    Cognitive and Metacognitive Factors in Reading Comprehension for Occupational Therapy Assistant Students

    Difficulties with reading comprehension among occupational therapy assistant (OTA) students can diminish their ability to understand didactic textual information limiting clinical performance and can impact OTA attrition rates and limit graduate success when completing the national certification examination. Participants were one cohort of OTA students from a rural public community college. This was a convergent mixed-method design (Creswell & Creswell, 2018). The students’ perceptions of cognitive and metacognitive factors affecting reading and the students’ preferences for cognitive and metacognitive strategies were assessed through a 59-item survey assessment combining the Text-Learning Strategies Inventory (TLSI) (Merchie, Van Keer, & Vandevelde, 2014), the Metacomprehension Scale (MCS) (Moore, Zabrucky, & Commander, 1993) and three open-ended questions, respectively. Two course examinations were also reviewed, pre- and post reading comprehension strategy instruction. After the instruction, perceptions increased for 50 percent of survey items with a p=value of .001 and a Cohen’s d medium effect size (.314). Course exams scores increased for 70 percent of participants with a p=value of .047 and a Cohen’s d large effect size (.608). Error analysis of open-ended questions also showed a change in students’ preferences moving toward higher order thinking strategies. Participants examination performance improved after reading strategy instruction particular to OTA academic material. The participants changed their perceptions and preferences for reading and demonstrated a deeper reading level with text-based information and examination questions. Data indicated a significant improvement in OTA student examination performance and change of OTA students’ perceptions of cognitive factors and metacognitive factors associated with reading comprehension

    DataComm in Flight Deck Surface Trajectory-Based Operations

    The purpose of this pilot-in-the-loop aircraft taxi simulation was to evaluate a NextGen concept for surface trajectory-based operations (STBO) in which air traffic control (ATC) issued taxi clearances with a required time of arrival (RTA) by Data Communications (DataComm). Flight deck avionics, driven by an error-nulling algorithm, displayed the speed needed to meet the RTA. To ensure robustness of the algorithm, the ability of 10 two-pilot crews to meet the RTA was tested in nine experimental trials representing a range of realistic conditions including a taxi route change, an RTA change, a departure clearance change, and a crossing traffic hold scenario. In some trials, these DataComm taxi clearances or clearance modifications were accompanied by preview information, in which the airport map display showed a preview of the proposed route changes, including the necessary speed to meet the RTA. Overall, the results of this study show that with the aid of the RTA speed algorithm, pilots were able to meet their RTAs with very little time error in all of the robustness-testing scenarios. Results indicated that when taxi clearance changes were issued by DataComm only, pilots required longer notification distances than with voice communication. However, when the DataComm was accompanied by graphical preview, the notification distance required by pilots was equivalent to that for voice

    Near-Infrared Spectroscopy of Low Mass X-ray Binaries : Accretion Disk Contamination and Compact Object Mass Determination in V404 Cyg and Cen X-4

    We present near-infrared (NIR) broadband (0.80--2.42 μ\mum) spectroscopy of two low mass X-ray binaries: V404 Cyg and Cen X-4. One important parameter required in the determination of the mass of the compact objects in these systems is the binary inclination. We can determine the inclination by modeling the ellipsoidal modulations of the Roche-lobe filling donor star, but the contamination of the donor star light from other components of the binary, particularly the accretion disk, must be taken into account. To this end, we determined the donor star contribution to the infrared flux by comparing the spectra of V404 Cyg and Cen X-4 to those of various field K-stars of known spectral type. For V404 Cyg, we determined that the donor star has a spectral type of K3 III. We determined the fractional donor contribution to the NIR flux in the H- and K-bands as 0.98±.050.98 \pm .05 and 0.97±.090.97 \pm .09, respectively. We remodeled the H-band light curve from \citet{sanwal1996} after correcting for the donor star contribution to obtain a new value for the binary inclination. From this, we determined the mass of the black hole in V404 Cyg to be MBH=9.0.6+.2MM_{BH}= 9.0^{+.2}_{-.6}M_{\odot}. We performed the same spectral analysis for Cen X-4 and found the spectral type of the donor star to be in the range K5 -- M1V. The donor star contribution in Cen X-4 is 0.94±.140.94\pm.14 in the H-band while in the K-band, the accretion disk can contribute up to 10% of the infrared flux. We remodeled the H-band light curve from \citet{shahbaz1993}, again correcting for the fractional contribution of the donor star to obtain the inclination. From this, we determined the mass of the neutron star as MNS=1.5.4+.1MM_{NS}= 1.5^{+.1}_{-.4}M_{\odot}. However, the masses obtained for both systems should be viewed with some caution since contemporaneous light curve and spectral data are required to obtain definitive masses

