    Satisfação familiar em unidades de cuidados intensivos: revisão integrativa da literatura

    Background: due to the difficulty of evaluating and intensive care unit (ICU) patient’s satisfaction, family satisfaction must be taken into consideration when assessing the quality of the care provided in these units. Objectives: evaluate the feasibility of the quiz 'FS-ICU-24'as an instrument to assess family satisfaction; identify satisfaction levels through a questionnaire with an emphasis on items where satisfaction levels are lower and strategies to increase it. Methodology: an integrative review that analyzes relevant articles published in EBSCO and in FSICU.org data bases between 2014 and 2019. Key words: patient satisfaction; critical care; FS-ICU-24. In order to achieve systematized information, a table with the following points was prepared: authors and publication date, country, study objectives, results and participants. Results: ‘FS-ICU-24’ is a good instrument to assess family satisfaction in the ICU. Family satisfaction levels are high (above 70%). In open-ended questions relatives refer less positive aspects such as the physical environment and communication. Some improvement suggestions are the possibility to increase privacy at waiting rooms and provide communication skills training to the staff. Conclusion: the integrative review shows that the FS-ICU-24 allows family satisfaction assessment and identification of improvements points in ICU.Marco contextual: la dificultad para evaluar la satisfacción del paciente internado en una unidad de cuidados intensivos (UCI) determina que para averiguar la calidad de los cuidados prestados en este tipo de unidades sean consideradas las necesidades y satisfacción familiar. Objetivos: comprobar la viabilidad del 'FS-ICU-24' como instrumento para evaluar la satisfacción familiar; identificando los ítems donde esta satisfacción es más baja y qué estrategias se adoptan para aumentar. Metodología: revisión integradora donde se analizaron artículos (en inglés) publicados entre el 2014 y el 2019 en la base de datos EBSCO y sitio web organizacional FSICU.org. Se utilizaron como palabras clave: satisfacción del pacient, cuidados intensivos, FS-ICU-24.Para sistematizar esta información artículos se elaboró un instrumento con informaciones talescomo, autores y año de publicación, pais, objetivo de estúdio, resultados y participantes. Resultados: el ‘FS-ICU-24’ es un bueno instrumento para averiguar la satisfacción familiar en la UCI. Los niveles de satisfacción familiar en general son altos (superiores que 70%).En la respuesta abierta del cuestionario, los familiares indican aspectos menos positivos relacionados con el ambiente físico y con la comunicación, y también son sugeridas hipótesis de mejoría cómo proporcionar más privacidad en la sala de espera y capacitar al equipo de comunicación. Conclusión: la revisión integradora muestra que la FS-ICU-24 permite medir la satisfacción familiar e identificar puntos para mejorar en la UCI.Enquadramento: a dificuldade para avaliar a satisfação do doente numa unidade de cuidados intensivos (UCI) determina que para aferir a qualidade dos cuidados nestas unidades seja considerada a satisfação familiar. Objetivos: verificar a viabilidade do questionário ‘FS-ICU-24’ enquanto instrumento para avaliar a satisfação familiar; identificar níveis de satisfação através do questionário com enfase nos itens onde os níveis são mais baixos e estratégias para aumentar a satisfação. Metodologia: revisão integrativa com análise de artigos publicados entre 2014 e 2019 na base de dados EBSCO e no FSICU.org. Descritores: Satisfação do doente; Cuidados intensivos; FS-ICU-24. Para sistematizar a informação, foi elaborada uma tabela com os pontos: autores e ano; país, objetivos do estudo; resultados e participantes. Resultados: o FS-ICU-24 é um bom instrumento para aferir a satisfação familiar na UCI. Os níveis de satisfação familiar são altos (Superiores a 70%). Nas questões de resposta aberta, familiares indicam aspetos menos positivos como o ambiente físico e a comunicação. Algumas das hipóteses de melhoria sugeridas são proporcionar privacidade na sala de espera e treino à equipa em comunicação. Conclusão: a revisão integrativa evidencia que o FS-ICU-24 permite aferir a satisfação familiar e identificar pontos a melhorar na UCI

    Lazer: um contributo da Enfermagem de Reabilitação na autonomia da pessoa com DPOC

