Explainable artificial intelligence toward usable and trustworthy computer-aided early diagnosis of multiple sclerosis from Optical Coherence Tomography
Background: Several studies indicate that the anterior visual pathway
provides information about the dynamics of axonal degeneration in Multiple
Sclerosis (MS). Current research in the field is focused on the quest for the
most discriminative features among patients and controls and the development of
machine learning models that yield computer-aided solutions widely usable in
clinical practice. However, most studies are conducted with small samples and
the models are used as black boxes. Clinicians should not trust machine
learning decisions unless they come with comprehensive and easily
understandable explanations. Materials and methods: A total of 216 eyes from
111 healthy controls and 100 eyes from 59 patients with relapsing-remitting MS
were enrolled. The feature set was obtained from the thickness of the ganglion
cell layer (GCL) and the retinal nerve fiber layer (RNFL). Measurements were
acquired by the novel Posterior Pole protocol from Spectralis Optical Coherence
Tomography (OCT) device. We compared two black-box methods (gradient boosting
and random forests) with a glass-box method (explainable boosting machine).
Explainability was studied using SHAP for the black-box methods and the scores
of the glass-box method. Results: The best-performing models were obtained for
the GCL layer. Explainability pointed out to the temporal location of the GCL
layer that is usually broken or thinning in MS and the relationship between low
thickness values and high probability of MS, which is coherent with clinical
knowledge. Conclusions: The insights on how to use explainability shown in this
work represent a first important step toward a trustworthy computer-aided
solution for the diagnosis of MS with OCT

Tailored Graphenic Structures Directly Grown on Titanium Oxide Boost the Interfacial Charge Transfer
The successful application of titanium oxide-graphene hybrids in the fields
of photocatalysis, photovoltaics and photodetection strongly depends on the
interfacial contact between both materials. The need to provide a good coupling
between the enabling conductor and the photoactive phase prompted us to
directly grow conducting graphenic structures on TiO2 crystals. We here report
on the direct synthesis of tailored graphenic structures by using Plasma
Assisted Chemical Vapour Deposition that present a clean junction with the
prototypical titanium oxide (110) surface. Chemical analysis of the interface
indicates chemical bonding between both materials. Photocurrent measurements
under UV light illumination manifest that the charge transfer across the
interface is efficient. Moreover, the influence of the synthesis atmosphere,
gas precursor (C2H2) and diluents (Ar, O2), on the interface and on the
structure of the as-grown graphenic material is assessed. The inclusion of O2
promotes vertical growth of partially oxidized carbon nanodots/rods with
controllable height and density. The deposition with Ar results in continuous
graphenic films with low resistivity (6.8x10-6 ohm x m). The synthesis
protocols developed here are suitable to produce tailored carbon-semiconductor
structures on a variety of practical substrates as thin films, pillars or

Benefit-risk profile of cytoreductive drugs along with antiplatelet and antithrombotic therapy after transient ischemic attack or ischemic stroke in myeloproliferative neoplasms
We analyzed 597 patients with myeloproliferative neoplasms (MPN) who presented transient ischemic attacks (TIA, n =
270) or ischemic stroke (IS, n = 327). Treatment included aspirin, oral anticoagulants, and cytoreductive drugs. The
composite incidence of recurrent TIA and IS, acute myocardial infarction (AMI), and cardiovascular (CV) death was 4.21
and 19.2%, respectively at one and five years after the index event, an estimate unexpectedly lower than reported in
the general population. Patients tended to replicate the first clinical manifestation (hazard ratio, HR: 2.41 and 4.41 for
recurrent TIA and IS, respectively); additional factors for recurrent TIA were previous TIA (HR: 3.40) and microvascular
disturbances (HR: 2.30); for recurrent IS arterial hypertension (HR: 4.24) and IS occurrence after MPN diagnosis (HR: 4.47).
