    Rodent abundance, stone bund density and its effects on crop damage in the Tigray highlands, Ethiopia

    In areas of subsistence agriculture, a variety of soil conservation methods have been implemented in the last few decades to improve crop yields, however these can have unintended consequences such as providing habitat for rodent pests. We studied rodent population dynamics and estimated crop damage in high and low stone bund density fields for four cropping seasons in Tigray highlands, northern Ethiopia. Stone bunds are physical structures for soil and water conservation, and potentially habitat for rodents. We used a general model to relate the proportion of crop damage to rodent abundance, stone bund density and crop stages. Generally, rodent abundance remained relatively low during the study period, except during the fourth quarter of the 2010 cropping season. We found a positive correlation between rodent abundance and crop damage, and significant variation in rodent abundance and crop damage between high and low stone bund density fields. Furthermore, crop damage also varied significantly between crop stages. We concluded that Mastomys awashensis (Lavrenchenko, Likhnova and Baskevich 1998) and Arvicanthis dembeensis (Ruppel 1842) were the two most important crop pests in Tigray highlands causing significant damage. Fields with high stone bund density (~10m average distance apart) harbor more rodents and endure a significantly higher proportion of crop damage compared to fields with lower stone bund density (~15m average distance 4 apart). The fact that rodent abundances peaked during the reproductive stage of the crop and around harvest implies the need for management intervention before these crop stages are attained

    The global burden of adolescent and young adult cancer in 2019 : a systematic analysis for the Global Burden of Disease Study 2019

    Background In estimating the global burden of cancer, adolescents and young adults with cancer are often overlooked, despite being a distinct subgroup with unique epidemiology, clinical care needs, and societal impact. Comprehensive estimates of the global cancer burden in adolescents and young adults (aged 15-39 years) are lacking. To address this gap, we analysed results from the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2019, with a focus on the outcome of disability-adjusted life-years (DALYs), to inform global cancer control measures in adolescents and young adults. Methods Using the GBD 2019 methodology, international mortality data were collected from vital registration systems, verbal autopsies, and population-based cancer registry inputs modelled with mortality-to-incidence ratios (MIRs). Incidence was computed with mortality estimates and corresponding MIRs. Prevalence estimates were calculated using modelled survival and multiplied by disability weights to obtain years lived with disability (YLDs). Years of life lost (YLLs) were calculated as age-specific cancer deaths multiplied by the standard life expectancy at the age of death. The main outcome was DALYs (the sum of YLLs and YLDs). Estimates were presented globally and by Socio-demographic Index (SDI) quintiles (countries ranked and divided into five equal SDI groups), and all estimates were presented with corresponding 95% uncertainty intervals (UIs). For this analysis, we used the age range of 15-39 years to define adolescents and young adults. Findings There were 1.19 million (95% UI 1.11-1.28) incident cancer cases and 396 000 (370 000-425 000) deaths due to cancer among people aged 15-39 years worldwide in 2019. The highest age-standardised incidence rates occurred in high SDI (59.6 [54.5-65.7] per 100 000 person-years) and high-middle SDI countries (53.2 [48.8-57.9] per 100 000 person-years), while the highest age-standardised mortality rates were in low-middle SDI (14.2 [12.9-15.6] per 100 000 person-years) and middle SDI (13.6 [12.6-14.8] per 100 000 person-years) countries. In 2019, adolescent and young adult cancers contributed 23.5 million (21.9-25.2) DALYs to the global burden of disease, of which 2.7% (1.9-3.6) came from YLDs and 97.3% (96.4-98.1) from YLLs. Cancer was the fourth leading cause of death and tenth leading cause of DALYs in adolescents and young adults globally. Interpretation Adolescent and young adult cancers contributed substantially to the overall adolescent and young adult disease burden globally in 2019. These results provide new insights into the distribution and magnitude of the adolescent and young adult cancer burden around the world. With notable differences observed across SDI settings, these estimates can inform global and country-level cancer control efforts. Copyright (C) 2021 The Author(s).     Objective: information use is fundamental for evidence based decision making but data quality and information use in health facility is limited in low and middle income countries. Therefore, a facility based cross sectional using mixed approach was conducted to assess data quality and use for decision making in 12 selected health facilities of Tigray, Ethiopia, 2018.Results: District Health Information System 2 (DHIS2) software was practiced in 12 of the facilities. Completeness and consistency rate of register and reports was 53.5%, 56.3%, and 38.9%. Respectively. The lot of quality assurance system of service and disease report was 7(61.1%). The performance monitoring team is established decision-making rate on key coverage indicators was 42.9%. Comparisons of performance in a district /national targets were 5(41.5a high rate of incompleteness, inconsistencies, low lots of data assurance and low use of data for decision making. Therefore, DHIS2 tool needs to be used to generate a report, use data and disseminating platforms both inside and outside health facilities using the website and other channels.&nbsp;</p

