    The mean percentage of cryptogenic strokes among ischemic strokes is 31%, of which one-third may be associated with patent foramen ovale. The foramen ovale is required for blood flow through the fetal atrial septum.  It is formed as of the fourth week of gestation, and this leads to right-left interatrial shunt that allows the passage of oxygenated blood to systemic circulation. In 75% of cases, its closure is complete by 2 years of age, but it may persist in 25% of patients. We present the case of a patient with paradoxical embolism in the lower extremities and ischemic stroke in the clinical context of a patent foramen ovale

    Estudio retrospectivo de trauma dento-alveolar en escolares de Pasto, Colombia.

    Objective: determine the prevalence of dento-alveolar trauma in school children at Institución Educativa Municipal (INEM) from Pasto, Colombia, 2007-2008. Method: a cross-sectional descriptive study was designed to assess 325, aged 10 to 12, school children. Variables such as gender, place located, cause, dental anatomical structure, and etiological factors were used. Results: the prevalence of dento-alveolar trauma was 7.4 percent; a higher incidence in males (70.8 percent). A trauma peak was recorded in 11-year-old group (70.8 percent.) Male/female ratio was 2.3:1. Home was the place where dento-alveolar trauma was set, and accidental falls were the main injury cause. Twenty-five percent of school children traumatized had subluxation. The most affected teeth were the maxillary right central incisors (37.5 percent). An increased overbite (25 percent) was found in school children who presented dento-alveolar trauma. Conclusions: teeth injuries were more frequent on 11-year pupils in the assessed population. Dento-alveolar trauma could have an impact on dental aesthetic and the mouth function. Therefore, this trauma is an alarming condition and should have an emergency treatment provided.Objetivo: determinar la prevalencia de trauma dentoalveolar en escolares de la Institución Educativa Municipal (INEM) Pasto, Colombia,  entre el 2007 y el 2008. Metodología: se diseñó un estudio descriptivo transversal donde se evaluaron 325 escolares de 10 a 12  años de edad. Se utilizaron variables como sexo, lugar donde se estableció, causa, estructura anatómica dental y factores etiológicos.  Resultados: la prevalencia del trauma dentoalveolar fue del 7,4%, existió una mayor incidencia entre hombres (70,8%). Un pico de trauma  fue visto en el grupo de 11 años de edad (70,8%). La razón hombre/mujer fue 2,3:1. La casa fue el lugar donde se presentó el trauma  dentoalveolar y la principal causa de lesión fueron las caídas accidentales. El 25% de los escolares traumatizados tuvieron subluxación.  Los dientes más afectados fueron los incisivos centrales superiores derechos (37,5%). Un overbite aumentado (25%) fue encontrado en  los escolares, quienes presentaron trauma dentoalveolar. Conclusiones: en la población evaluada los casos de lesiones dentales fueron  más frecuentes en escolares de 11 años. El trauma dentoalveolar en los escolares podría tener un impacto sobre la estética dental y la  función de la boca. Por lo tanto, el trauma es una condición de alarma y debería ser provisto de un tratamiento de emergencia.  &nbsp

    Se ha observado que las especies forestales desarrollan adaptaciones anatómicas respondiendo a los cambios ambientales, el género Quercus especialmente, presenta características de resistencia a las sequías. En este estudio se compararon los rasgos funcionales relacionados con la conducción hidráulica como el diámetro y la frecuencia de poros, diámetro de punteaduras, espesor de la pared de las fibras y la conductividad hidráulica teórica en 16 individuos de bosque natural y arbolado urbano. Se encontraron diferencias significativas en las dos zonas en la mayoría de variables estudiadas, excepto en el diámetro de las punteaduras, estas diferencias responden a condiciones biofísicas propias en cada área de estudio, encontrando que de acuerdo a las características anatómicas del xilema, la capacidad de conductividad hidráulica de los individuos es mejor en los árboles de bosque natural que en los árboles de ciudad al igual que la resistencia al embolismo.&nbsp

    Successful outcomes of a new combined solution of hyaluronic acid, chondroitin sulfate and poloxamer 407 for submucosal injection: animal survival study

    Background: and study aims Endoscopic resection requires use of submucosal injection. This study was conducted to assess efficacy and impact on early healing of hyaluronic acid combined with chondroitin sulfate and poloxamer 407 (Ziverel) when used as a solution for submucosal injection. Materials: and methods Prospective and comparative study of gastric endoscopic mucosal resection (EMR) with three groups of two Yorkshire pigs. Six submucosal cushions were created in each animal by injecting 2 mL of Ziverel (Group 1) or succinylated gelatin (SG) (Group 2), enabling 12 EMR in each group. Submucosal cushions were created with Ziverel in Group 3, without resection. Electrosurgery unit settings were the same in all cases. EMR defects and injection sites were marked with clips. The animals were sacrificed 7 days later. EMR specimen size and duration of procedure were recorded. EMR specimens and EMR scars and injection sites were evaluated by a blinded pathologist. Results: We successfully performed 24 EMR (15 en-bloc and 9 piecemeal, without differences between groups 1 and 2). Mean EMR specimen dimensions were significantly larger in Group 1 (median 19 mm, range 6 - 40 vs 16.6 mm, range 5‑25; P = 0.019), without changing the electrocautery unit settings. Blinded histopathologist assessment of EMR specimens showed less fibrosis in the submucosa and a trend to fewer cautery artifacts with Ziverel and did not identify any significant differences in early healing of resection sites. Conclusion: The combination of Ziverel enables EMR and does not negatively affect early healing