    Association of prenatal exposure to early-life adversity with neonatal brain volumes at birth

    Importance: Exposure to early-life adversity alters the structural development of key brain regions underlying neurodevelopmental impairments. The association between prenatal exposure to adversity and brain structure at birth remains poorly understood. Objective: To examine whether prenatal exposure to maternal social disadvantage and psychosocial stress is associated with neonatal global and regional brain volumes and cortical folding. Design, Setting, and Participants: This prospective, longitudinal cohort study included 399 mother-infant dyads of sociodemographically diverse mothers recruited in the first or early second trimester of pregnancy and their infants, who underwent brain magnetic resonance imaging in the first weeks of life. Mothers were recruited from local obstetric clinics in St Louis, Missouri from September 1, 2017, to February 28, 2020. Exposures: Maternal social disadvantage and psychosocial stress in pregnancy. Main Outcomes and Measures: Confirmatory factor analyses were used to create latent constructs of maternal social disadvantage (income-to-needs ratio, Area Deprivation Index, Healthy Eating Index, educational level, and insurance status) and psychosocial stress (Perceived Stress Scale, Edinburgh Postnatal Depression Scale, Everyday Discrimination Scale, and Stress and Adversity Inventory). Neonatal cortical and subcortical gray matter, white matter, cerebellum, hippocampus, and amygdala volumes were generated using semiautomated, age-specific, segmentation pipelines. Results: A total of 280 mothers (mean [SD] age, 29.1 [5.3] years; 170 [60.7%] Black or African American, 100 [35.7%] White, and 10 [3.6%] other race or ethnicity) and their healthy, term-born infants (149 [53.2%] male; mean [SD] infant gestational age, 38.6 [1.0] weeks) were included in the analysis. After covariate adjustment and multiple comparisons correction, greater social disadvantage was associated with reduced cortical gray matter (unstandardized β = -2.0; 95% CI, -3.5 to -0.5; P = .01), subcortical gray matter (unstandardized β = -0.4; 95% CI, -0.7 to -0.2; P = .003), and white matter (unstandardized β = -5.5; 95% CI, -7.8 to -3.3; P \u3c .001) volumes and cortical folding (unstandardized β = -0.03; 95% CI, -0.04 to -0.01; P \u3c .001). Psychosocial stress showed no association with brain metrics. Although social disadvantage accounted for an additional 2.3% of the variance of the left hippocampus (unstandardized β = -0.03; 95% CI, -0.05 to -0.01), 2.3% of the right hippocampus (unstandardized β = -0.03; 95% CI, -0.05 to -0.01), 3.1% of the left amygdala (unstandardized β = -0.02; 95% CI, -0.03 to -0.01), and 2.9% of the right amygdala (unstandardized β = -0.02; 95% CI, -0.03 to -0.01), no regional effects were found after accounting for total brain volume. Conclusions and Relevance: In this baseline assessment of an ongoing cohort study, prenatal social disadvantage was associated with global reductions in brain volumes and cortical folding at birth. No regional specificity for the hippocampus or amygdala was detected. Results highlight that associations between poverty and brain development begin in utero and are evident early in life. These findings emphasize that preventive interventions that support fetal brain development should address parental socioeconomic hardships

    Alterations of immune response of non-small lung cancer with azacytidine

    Innovative therapies are needed for advanced Non-Small Cell Lung Cancer (NSCLC). We have undertaken a genomics based, hypothesis driving, approach to query an emerging potential that epigenetic therapy may sensitize to immune checkpoint therapy targeting PD-L1/PD-1 interaction. NSCLC cell lines were treated with the DNA hypomethylating agent azacytidine (AZA - Vidaza) and genes and pathways altered were mapped by genome-wide expression and DNA methylation analyses. AZA-induced pathways were analyzed in The Cancer Genome Atlas (TCGA) project by mapping the derived gene signatures in hundreds of lung adeno (LUAD) and squamous cell carcinoma (LUSC) samples. AZA up-regulates genes and pathways related to both innate and adaptive immunity and genes related to immune evasion in a several NSCLC lines. DNA hypermethylation and low expression of IRF7, an interferon transcription factor, tracks with this signature particularly in LUSC. In concert with these events, AZA up-regulates PD-L1 transcripts and protein, a key ligand-mediator of immune tolerance. Analysis of TCGA samples demonstrates that a significant proportion of primary NSCLC have low expression of AZA-induced immune genes, including PD-L1. We hypothesize that epigenetic therapy combined with blockade of immune checkpoints - in particular the PD-1/PD-L1 pathway - may augment response of NSCLC by shifting the balance between immune activation and immune inhibition, particularly in a subset of NSCLC with low expression of these pathways. Our studies define a biomarker strategy for response in a recently initiated trial to examine the potential of epigenetic therapy to sensitize patients with NSCLC to PD-1 immune checkpoint blockade

