurrent urinary tract infections (UTIs) or during screening of infants with a positive family history for significant VUR. The majority of infant patients who suffer from VUR will be cured without any intervention as they grow old. However, some of them will experience recurrent UTIs, which may result in kidney scarring. In terms of how to treat children with VUR, it is far from being a consensus among the experts. Some advocate a prophylactic antibiotic therapy only, whereas others believe that a surgical procedure is usually necessary. Our goal is to review the main aspects related to classification, clinical features, diagnosis, complications and treatment of children population affected with VUR.O Refluxo Vésico-Ureteral é definido como a entidade clínica em que ocorre fluxo retrógrado de urina da bexiga em direção aos ureteres. Conforme o grau do refluxo de urina e a severidade das alterações ocasionadas no sistema coletor renal demonstrado por uretrocistografia miccional, a doença é graduada de I a V, da menor para a maior gravidade. O diagnóstico pré-natal de RVU pode ser presumido através da evidência ultrassonográfica de hidronefrose fetal, enquanto que o diagnóstico pós-natal de RVU geralmente é feito após a ocorrência de infecção de trato urinário ou durante o rastreamento de crianças descendentes de famílias com altas taxas de RVU. A maioria dos pacientes apresentará resolução espontânea do RVU, alguns porém evoluem com infecções de trato urinário de repetição e formações de cicatrizes renais. O tratamento do refluxo vésico-ureteral em crianças é bastante controverso na literatura, variando desde conduta conservadora, com antibioticoprofilaxia de longo prazo, até procedimentos cirúrgicos abertos ou endoscópicos.     Food consumption and eating behavior among Brazilian adolescents: National Adolescent School-based Health Survey (PeNSE), 2009

    O objetivo deste artigo é descrever características de consumo e comportamento alimentar de adolescentes brasileiros e sua associação com fatores sociodemográficos. Estudou-se, em 2009, amostra representativa de alunos do 9º ano do ensino fundamental de escolas públicas e privadas das 26 capitais brasileiras e do Distrito Federal. Utilizou-se questionário autoaplicável sobre atributos sociodemográficos, consumo e comportamento alimentar, entre outros. Estimativas dos indicadores construídos foram apresentadas para o total da população e por sexo. A associação de cada um dos indicadores com variáveis sociodemográficas foi examinada por meio de regressão logística. A maioria dos adolescentes consumia regularmente feijão (62,6%), leite (53,6%) e guloseimas (50,9%), realizava pelo menos o almoço ou o jantar com a mãe ou responsável (62,6%) e comia assistindo televisão ou estudando (50,9%). Em geral, as meninas estavam mais expostas a práticas alimentares não desejáveis, e o melhor nível socioeconômico associou-se a maiores prevalências dos indicadores estudados. Os resultados revelaram consumo regular dos marcadores de alimentação não saudável e consumo inferior ao recomendado dos de alimentação saudável, apontando a necessidade de ações de promoção de saúde dirigidas a jovens.The objective of this article is to describe the characteristics of food consumption and eating behavior of adolescents and its association with socio-demographic factors. In 2009, a random sample of students in 9th grade of elementary education at public and private schools from 26 Brazilian state capitals and Federal District was studied. It was applied a self-administered questionnaire with socio-demographic attributes, food consumption and eating behavior, among others. Estimates of the constructed indicators were presented for the total population and by sex. The association of each indicator with socio-demographic variables was examined by logistic regression. The results showed that over half of adolescents presented frequent consumption of beans (62.6%), milk (53.6%) and sweets (50.9%), and held at least lunch or dinner with the mother or responsible (62.6%) and watching television or studying (50.9%). In general, girls were more exposed to undesirable eating habits and higher socioeconomic status was associated with a higher prevalence of the indicators studied. The results revealed regular consumption of unhealthy diet markers and consumption of less than the recommended for a healthy diet, pointing the need for strengthening health promotion activities targeting young people

    Molecular investigation of isolates from a multistate polymicrobial outbreak associated with contaminated total parenteral nutrition in Brazil

