    Application of the disposition model to breast cancer data

    In this paper, we have presented the second level nesting of Bonney's disposition model (Bonney, 1998) and examined the implications of higher level nesting of the disposition model in relation to the dimension of the parameter space. We have also compared the performance of the disposition model with Cox's regression model (Cox, 1972). It has been observed that the disposition model has a very large number of unknown parameters, and is therefore limited by the method of estimation used. In the case of the maximum likelihood method, reasonable estimates are obtained if the number of parameters in the model is at most nine. This corresponds to about four to seven covariates. Since each covariate in Cox's model provides a parameter, it is possible to include more covariates in the regression analysis. On the other hand, as opposed to Cox's model, the disposition model is fitted with parameters to capture aggregation in families, if there should be any.     Designing citizen science tools for learning: lessons learnt from the iterative development of nQuire

    This paper reports on a 4-year research and development case study about the design of citizen science tools for inquiry learning. It details the process of iterative pedagogy-led design and evaluation of the nQuire toolkit, a set of web-based and mobile tools scaffolding the creation of online citizen science investigations. The design involved an expert review of inquiry learning and citizen science, combined with user experience studies involving more than 200 users. These have informed a concept that we have termed ‘citizen inquiry’, which engages members of the public alongside scientists in setting up, running, managing or contributing to citizen science projects with a main aim of learning about the scientific method through doing science by interaction with others. A design-based research (DBR) methodology was adopted for the iterative design and evaluation of citizen science tools. DBR was focused on the refinement of a central concept, ‘citizen inquiry’, by exploring how it can be instantiated in educational technologies and interventions. The empirical evaluation and iteration of technologies involved three design experiments with end users, user interviews, and insights from pedagogy and user experience experts. Evidence from the iterative development of nQuire led to the production of a set of interaction design principles that aim to guide the development of online, learning-centred, citizen science projects. Eight design guidelines are proposed: users as producers of knowledge, topics before tools, mobile affordances, scaffolds to the process of scientific inquiry, learning by doing as key message, being part of a community as key message, every visit brings a reward, and value users and their time

    Molecular diagnosis for the novel coronavirus SARS-CoV-2: lessons learnt from the Ghana experience

    Background: A novel coronavirus, SARS-CoV-2 is currently causing a worldwide pandemic. The first cases of SARS-CoV-2 infection were recorded in Ghana on March 12, 2020. Since then, the country has been combatting countrywide community spread. This report describes how the Virology Department, Noguchi Memorial Institute for Medical Research (NMIMR) is supporting the Ghana Health Service (GHS) to diagnose infections with this virus in Ghana.Methods: The National Influenza Centre (NIC) in the Virology Department of the NMIMR, adopted real-time Polymerase Chain Reaction (rRT-PCR) assays for the diagnosis of the SARS-CoV-2 in January 2020. Samples from suspected cases and contact tracing across Ghana were received and processed for SARS-CoV-2. Samples were ‘pooled’ to enable simultaneous batch testing of samples without reduced sensitivity.Outcomes: From February 3 to August 21, the NMIMR processed 283 946 (10%) samples. Highest number of cases were reported in June when the GHS embarked on targeted contact tracing which led to an increase in number of samples processed daily, peaking at over 7,000 samples daily. There were several issues to overcome including rapid consumption of reagents and consumables. Testing however continued successfully due to revised procedures, additional equipment and improved pipeline of laboratory supplies. Test results are now provided within 24 to 48 hours of sample submission enabling more effective response and containment.Conclusion: Following the identification of the first cases of SARS-CoV-2infection by the NMIMR, the Institute has trained other centres and supported the ramping up of molecular testing capacity in Ghana. This provides a blueprint to enable Ghana to mitigate further epidemics and pandemics

    Political Regimes and Sovereign Credit Risk in Europe, 1750-1913

    This article uses a new panel data set to perform a statistical analysis of political regimes and sovereign credit risk in Europe from 1750 to 1913. Old Regime polities typically suffered from fiscal fragmentation and absolutist rule. By the start of World War I, however, many such countries had centralized institutions and limited government. Panel regressions indicate that centralized and?or limited regimes were associated with significant improvements in credit risk relative to fragmented and absolutist ones. Structural break tests also reveal close relationships between major turning points in yield series and political transformations

    The development of a business intelligence web application to support the decision-making process regarding absenteeism in the workplace

