    Interobserver and Intertest Agreement in Telemedicine Glaucoma Screening with Optic Disk Photos and Optical Coherence Tomography

Methods: A stratified sample of 4113 individuals was randomly selected, and those who accepted underwent examination including visual acuity, intraocular pressure (IOP), non-mydriatic retinography, and imaging using a portable OCT device. Participants’ data and images were uploaded and assessed by 16 ophthalmologists on a deferred basis. Two independent evaluations were performed for all participants. Agreement between methods was assessed using the kappa coefficient and the prevalence-adjusted bias-adjusted kappa (PABAK). We analyzed potential factors possibly influencing the level of agreement. Results: The final sample comprised 1006 participants. Of all suspected glaucoma cases (n = 201), 20.4% were identified in retinographs only, 11.9% in OCT images only, 46.3% in both, and 21.4% were diagnosed based on other data. Overall interobserver agreement outcomes were moderate to good with a kappa coefficient of 0.37 and a PABAK index of 0.58. Higher values were obtained by experienced evaluators (kappa = 0.61; PABAK = 0.82). Kappa and PABAK values between OCT and photographs were 0.52 and 0.82 for the first evaluation. Conclusion: In a telemedicine screening setting, interobserver agreement on diagnosis was moderate but improved with greater evaluator expertise.The study has been funded by the Fondo de InvestigacionesSanitarias of the Spanish Ministry of Health (PI15/00412)

    Subtypes of Severely Mentally Ill Violent Offenders in a Spanish Forensic Psychiatric Hospital

    Conduct disorder (CD) prior to age 15 identifies a subgroup of men with severe mental illness (SMI) who present a high risk for violence that persists across the life span. The present study examined male violent offenders with SMI in a forensic hospital in Spain, comparing those with SMI+CD and those without SMI-CD on the HCR-20 and PCL:SV. Violent offenders with SMI+CD obtained higher HCR-20 and PCL: SV total scores, and much higher H and factor 2 scores as compared to those without prior CD. Men with SMI+CD present a challenge to forensic psychiatric services

    Carrier Recombination in Highly Uniform and Phase-Pure GaAs/(Al,Ga)As Core/Shell Nanowire Arrays on Si(111): Mott Transition and Internal Quantum Efficiency

    GaAs-based nanowires are among the most promising candidates for realizing a monolithical integration of III-V optoelectronics on the Si platform. To realize their full potential for applications as light absorbers and emitters, it is crucial to understand their interaction with light governing the absorption and extraction efficiency, as well as the carrier recombination dynamics determining the radiative efficiency. Here, we study the spontaneous emission of zincblende GaAs/(Al,Ga)As core/shell nanowire arrays by μ\mu-photoluminescence spectroscopy. These ordered arrays are synthesized on patterned Si(111) substrates using molecular beam epitaxy, and exhibit an exceptionally low degree of polytypism for interwire separations exceeding a critical value. We record emission spectra over more than five orders of excitation density for both steady-state and pulsed excitation to identify the nature of the recombination channels. An abrupt Mott transition from excitonic to electron-hole-plasma recombination is observed, and the corresponding Mott density is derived. Combining these experiments with simulations and additional direct measurements of the external quantum efficiency using a perfect diffuse reflector as reference, we are able to extract the internal quantum efficiency as a function of carrier density and temperature as well as the extraction efficiency of the nanowire array. The results vividly document the high potential of GaAs/(Al,Ga)As core/shell nanowires for efficient light emitters integrated on the Si platform. Furthermore, the methodology established in this work can be applied to nanowires of any other materials system of interest for optoelectronic applications

    Adherence to the Mediterranean Diet in Pregnancy and Its Benefits on Maternal-Fetal Health: A Systematic Review of the Literature

