    The Influence of Measurement Scale and Uncertainty on Interpretations of River Migration

    Environmental scientists increasingly use remotely-sensed images to measure how rivers develop over time and respond to upstream changes in environmental drivers such as land use, urbanization, deforestation and agricultural practices. These measurements are subject to uncertainty that can bias conclusions. The first step towards accurate interpretation of river channel change is properly quantifying and accounting for uncertainty involved in measuring changes in river morphology. In Chapter 2 we develop a comprehensive framework for quantifying uncertainty in measurements of river change derived from aerial images. The framework builds upon previous uncertainty research by describing best practices and context-specific strategies, comparing each approach and outlining how to best handle measurements that fall below the minimum level of detection. We use this framework in subsequent chapters to reduce the impact of erroneous measurements. Chapter 3 evaluates how the time interval between aerial images influences the rates at which river channels appear to laterally migrate across their floodplains. Multiple lines of evidence indicate that river migration measurements obtained over longer time intervals (20+ years) will underestimate the ‘true’ rate because the river channel is more likely to have reversed the direction of migration, which erases part of the record of gross erosion as seen from aerial images. If the images don’t capture channel reversals and periodic episodes of fast erosion, the river appears to have migrated a shorter distance (which corresponds to a slower rate) than reality. Obtaining multiple measurements over shorter time intervals (\u3c 5 years) and limiting direct comparisons to similar time intervals can reduce bias when inferring how river migration rates may have changed over time. Chapter 4 explores the physical processes governing the relationship between river curvature and the rate of river migration along a series of meander bends. We used fine-scale empirical measurements and geospatial analyses to confirm theory and models indicating that migration and curvature exhibit a monotonic relationship. The results will improve models seeking to emulate river meander migration patterns

    The impact of a post-take ward round pharmacist on the risk score and enactment of medication-related recommendations

    There is a scarcity of published research describing the impact of a pharmacist on the post-take ward round (PTWR) in addition to ward-based pharmacy services. The aim of this paper was to evaluate the impact of clinical pharmacists' participation on the PTWR on the risk assessment scores of medication-related recommendations with and without a pharmacist. This includes medication-related recommendations occurring on the PTWR and those recommendations made by the ward-based pharmacist on the inpatient ward. A pre-post intervention study was undertaken that compared the impact of adding a pharmacist to the PTWR compared with ward-based pharmacist services alone. A panel reviewed the risk of not acting on medication recommendations that was made on the PTWR and those recorded by the ward-based pharmacist. The relationship between the risk scores and the number and proportion of recommendations that led to action were compared between study groups. There were more medication-related recommendations on the PTWR in the intervention group when a pharmacist was present. Proportionately fewer were in the 'very high and extreme' risk category. Although there was no difference in the number of ward pharmacist recommendations between groups, there was a significantly higher proportion of ward pharmacist recommendations in the "very high and extreme" category in those patients who had been seen on a PTWR attended by a pharmacist than when a pharmacist was not present. There were a greater proportion of "low and medium" risk actionable medication recommendations actioned on the PTWR in the intervention group; and no difference in the risk scores in ward pharmacist recommendations actioned between groups. Overall, the proportion of recommendations that were actioned was higher for those made on the PTWR compared with the ward. The addition of a pharmacist to the PTWR resulted in an increase in low, medium, and high risk recommendations on the PTWR, more very high and extreme risk recommendations made by the ward-based pharmacist, plus an increased number of recommendations being actioned during the patients' admission

    Evaluating the Relationship Between Meander-Bend Curvature, Sediment Supply, and Migration Rates

    River meander migration plays a key role in the unsteady “conveyor belt” of sediment redistribution from source to sink areas. The ubiquity of river meandering is evident from remotely sensed imagery, which has allowed for long-term, high-resolution studies of river channel change and form-process relationships. Empirical, experimental, and theoretical research approaches have described two distinct relationships between channel curvature and river channel migration rates. In this study, we employ a novel application of time-series algorithms to calculate migration rates and channel curvature at sub-meander bend length scales using 6 decades of aerial imagery spanning 205 km of the Minnesota River and Root River, Minnesota, USA. Results from the Minnesota River provide the first empirical evidence demonstrating how migration-curvature relations break down for rivers with low sediment supply, which is supported by the Root River data set. This not only highlights the importance of sediment supply as a driver of river migration, but also supports a simple means to detect river reaches lacking sediment supply. Furthermore, results from both rivers demonstrate that sub-meander bend measurement scales are most appropriate for studying channel migration rates and further indicate that a quasi-linear relationship—rather than the more commonly inferred peaked relationship—exists between channel curvature and migration rates. The highest migration rates are associated with the highest measured channel curvatures in our data set, after accounting for a spatial lag of channel widths. These findings are consistent with flume experiments and empirical data across diverse geologic and climatic environments

