    Prevalence of left ventricular diastolic dysfunction in European populations based on cross-validated diagnostic thresholds

    BACKGROUND: Different diagnostic criteria limit comparisons between populations in the prevalence of diastolic left ventricular (LV) dysfunction. We aimed to compare across populations age-specific echocardiographic criteria for diastolic LV dysfunction as well as their correlates and prevalence. METHODS: We measured the E and A peaks of transmitral blood flow by pulsed wave Doppler and the e' and a' peaks of mitral annular velocities by tissue Doppler imaging (TDI) in 2 cohorts randomly recruited in Belgium (n = 782; 51.4% women; mean age, 51.1 years) and in Italy, Poland and Russia (n = 476; 55.7%; 44.5 years). RESULTS: In stepwise regression, the multivariable-adjusted correlates of the transmitral and TDI diastolic indexes were similar in the 2 cohorts and included sex, age, body mass index, blood pressure and heart rate. Similarly, cut-off limits for the E/A ratio (2.5th percentile) and E/e' ratio (97.5th percentile) in 338 and 185 reference subjects free from cardiovascular risk factors respectively selected from both cohorts were consistent within 0.02 and 0.26 units (median across 5 age groups). The rounded 2.5th percentile of the E/A ratio decreased by ~0.10 per age decade in these apparently healthy subjects. The reference subsample provided age-specific cut-off limits for normal E/A and E/e' ratios. In the 2 cohorts combined, diastolic dysfunction groups 1 (impaired relaxation), 2 (possible elevated LV filling pressure) and 3 (elevated E/e' and abnormally low E/A) encompassed 114 (9.1%), 135 (10.7%), and 40 (3.2%) subjects, respectively. CONCLUSIONS: The age-specific criteria for diastolic LV dysfunction were highly consistent across the study populations with an age-standardized prevalence of 22.4% vs. 25.1%

    Assessing machine learning for diagnostic classification of hypertension types identified by ambulatory blood pressure monitoring

    Background: Inaccurate blood pressure classification results in inappropriate treatment. We tested if machine learning (ML), using routine clinical data, can serve as a reliable alternative to Ambulatory Blood Pressure Monitoring (ABPM) in classifying blood pressure status. Methods: This study employed a multi-centre approach involving three derivation cohorts from Glasgow, Gdańsk, and Birmingham, and a fourth independent evaluation cohort. ML models were trained using office BP, ABPM, and clinical, laboratory, and demographic data, collected from patients referred for hypertension assessment. Seven ML algorithms were trained to classify patients into five groups: Normal/Target, Hypertension-Masked, Normal/Target-White-Coat, Hypertension-White-Coat, and Hypertension. The 10-year cardiovascular outcomes and 27-year all-cause mortality risks were calculated for the ML-derived groups using the Cox proportional hazards model. Results: Overall XGBoost showed the highest AUROC of 0.85-0.88 across derivation cohorts, Glasgow (n=923; 43% females; age 50.7±16.3 years), Gdańsk (n=709; 46% females; age 54.4±13 years), and Birmingham (n=1,222; 56% females; age 55.7±14 years). But accuracy (0·57-0·72) and F1 scores (0·57-0·69) were low across the three patient cohorts. The evaluation cohort (n=6213, 51% females; age 51.2±10.8 years) indicated elevated 10-year risks of composite cardiovascular events in the Normal/Target-White-Coat and Hypertension-White-Coat groups, with heightened 27-year all-cause mortality observed in all groups except Hypertension-Masked, compared to the Normal/Target group. Conclusions: Machine learning has limited potential in accurate blood pressure classification when ABPM is unavailable. Larger studies including diverse patient groups and different resource settings are warranted

    Anatomical classification of the shape and topography of the stomach

    The aim of the study was to present the classification of anatomical variations of the stomach, based on the radiological and historical data. In years 2006–2010, 2,034 examinations of the upper digestive tract were performed. Normal stomach anatomy or different variations of the organ shape and/or topography without any organic radiologically detectable gastric lesions were revealed in 568 and 821 cases, respectively. Five primary groups were established: abnormal position along longitudinal (I) and horizontal axis (II), as well as abnormal shape (III) and stomach connections (IV) or mixed forms (V). The first group contains abnormalities most commonly observed among examined patients such as stomach rotation and translocation to the chest cavity, including sliding, paraesophageal, mixed-form and upside-down hiatal diaphragmatic hernias, as well as short esophagus, and the other diaphragmatic hernias, that were not found in the evaluated population. The second group includes the stomach cascade. The third and fourth groups comprise developmental variations and organ malformations that were not observed in evaluated patients. The last group (V) encloses mixed forms that connect two or more previous variations

    Revelations About Carotid Body Function Through its Pathological Role in Resistant Hypertension

    Much recent attention has been given to the carotid body because of its potential role in cardiovascular disease states. One disease, neurogenic hypertension, characterised by excessive sympathetic activity, appears dependent on carotid body activity that may or may not be accompanied by sleep-disordered breathing. Herein, we review recent literature suggesting that the carotid body acquires tonicity in hypertension. We predict that carotid glomectomy will be a powerful way to temper excessive sympathetic discharge in diseases such as hypertension. We propose a model to explain that signalling from the ‘hypertensive’ carotid body is tonic, and hypothesise that there will be a sub-population of glomus cells that channel separately into reflex pathways controlling sympathetic motor outflows