CV mortality was predicted by age over 60 years (HR: 3.98), an index IS (HR: 3.61), and the occurrence of index events
after MPN diagnosis (HR: 2.62). Cytoreductive therapy was a strong protective factor (HR: 0.24). The rate of major
bleeding was similar to the general population (0.90 per 100 patient-years). In conclusion, the long-term clinical
outcome after TIA and IS in MPN appears even more favorable than in the general population, suggesting an
advantageous benefit-risk profile of antithrombotic and cytoreductive treatment
Epidemiology of childhood tuberculosis in Spain: 2005-2009
BACKGROUND: European recent data about paediatric tuberculosis point out the importance of evaluate the trends of the disease to study the recent transmission, as well as the necessity of improving the microbiological diagnosis in paediatric cases. The aim of this paper is to study the epidemiology and trend evolution of paediatric tuberculosis in Spain during the period 2005-2009 and to establish the epidemiological differences between adult and paediatric tuberculosis. METHODS: Data reported to the National Surveillance Net (Red Nacional de Vigilancia Epidemiológica) in Spain was checked. Lineal regression was developed to establish the trend of the disease in all, adult and paediatric cases. Bivariate and multivariate logistic regression was used to compare paediatric and adult cases reported in 2009 and estimate the influence of different factors in the development of the disease. RESULTS: A total 39775, 2690 paediatrics (6.76%) cases of tuberculosis were reported during 2005-2009 period. Paediatric tuberculosis rates showed a slight increasing tendency (y=0.15x+7.8), while adult rates decrease during the period (y=-0.28x+20.2). In 2009, rates were 8.1 and 18.3 cases/100,000 inhab. for children and adults respectively. Paediatric cases presented higher proportion of pulmonary locations (84% vs. 76% in adults) and lower percentages of cases confirmed by culture (51% vs. 82% in adults) and of cases in non-Spanish population (25% vs. 34%). CONCLUSIONS: Paediatric tuberculosis rates showed a slight increasing tendency, while global and adult rates decrease slightly during the period. Tuberculosis disease shows different epidemiology in children and adults, what it is important to take into account to design public heh interventions
Investigaciones paleobotánicas en la cuenca central del Duero
El objetivo del trabajo es dar a conocer el estado actual de conocimientos cientÃficos sobre el pasado del paisaje vegetal (Cuaternario final) en los territorios interiores no montanos de la depresión del Duero. Se recogen todos los yacimientos cuyo estudio ya ha concluido asà como los que se encuentran en fase de investigación o prospección. Se precisa el tipo de informador en cada caso (polen, carbones, maderas, otros macrorrestos), el rango cronológico conocido hasta el momento asà como el grado o proporción de trabajo realizado en cada yacimiento en relación con las previsiones efectuadas. Se aporta una sÃntesis-resumen de los principales resultados obtenidos hasta el momento y de los aspectos más concluyentes de los mismos en relación con la elaboración de modelos de evolución del paisaje vegetal posteriores al último máximo glacial en la Meseta norte. A nuestro juicio debe destacarse, como uno de los resultados más relevantes, el conocimiento ya afianzado de que los pinares de meseta han sido el elemento más significativo en amplios sectores del sur y este de la cuenca a lo largo de todo o gran parte del Holoceno, circunstancia que contrasta con todas las propuestas de paisaje pretérito (preantrópico) existentes antes de la realización de las prospecciones paleobotánicas
SALMANTICOR study. Rationale and design of a population-based study to identify structural heart disease abnormalities: a spatial and machine learning analysis
[EN]Introduction: This study aims to obtain data on the prevalence and incidence of structural heart disease in a population setting and, to analyse and present those data on the application of spatial and machine learning methods that, although known to geography and statistics, need to become used for healthcare research and for political commitment to obtain resources and support effective public health programme implementation.
Methods and analysis: We will perform a cross-sectional survey of randomly selected residents of Salamanca (Spain). 2400 individuals stratified by age and sex and by place of residence (rural and urban) will be studied. The variables to analyse will be obtained from the clinical history, different surveys including social status, Mediterranean diet, functional capacity, ECG, echocardiogram, VASERA and biochemical as well as genetic analysis.
Ethics and dissemination: The study has been approved by the ethical committee of the healthcare community. All study participants will sign an informed consent for participation in the study. The results of this study will allow the understanding of the relationship between the different influencing factors and their relative importance weights in the development of structural heart disease
A Cautionary Tale: MARVELS Brown Dwarf Candidate Reveals Itself To Be A Very Long Period, Highly Eccentric Spectroscopic Stellar Binary
We report the discovery of a highly eccentric, double-lined spectroscopic
binary star system (TYC 3010-1494-1), comprising two solar-type stars that we
had initially identified as a single star with a brown dwarf companion. At the
moderate resolving power of the MARVELS spectrograph and the spectrographs used
for subsequent radial-velocity (RV) measurements (R ~ <30,000), this particular
stellar binary mimics a single-lined binary with an RV signal that would be
induced by a brown dwarf companion (Msin(i)~50 M_Jup) to a solar-type primary.
At least three properties of this system allow it to masquerade as a single
star with a very low-mass companion: its large eccentricity (e~0.8), its
relatively long period (P~238 days), and the approximately perpendicular
orientation of the semi-major axis with respect to the line of sight (omega~189
degrees). As a result of these properties, for ~95% of the orbit the two sets
of stellar spectral lines are completely blended, and the RV measurements based
on centroiding on the apparently single-lined spectrum is very well fit by an
orbit solution indicative of a brown dwarf companion on a more circular orbit
(e~0.3). Only during the ~5% of the orbit near periastron passage does the
true, double-lined nature and large RV amplitude of ~15 km/s reveal itself. The
discovery of this binary system is an important lesson for RV surveys searching
for substellar companions; at a given resolution and observing cadence, a
survey will be susceptible to these kinds of astrophysical false positives for
a range of orbital parameters. Finally, for surveys like MARVELS that lack the
resolution for a useful line bisector analysis, it is imperative to monitor the
peak of the cross-correlation function for suspicious changes in width or
shape, so that such false positives can be flagged during the candidate vetting