    Introduction: The impact of respiratory diseases on a person's physical and mental health is widely known. The Rehabilitation Nursing Specialist has always been sensitive to this problem, and the focus of intervention in its nursing care plan is the definition of adaptive strategies.For the person with Chronic Obstructive Pulmonary Disease, the current context of the pandemic situation has brought consequences at various levels, including a reduction in physical activity, social isolation and increased anxiety.This context reinforced the importance of reflecting on the adoption of strategies that promote the reduction of intolerance to the activity of the person with Chronic Obstructive Pulmonary Disease to carry out Activities of Daily Living , namely in activities in the field of “leisure”. This study aimed to assess the impact of a Respiratory Rehabilitation Program on the realization of Activities of Daily Living in “leisure” domain, measured by the London Chest Activity of Daily Living scale.Methodology: A retrospective correlational study was carried out, which included patients with Chronic Obstructive Pulmonary Disease admitted to the Respiratory Rehabilitation Program, the results of the Activities of Daily Living in the “Leisure” domain was evaluated using the London Chest Activity of Daily Living Scale, before and after the Respiratory Rehabilitation Program. 46 participants were included, (67.39% male and 32.6% female), with ages between 40 and 89 years, for an average age of 63 years old (SD = 10.5). Most patients had basic level education (52.2%).Results: Statistically significant improvements were identified (Z = -3.395, p = 0.001) in the “leisure” domain of the London Chest Activity of Daily Living Scale score after Respiratory Rehabilitation Program protocol completion.Conclusion: There was improvement in the leisure domain of London Chest Activity of Daily Living Scale score after the performance of the Respiratory Rehabilitation Program protocol.Introducción: Es ampliamente conocido el impacto de las enfermedades respiratorias en la salud física y mental de los pacientes. El Enfermero Especialista en Enfermería de siempre ha sido sensible a este tema, y parte de su intervención es la definición de estrategias de adaptación. Para la persona con Enfermedad Pulmonar Obstructiva Crónica, el contexto actual de la situación pandémica ha traído consecuencias a varios niveles, incluida la reducción de la actividad física, el aislamiento social y el aumento de la ansiedad. Este contexto reforzó la importancia de reflexionar sobre la adopción de estrategias que promuevan la reducción de la intolerancia a la actividad de la persona con Enfermedad Pulmonar Obstructiva Crónica para realizar Actividades de la Vida Diaria, concretamente en actividades del ámbito del “ócio”. Este estudio tuvo como objetivo evaluar el impacto de un Programa de Rehabilitación Respiratoria  en la realización de Actividades de la Vida Diaria  en el dominio del “ócio”, medido por la escala London Chest Activity of Daily Living. Metodología: Se realizó un estudio correlacional retrospectivo, que incluyó a pacientes con Enfermedad Pulmonar Obstructiva Crónica ingresados en el Programa de Rehabilitación Respiratoria, siendo los resultados del ámbito "ocio" evaluados a través de la Escala London Chest Activity of Daily Living, antes y después del Programa de Rehabilitación Respiratoria. Se incluyeron 46 participantes (67,39% hombres y 32,6% mujeres). La edad de los participantes fue entre 40 y 89 años, con un promedio de edad de 63 años (DE = 10,5). La mayoría de los participantes tenía la educación básica (52,2%). Resultados: Se identificaron diferencias estadísticamente significativas (Z = -3.395, p = 0.001) en el ámbito del “ocio” en la escala London Chest Activity of Daily Living. Conclusión: Se han comprobado ganancias clínicas en el dominio de “ocio” en la escala London Chest Activity of Daily Livin después de realizar la Programa de Rehabilitación Respiratoria.Introdução: É amplamente conhecido o impacto das doenças respiratórias na saúde física e mental da pessoa. O Enfermeiro Especialista em Enfermagem de Reabilitação sempre foi sensível a esta problemática, sendo foco de intervenção nos seus cuidados a definição de estratégias adaptativas.  Para a pessoa com Doença Pulmonar Obstrutiva Crónica, o contexto atual da situação pandémica trouxe consequências aos vários níveis entre as quais: redução da atividade física, isolamento social e aumento da ansiedade. Este contexto veio reforçar a importância de uma reflexão acerca da adoção de estratégias adaptativas que promovam a redução da intolerância à atividade da pessoa com Doença Pulmonar Obstrutiva Crónica na execução das Atividades de Vida Diárias nomeadamente as atividades no domínio do “lazer”. Este estudo teve como objetivo avaliar o impacto de um Programa de Reabilitação Respiratória na realização das Atividades de Vida Diárias no domínio do “lazer”, medido pela escala London Chest Activity of Daily Living. Metodologia: Efetuou-se um estudo retrospetivo correlacional, que incluiu utentes com Doença Pulmonar Obstrutiva Crónica admitidos no Programa de Reabilitação Respiratória, sendo os resultados do domínio “Lazer” avaliados, através da escala London Chest Activity of Daily Living, antes e após o Programa de Reabilitação Respiratória. Foram incluídos 46 participantes, (67,39% do género masculino e 32,6% do género feminino). A idade dos participantes situou-se entre os 40 e os 89 anos, para uma média de idade de 63 anos (DP=10,5). A maioria dos participantes possui o 1º ciclo do ensino básico (52,2%). Resultados: Foram identificadas diferenças estatisticamente significativas (Z=-3,395, p=0,001) no domínio do “lazer” da escala London Chest Activity of Daily Living. Conclusão: Verificaram-se ganhos clínicos no domínio “lazer” da escala London Chest Activity of Daily Living após a realização do Programa de Reabilitação Respiratória

