Advance care planning for patients with advanced illnesses attending hospital outpatient clinics study: A study protocol for a randomised controlled trial

Introduction It is unclear whether advance care planning (ACP) undertaken with patients living in the community can improve patient care and avoid unwanted interventions and hospital admissions. We have designed a randomised controlled trial (RCT) to examine if ACP undertaken with patients with advanced illnesses attending hospital outpatient clinics can reduce unplanned hospital admissions and improve patient and caregiver well-being. Methods and analysis Pragmatic RCT involving patients from subspecialty outpatient clinics at five clinical sites in Sydney, Australia. Participants will be ≥18 years screened as potentially having palliative care needs and at risk of dying in 6-12 months. The patients will be randomised to intervention or control group. Intervention group will undertake ACP discussions facilitated by a trained health professional. The control group will receive written information on ACP, representing the current standard of care. The primary outcome is the number of unplanned hospital admissions at the 6-month follow-up. Secondary outcomes include: (i) patient's health-related quality-of-life and quality of chronic disease care; (ii) caregiver's health-related quality-of-life and caregiver burden and (iii) other health outcomes including ambulance usage, emergency department presentations, hospital admissions, resuscitation attempts, intensive care unit admissions, deaths, documentation of patient wishes in patient records and audit of ACP discussions and documents. The staff's self-reported attitudes and knowledge of ACP will also be measured. The data will be collected using self-report questionnaires, hospital records audit, audit of ACP documentation and data linkage analysis. Semistructured interviews and focus group discussions with patients, caregivers and healthcare professionals will explore the acceptability and feasibility of the intervention. Ethics and dissemination Approved by South-East Sydney Local Health District Human Research Ethics Committee and NSW Population and Health Services Research Ethics Committee. HIV prevalence and undiagnosed infection among a community sample of gay and bisexual men in Scotland, 2005-2011: implications for HIV testing policy and prevention
<b>Objective</b><p></p>
To examine HIV prevalence, HIV testing behaviour, undiagnosed infection and risk factors for HIV positivity among a community sample of gay men in Scotland.<p></p>
<b>Methods</b><p></p>
Cross-sectional survey of gay and bisexual men attending commercial gay venues in Glasgow and Edinburgh, Scotland with voluntary anonymous HIV testing of oral fluid samples in 2011. A response rate of 65.2% was achieved (1515 participants).<p></p>
<b>Results</b><p></p>
HIV prevalence (4.8%, 95% confidence interval, CI 3.8% to 6.2%) remained stable compared to previous survey years (2005 and 2008) and the proportion of undiagnosed infection among HIV-positive men (25.4%) remained similar to that recorded in 2008. Half of the participants who provided an oral fluid sample stated that they had had an HIV test in the previous 12 months; this proportion is significantly higher when compared to previous study years (50.7% versus 33.8% in 2005, p<0.001). Older age (>25 years) was associated with HIV positivity (1.8% in those <25 versus 6.4% in older ages group) as was a sexually transmitted infection (STI) diagnosis within the previous 12 months (adjusted odds ratio 2.13, 95% CI 1.09–4.14). There was no significant association between age and having an STI or age and any of the sexual behaviours recorded.<p></p>
<b>Conclusion</b><p></p>
HIV transmission continues to occur among gay and bisexual men in Scotland. Despite evidence of recent testing within the previous six months, suggesting a willingness to test, the current opt-out policy may have reached its limit with regards to maximising HIV test uptake. Novel strategies are required to improve regular testing opportunities and more frequent testing as there are implications for the use of other biomedical HIV interventions.<p></p>
The Baryon Oscillation Spectroscopic Survey of SDSS-III
The Baryon Oscillation Spectroscopic Survey (BOSS) is designed to measure the
scale of baryon acoustic oscillations (BAO) in the clustering of matter over a
larger volume than the combined efforts of all previous spectroscopic surveys
of large scale structure. BOSS uses 1.5 million luminous galaxies as faint as
i=19.9 over 10,000 square degrees to measure BAO to redshifts z<0.7.
Observations of neutral hydrogen in the Lyman alpha forest in more than 150,000
quasar spectra (g<22) will constrain BAO over the redshift range 2.15<z<3.5.
Early results from BOSS include the first detection of the large-scale
three-dimensional clustering of the Lyman alpha forest and a strong detection
from the Data Release 9 data set of the BAO in the clustering of massive
galaxies at an effective redshift z = 0.57. We project that BOSS will yield
measurements of the angular diameter distance D_A to an accuracy of 1.0% at
redshifts z=0.3 and z=0.57 and measurements of H(z) to 1.8% and 1.7% at the
same redshifts. Forecasts for Lyman alpha forest constraints predict a
measurement of an overall dilation factor that scales the highly degenerate
D_A(z) and H^{-1}(z) parameters to an accuracy of 1.9% at z~2.5 when the survey