    Background: Between November 2013 and June 2014, 56 cases of bacteremia (15 deaths) associated with the use of Total Parenteral Nutrition (TPN) and/or calcium gluconate (CG) were reported in four Brazilian states. Methods: We analyzed 73 bacterial isolates from four states: 45 from blood, 25 from TPN and three from CG, originally identified as Acinetobacter baumannii, Rhizobium radiobacter, Pantoea sp. or Enterobacteriaceae using molecular methods. Results: The first two bacterial species were confirmed while the third group of species could not be identified using standard identification protocols. These isolates were subsequently identified by Multi-Locus Sequence Analysis as Phytobacter diazotrophicus, a species related to strains from similar outbreaks in the United States in the 1970’s. Within each species, TPN and blood isolates proved to be clonal, whereas the R. radiobacter isolates retrieved from CG were found to be unrelated. Conclusion: This is the first report of a three-species outbreak caused by TPN contaminated with A. baumannii, R. radiobacter and P. diazotrophicus. The concomitant presence of clonal A. baumannii and P. diazotrophicus isolates in several TPN and blood samples, as well as the case of one patient, where all three different species were isolated simultaneously, suggest that the outbreak may be ascribed to a discrete contamination of TPN. In addition, this study highlights the clinical relevance of P. diazotrophicus, which has been involved in outbreaks in the past, but was often misidentified as P. agglomerans

    Keras R-CNN: library for cell detection in biological images using deep neural networks

    Background: A common yet still manual task in basic biology research, high-throughput drug screening and digital pathology is identifying the number, location, and type of individual cells in images. Object detection methods can be useful for identifying individual cells as well as their phenotype in one step. State-of-the-art deep learning for object detection is poised to improve the accuracy and efficiency of biological image analysis. Results: We created Keras R-CNN to bring leading computational research to the everyday practice of bioimage analysts. Keras R-CNN implements deep learning object detection techniques using Keras and Tensorflow (https://github.com/broadinstitute/keras-rcnn). We demonstrate the command line tool’s simplified Application Programming Interface on two important biological problems, nucleus detection and malaria stage classification, and show its potential for identifying and classifying a large number of cells. For malaria stage classification, we compare results with expert human annotators and find comparable performance. Conclusions: Keras R-CNN is a Python package that performs automated cell identification for both brightfield and fluorescence images and can process large image sets.     Arterial spin labelling MRI to measure renal perfusion: a systematic review and statement paper

    Renal perfusion provides the driving pressure for glomerular filtration and delivers the oxygen and nutrients to fuel solute reabsorption. Renal ischaemia is a major mechanism in acute kidney injury and may promote the progression of chronic kidney disease. Thus, quantifying renal tissue perfusion is critically important for both clinicians and physiologists. Current reference techniques for assessing renal tissue perfusion have significant limitations. Arterial spin labelling (ASL) is a magnetic resonance imaging (MRI) technique that uses magnetic labelling of water in arterial blood as an endogenous tracer to generate maps of absolute regional perfusion without requiring exogenous contrast. The technique holds enormous potential for clinical use but remains restricted to research settings. This statement paper from the PARENCHIMA network briefly outlines the ASL technique and reviews renal perfusion data in 53 studies published in English through January 2018. Renal perfusion by ASL has been validated against reference methods and has good reproducibility. Renal perfusion by ASL reduces with age and excretory function. Technical advancements mean that a renal ASL study can acquire a whole kidney perfusion measurement in less than 5–10min. The short acquisition time permits combination with other MRI techniques that might inform drug mechanisms and renal physiology. The flexibility of renal ASL has yielded several variants of the technique, but there are limited data comparing these approaches. We make recommendations for acquiring and reporting renal ASL data and outline the knowledge gaps that future research should address

    Technical recommendations for clinical translation of renal MRI: a consensus project of the Cooperation in Science and Technology Action PARENCHIMA.

    PURPOSE: The potential of renal MRI biomarkers has been increasingly recognised, but clinical translation requires more standardisation. The PARENCHIMA consensus project aims to develop and apply a process for generating technical recommendations on renal MRI. METHODS: A task force was formed in July 2018 focused on five methods. A draft process for attaining consensus was distributed publicly for consultation and finalised at an open meeting (Prague, October 2018). Four expert panels completed surveys between October 2018 and March 2019, discussed results and refined the surveys at a face-to-face meeting (Aarhus, March 2019) and completed a second round (May 2019). RESULTS: A seven-stage process was defined: (1) formation of expert panels; (2) definition of the context of use; (3) literature review; (4) collection and comparison of MRI protocols; (5) consensus generation by an approximate Delphi method; (6) reporting of results in vendor-neutral and vendor-specific terms; (7) ongoing review and updating. Application of the process resulted in 166 consensus statements. CONCLUSION: The process generated meaningful technical recommendations across very different MRI methods, while allowing for improvement and refinement as open issues are resolved. The results are likely to be widely supported by the renal MRI community and thereby promote more harmonisation