    Brazilian phonoaudiology telepractice before and during the COVID-19 pandemic

    Purpose: to analyze the use of digital resources by Brazilian phonoaudiologists before and during the period of the COVID-19 pandemic. Methods: cross-sectional study carried out with a representative sample of Brazilian phonoaudiologists, with at least one year of practical experience. An online questionnaire consisting of 28 questions divided into eight sections was prepared. Data were analyzed and are reported as absolute and relative frequency regarding the variables: respondent’s age, highest degree and years of experience; weekly working hours; region of the country; type of service and area of expertise; use of digital means before and during the pandemic and willingness to use the same kind of resource after the pandemic. Results: telehealth technologies were used by 90% of phonoaudiologists in Brazil since before the COVID-19 pandemic. A change in the profile of technologies used was observed: after the pandemic, videoconferences gained more use in contact with patients compared to the pre-pandemic period even though contact via mobile phone and instant messaging applications are the most used, both before and during the pandemic. Conclusion: the study’s findings demonstrate that 90% of phonoaudiologists who answered the questionnaire in Brazil used and still make use of telehealth technologies to contact patients

    The COVID-19 pandemic brought challenges and the need for adaptation on several fronts. Updating the ways of working is necessary and promoting education and care in the health sector in line with the 21st century is essential. In addition to undergraduate students training in Speech-Language Pathology (SLP) field, based on the necessary opportunities for practical experience with patients during undergraduation, a significant portion of the population is attended by school clinics linked to universities and training centers, which work in conjunction with public health services. In this text we will bring some reflections on the relevance of using telehealth technologies for speech therapists training, considering the articulation between national curricular guidelines, professional training and ethical aspects.A pandemia da COVID-19 trouxe desafios e necessidade de adaptação em diversas frentes. A atualização das formas de trabalho é necessária e promover ensino e atendimento no setor saúde condizentes com o século XXI é fundamental. Não estamos apenas tratando da formação de graduandos na área de Fonoaudiologia, mas do atendimento clínico a uma parcela significativa da população que depende dos serviços de clínicas-escola vinculadas às universidades e centros de formação em nossa profissão.Neste texto traremos algumas reflexões sobre a relevância do uso das tecnologias de telessaúde para a formação de fonoaudiólogos, considerando a articulação entre as diretrizes curriculares nacionais, a formação profissional e aspectos éticos

    Lower insulin-dose adjusted A1c (IDAA1c) is associated with less complications in Individuals with Type 1 Diabetes treated with hematopoetic stem-cell transplantation and conventional therapy

    Objective: To evaluate the association between insulin-dose adjusted A1C (IDAA1c) and microvascular complications (MC) and hypoglycemia in a representative Brazilian population of Type 1 diabetes mellitus (T1DM) patients. Research Design and Methods: This was a cross-sectional study based on a previous study, “Microvascular Complications in Type 1 Diabetes: a comparative analysis of patients treated with autologous nonmyeloablative hematopoietic stem-cell transplantation (AHST) and conventional medical therapy (CT)”. The 168 patients in that study (144 from CT plus 24 from AHST) were re-subdivided into two groups, according to their IDAA1c values (30 patients had IDAA1c ≤ 9; 138 had IDAA1c > 9). Then, the prevalence of MC (diabetic renal disease, neuropathy, and retinopathy), hypoglycemia (blood glucose <60 mg/dL), and severe hypoglycemic (episode of hypoglycemia that required the assistance of another person to treat) events were compared between the groups. The groups were well-matched on these factors: duration of disease, sex, and age at the time of diagnosis of T1DM. Results: After an average of 8 years after diagnosis, only 6.6% (2/30) of the patients from IDAA1c ≤ 9 group developed any MC, whereas 21.0% (29/138) from the IDAA1c > 9 group had at least one complication (p = 0.044). Regarding hypoglycemic events, the proportion of individuals who reported at least 1 episode of hypoglycemia in the last month was 43.3 and 64.7% from the IDAA1c ≤ 9 and IDAA1c > 9 groups, respectively (p = 0.030). Regarding severe hypoglycemia, the proportion of patients presenting at least one episode in the last month and the rate of episode/patient/month were similar between groups (6.7 vs. 13.2%; p = 0.535; and 0.1/patient/month vs. 0.25/patient/month; p = 0.321). Conclusion: In a representative Brazilian population of T1DM patients, those with IDAA1c ≤ 9 presented a lower frequency of MC, as well as fewer episodes of hypoglycemia, in the month prior to the analysis.publishedVersio

    Denture stomatitis treated with photodynamic therapy: five cases

    Objective: Photodynamic therapy (PDT) is an effective method for Candida spp. inactivation in vitro and in vivo, but as yet, no clinical trial has been conducted. This report describes 5 cases of denture stomatitis (DS) treated with PDT.\ud \ud Study design: Five subjects with clinical and microbiologic diagnosis of DS were submitted to 6 sessions of PDT 3 times a week for 15 days. In each session, patients' dentures and palates were sprayed with 500 mg/L Photogem, and, after 30 minutes of incubation, irradiated by light-emitting diode light source at 455 nm (37.5 and 122 J/cm2, respectively). Cultures of Candida spp. from dentures and palates and standard photographs of the palates were taken at baseline (day 0), at the end of the treatment (day 15), and at follow-up time intervals (days 30 and 60).\ud \ud Results: Four patients showed clinical resolution of DS (no inflammation) after PDT sessions, and only 1 subject demonstrated reduction in palatal inflammation.