    Application of quality by design concepts and automation to improve manufacturing process consistency of development and clinical-stage cell therapies

    Designing manufacturing processes to reproducibly generate process-sensitive human cells of sufficient quantity and quality for clinical application is challenging and complex. Manufacture of cell therapies in manual flask based processes is controlled primarily through adherence to detailed SOPs which may contain subjective user interventions and relatively poorly defined operating controls. This situation can lead to clinical production processes with limited control of critical quality attributes, significant reliance on endpoint quality testing and consequent product wastage. Applying systematic and data driven approaches to process development, many of which form part of the Quality by Design (QbD) toolset, reduces manufacturing process risk. We have applied these approaches with a series of partners and cell types to demonstrate application of QbD tools to cell therapies. This includes statistical capability analysis to define process confidence limits for expansion processes, to identify sources of process variability, and to quantify process performance in relation to the process specification and necessary scale. This further enables risk assessment and gap analysis to identify and prioritise key manufacture process risks with common recurrent elements including input materials, cryo-strategy, and operational parameters pertaining to culture and medium supply strategy. Key variable screening via statistically designed experiments has enabled improvement in process consistency across multiple operations and an improved understanding of process tolerance to parameter levels. It also highlights where automation could be applied to enhance process reproducibility and increase process scale whilst retaining process format with comparability to prior manufacturing development. CompacT SelecT automated manufacturing processes have demonstrated consistently greater cell yields than manual processes with statistical analysis showing significantly improved confidence intervals between multiple production batches and facilitating identification of remaining sources of variation for further targeted process improvement. Example case studies include a partnership with ReNeuron, a UK-based stem cell therapy business currently undergoing a phase II clinical trial with its CTX cell therapy candidate to enhance motor recovery in disabled stroke patients, to develop scalable robust production processes for the CTX cell line

    Sediment Dynamics in the Bear River-Mud Lake-Bear Lake System

    The overarching goal of this project was to compile and analyze a variety of existing datasets, and generate several new datasets, to advance our understanding of how the Bear River Mud Lake-Bear Lake system functions, how it has, or is expected to change, identify which components are degraded or vulnerable to degradation, and determine if/where critical data and/or knowledge gaps exist. We conducted a series of analyses to evaluate changes in hydrology and suspended sediment, collected sediment cores from nine locations in Mud Lake to evaluate how sedimentation rates, sediment sources and water quality have changed over time, and utilized historical air photos and satellite imagery to document changes in Bear Lake’s shoreline. Hydrologic analyses indicate that low, median and high flows have not changed systematically at the Inlet Canal in terms of their long-term averages, since the 1940s. However, all three flow metrics have increased in terms of variability and have experienced longer duration wet and dry periods over the past three decades. We note a paucity of long-term hydrologic datasets for the Bear River-Dingle Marsh-Bear Lake system and additional monitoring would greatly help ensure that we are able to monitor trends throughout the system more carefully. We compiled suspended sediment data from all available sources and concluded, similar to previous studies, that Mud Lake appears to serve as a sediment sink for sediment, but the sediment trapping efficiency appears to vary considerably within and among years. Similar to the flow data, we note an unfortunate paucity of suspended sediment data and strongly recommend more rigorous and continuous monitoring of sediment in all parts of the Bear River-Mud Lake-Bear Lake system. Existing data and monitoring programs are insufficient to identify trends over time. The nine sediment cores extracted from Mud Lake provide a longer-term perspective on sediment dynamics. Results demonstrate that Mud Lake has historically and continues to serve as a net sediment sink. Two of the six dated cores document continuous deposition over the past 120 years, while the other four cores show truncated profiles in the 1950s. Visual inspection of the cores, as well as analysis of organic, calcium carbonate and mineral fractions occurring in the cores demonstrate highly variable history of sediment sources and water quality conditions in Mud Lake. Analysis of diatom algae species provides more detailed information regarding water quality conditions, indicating that Mud Lake has changed from a planktonic glacial lake, to a cold water, low nutrient environment and has existed as a mesotrophic environment with moderate water quality over the past century. Given the detailed information that diatoms can provide regarding historical water quality, we suggest that a similar diatom study examining the past 150 years in Bear Lake’s history could be worthwhile. Elemental analysis of Mud Lake sediments indicate two significant shifts in sediment sources, one coincident with diversion of Bear River into Mud Lake approximately 100 years ago, and a recent shift, within the past 10 years as silver, mercury and rare earth elements have increased considerably. Analysis of Bear Lake’s shoreline from historical imagery shows considerable amount of deposition has occurred in most areas around the lake in the past several decades. The shoreline at low water levels has moved lakeward by 30 to 50 meters (100 to 160 feet) in several locations and as much as 500 m (1600 feet) in the northwest corner of the lake, near St. Charles Creek. Notably, the only location where we document shoreline erosion (i.e., the shoreline moving landward for a given water elevation) is along the eastern edge of the lake, near Porcupine Hollow, Peterson Hollow and Bear Lake State Park. Further, we document that approximately 10% of the beach area in the northwest corner of the lake near St. Charles Creek has transitioned to vegetation cover between 2003 and 2016

