    Adult Attachment, Differentiation of Self, and Relationship Satisfaction in Lesbians and Gay Men

    AbstractWe examined adult attachment styles, differentiation of self, and relationship satisfaction in a sample of 298 Italian lesbians (48%) and gay men (52%), all of whom were ages 19–71 years (M = 36.1; SD = 11.8) and in a couple relationship for at least six months. Participants were recruited via the Internet and completed a web-based survey. We tested the hypotheses that attachment insecurity and differentiation of self would predict relationship satisfaction, as well as that differentiation of self would mediate the relationship between adult attachment and relationship satisfaction. Results supported the hypotheses, thus indicating that attachment insecurity and differentiation of self were correlated with and predicted relationship satisfaction. Furthermore, results supported the hypothesized mediating role of differentiation of self. Finally, results indicated that younger participants, lesbians, and participants in civil unions reported higher levels of relationship satisfaction. Implications for counselors and therapists working with LG populations are discussed

    On the occurrence of the Neapolitan Yellow Tuff tephra in the Northern Phlegraean Fields offshore (Eastern Tyrrhenian margin; Italy)

    A main volcanic marker has been identified for the first time on the continental shelf of the northern Phlegraean Fields in the Gaeta Gulf (Campania region, eastern Tyrrhenian margin, Italy) by means of Subbottom Chirp profile grid and stratigraphic analysis of a core collected on the slope. In the seismic sections, the core bottom corresponds to the top of a continuous and parallel reflector (V) interbedded within the transgressive deposits of the Late Quaternary-Holocene depositional sequence. The Transgressive System Tract deposits are particularly thick compared to the majority of the transgressive deposits of other shelf settings. This might be due to the input of pyroclastic and volcanoclastic deposits related to the intense eruptive activity of the Campania Plain during the Late Pleistocene-Holocene time span. Undulations and pockmarks are the main morphological features of the sea floor and they might be linked to gas uprising, widely detected in the study area. The V reflector is located on the shelf from northeast to southwest at different depths, ranging from 10 ms (about 8 m) to 30 ms (about 25 m) below sea floor and it can be mapped down to the continental slope. The geological calibration of this continuous reflector coupled with tephrostratigraphic analysis, allowed to correlate it with the Neapolitan Yellow Tuff deposits emplaced at Phlegraean Fields at ca. 15 ka

    Retinitis pigmentosa is a group of diseases caused by genetic changes that lead to progressive degeneration of photoreceptors, rods mainly. In general, it has bilateral presentation. This study is a case report of a patient with unilateral involvement of the retina, similar to the characteristics of retinitis pigmentosa, and an old ocular trauma history. It describes her history and ophthalmologic findings.A retinose pigmentada constitui um grupo de doenças causadas por alterações genéticas que levam à degeneração progressiva dos fotorreceptores, principalmente bastonetes.     Comparison of Monocyte Distribution Width (MDW) and Procalcitonin for early recognition of sepsis

    We carried out a prospective observational study to evaluate whether Monocyte Distribution Width (MDW) may play a role in identifying patients with sepsis in comparison with Procalcitonin (PCT). We prospectively enrolled all consecutive patients hospitalized at the Infectious Diseases Unit of Pescara General Hospital for bacterial infection or sepsis. MDW values were collected for all patients. Clinical characteristics, demographic data, past and present medical history, microbiological results, PCT, as well as neutrophil and monocytes indices at entry were compared in the 2 groups. Two-hundred-sixty patients were enrolled, 63.5% males, aged 59.1±19.5 years. Sepsis was diagnosed in 105 (40.4%); in 60 (57.1%) at least 1 microorganism was isolated from blood cultures. In multivariate models, MDW as a continuous variable (OR:1.57 for each unit increase; 95%CI: 1.31-1.87, p<0.001) and PCT˃1 ng/mL (OR: 48.5; 95%CI: 14.7-160.1, p<0.001) were independently associated with sepsis. Statistical best cut points associated with sepsis were 22.0 for MDW and 1.0 ng/mL for PCT whereas MDW values<20 were invariably associated with negative blood cultures. At ROC curve analysis, the AUC of MDW (0.87) was nearly overlapping that of PCT (0.88). Our data suggest that incorporating MDW within current routine WBC counts and indices may be of remarkable use for detection of sepsis. Further research is warranted

    Targeted gene panel screening is an effective tool to identify undiagnosed late onset Pompe disease