    Introduction: Pregnancy is a transcendent period for the mother and the fetus, characterized by an increase on energy requirements. Mediterranean diet (MD) is considered a healthy eating pattern that can provide the nutritional requirements of pregnancy and protect from the development of obstetric pathologies. Objective: To know the relationship between adherence to the MD and its maternal-fetal benefits. Methodology: A systematic review was conducted by identifying articles in the PubMed and Cochrane databases. The publication date of the studies was between 2010 and 2020, and the inclusion criteria established were that the articles were written in English and Spanish and were accessible in full text. Studies concerning assisted reproduction, gene modulation, conference abstracts, systematic reviews, and pilot studies were excluded. Results: Finally, a total of 14 studies were included in the review. The association between the MD and the reduction of some pathologies of pregnancy, such as gestational diabetes, overweight or obesity, sleep quality, complications of childbirth, urinary tract infections (UTIs), and alterations in fetal growth was demonstrated, as well as perinatal problems, including birth weight, prematurity, gastroschisis, and other childhood problems. Conclusion: The MD is an optimal diet to consume during pregnancy

    Effect of molasses application alone or combined with trichoderma asperellum T-34 on Meloidogyne spp. management and soil microbial activity in organic production systems

    The effect of molasses alone or combined with Trichoderma asperellum T34 Biocontrol® was assessed on Meloidogyne reproduction, disease severity, and density and activity of soil microor- ganisms in pot and field experiments. Firstly, molasses application at 1 mL m−2 was assessed in four different textured soils. Secondly, molasses application at 5, 10, 20, and 40 mL m−2, alone or combined with T34, was assessed in pot and field experiments at 10 mL m−2 in two different textured soils. The application of 1 mL m−2 of molasses was effective in reducing nematode reproduction in the loam textured soil but not in sandy clay loam, sandy loam, or clay loam textured soils. Increasing molasses dosage reduced the tomato dry shoot and fresh root weights, producing phytotoxicity at 40 mL m−2.     El diseño de prácticas docentes de realidad simulada constituye un avance en el proceso de enseñanza-aprendizaje en el alumnado de Criminología. Mediante este tipo de actividades, los estudiantes desarrollan las competencias académicas y profesionales adquiridas durante su formación, recreando contextos y situaciones reales en un entorno de aprendizaje. El objetivo de este trabajo es evaluar las competencias adquiridas por el alumnado de último curso del Grado en Criminología, tras la implementación de una práctica de realidad simulada basada en la recreación de un Juicio donde el alumnado deberá poner en práctica los conocimientos y técnicas de evaluación criminológica aplicados al ámbito de la Justicia Penal. Se describen los resultados de aprendizaje y la satisfacción del alumnado con la incorporación de esta experiencia piloto mediante el análisis de respuestas de un cuestionario informatizado ad hoc

    Helios expression coordinates the development of a subset of striatopallidal medium spiny neurons

    Here we unravel the mechanism of action of Helios (He) during the development of striatal medium spiny neurons (MSNs). He regulates the second wave of striatal neurogenesis involved in the generation of striatopallidal neurons that express dopamine 2 receptor (D2R) and enkephalin (ENK). To exert this effect He is expressed in neural progenitor cells (NPCs) retaining them into the G1/G0 phase of the cell cycle. Thus, the lack of He produces an increase of S-phase entry and S-phase length of NPCs which in turn impairs striatal neurogenesis and produces an accumulation of the number of cycling NPCs in the germinal zone (GZ) that end up dying at postnatal stages. Therefore, He-/- mice show a reduction in the number of Dorso-Medial Striatal MSNs in the adulthood that produces deficits in motor skills acquisition. In addition, overexpression of He in NPCs induce DARPP32 phenotype when transplanted in mouse striatum.Present findings demonstrate that He is involved in the correct development of a subset of striatopallidal MSNs and reveal new cellular mechanisms for neuronal development

    Designing for user attention: a method for supporting unobtrusive routine tasks

However, this automation may be annoying since some tasks may grab users attention in inappropriate situations. Since user attention is a valuable resource, task automation must behave in a considerate manner demanding user attention only when it is required. To address this issue, this work presents a systematic method for supporting the design and automation of unobtrusive routine tasks that can adjust their obtrusiveness level at runtime according to the user attentional resources and context. This method proposes to design the routine tasks that the system must carry out and how they must interact with users in terms of obtrusiveness. The method also provides a software infrastructure that makes the execution of the tasks at the appropriate obtrusiveness degree a reality.