    Neonatal motor functional connectivity and motor outcomes at age two years in very preterm children with and without high-grade brain injury

    Preterm-born children have high rates of motor impairments, but mechanisms for early identification remain limited. We hypothesized that neonatal motor system functional connectivity (FC) would relate to motor outcomes at age two years; currently, this relationship is not yet well-described in very preterm (VPT; born \u3c32 weeks\u27 gestation) infants with and without brain injury. We recruited 107 VPT infants - including 55 with brain injury (grade III-IV intraventricular hemorrhage, cystic periventricular leukomalacia, post-hemorrhagic hydrocephalus) - and collected FC data at/near term-equivalent age (35-45 weeks postmenstrual age). Correlation coefficients were used to calculate the FC between bilateral motor and visual cortices and thalami. At two years corrected-age, motor outcomes were assessed with the Bayley Scales of Infant and Toddler Development, 3rd edition. Multiple imputation was used to estimate missing data, and regression models related FC measures to motor outcomes. Within the brain-injured group only, interhemispheric motor cortex FC was positively related to gross motor outcomes. Thalamocortical and visual FC were not related to motor scores. This suggests neonatal alterations in motor system FC may provide prognostic information about impairments in children with brain injury

    Monkeypox Transmission and Pathogenesis in Prairie Dogs

    During May and June 2003, the first cluster of human monkeypox cases in the United States was reported. Most patients with this febrile vesicular rash illness presumably acquired the infection from prairie dogs. Monkeypox virus was demonstrated by using polymerase chain reaction in two prairie dogs in which pathologic studies showed necrotizing bronchopneumonia, conjunctivitis, and tongue ulceration. Immunohistochemical assays for orthopoxviruses demonstrated abundant viral antigens in surface epithelial cells of lesions in conjunctiva and tongue, with less amounts in adjacent macrophages, fibroblasts, and connective tissues. Viral antigens in the lung were abundant in bronchial epithelial cells, macrophages, and fibroblasts. Virus isolation and electron microscopy demonstrated active viral replication in lungs and tongue. These findings indicate that both respiratory and direct mucocutaneous exposures are potentially important routes of transmission of monkeypox virus between rodents and to humans. Prairie dogs offer insights into transmission, pathogenesis, and new vaccine and treatment trials because they are susceptible to severe monkeypox infection

    Exclusive Photoproduction of the Cascade (Xi) Hyperons

    We report on the first measurement of exclusive Xi-(1321) hyperon photoproduction in gamma p --> K+ K+ Xi- for 3.2 < E(gamma) < 3.9 GeV. The final state is identified by the missing mass in p(gamma,K+ K+)X measured with the CLAS detector at Jefferson Laboratory. We have detected a significant number of the ground-state Xi-(1321)1/2+, and have estimated the total cross section for its production. We have also observed the first excited state Xi-(1530)3/2+. Photoproduction provides a copious source of Xi's. We discuss the possibilities of a search for the recently proposed Xi5-- and Xi5+ pentaquarks.Comment: submitted to Phys. Rev.

    The Prevalence and Influence of the Combination of Humor and Violence in Super Bowl Commercials

    The growing concern over violence in the media has led to vast amounts of research examining the effects of violent media on viewers. An important subset of this research looks at how humor affects this relationship. While research has considered this subset in television programming, almost no research has explored this in the context of advertising. This paper builds on the little research that exists by examining the effects of combining humor and violence, as well as the theoretical approaches that underlie these effects. A content analysis is conducted to identify the prevalence of violence, humor, and the combination of these elements in a longitudinal sample of Super Bowl commercials (2005, 2007, and 2009). Further, we investigate the relationship between the joint occurrence of humor and violence in ads and ad popularity. We conclude that violent acts are rampant in these commercials and that many acts are camouflaged by the simultaneous presence of humor, especially in the most popular ads