    The impact of a pharmacist on post-take ward round prescribing and medication appropriateness

    Background Medication communication and prescribing on the post-take ward round following patient admission to hospital can be suboptimal leading to worse patient outcomes. Objective To evaluate the impact of clinical pharmacist participation on the post-take ward round on the appropriateness of medication prescribing, medication communication, and overall patient health care outcomes. Setting Tertiary referral teaching hospital, Brisbane, Australia. Method A pre-post intervention study was undertaken that compared the addition of a senior clinical pharmacist attending the post-take ward was compared to usual wardbase pharmacist service, with no pharmacist present of the post-take ward round. We assessed the proportion of patients with an improvement in medication appropriateness from admission to discharge, using the START/STOPP checklists. Medication communication was assessed by the mean number of brief and in-depth discussions, with health care outcomes measured by comparing length of stay and 28-day readmission rates. Main outcome measures: Medication appropriateness according to the START/STOPP list, number and type of discussions with team members and length of stay and readmission rate. Results Two hundred and sixty patients were recruited (130 pre- and 130-post-intervention), across 23 and 20 post-take ward rounds, respectively. Post-intervention, there was increase in the proportion of patients who had an improvement medication appropriateness (pre-intervention 25.4%, post-intervention 36.9%; p = 0.004), the number of in-depth discussions about patients’ medication (1.9 ± 1.7 per patient pre-intervention, 2.7 ± 1.7 per patient post-, p < 0.001), and the number relating to high-risk medications (0.71 ± 1.1 per patient pre-intervention, to 1.2 ± 1.2 per patient post-, p < 0.05). Length of stay and 28-day mortality were unchanged. Conclusion Clinical pharmacist participation on the post-take ward round leads to improved medication-related communication and improved medication appropriateness but did not significantly improve health care outcomes

    Patient-reported depression measures in cancer: a meta-review

    The patient-reported depression measures that perform best in oncology settings have not yet been identified. We did a meta-review to integrate the findings of reviews of more than 50 depression measures used in adults with, or recovering from, any type of cancer. We searched Medline, PsycINFO, Embase, and grey literature from 1999 to 2014 to identify 19 reviews representing 372 primary studies. 11 reviews were rated as being of high quality (defined as meeting at least 20 criteria in the Preferred Reporting Items for Systematic Reviews and Meta-Analyses statement). The Hospital Anxiety Depression Scale (HADS) was the most thoroughly evaluated measure, but was limited by cutpoint variability. The HADS had moderate screening utility indices and was least recommended in advanced cancer or palliative care. The Beck Depression Inventory was more generalisable across cancer types and disease stages, with good indices for screening and case finding. The Center for Epidemiologic Studies Depression Scale was the best-weighted measure in terms of responsiveness. This meta-review provides a comprehensive overview of the strengths and limitations of available depression measures. It can inform the choice of the best measure for specific settings and purposes

    AimsTo investigate the impact of alcohol consumption on risk decisions taken both individually and while part of a four- to six-person ad-hoc group.DesignA 2 (alcohol: consuming versus not consuming alcohol) x 2 (decision: individual, group) mixed-model design; decision was a repeated measure. The dependent variable was risk preference, measured using choice dilemmas.SettingOpportunity sampling in campus bars and a music event at a campus-based university in the United Kingdom.ParticipantsA total of 101 individuals were recruited from groups of four to six people who either were or were not consuming alcohol.MeasurementsParticipants privately opted for a level of risk in response to a choice dilemma and then, as a group, responded to a second choice dilemma. The choice dilemmas asked participants the level of accident risk at which they would recommend someone could drive while intoxicated.FindingsFive three-level multi-level models were specified in the software program HLM 7. Decisions made in groups were less risky than those made individually (B = -0.73, P < 0.001). Individual alcohol consumers opted for higher risk than non-consumers (B = 1.27, P = 0.025). A significant alcohol?×?decision interaction (B = -2.79, P = 0.001) showed that individual consumers privately opted for higher risk than non-consumers, whereas risk judgements made in groups of either consumers or non-consumers were lower. Decisions made by groups of consumers were less risky than those made by groups of non-consumers (B = 1.23, P < 0.001).ConclusionsModerate alcohol consumption appears to produce a propensity among individuals towards increased risk-taking in deciding to drive while intoxicated, which can be mitigated by group monitoring processes within small (four- to six-person) groups