    Nowadays, one of the biggest concerns of industries all over the world is situations regarding absenteeism, since it has a great impact on the productivity and economy of companies, as well as on the health of their employees. The major causes of absenteeism appear to be work accidents and sickness leaves, which lead to the attempt by companies of understanding how the workload is related to the health of their collaborators and, consequently, to absenteeism. Thus, this paper proposes the design and development of a Web Application based on Business Intelligence indicators in order to help the health and human resources professionals of a Portuguese company analyse the relation between absenteeism and the health and lifestyle of employees, with the intention of concluding whether the work executed on the company is harming workers’ health. Furthermore, it is intended to discover the principal motives for the numerous and more frequent absences in this company, so that it is possible to decrease the absenteeism rate and, hence, improve the decision-making process.     Diffusion Tensor Imaging determines three-dimensional architecture of human cervix: a cross sectional study

    Objective To determine the microarchitecture of the cervix using high-resolution diffusion tensor (DT) magnetic resonance imaging (MRI). Design Cross-sectional study. Setting Leeds, UK. Sample Women undergoing hysterectomy for benign pathology. Methods Ex-vivo DT-MRI measurements were obtained using a 9.4-T Bruker nuclear magnetic resonance (NMR) spectrometer on seven fixed human cervices obtained at hysterectomy. A deterministic fibre-tracking algorithm was used to indirectly visualise underlying fibre organisation. Inter-regional differences in tissue structure were sought using quantitative measurements of diffusion. Main outcome measure The identification of an occlusive structure in the region corresponding to the internal cervical os. Results Fibre tracking demonstrated two regions: an outer circular and inner longitudinal layer. The total circumferential tract volume (TV) was greatest in the proximal region of the cervix (TV: proximal, 271 ± 198 mm3; middle, 186 ± 119 mm3; distal, 38 ± 36 mm3). Fractional anisotropy (FA) and apparent diffusion coefficient (ADC) measurements were significantly different between regions in all samples (P < 0.0005), indicating greater tract density and organisation towards the internal os. Conclusion Fibre tracking infers a system of dense, well-defined, encircling fibres in the proximal region of the cervix, corresponding to the location of the internal os. These findings may provide evidence of specific anatomic microarchitecture within the cervix able to resist intrauterine forces associated with pregnancy

    Development of a digital twin operational platform using Python Flask

    The digital twin concept has developed as a method for extracting value from data, and is being developed as a new technique for the design and asset management of high-value engineering systems such as aircraft, energy generating plant, and wind turbines. In terms of implementation, many proprietary digital twin software solutions have been marketed in this domain. In contrast, this paper describes a recently released open-source software framework for digital twins, which provides a browser-based operational platform using Python and Flask. The new platform is intended to maximize connectivity between users and data obtained from the physical twin. This paper describes how this type of digital twin operational platform (DTOP) can be used to connect the physical twin and other Internet-of-Things devices to both users and cloud computing services. The current release of the software—DTOP-Cristallo—uses the example of a three-storey structure as the engineering asset to be managed. Within DTOP-Cristallo, specific engineering software tools have been developed for use in the digital twin, and these are used to demonstrate the concept. At this stage, the framework presented is a prototype. However, the potential for open-source digital twin software using network connectivity is a very large area for future research and development

    Unwillingness of patients in Ghana to interrupt antiretroviral therapy for HIV cure research

    OBJECTIVES: Though antiretroviral therapy (ART) has reduced HIV infection into a manageable chronic disease, it does not provide for a cure. HIV cure trials may carry risks for patients who are generally doing well on ART, making it imperative that their input is sought as various types of cure methods and trials are designed. Few studies have sought the views of African patients on HIV cure studies. The objective of this study was to determine the views and preferences of people living with HIV (PLWH) in Ghana on cure research. METHODS: We used a questionnaire to interview 251 PLWH in Ghana about their willingness to engage in HIV cure research. We investigated their motivations, the types of cure they would prefer and which risks were acceptable to them. RESULTS: Most participants were enthusiastic about participating in cure research and driven by both altruistic and personal motives. Patients preferred a cure where they would continue follow-up with their doctor (88%) compared to being assured that they have been completely cured and did not need further follow-up (11%). The vast majority of the respondents were risk averse. Most patients (67%) would decline to interrupt ART as part of a protocol for HIV cure research. In bivariate analysis, participants above the age of 40 years were more likely to agree to treatment interruption during cure studies (OR 2.77; 95% CI 1.21-.6.34. CONCLUSIONS: Our results show that preferred cure modalities and risk tolerance for patients in Africa may be different from those of other parts of the world. Extensive social science and behavioural studies are needed on the continent to help inform future cure trials