    Opposing Age-Related Trends in Absolute and Relative Risk of Adverse Health Outcomes Associated With Out-of-Office Blood Pressure

    Participant-level meta-analyses assessed the age-specific relevance of office blood pressure to cardiovascular complications, but this information is lacking for out-of-office blood pressure. At baseline, daytime ambulatory (n=12 624) or home (n=5297) blood pressure were measured in 17 921 participants (51.3% women; mean age, 54.2 years) from 17 population cohorts. Subsequently, mortality and cardiovascular events were recorded. Using multivariable Cox regression, floating absolute risk was computed across 4 age bands (80 years). Over 236 491 person-years, 3855 people died and 2942 cardiovascular events occurred. From levels as low as 110/65 mm Hg, risk log-linearly increased with higher out-of-office systolic/diastolic blood pressure. From the youngest to the oldest age group, rates expressed per 1000 person-years increased (P<0.001) from 4.4 (95% CI, 4.0-4.7) to 86.3 (76.1-96.5) for all-cause mortality and from 4.1 (3.9-4.6) to 59.8 (51.0-68.7) for cardiovascular events, whereas hazard ratios per 20-mm Hg increment in systolic out-of-office blood pressure decreased (P <= 0.0033) from 1.42 (1.19-1.69) to 1.09 (1.05-1.12) and from 1.70 (1.51-1.92) to 1.12 (1.07-1.17), respectively. These age-related trends were similar for out-of-office diastolic pressure and were generally consistent in both sexes and across ethnicities. In conclusion, adverse outcomes were directly associated with out-of-office blood pressure in adults. At young age, the absolute risk associated with out-of-office blood pressure was low, but relative risk high, whereas with advancing age relative risk decreased and absolute risk increased. These observations highlight the need of a lifecourse approach for the management of hypertension

    Układ współczulny odgrywa istotną rolę w regulacji ciśnienia tętniczego i etiopatogenezie chorób układu krążenia. Zwiększenie aktywności układu współczulnego stwierdza się u chorych z niewydolnością krążenia, choroby wieńcową i zaburzeniami rytmu. Celem mniejszego omówienia jest przedstawienie znaczenia układu współczulnego w rozwoju nadciśnienia tętniczego. W pierwszej kolejności zostaną szczegółowo omówione metody badania układu współczulnego, ze szczególnym uwzględnieniem znaczenia mikroneurografii. W dalszej kolejności zostaną przedstawione efekty pobudzenia układu współczulnego oraz dowody wskazujące na zwiększenie aktywności tego układu u chorych z nadciśnieniem tętniczym. Przedstawione zostaną także mechanizmy mogące prowadzić do zwiększonej aktywności układu współczulnego u chorych z nadciśnieniem tętniczym. Ostatnia część niniejszego omówienia zostanie poświęcona zaburzeniom metabolicznym. Szczególny nacisk zostanie położony na potencjalne znaczenie insuliny i leptyny w etiopatogenezie nadciśnienia tętniczego.The sympathetic nervous system plays a central role in cardiovascular regulation in both health and disease. Sympathetic activation may be implicated in the pathogenesis of hypertension, coronary artery disease, cardiac arrhythmias and heart failure. This review focuses on sympathetic neural mechanisms in hypertension. We will review first, methods for studying sympathetic activity in humans; second, the cardiovascular ef fects of sympathetic activation; third, the evidence for sympathetic overactivity in hypertension; fourth, the nęural mechanisms enhancing sympathetic drive in hypertension; and fifth, the importance of humoral and metabolic mechanisms in sympathetic activation. While fundamental concepts outlined in earlier reviews [l, 2] will be revisited, emphasis will be placed on more recent developments

    Serum of obstructive sleep apnea patients impairs human coronary endothelial cell migration

    Introduction: Endothelial cell migration and proliferation play an important role in the growth and development of new blood vessels and endothelium healing. This process occurs in response to injury, inflammation and immune reactions. Dysfunction of the endothelium may play a significant role in development and progression of cardiovascular disease related to sleep-disordered breathing. The aim of our study was to evaluate the chemo-attractant activity of serum from obstructive sleep apnea (OSA) and normal subjects on coronary artery endothelial cell migration. Material and methods: We studied 12 severe OSA patients, free of other co-morbidities and on no treatment, along with 12 age-, body mass index, and gender matched healthy controls. Blood was collected at three time points: at 21:00 before sleep, at 6:00 after waking from sleep, and at 11.00 (after 5 h of normal daytime activity). Serum chemo-attractant activity for human coronary endothelial cells was assessed using a colorimetric cell migration assay kit. Results : In healthy subjects, serum chemo-attractant activity peaked in the morning after waking from sleep (p = 0.02). This early morning increase was blunted in severe OSA subjects, in whom chemo-attractant activity was weaker than in normal controls (p = 0.02), and did not change significantly at the different time-points (p < 0.001 vs. controls). Conclusions : Chemo-attractant activity of the serum from OSA patients is lower compared to serum from healthy subjects, especially in the morning. Altered chemo-attractant serum activity may conceivably contribute to the impairment of endothelial function in obstructive sleep apnea patients