    Mutations in the GAA gene may cause a late onset Pompe disease presenting with proximal weakness without the characteristic muscle pathology, and therefore a test for GAA activity is the first tier analysis in all undiagnosed patients with hyperCKemia and/or limb-girdle muscular weakness. By using MotorPlex, a targeted gene panel for next generation sequencing, we analyzed GAA and other muscle diseasegenes in a large cohort of undiagnosed patients with suspected inherited skeletal muscle disorders (n = 504). In this cohort, 275 patients presented with limb-girdle phenotype and/or an isolated hyperCKemia. Mutational analysis identified GAA mutations in ten patients. Further seven affected relatives were identified by segregation studies. All the patients carried the common GAA mutation c.-32-13T > G and a second, previously reported mutation. In the subcohort of 275 patients with proximal muscle weakness and/or hyperCKemia, we identified late-onset Pompe disease in 10 patients. The clinical overlap between Pompe disease and LGMDs or other skeletal muscle disorders suggests that GAA and the genes causing a metabolic myopathy should be analyzed in all the gene panels used for testing neuromuscular patients.     Immunogenicity Is Not Improved by Increased Antigen Dose or Booster Dosing of Seasonal Influenza Vaccine in a Randomized Trial of HIV Infected Adults

    The risk of poor vaccine immunogenicity and more severe influenza disease in HIV necessitate strategies to improve vaccine efficacy.A randomized, multi-centered, controlled, vaccine trial with three parallel groups was conducted at 12 CIHR Canadian HIV Trials Network sites. Three dosing strategies were used in HIV infected adults (18 to 60 years): two standard doses over 28 days, two double doses over 28 days and a single standard dose of influenza vaccine, administered prior to the 2008 influenza season. A trivalent killed split non-adjuvanted influenza vaccine (Fluviral™) was used. Serum hemagglutinin inhibition (HAI) activity for the three influenza strains in the vaccine was measured to assess immunogenicity.297 of 298 participants received at least one injection. Baseline CD4 (median 470 cells/µL) and HIV RNA (76% of patients with viral load <50 copies/mL) were similar between groups. 89% were on HAART. The overall immunogenicity of influenza vaccine across time points and the three influenza strains assessed was poor (Range HAI ≥ 40 =  31-58%). Double dose plus double dose booster slightly increased the proportion achieving HAI titre doubling from baseline for A/Brisbane and B/Florida at weeks 4, 8 and 20 compared to standard vaccine dose. Increased immunogenicity with increased antigen dose and booster dosing was most apparent in participants with unsuppressed HIV RNA at baseline. None of 8 serious adverse events were thought to be immunization-related.Even with increased antigen dose and booster dosing, non-adjuvanted influenza vaccine immunogenicity is poor in HIV infected individuals.     Complicações pós-cirúrgicas da correção da atresia de esôfago

    Este artigo teve como objetivo compreender e evidenciar as complicações pós-cirúrgicas da correção da Atresia Esofágica (AE) e o papel crucial dessas condições na qualidade de vida dos pacientes submetidos a esse procedimento. O estudo enfatiza a importância do diagnóstico precoce para um prognóstico mais favorável, embora a taxa de detecção pré-natal ainda seja limitada. As técnicas cirúrgicas evoluíram, resultando no aumento da taxa de sobrevivência, embora as complicações persistam. Destaca-se a prática comum de inserção de sonda de alimentação transanastomótica, apesar de estar associada a um aumento significativo no risco de estenose anastomótica. As complicações pós-cirúrgicas, tanto precoces quanto tardias, incluem estenose de anastomose, fístulas traqueoesofágicas, disfagia e refluxo gastroesofágico, impactando a qualidade de vida em curto e longo prazo. A correlação entre complicações respiratórias tardias e disfunção das cordas vocais ressalta a complexidade do impacto sistêmico da AE. Em resumo, embora os avanços cirúrgicos tenham melhorado a sobrevida na AE, as complicações pós-cirúrgicas permanecem desafiadoras. Compreender essas complicações e buscar abordagens preventivas são cruciais para otimizar os resultados a longo prazo e a qualidade de vida. A investigação futura deverá centrar-se na identificação de marcadores de risco e estratégias de intervenção para mitigar tais problemas

    Methodological approach for evaluating the geo-exchange potential: VIGOR Project

    In the framework of VIGOR Project, a national project coordinated by the Institute of Geosciences and Earth Resources (CNR-IGG) and sponsored by the Ministry of Economic Development (MiSE), dedicated to the evaluation of geothermal potential in the regions of the Convergence Objective in Italy (Puglia, Calabria, Campania and Sicily), is expected to evaluate the ability of the territory to heat exchange with the ground for air conditioning of buildings. To identify the conditions for the development of low enthalpy geothermal systems collected and organized on a regional scale geological and stratigraphic data useful for the preparation of a specific thematic mapping, able to represent in a synergistic and simplified way the physical parameters (geological, lithostratigraphic, hydrogeological, thermodynamic) that most influence the subsoil behavior for thermal exchange. The litho-stratigraphic and hydrogeological database created for every region led to the production of different cartographic thematic maps, such as the thermal conductivity (lithological and stratigraphical), the surface geothermal flux, the average annual temperature of air, the climate zoning, the areas of hydrogeological restrictions. To obtain a single representation of the geo-exchange potential of the region, the different thematic maps described must be combined together by means of an algorithm, defined on the basis of the SINTACS methodology. The purpose is to weigh the contributions of the involved parameters and to produce a preliminary synthesis map able to identify the territorial use of geothermal heat pump systems, based on the geological characteristics and in agreement with the existing regulatory constraints