Search for Large Extra Dimensions in Dielectron and Diphoton Production
We report a search for effects of large extra spatial dimensions in ppbar
collisions at a center-of-mass energy of 1.8 TeV with the DZero detector, using
events containing a pair of electrons or photons. The data are in good
agreement with the expected background and do not exhibit evidence for large
extra dimensions. We set the most restrictive lower limits to date, at the 95%
confidence level, on the effective Planck scale between 1.0 TeV and 1.4 TeV for

Search for bottom squarks in pbarp collisions at sqrt(s)=1.8 TeV
We report on a search for bottom squarks produced in pbarp collisions at
sqrt(s) = 1.8 TeV using the D0 detector at Fermilab. Bottom squarks are assumed
to be produced in pairs and to decay to the lightest supersymmetric particle
(LSP) and a b quark with branching fraction of 100%. The LSP is assumed to be
the lightest neutralino and stable. We set limits on the production cross

Search for Squarks and Gluinos in Events Containing Jets and a Large Imbalance in Transverse Energy
Using data corresponding to an integrated luminosity of 79 pb-1, D0 has
searched for events containing multiple jets and large missing transverse
energy in pbar-p collisions at sqrt(s)=1.8 TeV at the Fermilab Tevatron
collider. Observing no significant excess beyond what is expected from the
standard model, we set limits on the masses of squarks and gluinos and on the
model parameters m_0 and m_1/2, in the framework of the minimal low-energy
supergravity models of supersymmetry. For tan(beta) = 2 and A_0 = 0, with mu <
0, we exclude all models with m_squark < 250 GeV/c^2. For models with equal

Differential Production Cross Section of Z Bosons as a Function of Transverse Momentum at sqrt{s}=1.8 TeV
We present a measurement of the transverse momentum distribution of Z bosons
produced in ppbar collisions at sqrt{s}=1.8 TeV using data collected by the D0
experiment at the Fermilab Tevatron Collider during 1994--1996. We find good
agreement between our data and a current resummation calculation. We also use
our data to extract values of the non-perturbative parameters for a particular
version of the resummation formalism, obtaining significantly more precise

Search for Top Squark Pair Production in the Dielectron Channel
This report describes the first search for top squark pair production in the
channel stop_1 stopbar_1 -> b bbar chargino_1 chargino_1 -> ee+jets+MEt using
74.9 +- 8.9 pb^-1 of data collected using the D0 detector. A 95% confidence
level upper limit on sigma*B is presented. The limit is above the theoretical
expectation for sigma*B for this process, but does show the sensitivity of the

Search for a Fourth Generation Charge -1/3 Quark via Flavor Changing Neutral Current Decay
We report on a search for pair production of a fourth generation charge -1/3
quark (b') in pbar p collisions at sqrt(s) = 1.8 TeV at the Fermilab Tevatron
using an integrated luminosity of 93 pb^-1. Both quarks are assumed to decay
via flavor changing neutral currents (FCNC). The search uses the signatures
gamma + 3 jets + mu-tag and 2 gamma + 2 jets. We see no significant excess of
events over the expected background. We place an upper limit on the production
cross section times branching fraction that is well below theoretical
expectations for a b' quark decaying exclusively via FCNC for b' quark masses