    Background Lipids and glucose concentrations in the blood rise during pregnancy period. Poor control of these analytes results in cardio metabolic dysfunction. Despite this, there are no documented studies which investigate lipids and glucose among pregnant women in Tigrai, northern Ethiopia. Objective The objective of this study was to assess lipid and glucose levels and identify their correlates among pregnant women in Tigrai, northern Ethiopia. Method We conducted a facility-based cross sectional study comprising of systematically selected 200 pregnant women from July to October 2021. Those who were severely ill were excluded from the study. We used a structured questionnaire to collect socio-demographic and clinical characteristics of pregnant women. Lipids such as triglycerides, low density lipoprotein, cholesterol and blood glucose were also measured using Cobas C311 chemistry machine from plasma samples. The data were analyzed using SPSS version 25. Logistic regression was performed and statistical significance was declared at p-value Result Proportion of pregnant women with cholesterol, triglyceride, low density lipoprotein and blood glucose levels above the upper limit of the normal range used for clinical decision were 26.5%, 43%, 44.5% and 21%, respectively. Pregnant women’s income > = 10,000 ETB (AOR = 3.35; 95%CI: 1.46–7.66), age (AOR = 3.16; 95%CI: 1.03–9.68), gestational age 29–37 weeks (AOR = 8.02; 95%CI: 2.69–23.90) and having systolic blood pressure greater than 120 mmHg (AOR = 3.99; 95%CI: 1.64–9.75) demonstrated statistically significant association with raised levels of lipids. Conclusion and recommendation Proportion of pregnant women with out of normal range values of lipids, particularly triglycerides and low density lipoprotein, is high. Gestational age is a strong predictor of increase in blood levels for both lipids. Provision of life style related health education and dietary intake to pregnant mothers matters. Moreover, monitoring lipid profile and glucose level during antenatal care period is essential

    Immediate health and economic impact of the Tigray war on internally displaced persons and hosting households

    Abstract Globally, war is the major cause of displacement from the usual place of the biological environment. The war of Tigray exposed thousands of people to internal displacement and migration. Evidence has shown that displaced people and migrants shoulder the health and economic burden to ensure survival. However, evidence of the impact of the war on health and the economy related to the displaced people and their hosting communities is not documented. Thus, this study aimed to investigate the health and economic impact of the war on displaced people and the hosting community. A community-based survey was conducted among randomly selected 3572 households of 48 woredas/districts from August 06 to 30/2021 in Tigray. Each district had 4 enumeration sites and there were 20 households (HHs) to be sampled per each enumeration site. Data were collected using a pretested structured questionnaire using face-to-face interviews of displaced and hosting household heads. The entered data is exported to SPSS version 26 statistical packages for data analysis. Summary statistics and geo-spatial analysis was computed. The war had a significant impact on the health and economy of the community of Internally Displaced People (cIDPs) and hosting households. There were 12,691 cIDPs and 3572 hosting HHs. About 12.3% had chronic illness12.3% of (cIDP) who had chronic diseases and follow-up medication was forced to stop their medication. 536 (15%) civilian family members of cIDPs were killed at their homes. During the war, 244 (6.83%) of civilian family members faced physical disability. Consequentially, 43.8% and 58.8% of respondents of cIDPs suffered from severe depression and post-traumatic stress disorder. The war had a significant amount of personal resources such as domestic animals, cereals, cars, machinery, and HH furniture was looted and vandalized by the perpetrator forces from the cIDPs and hosting HHs. The range of family size in the hosting households was 3 to 22. The war had a significant health and economic impact on both cIDPs and hosting HHs. cIDPs suffered from various illnesses and disabilities related to the war with no medical access and follow-up care leading them to stressful situations such as depression and PTSD. There was also a huge economic damage and distraction which threatens the survival of the survivors

    Crop damage, rodent abundance and stone bund density in Tigray highlands, Ethiopia

    Farmers in Tigray highlands have ranked rodents as the number one pre- and post-harvest crop pests. We investigated temporal dynamics of rodent populations in rainfed crop fields with low stone bund density (LSBD) and high stone bund density (HSBD) grids near the town of Hagere Selam, and relate this with the level of pre-harvest damage to mixed barley and wheat crop. Rodent population changes were also compared with changes in crop developmental stages, density of stone bunds, and meteorological information (rainfall and temperature). We report that the multimammate mouse Mastomys awashensis and the African grass rat Arvicantis dembeensis were the two dominant pest species (accounting for 86.6% of the capture) in the crop fields. Monthly paired abundance estimates of the two grids revealed that indeed the abundance of rodents in HSBD grid was significantly higher than in LSBD grid. The generalized mixed model fitted to the data clearly showed correlation between the proportion of crop damage and abundance of rodents. We found significantly higher proportion of damage in HSBD grid, at both milky and maturation stages, than in LSBD grids. More significant damage was particularly observed at milky stage, where about 50% more damage difference was estimated in HSBD grid at a density of 40 animals ha-1. We could not find correlation between rodent abundance and weather data. However, seasonality and interannual variations in abundance were evident. Our findings show more damage at the reproductive stages of the crops. This implies obvious pest control significance