    Revisiting niche fundamentals with Tukey depth

    1. The first attempts to describe species ecological niches were simple geometric procedures that depict the niche boundaries directly from environmental data. The convex hull was one of such procedures, popular for its simplicity, clear ecological rational and precise definition of the niche. However, it lacked the ability to differentiate areas of the niche with different probabilities of occurrence according to environmental suitability. 2. We incorporate the Tukey depth, a mathematical tool to measure the centrality of a point within a cloud of points on a multidimensional space, in the convex hull approach to (i) propose a new procedure (CH-Tukey) to estimate species’ environmental suitability, and (ii) estimate niche overlap coherently. In addition to a clear ecological rational and simplicity the CHTukey procedure has a number of attractive features: use of presence-only data; independence from background data; invariance to scale; robustness to outliers; and the decomposition of the niche into a finite number of isosuitability levels, permitting the computation of consistent overlap indices. We illustrate the use of CH-Tukey, using occurrence data of the main Quercus species and subspecies from Western Mediterranean Europe, comparing its outputs with BIOCLIM and MAXENT. 3. Results showed distinct niche geometries among the different approaches. BIOCLIM produced rectilinear niches reflecting the assumption that ecological variables are independent in their action on the species. CHTukey, relaxing this assumption, adjusts niche outer boundary and the inner suitability levels to the known occurrences. MAXENT produced unbounded niche geometries, showing abrupt shifts in the species response to the environmental variables. 4. The niche predictions obtained with geometric approaches, BIOCLIM and CH Tukey, are simpler but better aligned with Hutchinson’s niche concept than those obtained with MAXENT, this latter showing ecologically implausible relationships with the environmental variables. CH-Tukey and the related overlap measures provide an adequate tool to explore niche properties and species-environment relationships

    In Vitro Toxicity of Industrially Relevant Engineered Nanoparticles in Human Alveolar Epithelial Cells: Air-Liquid Interface versus Submerged Cultures

    This article belongs to the Special Issue Engineered Nanomaterials Exposure and Risk Assessment: Occupational Health and SafetyDiverse industries have already incorporated within their production processes engineered nanoparticles (ENP), increasing the potential risk of worker inhalation exposure. In vitro models have been widely used to investigate ENP toxicity. Air-liquid interface (ALI) cell cultures have been emerging as a valuable alternative to submerged cultures as they are more representative of the inhalation exposure to airborne nano-sized particles. We compared the in vitro toxicity of four ENP used as raw materials in the advanced ceramics sector in human alveolar epithelial-like cells cultured under submerged or ALI conditions. Submerged cultures were exposed to ENP liquid suspensions or to aerosolised ENP at ALI. Toxicity was assessed by determining LDH release, WST-1 metabolisation and DNA damage. Overall, cells were more sensitive to ENP cytotoxic effects when cultured and exposed under ALI. No significant cytotoxicity was observed after 24 h exposure to ENP liquid suspensions, although aerosolised ENP clearly affected cell viability and LDH release. In general, all ENP increased primary DNA damage regardless of the exposure mode, where an increase in DNA strand-breaks was only detected under submerged conditions.
